Background:Information on the association between physical activity(PA)and the risk of chronic kidney disease(CKD)is *** aimed to explore the associations of total,domain-specific,and intensity-specific PA with CKD and its subtypes in ***:The study included 475,376 adults from the china Kadoorie Biobank aged 30-79 years during 2004-2008 at *** interviewer-administered questionnaire was used to collect the information about PA,which was quantified as metabolic equivalent of task hours per day(MET-h/day)and categorized into 4 groups based on *** regression was used to analyze the association between PA and CKD ***:During a median follow-up of 12.1 years,5415 incident CKD cases were documented,including 1159 incident diabetic kidney disease(DKD)cases and 362 incident hypertensive nephropathy(HTN)*** PA was inversely associated with CKD risk,with an adjusted hazard ratio(HR,95%confidence interval(95%CI))of 0.83(0.75-0.92)for incident CKD in the highest quartile of total PA as compared with participants in the lowest *** results were observed for risk of DKD and HTN,and the corresponding HRs(95%CIs)were 0.75(0.58-0.97)for DKD risk and 0.56(0.37-0.85)for HTN *** nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA were significantly associated with a decreased risk of CKD,with HRs(95%CIs)of 0.80(0.73-0.88),0.85(0.77-0.94),and 0.85(0.76-0.95)in the highest quartile,***:PA,including nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA,was inversely associated with the risk of CKD,including DKD,HTN,and other CKD,and such associations were dose dependent.

目的分析影响中国人群长寿的社会人口学特征和生活方式相关因素。方法基于中国慢性病前瞻性研究(CKB),排除1938年12月31日之后出生者,纳入分析51870人,将到达年龄定义为研究对象在随访期间死亡时的年龄或截至2018年12月31日的年龄,根据到达年龄对研究对象进行分组:非长寿组(80岁以前死亡)和长寿组(到达年龄≥80岁),长寿组进一步分组为长寿但死亡、长寿且存活。社会人口学和生活方式信息为2004-2008年基线调查时采集。采用无序多分类logistic回归模型分析暴露因素与结局的关联,以非长寿组为参照组。结果纳入分析的51870人的基线年龄范围为65~79岁,经过(10.2±3.5)年的随访,38841人长寿,其中30354人存活。以男性、农村人群、非在婚者、家庭年收入<10000元者和小学及以下文化程度者为参照,女性、城市人群、在婚者、家庭年收入≥20000元者、大专及以上文化程度者长寿且存活的OR值(95%CI)分别为1.68(1.58~1.78)、1.69(1.61~1.78)、1.15(1.10~1.21)、1.44(1.36~1.53)和1.32(1.19~1.48)。对于生活方式相关因素,总体力活动水平每增加4 MET-h/d,长寿且存活的OR值(95%CI)为1.09(1.08~1.10)。分别以从不或偶尔吸烟者、非每周饮酒者、正常体重者(BMI 18.5~23.9 kg/m2)和腰围<85 cm(男性)/<80 cm(女性)者为参照组,吸烟或因病戒烟者中每日吸烟≥20支者长寿且存活的OR值(95%CI)为0.64(0.60~0.69),每周但非每日饮酒者和每日纯酒精摄入量<30 g者为1.29(1.14~1.46)和1.13(1.01~1.26),低体重、超重和肥胖者依次为0.56(0.52~0.61)、1.27(1.19~1.36)和1.23(1.11~1.36),中心性肥胖者为0.86(0.79~0.93)。进一步按腰围进行分层分析,腰围<85 cm(男性)/<80 cm(女性)者中,BMI每增加5 kg/m2,长寿且存活的OR值(95%CI)为1.80(1.69~1.92);而在腰围≥85 cm(男性)/≥80 cm(女性)者中,对应的OR值(95%CI)为1.02(0.96~1.08),两层关联效应值差异有统计学意义(交互检验P<0.001)。结论本研究发现女性、在婚者、高社会经济水平者、文化程度较高者以及生活方式健康者更有可能长寿。

受限于传统自我报告问卷测量方法的信、效度,身体行为(身体活动、久坐行为和睡眠)的准确测量受到很大限制。随着科学技术的发展,加速度计等客观测量设备能够更加准确、全面地测量身体行为相关特征,并逐渐为大规模流行病学研究所用。然而,我国乃至亚洲人群尚缺乏采集身体行为客观测量数据的大型队列研究。中国慢性病前瞻性研究(CKB)项目在第三次重复调查(2020年8月至2021年12月)使用Axivity AX3腕式三轴加速度计测量研究对象的日常活动及睡眠情况,最终共纳入全国10个项目地区20370名研究对象,其中女性占65.2%,年龄(65.4±9.1)岁,不同项目地区研究对象的身体行为活动水平具有较大差异。CKB项目开展的身体行为客观测量数据为描述我国中老年人24 h身体行为特征、探究身体活动、久坐行为、睡眠的健康效应以及其与疾病的深入研究提供了具有重要价值的科研资源。

The shear strength parameters property of rock-soil aggregates in embankment slope of reservoir,that is,the relationship between cohesion and gravel content,between friction angle and gravel content,and the relationship between cohesion and water content,between friction angle and water content,is studied based on the direct shear test results,the shear strength change law of the rock-soil aggregates is given,and the unsaturated shear strength formulation of rock-soil aggregates that could consider suction and saturation degree influence is put forward in this paper,through which the sliding or failure physical mechanism of this type of slope under the condition of rainfall infiltration is studied. Also the 3D unsteady saturated-unsaturated seepage field and its FEM resolving mode are established based on the analysis of the slope rainfall infiltration process. Case study with this method indicates that the minimum safety factor of the accumulated rock-soil aggregates dose not arrive at the moment of rainfall cessation,but appears several hours after the rainfall cessation,this phenomenon is in accordance with the practical slope engineering's failure process and could explain appropriately the slope failure caused by rainfall infiltration. Research results in this paper have an important reference value for the research on stability of the accumulated rock-soil aggregates in embankment slope of reservoir,and can enrich the stability analysis method and relevant theory of reservoir embankment slope.

