BACKGROUND Patients with intraductal papillary mucinous neoplasm(IPMN)have an increased risk of pancreatic and extrapancreatic malignancies.Lymphomas are rare extrapancreatic malignancies,and in situ collisions of early gastric cancer and diffuse large B-cell lymphoma(DLBCL)are even rarer.Here,we report the first case of pancreatic cancer comorbid with in situ collision of extrapancreatic malignancies(early gastric cancer and DLBCL)in a follow-up IPMN patient.Furthermore,we have made innovations in the treatment of such cases.CASE SUMMARY An 81-year-old Japanese female diagnosed with IPMN developed elevated carbohydrate antigen(CA)19-9 levels during follow-up.Because her CA19-9 levels continued to rise,endoscopic ultrasound(EUS)was performed and revealed a suspicious lesion at the pancreatic tail.However,lesions in the pancreas were not found by computed tomography,magnetic resonance imaging,or endoscopic retrograde cholangiopancreatography.To make an exact pathological diagnosis,EUS-guided fine needle aspiration was performed.To our supprise,early gastric cancer was found in preoperative gastroscopy.The gastric cancer was completely resected through endoscopic submucosal dissection before postoperative pathology identified early adenocarcinoma collided with DLBCL.Subsequent EUS-guided fine needle aspiration provided pathological support forthe pancreatic cancer diagnosis, and then laparoscopic distal pancreatectomy andsplenectomy were performed. CA19-9 levels returned to normal postoperatively.CONCLUSION Endoscopic submucosal dissection is appropriate for submucosal lymphomas inpatients intoleratant of chemotherapy. EUS can detect small IPMN-relatedpancreatic tumors.

Stress and strain in the structure of self-assembled quantum dots constructed in the Ge/Si(001) system is calculated by using molecular dynamics simulation. Pyramidal hut cluster composed of Ge crystal with {105} facets surfaces observed in the early growth stage are computationally modeled. We calculate atomic stress and strain in relaxed pyramidal structure. Atomic stress for triplet of atoms is approximately defined as an average value of pairwise (virial) quantity inside triplet, which is the product of vectors between each two atoms. Atomic strain by means of atomic strain measure (ASM) which is formulated on the Green’s definition of continuum strain. We find the stress (strain) relaxation in pyramidal structure and stress (strain) concentration in the edge of pyramidal structure. We discuss size dependency of stress and strain distribution in pyramidal structure. The relationship between hydrostatic stress and atomic volumetric strain is basically linear for all models, but for the surface of pyramidal structure and Ge-Si interface. This means that there is a reasonable correlation between atomic stress proposed in the present study and atomic strain measure, ASM.

Successful treatment of conversion chemotherapy for initially unresectable synchronous colorectal liver metastasis

A 72-year-old woman with a sigmoid colon cancer and a synchronous colorectal liver metastasis(CRLM),which involved the right hepatic vein(RHV)and the inferior vena cava(IVC),was referred to our hospital.The metastatic lesion was diagnosed as initially unresectable because of its invasion into the confluence of the RHV and IVC.After she had undergone laparoscopic sigmoidectomy for the original tumor,she consequently had 3 courses of modified 5-fluorouracil,leucovorin,and oxaliplatin(m FOLFOX6)plus cetuximab.Computed tomography revealed a partial response,and the confluence of the RHV and IVC got free from cancer invasion.After 3 additional courses of m FOLFOX6 plus cetuximab,preoperative percutaneous transhepatic portal vein embolization(PTPE)was performed to secure the future remnant liver volume.Finally,a right hemihepatectomy was performed.The postoperative course was uneventful.The patient was discharged from the hospital on postoperative day 13.She had neither local recurrence nor distant metastasis 18 mo after the last surgical intervention.This multidisciplinary strategy,consisting of conversion chemotherapy using FOLFOX plus cetuximab and PTPE,could contribute in facilitating curative hepatic resection for initially unresectable CRLM.

Targeting tight junctions during epithelial to mesenchymal transition in human pancreatic cancer

Mechanical property and structural evolution process of isotactic polypropylene (iPP) film with different molecular weight distribution (MWD) and isotacticity has been investigated by carrying out the in-situ time-resolved measurements of synchrotron wide-angle X-ray diffraction (WAXD) and small-angle X-ray scattering (SAXS).In the uniaxial hot drawing process the stress was found to increase with an increment of drawing ratio,which was more remarkable for the sample with narrower MWD and higher isotacticity.The SAXS and WAXD patterns were found to change remarkably in this process,from which the deformation of lamellar stacking structure and crystallites were derived concretely.The sample with narrower MWD and higher isotacticity showed the homogeneous lamellar stacking structure and lamellar deformation behavior.On the other hand the sample with wider MWD and isotacticity showed heterogeneous lamellar stacking structure and deformation behavior.The mechanical property and the deformation behavior of crystalline lamellae in the hot drawing process were found to be affected significantly and sensitively by MWD and isotacticity distribution of iPP samples.

Pancreatic cancer continues to be a leading cause of cancer-related death worldwide and there is an urgent need to develop novel diagnostic and therapeutic strategies to reduce the mortality of patients with this disease. In pancreatic cancer, some tight junction proteins, including claudins, are abnormally regulated and therefore are promising molecular targets for diagnosis, prognosis and therapy. Claudin-4 and-18 are overexpressed in human pancreatic cancer and its precursor lesions. Claudin-4 is a high affinity receptor of Clostridium perfringens enterotoxin(CPE). The cytotoxic effects of CPE and monoclonal antibodies against claudin-4 are useful as novel therapeutic tools for pancreatic cancer. Claudin-18 could be a putative marker and therapeutic target with prognostic implications for patients with pancreatic cancer. Claudin-1,-7, tricellulin and marvelD3 are involved in epithelial to mesenchymal transition(EMT) of pancreatic cancer cells and thus might be useful as biomarkers during disease. Protein kinase C is closely related to EMT of pancreatic cancer and regulates tight junctions of normal human pancreatic duct epithelial cells and the cancer cells. This review focuses on the regulation of tight junctions via protein kinase C during EMT in human pancreatic cancer for the purpose of developing new diagnostic and therapeutic modalities for pancreatic cancer.

