Clinical usefulness of biochemical markers of liver fibrosis in patients with nonalcoholic fatty liver disease

AIM: Nonalcoholic steatohepatitis (NASH) is a severe form of nonalcoholic fatty liver disease (NAFLD), and progresses to the end stage of liver disease. Biochemical markers of liver fibrosis are strongly associated with the degree of histological liver fibrosis in patients with chronic liver disease. However, data are few on the usefulness of markers in NAFLD patients. The aim of this study was to identify better noninvasive predictors of hepatic fibrosis, with special focus on markers of liver fibrosis, type VI collagen 7S domain and hyaluronic acid. METHODS: One hundred and twelve patients with histologically proven NAFLD were studied. RESULTS: The histological stage of NAFLD correlated with several clinical and biochemical variables, the extent of hepatic fibrosis and the markers of liver fibrosis were relatively strong associated. The best cutoff values to detect NASH were assessed by using receiver operating characteristic analysis: type VI collagen 75 domain ≥5.0 ng/mL, hyaluronic acid ≥43 ng/mL. Both markers had a high positive predictive value: type VI collagen 7S domain, 86% and hyaluronic acid, 92%. Diagnostic accuracies of these markers were evaluated to detect severe fibrosis. Both markers showed high negative predictive values: type VI collagen 7S domain (≥5.0 ng/mL), 84% and hyaluronic acid (≥50 ng/mL), 78%, and were significantly and independently associated with the presence of NASH or severe fibrosis by logistic regression analysis. CONCLUSION: Both markers of liver fibrosis are useful in discriminating NASH from fatty liver alone or patients with severe fibrosis from patients with non-severe fibrosis.

MIM： To investigate the different impact of genotypes E and C on the development of liver cirrhosis （LC） among different age groups of patients with chronic hepatitis （CH-B）.METHODS： We examined the outcome of 121 patients with CH-B, divided by age and genotype. Univariate analyses were used to compare different groups. The Cox proportional hazard model was employed to evaluate factors affecting the development of LC.RESULTS： In patients 〈 30 years old, there were no significant predictors for development of LC. However, in patients ≥ 30 years old, genotype C was the only significant predictor. In the genotype C group, 8 of 12 patients who progressed to LC were 30-49 years old at initial diagnosis of chronic hepatitis （7 patients were positive for HBeAg）. In the genotype B group, 4 of 8 patients who developed LC were ≥50 years old at initial diagnosis and were HBeAg-negative.CONCLUSION： The rate of development of LC was comparable in patients infected with genotypes B and C when CH-B occurred at 〈 30 years old. However, CH-B patients infected with genotype C showed poor prognosis if they were 30-49 years old and were positive for HBeAg. Age-specific natural course of CH-B should be considered when patients with CH-B are treated with antiviral drugs.

AIM:To investigate the clinical and biochemical factors associated with visceral fat accumulation in the general ***:We enrolled 1004 subjects who underwent a medical health checkup between April 2008 and March *** medical health checkup included the following tests:Height,body weight,waist circumference(WC),systolic blood pressure,diastolic blood pressure,urinalysis,blood-cell counts,blood chemistry,electrocardiography,chest radiography,and abdominal computed tomography(CT)for visceral fat *** patients’medical history and lifestyle factors were collected privately by nurses using a selfadministered questionnaire,and they included questions regarding physical activity,sleep duration,dietary habits,smoking,and alcohol *** fat area(VFA)was defined as the sum of the intraperitoneal fat area at the level of the umbilicus with CT density in the range of-150 to-50 Hounsfield ***:The mean age and body mass index(BMI)of the study subjects were 57.0 years and 24.4 kg/*** both male and females,v FA was significantly andpositively correlated with WC(r=0.532,P<0.01;r=0.612,P<0.01).Subjects with high levels of v FA were primarily male with significantly higher age,height,body weight,BMI,systolic blood pressure(BP),diastolic BP,and hemoglobin in all subjects(P<0.05).A multivariate logistic regression analysis revealed that vF A had a positive relationship with age≥56,BMI≥25 kg/m2,and triglyceride level≥149 in males(P<0.05),whereas it had a positive relationship with age≥58,BMI≥24.4 kg/m2,high-density lipoprotein cholesterol level<40 mg/d L,and current drinking in females(P<0.05).CONCLUSION:These results suggest that gender differences exist in the clinical and biochemical parameters associated with visceral fat accumulation.

