Quality indicators in pediatric colonoscopy in a low-volume center:Implications for training

AIM To study implications of measuring quality indicators on training and trainees' performance in pediatric colonoscopy in a low-volume training *** We reviewed retrospectively the performance of pediatric colonoscopies in a training center in Malaysia over 5 years(January 2010-December 2015), benchmarked against five quality indicators: appropriateness of indications, bowel preparations, cecum and ileal examination rates, and complications. The European Society of Gastrointestinal Endoscopy guideline for pediatric endoscopy and North American Society for Pediatric Gastroenterology, Hepatology and Nutrition training guidelines were used as *** Median(± SD) age of 121 children [males = 74(61.2%)] who had 177 colonoscopies was 7.0(± 4.6) years. On average, 30 colonoscopies were performed each year(range: 19-58). Except for investigations of abdominal pain(21/177, 17%), indications for colonoscopies were appropriate in the remaining 83%. Bowel preparation was good in 87%. One patient(0.6%) with severe Crohn's disease had bowel perforation. Cecum examination and ileal intubation rate was 95% and 68.1%. Ileal intubation rate was significantly higher in diagnosing or assessing inflammatory bowel disease(IBD) than non-IBD(72.9% vs 50.0%; P = 0.016). Performance of four trainees was consistent throughout the study period. Average cecum and ileal examination rate among trainees were 97% and 77%.CONCLUSION Benchmarking against established guidelines helps units with a low-volume of colonoscopies to identify area for further improvement.

A biosynthetic gene cluster for the bioactive fungal sesterterpenoids variecolin(1)and variecolactone(2)was identified in Aspergillus aculeatus ATCC *** production of 1 and 2 was achieved in Aspergillus oryzae by expressing the sesterterpene synthase VrcA and the cytochrome P450 ***,the replacement of VrcB with homologous P450s from other fungal terpenoid pathways yielded three new variecolin analogues(5-7).Analysis of the compounds’anticancer activity in vitro and in vivo revealed that although 5 and 1 had comparable activities,5 was associated with significantly reduced toxic side effects in cancer-bearing mice,indicating its potentially broader therapeutic *** study describes the first tests of variecolin and its analogues in animals and demonstrates the utility of synthetic biology for creating molecules with improved biological activities.

Absent middle hepatic vein in a right liver graft donor

BACKGROUND: The middle hepatic vein (MHV) is normally in form of a large trunk lying within the midplane of the liver. An anomaly in form of two separate trunks, each draining segment V/VIII and segment IV, has been described by Couinaud but not been well documented in the literature. METHOD: We report a right liver donor in whom the MHV was absent and not encountered during liver transection along the midplane of the liver. RESULTS: On computed tomography (CT) scan and intraoperative ultrasonography, there was a large segment VIII hepatic vein mistaken as the MHV on preoperative assessment and a large segment IV hepatic vein close to the ligamenturn venosum. CT volumetry based on either segment VIII or IV hepatic vein led to major error in liver volume calculation. Transection of the liver guided by segment VIII or IV hepatic vein would lead to sacrifice of liver parenchyma unnecessarily or presence of necrotic liver in the graft. CONCLUSION: Absent MHV is a rare anomaly. It is revealed by careful study of the CT scan.

目的:报告1例免疫功能正常的患者患复发性后天性眼弓形体病的异常恶化及治疗后视力的改善。方法:病例报告。结果:比沙亚族女孩,21岁,其它方面健康,有与猫接触史,表现为右眼无痛、渐进性视力模糊2wk。视力右眼6/15,左眼6/6。眼底检查呈右眼后极部局灶性视网膜炎,伴视盘肿胀充血,双眼周边陈旧性视网膜脉络膜疤痕。经弓形虫血清检查证实其右眼患复发性眼弓形虫病。起初,患者对复方新诺明和阿奇霉素两种药物反应良好。然而在治疗过程中患者右眼弓形体病两次恶化,右眼视力下降到6/60,随后进行持续6wk的口服阿奇霉素和逐渐减量的皮质类固醇药物治疗,右眼弓形虫性视网膜炎治愈,随访1a未见复发。患者最终视力,右眼6/30,左眼6/6。结论:患复发性后天性眼弓形体病在免疫功能正常的个体并不常见。延误诊断和治疗导致永久性失明。及时、充分的治疗和良好的治疗依从性对防止并发症毁坏视觉极其重要。

