In this study, we modified a polytetrafluoroethylene （PTFE） hollow-fiber membrane element used for submerged membrane bioreactors （MBRs） to reduce the energy consumption during MBR processes. The high mechanical strength of the PTFE membrane made it possible to increase the effective length of the membrane fiber from 2 to 3 m. In addition, the packing density was increased by 20% by optimizing the membrane element configuration. These modifications improve the efficiency of membrane cleaning associated with aeration. The target of specific energy consumption was less than 0.4 kWh·m^-3 in this study. The continuous operation of a pilot MBR treating real municipal wastewater revealed that the MBR utilizing the modified membrane element can be stably operated under a specific air demand per membrane surface area （SADm） of 0.13 m^3·m^-2. hr I when the daily- averaged membrane fluxes for the constant flow rate and flow rate fluctuating modes of operation were set to 0,6 and 0.5m^3·m^-2·d^-1 respectively. The specific energy consumption under these operating conditions was estimated to be less than 0.37 kWh.m^-3. These results strongly suggest that operating an MBR equipped with the modified membrane element with a specific energy consumption of less than 0.4 kWh·m^-3 is highly possible.

OBJECTIVE： Hie （cold sensation） is one of the most common health complaints in Japan. Those who suffer from severe hie are considered as having hiesho （cold disorder）. However, exact hiesho symptoms have not been defined clearly and the decision as to whether a person suffers from hiesho is subjective and based on self-awareness. The study was conducted in attempt to develop a standardized hiesho diagnostic scale. METHODS： Subjects comprised 1 146 students. From the self-awareness of hiesho symptoms, males and females were divided into hiesho and non-hiesho groups. Physical, behavioral and adaptive characteristics were compared using the 24-item questionnaire （four-grade survey） and indicators for hiesho symptoms were extracted. Based on the scores, a receiver operating characteristic curve was drawn for the total ordinal scale score of the extracted items in relation to the presence and absence of hiesho symptoms, and an optimal cutoff value was determined. RESULTS： The self-awareness of having hiesho was found in 23.2% males and in 55.6% females. The sensitivity was 84.5% for males and 83.3% for females in the hiesho groups, and the specificity was 86.0% for males and 85.2% for females in the non-hiesho groups. CONCLUSION： A questionnaire consisting of the extracted items may be useful to identify hiesho in young males and females with a high level of accuracy.

Dear Editor,Sperm in patients with obstructive azoospermia （OA） have been retrieved from the reproductive tract and/or from the testis for intracytoplasmic sperm injection （ICSI） in several ways, including microsurgical epididymal sperm aspiration （MESA）, percutaneous epididymal sperm aspiration （PESA）, testicular sperm extraction （TESE）, and other techniques） Each of these has advantages and drawbacks in terms of microsurgical expertise or equipment, number of sperm obtained, invasiveness, and cost. Therefore, we developed a safe, simple, feasible, and low-cost modified MESA technique to collect epididymal sperm under direct vision, which we have labeled macrosurgical epididymal sperm imprint collection （MESIC）. Herein, we present the details of the MESIC technique and our initial experience in patients with suspected OA.

Tumor markers comprise a wide spectrum of biomacromolecules excessively synthesized by a variety of neoplastic cells. These markers can be endogenous products of highly active metabolites from malignant neoplastic cells or the products of newly activated genes. Ideally, tumor markers should be highly sensitive, specific, and reliable with a high prognostic value and organ specificity. In addition, they should reflect the tumor stage. However, no tumor markers identified thus far have all of these characteristics. Nevertheless, most tumor markers show excellent clinical relevance for monitoring the efficacy of a variety of therapies. We herein review how to use the recommended tumor markers to diagnose malignancies, such as gastrointestinal carcinoma, liver cancer, bile duct/pancreatic cancer, lung cancer, breast cancer, gynecologic cancer, and urologic cancer.

Evaluation of a Dose-Monitoring Method for Prophylactic Anticoagulant Therapy with Low-Molecular-Weight Heparin

Objective: In the present study, we report on the results of our investigation of optimum dose monitoring using coagulation and fibrinolytic system indicators during obstetric prophylactic anticoagulant therapy with enoxaparin. Study Design: Of 103 cases of cesarean section performed at our hospital, 37 cases were selected for this study after obtain ing their consent for blood collection. Variables of the coagulation and fibrinolytic systems [anti-factor Xa activity, endogenous thrombin potential (ETP), prothrombin time (PT) or international normalized ratio (INR), activated partial thromboplastin time (APTT) and D-dimer levels] were determined. Results: In the 5-day administration group, the anti-factor Xa activitywas 0.0 U/ml on the postoperative day 1, increased to 0.05 U/ml ± 0.04 U/ml on the postoperative day 3, and mildly increased to 0.06 U/ml ± 0.05 U/ml on the postoperative day 5. On the other hand, the anti-factor Xa activity in the 3-day administration group was 0.0 U/ml on the postoperative day 1 (before enoxaparin administration), increased to 0.06 U/ml ± 0.05 U/ml on the postoperative day 3, and significantly decreased to 0.02 U/ml ± 0.03 U/ml on the postoperative day 5 (p = 0.003);thus, the pattern of change was significantly different from that in the 5-day administration group (p = 0.004). Enoxaparin administration did not result in any significant fluctuation of the ETP, and no significant difference was observed between the 5-day and 3-day administration groups. Conclusion: Enoxaparin administration was associated with increase of the anti-factor Xa activity, and prolonged administration led to more sustained increase of the activity.
