Introduction: One of the most common causes of hyperthyroidism is Graves’ disease. The malignant exophthalmos is a rare condition characterized by the loss of visual function that doesn’t adequately respond to various forms of treatment, requiring, in most cases, surgical correction. Objective: We report a case of conjunctival edema and protrusion of the eyeball in patients secondary to Graves’ disease, characterizing malignant ophthalmopathy and discuss their surgical treatment. Case Report: A 73-year-old male patient presenting bilateral conjunctive hyperaemia, decreased visual acuity, exophtalmos with progressive deterioration. Pulse therapy was performed without improvement and then underwent bilateral ethmoidectomy. Discussion: Late diagnosis is harmful due to the fact that malignant disease is extremely aggressive and fast, as reported in this case. Most often, the ophthalmopathy follows a relatively mild and self-limiting course, making a proper diagnosis difficult if the disease progresses. In extreme cases acute orbital decompression is indicated complementing appropriate medical treatment, which will allow herniation of orbital contents. Many orbital decompression techniques have been used in the treatment of Graves’ ophthalmopathy. The most common of these is the endoscopic sinus surgery which potentiated act in decompression of the medial wall safely and minimally invasive. In our case, open bilateral ethmoidectomy was used, due to the fact that surgeons are most experienced in this technique. Although the post-operative has been successfully submitted, no signs of recurrence were observed after fifteen days due to the evolution of Graves’ disease. Even with outpatient treatment, he walked to the total visual loss. Conclusion: We observed that the malignant ophthalmopathy is rare and aggressive;however, in spite of therapeutic resources used, it can progress to total visual loss.

Cloud computing is a dynamic and rapidly evolving field,where the demand for resources fluctuates *** paper delves into the imperative need for adaptability in the allocation of resources to applications and services within cloud computing *** motivation stems from the pressing issue of accommodating fluctuating levels of user demand *** adhering to the proposed resource allocation method,we aim to achieve a substantial reduction in energy *** reduction hinges on the precise and efficient allocation of resources to the tasks that require those most,aligning with the broader goal of sustainable and eco-friendly cloud computing *** enhance the resource allocation process,we introduce a novel knowledge-based optimization *** this study,we rigorously evaluate its efficacy by comparing it to existing algorithms,including the Flower Pollination Algorithm(FPA),Spark Lion Whale Optimization(SLWO),and Firefly *** findings reveal that our proposed algorithm,Knowledge Based Flower Pollination Algorithm(KB-FPA),consistently outperforms these conventional methods in both resource allocation efficiency and energy consumption *** paper underscores the profound significance of resource allocation in the realm of cloud *** addressing the critical issue of adaptability and energy efficiency,it lays the groundwork for a more sustainable future in cloud computing *** contribution to the field lies in the introduction of a new resource allocation strategy,offering the potential for significantly improved efficiency and sustainability within cloud computing infrastructures.

Multicentric Cervical Paraganglioma. Rare Case of Five Locations in One Patient. Case Report and Literature Review

Objective: To report a case of a multiple paraganglioma with 5 concurrent locations and also describe a complication not found in the literature, spontaneous tumor bleeding. Introduction: The paragangliomas are mostly benign and more frequent in women. Can be multiple, and these are considered as the most likely source of familial origin (30%-35%). The multicentric lesions can occur at different life stages not being necessarily simultaneous, a fact that prevents image exam research to be conclusive for tumor presence or absence. In this way the examination of the family members of patients, specially young ones, diagnosed with multicentric tumors may lead to negative results which doesn’t meant that the disease can be developed at a later time period. Therefore routine examination is not justifiable when symptoms relative to tumor presence are not present. Case report: A female patient of 32 years old complaining of tinnitus and diagnosed with five paragangliomas (jugulo-timpanic, bilateral carotid body and bilateral brachiocephalic trunk) through imaging studies. She reported suffering relatives of paragangliomas. Conclusion: The paper reports a rare case and perhaps unique in the literature, multiple paraganglioma with five locations and discusses difficult choice of treatment and complications.

This study investigated the relationships between human resource(HR)practices,employee psychological empowerment,employee innovative work behavior,and firm innovation capability in the context of the Information and Communication Technology(ICT)sector in *** the Social Exchange Theory(SET)as a guiding framework,this study explored how employee psychological empowerment and innovative work behavior mediated the influence of HR practices on a firm’s innovative *** on extensive data collected from ICT organizations in Jordan,our analysis had several key findings.A significant positive correlation between HR practices and psychological empowerment was *** design emerged as the most influential factor affecting psychological empowerment,whereas compensation had a weaker ***,we established that psychological empowerment significantly enhanced innovative work behavior,underscoring its role as a catalyst for innovative work behavior within the ***,our study supported the theory that psychological empowerment influenced a firms’innovation capabilities,emphasizing the pivotal role of employee empowerment in driving firm-level innovation in the Jordanian ICT *** findings provide insights for academic and practical audiences,clarifying how HR practices,employee psychological empowerment,and innovative work behavior collectively influenced firm innovation capability within the unique context of the Jordanian ICT sector.

