mathematical models and simulation are considered a powerful tool in engineering practice. Those tools are becoming increasingly used for the improvement of wastewater treatment plants design because the conceptual design is complex and ill-defined. In this paper, three alternatives: 1) complete mix activated sludge without nitrogen removal (CAS);2) complete mix activated sludge with nitrogen removal (CAS-N) and;3) membrane bioreactor (mBR) processes were designed into two steps: first concept design to calculate the size of process units, then second implement modeling and simulation to improve the accuracy of the conceptual design. In brief, the treatment process design has been verified by using the activated sludge model No. 1 (ASm1) in GPS-X (v.7) simulation software. This application helps not only in sizing the treatment units but also in understanding the plant’s capacity. In the same time, it can assist in studying the future expansion works required for increased hydraulic and organic loadings. For this purpose, Tikrit WWTP was selected as a case study. The used model was validated by comparing the designed values of the plant and the modeling data. The verification of the obtained results from both hand calculations and the results of the program showed a good agreement. A significant difference in the volume of secondary treatment was obtained from design calculations, where the CAS without denitrification system was 9244 m3 (aerobic and secondary tanks), CAS with denitrification system was 11,324 m3 (anoxic, aerobic and secondary tanks) and for mBR system was 7468 m3 (anoxic, aerobic and immersed membrane tanks). From the obtained results point of view, it can be concluded that mathematical models can be considered as worthy tools to complement the established wastewater treatment plant design procedures.

Background: Recent advances in breast reconstruction and the introduction of oncoplastic techniques have resulted in significant improvement in quality of life and psychological well-being of most patients. Nipple sparing mastectomy is a surgical technique that removes breast tissue while preserving the native skin envelope, infra-mammary fold and the NAC, which allows immediately reconstructed breasts to have an excellent cosmetic outcome. Aim: Our primary end point was for objective assessment of aesthetic outcome after NSm via more accurate new method and subsiding bias in that assessment and the secondary end point was for evaluating the influence of incision choice and recommending which incision is the best for each patient putting in mind cup size, degree of ptosis and body mass index of Egyptian patients. methods: Starting January 2013 to November 2015, 74 patients with breast cancer underwent NSm with immediate reconstruction using LD flap with or without implant augmentation. Results: Incisions used are elliptical (37.8%), lateral (27%), peri-areolar (21.6%) and infra-mammary (13.5%). In 81.1% of the patients, the procedure was performed using extended LD flaps only, while in the remaining 18.9% the flaps were augmented using implant insertion. Axillary dissection was done in 68.9% of patients and SLN in 24.3% of patients. Overall aesthetic results were done by patient self-assessment, assessment by the surgeon, assessment by professional plastic breast surgeon and assessment by onco-plastic surgeon, and this was followed by statistical analysis of the agreement between the plastic surgeon and the onco-plastic surgeon. Conclusion: NSm is safe, feasible and offers adequate oncologic results along with excellent cosmetic outcome. Choice of incision and reconstruction should be tailored to suit each patient. Breast cancer patients can benefit from sound resection and enjoy a sense of wholeness.

Impact of Postoperative Radiotherapy and Chemotherapy on Survival for Patients with Node Positive Oral Cancer

Purpose: The aim of the present study is to compare between the use of post-operative concurrent chemotherapy and radiotherapy alone on survival for patients with high risk oral cavity tumors at the National cancer Institute of Egypt, undergoing surgery and receiving adjuvant treatment. Patients and methods: This is a retrospective study, which was carried out at the National Cancer Institute (Cairo University) on patients with node positive oral cavity cancer diagnosed between the year 2000 and 2008. The study included 60 patients (45 males and 15 females) with median age 57 years old. The patients underwent surgery, followed by postoperative radiotherapy 60 Gy/6 weeks versus postoperative radiotherapy 60 Gy/6 weeks with concurrent cisplatin 100 mg/m2 at day 1, day 22 and day 43. Results: Regarding use of concurrent chemotherapy, there was a significant difference in overall survival rate and locoregional control favoring patients who received concurrent chemotherapy and radiotherapy, 3 years and 5 years overall survival rates respectively were 53.8% and 40.4% compared to 37.5% and 26.3% for patients who didn’t receive any chemotherapy with (p 0.038) for 5 years. Regarding age, there was a significant difference in overall survival rate favoring patients ≤57 years in both arm groups, 3 years and 5 years overall survival rates respectively were 51.6% and 38.9% compared to 28.3% and 18.9% for patients >57 years with (p 0.028) for 5 years. Conclusion: We recommend for oral cavity tumor patients at the NCI of Egypt who have positive neck nodes to be treated with concurrent chemo-radiotherapy rather than radiotherapy alone especially by using the new techniques as intensity modulated radiotherapy (ImRT) and image guided radiotherapy (IGRT).

