Reduction of ischemia reperfusion injury after liver resection and hepatic inflow occlusion by α-lipoic acid in humans

AIM: To evaluate the protective effects of precondition- ing by α-lipoic acid (LA) in patients undergoing hepatic resection under inflow occlusion of the liver. METHODS: Twenty-four patients undergoing liver re- section for various reasons either received 600 mg LA or NaCl 15 min before transection performed under inflow occlusion of the liver. Blood samples and liver wedge bi- opsy samples were obtained after opening of the abdo- men immediately after inflow occlusion of the liver, and 30 min after the end of inflow occlusion of the liver. RESULTS: Serum levels of aspartate transferase and alanine transferase were reduced at all time points in patients who received LA in comparison to those who received NaCL. This was accompanied by reduced histo- morphological features of oncosis. We observed TUNEL- positive hepatocytes in the livers of the untreated patients, especially after 30 min of ischemia. LA attenu- ated this increase of TUNEL-positive hepatocytes. Under preconditioning with LA, ATP content was significantly enhanced after 30 min of ischemia and after 30 min of reperfusion. CONCLUSION: This is the first report on the poten- tial for LA reducing ischemia/reperfusion injury (IRI) of the liver in humans who were undergoing liver surgery. Beside its simple and rapid application, side effects did not occur. LA might therefore represent a new strategy against hepatic IRI in humans.

AIM: To compare different preconditioning strategies to protect the liver from ischemia/reperfusion injury focusing on the expression of pro- and anti-apoptotic proteins. Interventions comprised different modes of ischemic preconditioning (IP) as well as pharmacologic pretreatment by α-lipoic acid (LA). METHODS: Several groups of rats were compared: sham operated animals, non-pretreated animals (nt), animals receiving IP (10 min of ischemia by clamping of the portal triad and 10 min of reperfusion) prior to sustained ischemia, animals receiving selective ischemic preconditioning (IPsel, 10 min of ischemia by selective clamping of the ischemic lobe and 10 min of reperfusion) prior to sustained ichemia, and animals receiving 500 μmol α-LA injected i.v. 15 min prior to the induction of 90 min of selective ischemia. RESULTS: Cellular damage was decreased only in the LA group. TUNEL-positive hepatocytes as well as necrotic hepatocyte injury were also decreased only by LA (19 ± 2 vs 10 ± 1, P < 0.05 and 29 ± 5 vs 12 ± 1, P < 0.05). Whereas caspase 3- activities in liver tissue were unchanged, caspase 9- activity in liver tissue was decreased only by LA pretreatment (3.1 ± 0.3 vs 1.8 ± 0.2, P < 0.05). Survival rate as the endpoint of liver function was increased after IP and LA pretreatment but not after IPsel. Levels of lipid peroxidation (LPO) in liver tissue were decreased in the IP as well as in the LA group compared to the nt group. Determination of pro- and anti-apoptotic proteins showed a shift towardsanti-apoptotic proteins by LA. In contrast, both our IP strategies failed to influence apototic cell death. CONCLUSION: IP, consisting of 10 min of ischemia and 10 min of reperfusion, protects only partly against ischemia/reperfusion injury of the liver prior to 90 min of selective ischemia. IPsel did not influence ischemic tolerance of the liver. LA improved tolerance to ischemia, possibly by downregulation of pro-apoptotic Bax.

AIM: To define the (co-)expression pattern of target receptor-tyrosine-kinases (RTK) in human gastric adenocarcinoma. METHODS: The (co-)expression pattern of VEGFR1-3,PDGFRα/b and EGFR1 was analyzed by RT-PCR in 51 human gastric adenocarcinomas. In addition,IHC staining was applied for confirmation of expression and analysis of RTK localisation. RESULTS: The majority of samples revealed a VEGFR1 (98%),VEGFR2 (80%),VEGFR3 (67%),PDGFRα (82%) and PDGFRβ(82%) expression,whereas only 62% exhibited an EGFR1 expression. 78% of cancers expressed at least four out of six RTKs. While VEGFR1-3 and PDGFRα revealed a predominantly cytoplasmatic staining in tumor cells,accompanied by an additional nuclear staining for VEGFR3 ,EGFR1 was almost exclusively detected on the membrane of tumor cells. PDGFRβ was restricted to stromal pericytes,which also depicted a PDGFRα ***-tyrosine-kinases coexpression in gastric adenocarcinoma and might therefore encourage an application of multiple-target RTK-inhibitors within a combination therapy.

