Introduction: Vulnerable is someone who can be hurt or receive injury, physically or morally. The work environment appears as one of the dimensions in which social vulnerability develops and social and occupational vulnerability is defined. The literature does not include an assessment of quantified occupational vulnerability as it already exists in the case of social vulnerability. The aim of this paper is to identify and quantify the variables included in the Protocol for the Assessment of Vulnerability to COVID-19 in such a way that the result can be quantified and allows for a predictive effect on the degree of vulnerability. Methodology: The starting point is the design of a protocol proposal that includes 29 variables. Data is collected from a sample of 420 workers, quantifying the results according to each variable, the overall scores for each group of aspects assessed and, finally, the total score that estimates the degree of vulnerability according to established ranges. Results: Variables that have the highest coefficient and, therefore, the greatest weight in the probability of reaching a group of high or medium vulnerability are cardiovascular disease, cancer, and coagulation alteration. The weight of the labour aspects due to inadequate working conditions stands out. Conclusions: The results obtained with this protocol allow us to make a quantified assessment of labour vulnerability to COVID-19 by integrating individual variables of the worker, his working conditions and the preventive actions of his company against COVID-19, and can be useful as an Occupational Vulnerability Index (OVI).

The thyroid hormones(THs), triiodothyronine(T3) and thyroxine(T4), are essential for survival; they are involved in the processes of development, growth, and metabolism. In addition to hyperthyroidism or hypothyroidism, THs are involved in other diseases. The role of THs in the development and differentiation of mammary epithelium is well established; however, their specific role in the pathogenesis of breast cancer(BC) is controversial. Steroid hormones affect many human cancers and the abnormal responsiveness of the mammary epithelial cells to estradiol(E2) in particular is known to be an important cause for the development and progression of BC. The proliferative effect of T3 has been demonstrated in various types of cancer. In BC cell lines, T3 may foster the conditions for tumor proliferation and increase the effect of cell proliferation by E2; thus, T3 may play a role in the developmentand progression of BC. Studies show that T3 has effects similar to E2 in BC cell lines. despite controversy regarding the relationship between thyroid disturbances and the incidence of BC, studies show that thyroid status may influence the development of tumor, proliferation and metastasis.

Psoriasis is a chronic inflammatory skin disease with a strong genetic background and is triggered by environmental *** evidence supports CD6,a lymphocyte surface receptor mostly expressed by T cells,as a putative target in ***,a humanized anti-CD6 antibody has been assayed for the treatment of certain autoimmune disorders,including ***,we present novel evidence in mice and humans for a direct involvement of CD6 in psoriasis ***,an attenuated form of imiquimod-induced psoriasis-like skin inflammation was demonstrated in CD6-deficient mice,as deduced from lower epidermal thickness and local reduced production of pro-inflammatory cytokines,namely,***,isolated CD4+CD62L+T cells from CD6-deficient mice displayed decreased in vitro T-helper type 17 ***,a statistically significant association between CD6 single-nucleotide polymorphisms(rs17824933,rs11230563 and rs12360861)and more severe forms of psoriasis was demonstrated in a cohort of 304 patients at three public hospitals from the metropolitan area of *** together,these results provide new supportive evidence of the contribution of the CD6 lymphocyte receptor in psoriasis at both experimental and clinical levels.

Comparison between standard and wide-field autofluorescence in detection of retinal displacement after rhegmatogenous retinal detachment surgery

Purpose To analyse the differences between 30°blue autofluorescence(BAF30),55°blue autofluorescence(BAF55)and 200°green Ultra-Wide Field autofluorescence(UWF200)imaging to detect retinal displacement(RD)after vitrectomy surgery for rhegmatogenous retinal detachment(RRD).Methods This cross-sectional study considers forty-nine consecutive patients who underwent RRD surgery in the time period 4–8 months previous to image *** exclusion criteria contemplate previous retinal pathology,under 18-year-olds and non-assessable images in any of the 3 *** images were analysed by two masked graders that assessed either the presence or absence of retinal displacement.A third observer reviewed the images that presented *** A total of forty-nine patients were analysed.7 eyes were excluded due to poor quality in either of the imaging *** final analysis included 42 eyes of forty-two patients with a mean age of 60.3±11.9 *** patients underwent a 3 port 23-gauge pars plana vitrectomy as the technique of *** grade of RD was detectable in 45.2% of *** was similar between BAF30 and UWF200(42.9% of eyes).BAF55 showed RD to a lesser extent(38.1%).Agreement index between BAF30 and BAF55 was 0.901,0.903 between BAF30 and UWF200 and 0.803 between BAF55 and *** agreement index between graders was 0.775 for BAF30,0.798 for BAF50 and 0.808 for UWF200 *** All imaging modalities were able to detect RD after vitrectomy for RRD,with no inferiority of BAF30 and BAF55 over UWF200.

