Factors associated with residual gastroesophageal reflux disease symptoms in patients receiving proton pump inhibitor maintenance therapy

AIM To elucidate the factors associated with residual gastroesophageal reflux disease(GERD) symptoms in patients receiving proton pump inhibitor(PPI) maintenance therapy in clinical practice.METHODS The study included 39 GERD patients receiving maintenance PPI therapy. Residual symptoms were assessed using the Frequency Scale for Symptoms of GERD(FSSG) questionnaire and the Gastrointestinal Symptom Rating Scale(GSRS). The relationships between the FSSG score and patient background factors, including the CYP2C19 genotype, were analyzed.RESULTS The FSSG scores ranged from 1 to 28 points(median score: 7.5 points), and 19 patients(48.7%) had a score of 8 points or more. The patients' GSRS scores were significantly correlated with their FSSG scores(correlation coefficient = 0.47, P < 0.005). In erosive esophagitis patients, the FSSG scores of the CYP2C19 rapid metabolizers(RMs) were significantly higher than the scores of the poor metabolizers and intermediate metabolizers(total scores: 16.7 ± 8.6 vs 7.8 ± 5.4, P < 0.05; acid reflux-related symptom scores: 12 ± 1.9 vs 2.5 ± 0.8, P < 0.005). In contrast, the FSSG scores of the CYP2C19 RMs in the non-erosive reflux disease patients were significantly lower than those of the other patients(total scores: 5.5 ± 1.0 vs 11.8 ± 6.3, P < 0.05; dysmotility symptom-related scores: 1.0 ± 0.4 vs 6.0 ± 0.8, P < 0.01). CONCLUSION Approximately half of the GERD patients receiving maintenance PPI therapy had residual symptoms associated with a lower quality of life, and the CYP2C19 genotype appeared to be associated with these residual symptoms.

Factors Affecting Behaviors that Precede Evacuation at the Onset of a Heavy Rainstorm in Japan

There exist certain behaviors that people tend to do in disaster situations before evacuation. Such behaviors include warning confirmation behavior(for example, seeking information) and family-oriented behavior(for example,contacting one’s family). Identifying factors that affect these behaviors is of particular importance in building a better understanding of why people often fail to respond quickly to evacuation orders. For this purpose, the present study employed some of the established factors affecting evacuation behavior as predictor variables along with the timing of an evacuation order. A total of 518 participants took part in a 12-item online questionnaire survey that contained a hypothetical disaster scenario. The results of ordinal logistic regression analyses revealed that only risk area residence and disaster preparedness were associated with warning confirmation behavior, while gender, age, disaster preparedness,and risk perception had some associations with family-oriented behavior. Also, the participants were not more likely to engage in these behaviors in the morning and the afternoon than the evening in the hypothetical scenario. These findings imply the possibility that people engage in warning confirmation behavior and family-oriented behavior before evacuation regardless of individual characteristics and the circumstances surrounding them.

Thermal-hydro-mechanical coupled analysis of unsaturated frostsusceptible soils

Damage caused by frost heave leads to costly maintenance in cold regions, like Hokkaido, Japan. Therefore, thestudy of the frost mechanism with experimental and numerical methods has been of great interest. Numerousmodels have been developed to describe the freezing process of saturated soil, which differs from the partiallysaturated conditions in the field. In fact, most subsurface soils are unsaturated. The freezing process of partiallysaturated soils is more complex than saturated soils, as the governing equations show strongly nonlinear characteristics. This study proposes a thermo-hydro-mechanical coupled model considering the heat transfer, waterinfiltration, and deformation of partially saturated soil to reproduce the freezing process of partially saturatedfrost susceptible soils distributed in Hokkaido. This model better considers the water-ice phase change and soilfreezing characteristic curve (SFCC) during freezing under field conditions. The results from the multiphysicssimulations agree well with the frost heave and water migration data from frost heave tests of Touryo soil andFujinomori soil. In addition, this study discussed the influence of the various factors on frost heave amount,including temperature gradients, overburden pressures, water supply conditions, cooling rates, and initial saturation. The simulation results indicate that the frost heave ratio is proportional to the initial degree of saturationand is inversely proportional to the cooling rate and overburden pressure.Moreover, simulation under the open system generates much more frost heave than under the closed system.Finally, the main features of the proposed model are revealed by simulating a closed-system frost heave test. Thesimulation results indicate that the proposed model adequately captures the coupling characteristics of water andice redistribution, temperature development, hydraulic conductivity, and suction in the freezing process. X-ray CT scanners,due to the transmissive nature of X-rays,have enabled the non-destructive evaluation of industrial products,even inside their bodies.In light of its effectiveness,this study introduces a new approach to accelerate the inspection of many mechanical parts with the same shape in a bin.The input to this problem is a volumetric image(i.e.,CT volume)of many parts obtained by a single CT scan.We need to segment the parts in the volume to inspect each of them;however,random postures and dense contacts of the parts prohibit part segmentation using traditional template matching.To address this problem,we convert both the scanned volumetric images of the template and the binned parts to simpler graph structures and solve a subgraph matching problem to segment the parts.We perform a distance transform to convert the CT volume into a distance field.Then,we construct a graph based on Morse theory,in which graph nodes are located at the extremum points of the distance field.The experimental evaluation demonstrates that our fully automatic approach can detect target parts appropriately,even for a heap of 50 parts.Moreover,the overall computation can be performed in approximately 30 min for a large CT volume of approximately 2000×2000×1000 voxels.