预防和控制慢性非传染性疾病(慢性病)是我国面临的重要公共卫生问题。西北区域自然环境独特,人群健康状况与疾病特征具有鲜明的地域特色,为了满足深入研究该地区慢性病病因、机制和转归的迫切需求,根据西北区域特殊生态环境、人口规模和民族分布,于2017-2019年在陕西省、甘肃省、青海省、宁夏回族自治区和新疆维吾尔自治区建立西北区域自然人群队列,招募了117644名35~74岁的多民族(汉族、回族、维吾尔族、哈萨克族和藏族)自然人群,调查队列成员的个体、环境及社会等多方面的基线暴露信息,采集外周血,分离血浆、血清和白膜层,形成各类生物样本90余万份,以-80℃储存于标准生物样本库。已开始用常规监测和主动随访相结合的方式进行长期随访观察。队列成员的平均年龄52.43岁,女性70391名(59.8%),不同民族间的社会经济状况和生活方式存在一定差异,健康状况具有相似性但呈现出各自不同的特点。该队列为深入研究环境、生活方式及遗传因素与重要高发慢性病的关系提供了研究平台,为明确该地区不同民族慢性病的病因及长期健康危害提供重要流行病学证据,为国家制定慢性病防治战略提供西北区域参考。本文简要介绍西北区域自然人群队列的建设概况和实践思考。

随着生活行为方式、自然和社会环境的急剧变化,我国慢性非传染性疾病(慢性病)的流行现状及相关危险因素也发生了巨大的改变。全国各地各类慢性病的流行特征及其危险因素分布不尽相同,而华南地区独特的气候、饮食及生活方式对慢性病发生发展的影响至今尚不明确。因此,亟需通过区域性大型队列的建设,为区域重大疾病的病因学研究、早期预测和干预策略提供本土化人群证据,也为国家重大慢性病防治策略的制定提供依据。华南区域自然人群慢性病前瞻性队列于2017年12月正式启动,覆盖广东省、广西壮族自治区、福建省和海南省,以35~74岁常住居民为主要研究对象,包括城市与农村、汉族与以壮族为代表的少数民族多类型自然人群。本队列围绕华南区域常见重大慢性病建立集健康信息和生物样本为一体的精准医学大数据平台,并开展长期随访。现已建立116520人的基线数据库,年龄为(54.9±12.5)岁,女性71077人(61.0%)。基线数据库包括问卷调查、体格检查数据以及生物样本。本文对华南区域自然人群慢性病前瞻性队列设计的理念、研究进展和随访设想作简要介绍。

近年来我国慢性非传染性疾病(慢性病)负担逐年加重,且存在区域异质性。我国西南地区地域辽阔,民族众多,地域特色鲜明,慢性病负担沉重,但卫生资源相对匮乏,高危慢性病防控形势严峻。为全面深入解析我国西南地区多民族人群疾病分布模式和潜在暴露风险规律,西南区域自然人群队列项目组受国家重点研发计划“精准医学研究”重点专项资助,于2017年启动西南区域自然人群队列建设。目前,西南区域自然人群队列在四川省、云南省、贵州省、西藏自治区、重庆市5省/市/自治区共招募119556名30~79岁居民(西藏自治区为18~79岁),已建成大规模、多民族、高质量自然人群队列及生物样本库。项目组围绕西南自然环境、生活方式的健康效应已取得初步研究进展,并对欠发达地区大型队列的构建模式进行了创新,可为复杂地理场景的大规模自然人群队列建立与研究提供借鉴,为我国精准医学研究与多民族疾病防控决策提供高质量基础资源。

Background: Existing evidence suggests that fruit consumption is a significant influencing factor for chronic obstructive pulmonary disease (COPD), but this is unclear in the Chinese population. We examined the association of fresh fruit consumption with the risk of COPD-related hospitalization and death in a nationwide, population-based prospective cohort from china. Methods: Between 2004 and 2008, the china Kadoorie Biobank recruited >0.5 million adults aged 30 to 79 years from ten diverse regions across china. After excluding individuals diagnosed with major chronic diseases and prevalent COPD, the prospective analysis included 421,428 participants. Cox regression was used to calculate the hazard ratios (HRs) for the association between fresh fruit consumption and risk of COPD-related hospitalization and death, with adjustment for established and potential confounders. Results: During a mean follow-up of 10.9 years, 11,292 COPD hospitalization events and deaths were documented, with an overall incidence rate of 2.47/1000 person-years. Participants who consumed fresh fruit daily had a 22% lower risk of COPD-related hospitalization and death compared with non-consumers (HR= 0.78, 95% confidence interval [CI]: 0.71–0.87). The inverse association between fresh fruit consumption and COPD-related hospitalization and death was stronger among non-current smokers and participants with normal body mass index (BMI) (18.5 kg/m 2 ≤ BMI cessation and weight control, should be considered in the prevention and management of COPD.