Efficacy and safety of dual therapy with daclatasvir and asunaprevir in elderly patients

To survey the efficacy and safety of dual therapy with daclatasvir and asunaprevir in the elderly hepatitis C virus (HCV) patients multicentricity. METHODSInterferon-ineligible/intolerant patients and non-responders to previous pegylated-interferon/ribavirin therapy with chronic HCV genotype 1b infection were enrolled. Child B, C cirrhotic patients were excluded. Patients received oral direct acting antiviral treatment consisting of 60 mg daclatasvir once daily plus 200 mg asunaprevir twice daily for 24 wk. We divided the patients into two groups of 56 elderly patients (≥ 75 years-old) and 141 non-elderly patients (24). In the former, 1.8% experienced viral breakthrough, as compared with 3.5% in the latter (not significant). Adverse events occurred in 55.4% of the former and 56.0% of the latter. In the former, 7 cases (12.5%) were discontinued due to adverse events, and in the latter 9 cases were discontinued (6.4%, not significant). CONCLUSIONDual therapy with daclatasvir and asunaprevir achieved the same high rates of SVR24 in HCV elderly patients without more adverse events than in the non-elderly patients.

Antifungal activity of chemotype essential oils from rosemary against <i>Candida albicans</i>

Oral candidiasis is caused by Candida albicans. The symptoms including pain in the oral cavity affect our eating function. While some antifungal agents are commonly used for the treatment of candidiasis, occasional consumption of the effective dose cannot be tolerated due to side effects. There is also a possibility of induction of antifungal resistance, thus the development of replacement agents has been awaited. We aim to explore antimicrobial activities of essential oils. We evaluated the antifungal activities against C. albicans of essential oils from seven aromatic plants from three manufacturers, and of three chemotype essential oils from rosemary (Rosmarinus officinalis). As a result, we found that the antifungal activity was increased several times by the addition of Tween 80. All the tested essential oils showed stable antifungal activity, however, the variation was observed among the manufacturers of rosemary and eucalyptus. Rosemary has three chemotypes;CINEOL, CAMPHOR and VERBENON. They derived from a same plant species, but contain different chemical components. The CINEOL, dose-dependently decreased the number of C. albicans in the time-kill assay. Hence we concluded that the components of rosemary essential oil would have an effect on its antifungal activity. A chemotype is the first to consider in measuring antifungal activities of rosemary oil.

Lamiaceae Peppermint Oil with Surfactant Showing Equal Antifungal Activity against <i>Candida albicans</i>to Rosemary Chemotype CINEOL

Oral candidiasis causes pain in oral cavity or tongue. Antifungal drugs are commonly used for the treatment of candidiasis, however, there could be several problems such as side effects, drug resistance, or contraindication to concomitant use with drugs already taken. The development of substituting antimicrobial agents, which could be used internally or as gargling agents, is expected. Some essential oils have been known for the activities against microbes. We focused on the three chemotype essential oils from rosemary and five essential oils from Lamiaceae plants including rosemary, and examined their antifungal activity against Candida albicans by broth microdilution method. The activity with or without addition of Tween 80 or Tween 20 was statistically analyzed. Minimal inhibitory concentration (MIC) values of the essential oils were significantly decreased when either of the two surfactants was added. Among the tested oils, only CAMPHOR, a rosemary chemotype, showed a significantly lower MIC with addition of Tween 20 than Tween 80. It is inferred that the antifungal activity of essential oils is influenced by the type of surfactants. Some of the essential oils showed the same values of MIC and the minimal fungicidal concentration (MFC) when surfactant was added. Peppermint showed the same MICs and MFCs in most cases, and its low MFC indicated potent fungicidal activity against C. albicans, as observed in CINEOL. We will discuss peppermint oil as one of the potential candidates for antifungal agents against C. albicans.