The Si tandem solar cells are very attractive for realizing high efficiency and low cost. This paper overviews current status of III-V/Si tandem solar cells including our results. The analytical results for efficiency potential of Si tandem solar cells and loss analysis of Si bottom cells as well as bandgap energy optimization of sub-cells are presented. The 2-junction and 3-junction Si tandem solar cells have potential efficiencies of 36% and 42%, respectively. ERE (external radiative efficiency) analysis for Si solar cells is analyzed in order to clarify properties of Si bottom solar cells. Properties of single-crystalline Si heterojunction solar cell fabricated in this study were analyzed. The current status of efficiencies of our Si bottom cell, upper III-V 2-junction solar cell and III-V/Si 3-junction tandem solar cell was shown to be 5.2% and 28.6% and 33.8%. Achievement of Jsc of 12 mA/cm2 for Si bottom cell is necessary to realize high-efficiency 3-junction Si tandem solar cells with an efficiency of more than 37%. In addition, this paper presents ERE analysis of III-V 2-junction upper solar cells for improving III-V/Si 3-junction tandem solar cells. Several ways to improve efficiency of III-V/Si 3-junction tandem solar cells by reducing non-radiative recombination, optical and resistance losses are shown.

AIM:To study how lymph node metastasis(LNM) risk is stratified in undifferentiated-type early gastric cancer(undiff-EGC) dependent on combinations of risk factors.METHODS:Five hundred and sixty-seven cases with undiff-EGC undergoing gastrectomy with lymphadenectomy were examined retrospectively.Using clinicopathological factors of patient age,location,size,an endoscopic macroscopic tumor form,ulceration,depth,histology,lymphatic involvement(LI) and venous involvement(VI),LNM risk was examined and stratified by conventional statistical analysis and datamining analysis.RESULTS:LNM was positive in 44 of 567 cases(7.8%).Univariate analysis revealed > 2 cm,protrusion,submucosal(sm),mixed type,LI and VI as significant prognostic factors and > 2 cm and LI-positive were independent factors by multivariate analysis.In preoperatively evaluable factors excluding LVI,sm and > 2 cm were independent factors.According to the depth and size,cases were categorized into the low-risk group [m and ≤ 2 cm,0%(LNM incidence)],the moderaterisk group(m and > 2 cm,5.6%; and sm and ≤ 2 cm,6.0%),and the high-risk group(sm and > 2 cm,19.3%).On the other hand,LNM occurred in 1.4% in all LI-negative cases,greatly lower than 28.2% in all LI-positive cases,and LNM incidence was low in LInegative cases even in the moderate- and high-risk groups.CONCLUSION:LNM-related factors in undiff-EGC were depth and size preoperatively while those were LI and size postoperatively.Among these factors,LI was the most significantly correlated factor.

AIM:To investigate ethanol-induced hepatic steatosis after liver resection and the mechanisms behind it.METHODS:First,the preliminary examination was performed on 6 sham-operated(Sham)and 30 partial hepatectomy(PH)male Wistar rats(8-wk-old)to evaluate the recovery of the liver weight and liver function after liver resection.PH rats were sacrificed at the indicated time points(4,8,and 12 h;1,3,and 7d)after PH.Second,the time point for the beginning of the chronic ethanol exposure(1 wk after sham-or PH-operation)was determined based on the results of the preliminary examination.Finally,pair-feeding was performed with a controlled diet or with a 5-g/d L ethanol liquid diet for 28 d in another 35 age-matched male Wistar rats with a one-week recovery after undergoing a sham-(n=15)or PH-operation(n=20)to evaluate the ethanol-induced liver injury after liver resection.Hepatic steatosis,liver function,fatty acid synthase(Fas)gene expression level,the expression of lipid metabolism-associated enzyme regulator genes[sterol regulatory element binding protein(Srebp)-1and pe roxis ome prol i fe rat or-ac t ivat e d re c e pt or(Ppar)-α],the mediators that alter lipid metabolism[plasminogen activator(Pai)-1 gene expression level and tumor necrosis factor(Tnf)-αproduction],and hepatic class-1 alcohol dehydrogenase(Adh1)-associated ethanol elimination were investigated in the4 groups based on histological,immunohistochemical,biochemical,Western blotting,reverse transcriptase chain reaction,and blood ethanol concentration analyses.The relevant gene expression levels,liver weight,and liver function were assessed before and 1wk after surgery to determine the subject’s recovery from the liver resection using the rats that had been subjected to the preliminary examination.RESULTS:In the PH rats,ethanol induced marked hepatic steatosis with impaired liver functioning,as evidenced by the accumulation of fatty droplets within the hepatocytes,the higher increases in their hepatic triglyceride and blood alanine aminotransferase and blood

Development of highly-efficient photovoltaic (PV) modules and expanding its application fields are significant for the further development of PV technologies and realization of innovative green energy infrastructure based on PV. Especially, development of solar-powered vehicles as a new application is highly desired and very important for this end. This paper presents the impact of PV cell/module conversion efficiency on reduction in CO2 emission and increase in driving range of the electric based vehicles.