As cloud computing usage grows,cloud data centers play an increasingly important *** maximize resource utilization,ensure service quality,and enhance system performance,it is crucial to allocate tasks and manage performance *** purpose of this study is to provide an extensive analysis of task allocation and performance management techniques employed in cloud data *** aim is to systematically categorize and organize previous research by identifying the cloud computing methodologies,categories,and gaps.A literature review was conducted,which included the analysis of 463 task allocations and 480 performance management *** review revealed three task allocation research topics and seven performance management *** allocation research areas are resource allocation,load-Balancing,and *** management includes monitoring and control,power and energy management,resource utilization optimization,quality of service management,fault management,virtual machine management,and network *** study proposes new techniques to enhance cloud computing work allocation and performance ***-comings in each approach can guide future *** research’s findings on cloud data center task allocation and performance management can assist academics,practitioners,and cloud service providers in optimizing their systems for dependability,cost-effectiveness,and *** methodologies can steer future research to fill gaps in the literature.

Burnishing is a profitable process of surface finishing due to its ability to be automated,which makes burnishing method more desirable than other finishing *** obtain high surface finish,non-stop operation is required for CNC machine and we can attain that by choosing a suitable trajectory of the finishing *** other words,burnishing paths should be multidirectional rather than monotonic,in order to cover uniformly the ***,the burnishing force is also a key parameter of the burnishing process because it determines the degree of plastic deformation,and that makes determining the optimum burnishing force an essential step of the burnishing process a ***,we consider the strategy of ball burnishing path and the burnishing force as variable parameters in this *** this paper,we propose a new strategy of burnishing tool path with trochoid cycles that achieves a multidirectional burnishing of the surface according to various *** into consideration the optimum burnishing force,to improve flat surface finish of AL6061-O samples by reducing the surface roughness parameter(Rz).Experiments carried out on 3-axis milling machine show that the proposed trochoidal path is more effective than the conventional one.

Ketamine and midazolam sedation for pediatric gastrointestinal endoscopy in the Arab world

AIM:To evaluate the safety and effectiveness of intravenous ketamine-midazolam sedation during pediatric endoscopy in the Arab ***:A retrospective cohort study of all pediatric endoscopic procedures performed between 2002-2008 at the shared endoscopy suite of King Abdullah University Hospital,Jordan University of Science & Technology,Jordan was *** children were > 1 year old and weighed > 10 kg with American Society of Anesthesiologists class 1 or *** was performed in terms of sedation-related complications(desaturation,respiratory distress,apnea,bradycar-dia,cardiac arrest,emergence reactions),adequacy of sedation,need for sedation reversal,or failure to complete the ***:A total of 301 patients(including 160 males) with a mean age of 9.26 years(range,1-18 years) were *** were premedicated with atropine;and 79.4%(239/301) had effective and uneventful *** 248(82.4%) of the 301 patients received a mean dose of 0.16 mg/kg(range,0.07-0.39) midazolam and 1.06 mg/kg(range,0.31-2.67) ketamine,respectively within the recommended dosage *** maximum midazolam dose was exceeded in 17.6% patients [34 female(F):19 male(M),P = 0.003] and ketamine in 2.7%(3 M:5 F).Maximum midazolam dose was more likely to be exceeded than ketamine(P 1 year and weighing > 10 kg without co-morbidities.

INTRODUCTION Despite the known benefits of antipsychotic medications,they have been associated with increased risks of cardiometabolic side ***-induced adverse cardiometabolic effects,such as weight gain,high blood pressure,alterations in glucose metabolism and lipid dysregulation,are well-recognised risk factors for cardiovascular diseases(CVDs)and diabetes.1 Clinical guidelines call for regular physical health assessments and laboratory monitoring as a means to prevent and detect adverse cardiometabolic effects of antipsychotic ***,recent evidence suggests that cardiovascular condi-tions remain the key causes of death among people using antipsychotic medications,suggesting suboptimal screening and moni-toring *** cardiometabolic screening among antipsychotic users can hinder the early detection of high-risk people and delay receiving proper management.
