Survival analysis of breast cancer liver metastasis treated by hepatectomy: A propensity score analysis for Chinese women in Hong Kong

Background: Survival of patients with breast cancer liver metastasis is very poor. This study aimed to analyze the survival outcome of hepatectomy for this patient population. Methods: From January 1995 to December 2014, 2522 patients with liver cancer received hepatectomy at our hospital. Twenty-one of them, all female, received the operation for breast cancer liver metastasis. Performance was compared with patients with colorectal liver metastasis treated with hepatectomy after propensity score analysis in a ratio of 1:3. Results: Twenty-one patients received hepatectomy for breast cancer. After propensity score matching, 63 patients who had hepatectomy for colorectal cancer were selected for comparison. There was no significant difference in immediate or short-term outcomes between the two groups of patients in terms of operative time, blood loss and surgical morbidities. All patients with breast cancer had R0 resection. No hospital death occurred. After hepatectomy, the 1-, 3- and 5-year overall survival rates were 100.0%, 58.9% and 58.9% respectively in patients with breast cancer. The 1-, 3- and 5-year overall survival rates were 95.0%, 57.2% and 39.7% respectively in patients with colorectal cancer (P = 0.572). On multivariate analysis, triple negative status was the only independent poor prognostic factor in breast cancer liver metastasis (OR = 6.411;95% CI: 1.351–30.435;P = 0.019). Conclusions: Hepatectomy is a safe and effective way of treating breast cancer liver metastasis at experienced centers where multidisciplinary adjuvant treatments are available. It can be considered more frequently as part of the multidisciplinary care for this patient population.

Evolution of systemic therapy of advanced hepatocellular carcinoma

Hepatocellular carcinoma (Hcc) commonly occurs in hepatitis B endemic areas, especially in Asian countries. Hcc is highly refractory to cytotoxic chemotherapy. This resistance is partly related to its tumor biology, pharmacokinetic properties, and both intrinsic and acquired drug resistance. There is no convincing evidence thus far that systemic chemotherapy improves overall survival in advanced Hcc patients. Other systemic approaches, such as hormonal therapy and immunotherapy, have also disappointing results. Recently, encouraging results have been shown in using sorafenib in the treatment of advanced Hcc patients. In this review, we concisely summarize the evolution of developments in the systemic therapy of advanced Hcc.

Colon cancer in a patient with underlying aplastic anemia: A clinical challenge

The association of gastrointestinal malignancy with aplastic anemia has rarely been reported in the literature. Although it is not clear whether there is any direct relationship between aplastic anemia and gastrointestinal cancers, a retrospective analysis did suggest the notion that patients with aplastic anemia might have a higher incidence of colorectal cancer. Here, we report the diagnostic and therapeutic challenges in managing a patient with aplastic anemia and advanced colorectal cancer. Early diagnosis is challenging due to overlapping symptomatology and clinical features, increased risk of diagnostic procedures, and confounding complications arising from aplastic anemia and its treatment. A high index of suspicion and multidisciplinary input are essential.

A two-level method for sparse time-frequency representation of multiscale data Dedicated to Professor LI TaTsien on the Occasion of His 80th Birthday

Based on the recently developed data-driven time-frequency analysis(Hou and Shi, 2013), we propose a two-level method to look for the sparse time-frequency decomposition of multiscale data. In the two-level method, we first run a local algorithm to get a good approximation of the instantaneous frequency. We then pass this instantaneous frequency to the global algorithm to get an accurate global intrinsic mode function(IMF)and instantaneous frequency. The two-level method alleviates the difficulty of the mode mixing to some *** also present a method to reduce the end effects.

Evaluating the Spatial Uncertainty of Future Land Abandonment in a Mountain Valley(Vicdessos, Pyrenees-France): Insights from Model Parameterization and Experiments

European mountains are particularly sensitive to climatic disruptions and land use *** latter leads to high rates of natural reforestation over the last 50 years. Faced with the challenge of predicting possible impacts on ecosystem services,LUcc models offer new opportunities for land managers to adapt or mitigate their *** the spatial uncertainty of future LUcc is crucial for the definition of sustainable land use strategies. However, the sources of uncertainty may differ, including the input parameters, the model itself, and the wide range of possible futures. The aim of this paper is to propose a method to assess the probability of occurrence of future LUcc that combines the inherent uncertainty of model parameterization and the ensemble uncertainty of the future based scenarios. For this purpose, we used the Land Change Modeler tool to simulate future LUcc on a study site located in the Pyrenees Mountains(France) and two scenarios illustrating two land use strategies. The model was parameterized with the same driving factors used for its calibration. The definition of ‘static vs. dynamic' and ‘quantitative ***(discretized)' driving factors, and their combination resulted in four parameterizations. The combination of model outcomes produced maps of the spatial uncertainty of future LUcc. This work involves adapting the definition of spatial uncertainty in the literature to future-based LUcc studies. It goes beyond the uncertainty of simulation models by integrating the uncertainty of the future to provide maps to help decision makers and land managers.

We prove that for all n = 4k- 2 and k 2 there exists a closed smooth complex hyperbolic manifold M with real dimension n having non-trivial π1(T<0(M)). T<0(M) denotes the Teichm¨uller space of all negatively curved Riemannian metrics on M, which is the topological quotient of the space of all negatively curved metrics modulo the space of self-diffeomorphisms of M that are homotopic to the identity.

Background and Objective:Hong Kong,like many parts of Asia,faces a high burden of hepatocellular carcinoma(Hcc)caused by high endemic rates of hepatitis B virus *** Kong clinicians have developed a high level of expertise in Hcc treatment across surgical,transarterial,ablative,radiotherapeutic and systemic *** publication summarizes the latest evidence-based recommendations on how these modalities should be ***:In two meetings held in 2020,a multidisciplinary panel of surgeons,oncologists and interventional radiologists performed a narrative review of evidence on the management of Hcc,with an emphasis on treatment of Hcc not amenable to surgical *** attention was paid to new evidence published since the previous version of these statements in *** Content and Findings:The expert panel has formulated 60 consensus statements to guide the staging and treatment of unresectable *** the previous version of these statements,considerable additions have been made to the recommendations on use of targeted therapies and immunotherapies because of the large volume of new ***:Our consensus statements offer guidance on how to select Hcc patients for surgical or non-surgical treatment and for choosing among non-surgical modalities for patients who are not candidates for *** particular,there is a need for more evidence to aid physicians in the selection of second-line systemic therapies,as currently most data are limited to patients with disease progression on first-line sorafenib.