AIM: To present 18 new cases of human immunodeficiency virus(HIV)-related pulmonary arterial hypertension(PAH) with presenting features,treatment options and follow-up ***: This is a single-centre,retrospective,observational study that used prospectively collected data,conducted during a 14-year period on HIV-related PAH patients who were referred to a pulmonary hy-pertension unit. All patients infected with HIV were consecutively admitted for an initial evaluation of PAH during the study period and included in our study. Right heart catheterisation was used for the diagnosis of PAH. Specific PAH treatment was started according to the physician's judgment and the recommendations for idiopathic PAH. The data collected included demographic characteristics,parameters related to both HIV infection and PAH and disease ***: Eighteen patients were included. Intravenous drug use was the major risk factor for HIV infection. Risk factors for PAH,other than HIV infection,were present in 55.5% patients. The elapsed time between HIV infection and PAH diagnoses was 12.2 ± 6.9 years. At PAH diagnosis,94.1% patients had a CD4 cell count > 200 cells/μL. Highly active antiretroviral therapy(present in 47.1% patients) was associated with an accelerated onset of PAH. Survival rates were 93.8%,92.9% and 85.7% at one,two and three years,respectively. Concerning specific therapy,33.3% of the patients were started on a prostacyclin analogue,and the rest were on oral drugs,mainly phosphodiesterase-5 inhibitors. During the follow-up period,specific therapy was de-escalated to oral drugs in all of the living ***: The survival rates of HIV-related PAH patients were higher,most likely due to new aggressive specific therapy. The majority of patients were on oral specific therapy and clinically stable. Moreover,sildenafil appears to be a safe therapy for less severe HIVrelated PAH.

Adams-Oliver Syndrome (AOS) is a rare genetic disease characterized by combination of aplasia cutis congenita (ACC) and terminal transverse limb defects (TTLD), often accompanied by defects in scalp and skull ossification. Different clinical phenotypes may be related to variable severity both of aplasia cutis and TTLD, and of minor clinical features as cutis marmorata telangiectatica congenita (CMTC), congenital cardiac defect and vascular anomalies. The treatment is multidisciplinary: dermatologic, orthopedic and surgical consult should be required. It still remains unclear how to treat patients with a large skin defect that can‘t be closed primarly and may require both surgical and conservative management. We report two cases of AOS with typical limb defects and an area of aplasia cutis over vertex of the scalp managed conservatively with two different dermatologic devices.

Predictive indicators of successful tyrosine kinase inhibitor discontinuation in patients with chronic myeloid leukemia

Clinical trials have demonstrated that some patients with chronic myeloid leukemia(CML)treated for several years with tyrosine kinase inhibitors(TKIs)who have maintained a molecular response can successfully discontinue treatment without *** free remission(TFR)can be reached by approximately 50%of patients who *** having similar levels of deep molecular response and an identical duration of treatment,the factors that influence the successful discontinuation of CML patients remain to be *** this review we will explore the factors identified to date that can help predict whether a patient will successfully achieve *** will also discuss the need for the identification of predictive biomarkers associated with a high probability of achieving TFR for the future personalized identification of patients who are suitable for the discontinuation of TKI treatment.

Background:The aim of this trial was to investigate the effects of different inclusion levels of a partially defatted black soldier fly (BSF,Hermetia illucens L.) larva meal on the growth performance,nutrient digestibility,blood profile,gut morphology and histological features of piglets.A total of 48 newly weaned piglets were individually weighed (initial body weight (IBW):6.1 ± 0.16 kg) and randomly allocated to 3 dietary treatments (4 boxes as replicates/treatment and 4 animals/box).BSF larva meal was included at increasing levels (0% [BSF0],5% [BSFS] and 10% [BSF10]) in isonitrogenous and isoenergetic diets formulated for two feeding phases:Ⅰ (from d 1 to d 23) and Ⅱ (from d 24 to d 61).The weight gain (WG),average daily gain (ADG),average daily feed intake (ADFI) and feed conversion ratio (FCR) were calculated for each feeding phase and for the whole *** haematochemical parameters and nutrient digestibility of the piglets were also evaluated.A total of 3 piglets per box were slaughtered on d 61 and the slaughtered piglets were submitted to morphometric investigations and histopathological ***:No overall significant differences were observed for growth performance (P > 0.05),except for the ADFI of phase Ⅱ,which showed a linear response to increasing BSF meal levels (P 0.05).Conclusions:The obtained results show that a partially defatted BSF larva meal can be used as a feed ingredient in diets for weaned piglets without negatively affecting their growth performance,nutrient digestibilit

depending on the treatment and weight of the breasts or abdomen, they may exceed volumes considered giant and morbidly obese. In these cases, and when the patient’s BMI is high above 40 kg/m2, the weight of the breasts or abdomen produces what we consider suffocation when the patient is placed in horizontal position on surgical tables, decreasing his respiratory capacity and increasing the difficulty in treating respiratory or embolic risks. An 8-kg breast on the patient’s chest prevents normal breathing. An abdomen with a volume of 30 kg causes difficulties in all senses, making the physiological expansion of the lungs impossible and even preventing surgical assistance to patients. These patients are almost always customers who sleep in the sitting position to breathe better. The gigantic extirpation of the surgical parts facilitates a better respiratory expansion reducing by a large percentage the risk of death, what we call body relief. This relief does not free the patient from bariatric surgery for a possible weight loss, which is vital for the proper functioning of the organs and decreasing arterial hypertension and diabetes.

Can the detection of IgA anti-Mycoplasma pneumoniae added to IgM increase diagnostic accuracy in patients with infections of the lower respiratory airways?

AIM To evaluate the increase in diagnostic yield, by using IgA in addition to IgM, instead of IgM alone, in relation to the age of the *** The study considered 1067 blood samples from patients with clinical signs of lower respiratory tract infections, tested for anti-Mycoplasma IgG, IgM and IgA *** The increase in diagnostic yield with IgA, compared to IgM detection alone was of 3.5% with statistically significant differences between age groups(0.8% for those equal/under 50 years of age and 4.3% for those over 50).CONCLUSION Our findings demonstrate that IgA detection lead to a twofold increase in the number of diagnoses among the older age groups, but it did not result in relevant increase among the younger age groups.