Towards a respondent-preferred k_i-anonymity model

Recently, privacy concerns about data collection have received an increasing amount of attention. In data collection process, a data collector （an agency） assumed that all respondents would be comfortable with submitting their data if the published data was anonymous. We believe that this assumption is not realistic because the increase in privacy concerns causes some re- spondents to refuse participation or to submit inaccurate data to such agencies. If respondents submit inaccurate data, then the usefulness of the results from analysis of the collected data cannot be guaranteed. Furthermore, we note that the level of anonymity （i.e., k-anonymity） guaranteed by an agency cannot be verified by respondents since they generally do not have access to all of the data that is released. Therefore, we introduce the notion of ki-anonymity, where ki is the level of anonymity preferred by each respondent i. Instead of placing full trust in an agency, our solution increases respondent confidence by allowing each to decide the preferred level of protection. As such, our protocol ensures that respondents achieve their preferred kranonymity during data collection and guarantees that the collected records are genuine and useful for data analysis.

Return to sport following tibial plateau fractures: A systematic review

AIM To systemically review all studies reporting return to sport following tibial plateau fracture, in order to provide information on return rates and times to sport, and to assess variations in sporting outcome for different treatment *** A systematic search of CINAHAL, Cochrane, EMBASE, Google Scholar, MEDLINE, PEDro, Scopus, SPORTDiscus and Web of Science was performed in January 2017 using the keywords "tibial", "plateau", "fractures", "knee", "athletes", "sports", "non-operative", "conservative", "operative", "return to sport". All studies which recorded return rates and times to sport following tibial plateau fractures were included. RESULTS Twenty-seven studies were included: 1 was a randomised controlled trial, 7 were prospective cohort studies, 16 were retrospective cohort studies, 3 were case series. One study reported on the outcome of conservative management(n = 3); 27 reported on the outcome of surgical management(n = 917). Nine studies reported on Open Reduction Internal Fixation(ORIF)(n = 193), 11 on Arthroscopic-Assisted Reduction Internal Fixation(ARIF)(n = 253) and 7 on Frame-Assisted Fixation(FRAME)(n = 262). All studies recorded "return to sport"rates. Only one study recorded a "return to sport" time. The return rate to sport for the total cohort was 70%. For the conservatively-managed fractures, the return rate was 100%. For the surgically-managed fractures, the return rate was 70%. For fractures managed with ORIF, the return rate was 60%. For fractures managed with ARIF, the return rate was 83%. For fractures managed with FRAME was 52%. The return rate for ARIF was found to be significantly greater than that for ORIF(OR 3.22, 95%CI: 2.09-4.97, P < 0.001) and for FRAME(OR 4.33, 95%CI: 2.89-6.50, P < 0.001). No difference was found between the return rates for ORIF and FRAME(OR 1.35, 95%CI: 0.92-1.96, P = 0.122). Perpendicular magnetic clusters with configurable domain structures via dipole-dipole interactions

Magnetic single-domain islands based on in-plane anisotropy （usually, shape anisotropy） and their dipole-dipole interactions have been investigated extensively in recent years. This has been driven by potential applications in magnetic recording, spintronics, magneto-biology, etc. Here, we propose a concept of out- of-plane magnetic dusters with configurable domain structures （multi-flux states） via dipole-dipole interactions. Their flux stages can be switched through an external magnetic field. The concept has been successfully demonstrated by patterned [Co/Pd] islands. A [Co/Pd] multilayer exhibits a large perpendicular anisotropy, a strong physical separation, and uniform intrinsic properties after being patterned into individual islands by electron beam lithography. A three- island cluster with six stable flux states has been realized by optimizing island size, thickness, gap, anisotropy, saturation magnetization, etc. Using [Co/Pd] multilayers, we have optimized the island structure by tuning magnetic properties （saturation magnetization and perpendicular anisotropy） using Landau-Liftshitz- Gilbert （LLG） simulation/calculation. Potential applications have been proposed, including a flexi-programmable logic device with AND, OR, NAND, and NOR functionalities and a magnetic domino, which can propagate magnetic current as far as 1 μm down from the surface via vertical dipole-dipole interactions.
