AIM: Infection with Helicobacter pylori(H pylori) possessing the cag pathogenicity island (PAI) has been associated with severe clinical outcome and CagA-antibody has been used to indicate cagPAI-positive infection. The aim of this study was to examine the accuracy of CagA seropositivity to indicate the virulence of the cagPAI in Japan. METHODS: Sixty isolates of H pylori cultured from gastric biopsies were examined by polymerase chain reaction assays for the presence of cagA, cagE and VirD4. Anti CagA IgG antibody in matching sera was tested by both ELISA and immunoblot assay. Histological grade of gastritis was graded according to the updated Sydney System. RESULTS: Amongst 53 patients infected with cagA+/cagE+/ VirD4+ strain, 38 were CagA seropositive. There were four patients infected with strains possessing incomplete cagPAI. Two out of three patients with cagA+/cagE-/VirD4- infection were CagA seropositive, while a patient with cagA-/cagE+/ VirD4+ infection was CagA seronegative. Accuracy of ELJSA to predict bacterial possession of cagA was 61.7% whereas 58.3% for cagE and VirD4. The immunoblot assay showed relatively higher sensitivity and showed better accuracy. The lower grade of gastric mucosal inflammatory infiltration was seen in false CagA-seronegative patients. CONCLUSION: Some serodiagnosis does not seem to have enough accuracy to indicate virulence of cagPAI, particularly in infection of strains with incomplete cagPAI. The degree of gastric mucosal inflammation may affect the results of CagA serodiagnosis.

We experienced a very rare complication of colonoscopy,a migration of stiffening tube into the colorectum. We herein introduce a withdrawing method of migrating stiffening tube incidentally inserted into the colorectum.A 65-year-old Japanese woman underwent colonoscopy because of abdominal discomfort. We used stiffening tube to insert the scope to the proximal colon because of her redundant sigmoid colon. When withdrawing the scope,we realized that the tube was fully inside the *** could not remove the tube instantly, and it reached the splenic flexure, finally. We reinserted the scope through the migrating tube, straightened the scope, and withdrew it holding a slight angle of the scope over the proximal end of the tube. Then, we could safely remove the tube along with the scope through the anus.

Acute esophageal necrosis (AEN) is extremely rare and the pathogenesis of this is still unknown. We report a case of AEN caused by alcohol abuse. In our case, the main pathogenesis could be accounted for low systemic perfusion caused by severe alcoholic lactic acidosis. After the healing of AEN, balloon dilatation was effective to manage the stricture.

AIM: To examine the long term survival of geriatric patients treated with percutaneous endoscopic gastrostomy (PEG) in Japan. METHODS: We retrospectively included 46 Japanese community and tertiary hospitals to investigate 931 consecutive geriatric patients (≥ 65 years old) with swallowing difficulty and newly performed PEG between Jan 1st 2005 and Dec 31st 2008. We set death as an outcome and explored the associations among patient’s characteristics at PEG using log-rank tests and Cox proportional hazard models. RESULTS: Nine hundred and thirty one patients were followed up for a median of 468 d. A total of 502 deaths were observed (mortality 53%). However, 99%, 95%, 88%, 75% and 66% of 931 patients survived more than 7, 30, 60 d, a half year and one year, respectively. In addition, 50% and 25% of the patients survived 753 and 1647 d, respectively. Eight deaths were considered as PEG-related, and were associated with lower serum albumin levels (P = 0.002). On the other hand, among 28 surviving patients (6.5%), PEG was removed. In a multivariate hazard model, older age [hazard ratio (HR), 1.02; 95% confidence interval (CI), 1.00-1.03; P = 0.009], higher C-reactive protein (HR, 1.04; 95% CI: 1.01-1.07; P = 0.005), and higher blood urea nitrogen (HR, 1.01; 95% CI: 1.00-1.02; P = 0.003) were significant poor prognostic factors, whereas higher albumin (HR, 0.67; 95% CI: 0.52-0.85; P = 0.001), female gender (HR, 0.60; 95% CI: 0.48-0.75; P < 0.001) and no previous history of ischemic heart disease (HR, 0.69; 95% CI: 0.54-0.88, P = 0.003) were markedly better prognostic factors. CONCLUSION: These results suggest that more than half of geriatric patients with PEG may survive longer than 2 years. The analysis elucidated prognostic factors.