The residual concentrations of HCHs, TC, DDTs, THCs and PCBs were measured in Sparus auratus collected from the eastern and western sectors of the Egyptian mediterranean Sea, as the most common fish species in the area. Gas chromatograph equipped with 63Ni-electron capture detector and 35% phenyl polysilphenylenesiloxane capillary column was used for the analysis and quantification after well-established extraction techniques. The concentrations (wet weight) of PCBs, DDTs, TCs, HCHs were: (1.87 - 616.66 ng·g-1), (0.42 - 98.28 ng·g-1), (0.06 - 2.94 ng·g-1), (0.35 - 11.77 ng·g-1), respectively in Sparus auratus collected from the eastern sector. However, these concentrations as wet weight in Sparus auratus from the western sector were: (5.75 - 605.53 ng·g-1), (1.53 - 226.47 ng·g-1), (0.09 - 8.12 ng·g-1), (0.26 - 5.80 ng·g-1), respectively. In general, concentrations of pesticides and PCBs in Sparus auratus either from the western or eastern Egyptian mediterranean coast were far below the international permissible levels.

Treatment Outcome of Pharmacokinetics-Based Dosing of Docetaxel and Fluorouracil in Advanced Head and Neck Cancer Patients

Introduction: Docetaxel, Cisplatin and 5-Fluorouracil (DPF) became the standard induction chemotherapy in advanced Head and Neck Cancer (HNC) but associated with high toxicity rate. Several studies reported higher response rates with better tolerability when chemotherapy dose is calculated based on Pharmacokinetics (PK) versus conventional Body Surface Area (BSA). Patients and methods: Thirty nine patients with stage III and IV HNC who received induction DPF were included in the study. Dose of cycle 1 was BSA-based then Docetaxel and 5-FU doses were PK-adjusted starting from cycle 2 whereas Cisplatin dose was BSA-based throughout the study. Results: After median follow up period of 14 months the median overall survival (OS) and progression free survival (PFS) were 15.1 and 10.6 months respectively. Twenty nine patients were available for response assessment. Seven patients (24.1%) achieved complete response while partial response encountered in 19 patients (65.5%) with and Overall response rate of 89.6%. Both treatment related side effects and mortality significantly decreased after the application of PK dose adjustments (p-value 0.007 and 0.01 respectively). Conclusion: PK-guided dose adjustments of 5-FU and Docetaxel in DPF regimen can significantly decrease the treatment related side effects and mortality without compromising the tumor response rate. A randomized clinical trial is needed to compare the PK-guided dose adjustment with the standard BSA based protocol.

Organotin and organolead compounds were determined in sediments of the Suez Gulf The concentrations of Tributyltin (TBT) ranged from 0.27 to 2.77 with an average value of 1.37 μgg-1;dry wt. However, the concentrations of dibutyltin (DBT) ranged from 0.07 to 2.27 with an average value of 0.58 μgg-1;dry wt. A significant correlation was found between TBT and DBT with r = 0.82, (p = 0.05) indicating that the occurrence of DBT is mainly related to the degradation of TBT. Generally, the high concentration of TBT was attributed to shipping activity in harbours. In addition, Diphenyltin (DPhT) concentrations ranged from not detected to 2.09 with an average of 1.10 μgg-1 dry wt. Antifouling agents, industrial discharge and the influence of sewage discharge are the main sources of pollution by DPhT compounds in Suez Gulf. On the other side, organolead (OLC) concentrations ranged from 10.88 - 440.2 with an average of 168.7 ngg-1;dry wt. A significant setting of OLC recorded in sediments of Suez Gulf was mainly attributed to cars exhaust and/or spelling and direct evaporation of fuels.

Background: Several Phase III randomized trials have demonstrated improved local control and survival for post-mastectomy radiotherapy in patients with high-risk pathologic features. Close or involved surgical margins were not included as high-risk in these protocols, but have been associated with increased risk of local failure;however, the impact of a boost dose following chestwall radiotherapy in this setting remains to be determined. methods: Retrospective single-institution outcomes analysis for patients with close or involved surgical margins treated with post-operative radiotherapy is followed by a boost. Results: Between 2003 and 2011, 34 patients were identified for inclusion in the present study. The median chestwall dose was 5040 cGy (range 5000 - 5040) and median boost dose was 1080 cGy (900 - 1620). At a median follow-up of 38.4 months (10.2 - 115.6;with 29% more than 5 years), 28 patients were alive without evidence of recurrence, 3 were alive with recurrent disease (1 chestwall), and 3 had died (none with recurrent disease). The 3-year local control, disease-free survival, and overall survivals were 96.9%, 93.9%, and 93.1%, respectively. Conclusion: Chestwall radiotherapy plus boost results in low risk of early locoregional recurrence for women with close or involved surgical margin(s) at mastectomy. Further investigation of PmRT with or without boost in this setting is warranted.

This paper investigates the performability of hierarchical Wireless Networked Control Systems (WNCS). The WNCS studied can operate in two modes: passive supervisor and active supervisor. It is first shown that the markov models for both modes are identical. Performability models are then developed and a case study shows how to use these models to help make design decisions. more specifically, it is observed that the performability of a passive supervisor system increases in time while that of an active supervisor system decreases in time.