Risk of conduction disturbances following different transcatheter aortic valve prostheses:the role of aortic valve calcifications

OBJECTIVES To assess the impact of prosthesis choice and aortic valve calcifications on the occurrence of conduction disturb-ances after transcatheter aortic valve implantation(TAVI).METHODS We retrospectively analyzed the preoperative clinical characteristics,electrocardiograms,contrast-enhanced mul-tidetector computed tomography scans and procedural strategies of patients who underwent TAVI in our center between January 2012 and June *** of calcium volume was performed for each aortic cusp above(aortic valve)and below(left ventricular outflow tract,LVOT)the basal *** analysis was performed to evaluate risk factors for the onset of new bundle branch block(BBB),transient and permanent atrioventricular block(tAVB,pAVB).RESULTS A total of 569 patients were included in the *** different prostheses were implanted(Edwards Sapien XT,n=162;Edwards Sapien 3,n=240;Medtronic CoreValve,n=27;Medtronic CoreValve Evolut R,n=21;Symetis Acurate,n=56;Sy-metis Acurate neo,n=63).The logistic regression analysis for BBB showed association with baseline left anterior *** logistic regression for tAVB,found the prior valvuloplasty and the balloon post-dilatation associated with the *** left and right BBB,degree of oversizing,and LVOT calcification beneath the non-coronary cusp were associated with *** the prosthesis model,nor the use of a self-expandable prosthesis showed statistical significance with the above-men-tioned outcomes on univariate *** LVOT calcification beneath the non-coronary cusp,baseline left anterior hemiblock,right BBB,balloon post-dilatation,prior valvuloplasty and oversizing are independently associated with postprocedural conduction disturbances after *** of a self-expandable prosthesis may show a lower incidence of AVB,if applied in lower calcified aortic valves.

End-Triassic ammonoid and bivalve faunas of the Germig area, Tibetan Himalaya, lived in a tropical, shallow-water environment during the Triassic-Jurassic boundary interval. High stratigraphic resolution based on ammonite-biochrons allows to tracing the place of origin of several faunal elements. The bivalves Aguilerella and Ctenostreon occurred first in the Tibetan Himalaya and migrated from there to the eastern South Pacific, exhibiting a pantropic dispersal pattern. This dispersal route is supported by the distribution pattern of the ammonites Choristoceras, Discamphiceras, Pleuroacanthites, and Psiloceras calliphyllum. A few taxa, which went extinct everywhere else by the end of the Triassic, survived in the Tibetan Himalaya into early Early Jurassic times. They include the ammonites Choristoceras and Eopsiloceras, and the bivalves Newaagia, Terquemia, Persia, Ryderia guangdongensis, and Cultriopsis angusta. This suggests that the Tibetan Himalaya may have played a refugia role in the course of the end-Triassic mass extinction.

Aim： To test the hypothesis that sildenafil （50 mg nightly for one year） can improve spontaneous erectile function （EF） in men with mild-to-moderate arteriogenic erectile dysfunction （ED） responsive to erectogenic treatment. Methods： In a prospective open-label trial, 112 men with ED were randomized to sildenafil 50 mg nightly or sildenafil 50 or 100 mg as needed for 12 months, followed by one-month and 6-month non-medicated periods. Non-randomized, non-medicated men with ED were also assessed. The EF domain of the International Index of Erectile Function （IIEF EF） and the peak systolic velocity （PSV） of penile cavernous arteries were used to measure the efficacy. Results： After sildenafil treatment and a subsequent non-medicated month, IIEF EF was normal in 29 of 48 （60.4%, 95% confidence interval [CI]： 45.3-74.2%） of the nightly group vs. 4 of 49 （8.2%, 95% CI： 2.3-19.6%） of the as-needed group. PSV improved by 11.2 cm/s （95% CI： 4.7-21.4; P = 0.012） in the nightly group but only by 3.4 cm/s （-5.1- 14.7; P = 0.435） in the as-needed group. IIEF EF normalized in 1 of 18 （5.6%, 95% CI： 0.1-27.3%） non-medicated men and the PSV declined slightly. Six months after treatment, the IIEF EF remained normal and PSV was stabilized in most （28/29, 97%） nightly group men who had initially normalized. Conclusion： Sildenafil nightly for one year resulted in ED regression that persisted well beyond the end of treatment, so that spontaneous EF was characterized as normal on the IIEF in most men. The results from this open-label, randomized trial warrant verification under double-blind, placebo-controlled conditions.

The Jurassic succession at Gebet Maghara, North Sinai, Egypt, represents a mixed carbonate-siticictastic sequence. Combining information from both fossits and rocks attowed a ptausibte reconstruction of the depositionat environments and of the basin evotution. The Ju- rassic succession of Gebel Maghara was deposited on a ramp, and the architecture of the ramp facies was strongty controtted not onty by sea-revel changes but atso by extensionat tectonics in connection with rifting of the Tethys, North Gondwana. Seven tectonicalty modified thirdorder sequences （DS 1-DS 7） have been recognized. The first three sequences （DS 1-DS 3）, ranging from the Toarcian to the Bajocian, record sea invasion （intertidat to shattow subtida[lconditions） across an intracratonic area as a resutt of eustatic sea-fever changes during a quiescent rift stage. The remaining sequences （DS 4-DS 7） reflect open marine mid to outer ramp settings. Non-marine conditions around the Bajocian-Bathonian boundary, documented by catiche, represent the maximum regression of the sea. During an active extensionat stage, horsts, which formerty acted as barriers separating the Maghara sub-basin from the main ocean, subsided. Subsequent rejuvenation and reactivation of fautts shifted the homoctinat physiography of the ramp to a distatty steepened ramp during the earty Bathonian, creating a 200-m-thick dettaic wedge. SimiLar processes during the earty Kimmeridgian created a catcirudite-catcarenite succession of stope origin. The diversity and the epifaunat/infaunat percentage of the macrofauna disptay a cyctic pattern which coincides more or tess with the sequence stratigraphic architecture.