Endoscopic management of unresectable malignant gastroduodenal obstruction with a nitinol uncovered metal stent: a prospective Japanese multicenter study

AIM: To determine the safety and efficacy of endoscopic duodenal stent placement in patients with malignant gastric outlet ***: This prospective, observational, multicenter study included 39 consecutive patients with malignant gastric outlet obstruction. All patients underwent endoscopic placement of a nitinol, uncovered, selfexpandable metal stent. The primary outcome was clinical success at 2 wk after stent placement that was defined as improvement in the Gastric Outlet Obstruction Scoring System score relative to the ***: Technical success was achieved in all duodenal stent procedures. Procedure-related complications occurred in 4 patients(10.3%) in the form of mild pneumonitis. No other morbidities or mortalitieswere observed. The clinical success rate was 92.3%. The mean survival period after stent placement was 103 d. The mean period of stent patency was 149 d and the patency remained acceptable for the survival period. Stent dysfunction occurred in 3 patients(7.7%) on account of tumor ***: Endoscopic management using duodenal stents for patients with incurable malignant gastric outlet obstruction is safe and improved patients' quality of life.

Destruction resistance such as start-up wear resistance characteristics of vitamin E(dl-α-tocopherol)blended ultrahigh molecular weight polyethylene(UHMWPE)were evaluated using the destruction energy index(DEI).The DEI is used to evaluate wear-like destruction,by minimising the effects of viscosity at the sliding interface,and is estimated using newly designed friction *** this experiment,silicone oils of different viscosities were used to determine micro displacement up to a start-up point,and the DEI of the UHMWPE was changed from 17.48 to 1.84μJ by adding vitamin *** results suggest that the blending with vitamin E reduces destruction of UHMWPE at start-up friction in silicone oils.

There exist certain behaviors that people tend to do in disaster situations before evacuation. Such behaviors include warning confirmation behavior(for example, seeking information) and family-oriented behavior(for example,contacting one’s family). Identifying factors that affect these behaviors is of particular importance in building a better understanding of why people often fail to respond quickly to evacuation orders. For this purpose, the present study employed some of the established factors affecting evacuation behavior as predictor variables along with the timing of an evacuation order. A total of 518 participants took part in a 12-item online questionnaire survey that contained a hypothetical disaster scenario. The results of ordinal logistic regression analyses revealed that only risk area residence and disaster preparedness were associated with warning confirmation behavior, while gender, age, disaster preparedness,and risk perception had some associations with family-oriented behavior. Also, the participants were not more likely to engage in these behaviors in the morning and the afternoon than the evening in the hypothetical scenario. These findings imply the possibility that people engage in warning confirmation behavior and family-oriented behavior before evacuation regardless of individual characteristics and the circumstances surrounding them.

The corrosion-protective performance of plasma-polymerized (PP) coatings on pretreated aluminum substrates has been investigated by cathodic polarization curve measurement. The surface composition and electronic properties of the pretreated and PP film coated metal surfaces were also characterized by XPS and the temperature-programmed photoelectron emission (TPPE). A PP coating was prepared on the pretreated surfaces by plasma polymerization of a mixture of tetraethoxysilane (TEOS) monomer vapor and oxygen using a 13.56MHz radiofrequency generator. The polarization curve of PP film coated samples was measured in NaCl aqueous solution. The weight loss rate calculated from the value of the corrosion current of the curve was used to estimate the protective performance of the PP film coated samples. Argon plasma treatment of the metal surface gave much better corrosion-protective performance than pretreatments such as oxidation by heating in air and diamond scratching. The XPS analysis indicated that the silicon oxide assigned to SiO2 was formed on the PP film coated surface. The TPPE analysis revealed that the electron emission characteristics for the metal surfaces pretreated only were strongly influenced by the pretreatments, while all the PP film coated samples exhibited nearly the same electron emission trend with a much decreased intensity.

The numerical and physical issues of simulations on compressible turbulence are reviewed in the present paper. An outline of the global spectral methods and the progress of recent local spectral methods are illustrated. Several typical subjects in this field are studied, including homogeneous isotropic turbulence, autoignition in premixed turbulence, interaction between flames and turbulence, and shock wave in turbulence. The results of the numerical simulations are discussed, enabling us to discover and to understand the physical phenomena which have not been solved by experiments.

X-ray CT scanners,due to the transmissive nature of X-rays,have enabled the non-destructive evaluation of industrial products,even inside their *** light of its effectiveness,this study introduces a new approach to accelerate the inspection of many mechanical parts with the same shape in a *** input to this problem is a volumetric image(i.e.,CT volume)of many parts obtained by a single CT *** need to segment the parts in the volume to inspect each of them;however,random postures and dense contacts of the parts prohibit part segmentation using traditional template *** address this problem,we convert both the scanned volumetric images of the template and the binned parts to simpler graph structures and solve a subgraph matching problem to segment the *** perform a distance transform to convert the CT volume into a distance ***,we construct a graph based on Morse theory,in which graph nodes are located at the extremum points of the distance *** experimental evaluation demonstrates that our fully automatic approach can detect target parts appropriately,even for a heap of 50 ***,the overall computation can be performed in approximately 30 min for a large CT volume of approximately 2000×2000×1000 voxels.