Structural Evolution Patterns of FCC-Type Gold Nanoclusters

Recent progress in the research of atomically-precise metal nanoclusters has identified a series of exceptionally stable nanoclusters with specific chemical compositions. Structural determination on such "magic size" nanoclusters revealed a variety of unique structures such as decahedron, icosahedron, as well as hexagonal close packing(hcp) and body-centered cubic(bcc) packing arrangements in gold nanoclusters, which are largely different from the face-centered cubic(fcc) structure in conventional gold nanoparticles. The characteristic geometrical structures enable the nanoclusters to exhibit interesting properties, and these properties are in close correlation with their atomic structures according to the recent studies. Experimental and theoretical analyses have been applied in the structural identification aiming to clarify the universal principle in the structural evolution of nanoclusters. In this mini-review, we summarize recent studies on periodic structural evolution of fcc-based gold nanoclusters protected by thiolates. A series of nanoclusters exhibit one-dimensional growth along the [001] direction in a layer-by-layer manner from Au_(23)(TBBT)_(20) to Au_(36)(TBBT)_(24),Au_(44)(TBBT)_(28), and to Au_(52)(TBBT)_(32)(TBBT: 4-tert-butylbenzenethiolate). The optical properties of these nanoclusters also evolve periodically based on steady-state and ultrafast spectroscopy. In addition, two-dimensional growth from Au_(44)(TBBT)_(28) toward both [100] and [010] directions leads to the Au_(92)(TBBT)_(44) nanocluster, and the recently reported Au_(52)(PET)_(32)(PET: 2-phenylethanethiol) also follows this growth pattern with partial removal of the layer. Theoretical predictions of relevant fcc nanoclusters include Au_(60)(SCH_3)_(36), Au_(68)(SCH_3)_(40), Au_(76)(SCH_3)_(44), etc, for the continuation of 1 D growth pattern, as well as Au_(68)(SR)_(38)mediating the 2 D growth pattern from Au_(44)(TBBT)_(28) to Au_(92)(TBBT)_(44). Characterizing human driver characteristics using an artificial neural network and a theoretical model

Human drivers seem to have different characteristics,so different drivers often yield different results from the same driving mode tests with identical vehicles and same chassis dynamometer.However,drivers with different experiences often yield similar results under the same driving conditions.If the features of human drivers are known,the control inputs to each driver,including warnings,will be customized to optimize each man–machine vehicle system.Therefore,it is crucial to determine how to characterize human drivers quantitatively.This study proposes a method to estimate the parameters of a theoretical model of human drivers.The method uses an artificial neural network(ANN)model and a numerical procedure to interpret the identified ANN models theoretically.Our approach involves the following process.First,we specify each ANN driver model through chassis dynamometer tests performed by each human driver and vehicle.Subsequently,we obtain the parameters of a theoretical driver model using the ANN model for the corresponding driver.Specifically,we simulate the driver’s behaviors using the identified ANN models with controlled inputs.Finally,we estimate the theoretical driver model parameters using the numerical simulation results.A proportional-integral-differential(PID)control model is used as the theoretical model.The results of the parameter estimation indicate that the PID driver model parameter combination can characterize human drivers.Moreover,the results suggest that vehicular factors influence the parameter combinations of human drivers.

Effect of die coating on surface crack depth of hot extruded 7075 aluminum alloy

The aluminum alloys belonging to the 7000 series are high-strength alloys used in a wide variety of products for weight reduction.They are primarily used in the field of transportation and aerospace.Among these,the A7075 alloy has the highest strength and is expected to be applicable in a wide range of fields,such as aircraft components and sports equipment.However,it has high deformation resistance and is prone to surface defects,which is called tearing.Tearing typically occurs at high temperatures and high ram speeds,and adversely affects productivity.The localized melting of Zn and additive compounds,due to the heat generated during the process,is considered to cause tearing.In this study,the effect of friction,heat,and tearing at the tool-metal interface was mitigated by improving the die surface quality.The reduced friction eliminated recrystallization by preventing the temperature from increasing to recrystallization temperature.In addition,an AlCrN coating was adopted instead of nitriding to improve the die surface quality.The tearing size and heat generated when using the AlCrN coating were found to be limited.Moreover,the grain size observed in the tearing region on the extruded surface was small.The simulations using the shear friction coefficient m observed from friction tests indicate that the use of the AlCrN coating improved the material flow.Thus,the AlCrN coating is considered effective for reducing friction at the interface and preventing the recrystallization of the extruded surface.From the aforementioned results,it can be inferred that a die coating can reduce the tearing sensitivity and increase the productivity of the A7075 alloy.

In this paper, we are concerned with the uniqueness and the non-degeneracy of positive radial solutions for a class of semilinear elliptic equations. Using detailed ODE anal- ysis, we extend previous results to cases where nonlinear terms may have sublinear growth. As an application, we obtain the uniqueness and the non-degeneracy of ground states for modified SchrSdinger equations.
