Risk-Adapted, Combined-Modality Therapy for Unfavorable Pediatric Hodgkin Lymphom

Background and Objectives: Risk-adapted therapy for children with HL is directed toward high survival, minimal toxicity and optimal quality of life, with long term follow up. We assess the impact of prognostic factors associated with local treatment failure of pediatric HL patients with unfavorable criteria treated with combined modality: Alternating ABVD (Doxorubicin, Bleomycin, Vinblastine and Decarbazine) and COEP (Cyclophosphamide, Oncovin, Etoposide and Prednisone) chemotherapy and response-based, involved-field radiation for newly diagnosed unfavorable pediatric HL patients, also will detect toxicities and long-term complications observed in the patients. Methods: This prospective study was carried out from January 2010 to January 2018, with a median follow up of 74 months (range 8 - 103 months). 54 patients were eligible for this study stratified into two groups: intermediate risk (IR) and high-risk group (HR). Patients were treated with (4 - 6 cycles) and (6 - 8 cycles) respectively of alternating ABVD/COEP chemotherapy followed by involved-field radiation therapy (IFRT): 15 Gy for patients achieved complete response, and 25.5 Gy for those achieved a partial response. Results: 27 patients were IR and 27 patients were HR. There were 16 treatment failures;5 patients had progressive disease;and 11 patients had a relapse. 9 patients died from their disease progression. The 5-year overall survival (OS) and event-free survival (EFS) rates (±SE) were 81.8% ± 5.7% and 71.8% ± 6.2% respectively. Multivariate analysis revealed that the only independent factor for inferior OS was radiotherapy. Conclusion: Treatment results of unfavourable HL patients in our study are satisfactory for with IR group but not for HR group who needs intensification of therapy. Radiotherapy is considered as a cornerstone in the treatment of the patients with unfavourable criteria with better assessment of early responders needed by PET-CT to identify patients at risk for relapse.

Aesthetic Outcome after Nipple Sparing Mastectomy for Cancer Patient Egyptian Pattern

Background: Recent advances in breast reconstruction and the introduction of oncoplastic techniques have resulted in significant improvement in quality of life and psychological well-being of most patients. Nipple sparing mastectomy is a surgical technique that removes breast tissue while preserving the native skin envelope, infra-mammary fold and the NAC, which allows immediately reconstructed breasts to have an excellent cosmetic outcome. Aim: Our primary end point was for objective assessment of aesthetic outcome after NSM via more accurate new method and subsiding bias in that assessment and the secondary end point was for evaluating the influence of incision choice and recommending which incision is the best for each patient putting in mind cup size, degree of ptosis and body mass index of Egyptian patients. Methods: Starting January 2013 to November 2015, 74 patients with breast cancer underwent NSM with immediate reconstruction using LD flap with or without implant augmentation. Results: Incisions used are elliptical (37.8%), lateral (27%), peri-areolar (21.6%) and infra-mammary (13.5%). In 81.1% of the patients, the procedure was performed using extended LD flaps only, while in the remaining 18.9% the flaps were augmented using implant insertion. Axillary dissection was done in 68.9% of patients and SLN in 24.3% of patients. Overall aesthetic results were done by patient self-assessment, assessment by the surgeon, assessment by professional plastic breast surgeon and assessment by onco-plastic surgeon, and this was followed by statistical analysis of the agreement between the plastic surgeon and the onco-plastic surgeon. Conclusion: NSM is safe, feasible and offers adequate oncologic results along with excellent cosmetic outcome. Choice of incision and reconstruction should be tailored to suit each patient. Breast cancer patients can benefit from sound resection and enjoy a sense of wholeness.

Role of Nurse in Administrating Induction of Ovulation Medications at Assisted Reproduction Center versus at Home

Background: The response of ovaries during controlled ovarian stimulation is the most important factor for evaluating the pregnancy outcome in assisted reproductive techniques. Aim: The study is to assess the role of a nurse in giving induction of ovulation medications at assisted reproduction university center versus home medications by private In Vitro Fertilization and infertility center on clinical outcomes and patient satisfaction. Subjects and Methods: Case-control research design, conducted at two centers (Banon private center for IVF and Assisted Reproductive Unit at Women health hospital, Assiut University) during the period from February 2018 to November 2018. The sample size included 150 women undergoing IVF for each group. Results: There is no statistically significant difference between women given IVF medications by the nurse at the public IVF center and women take IVF medication in the home at Banon IVF center as regards the outcome of IVF. Majority of infertile patients are satisfied with the care, they received and nearly third of them are satisfied with outcome of IVF in both groups, and there is statistically significant difference between satisfaction in public IVF center and satisfaction in private center in relation to information provided about IVF procedure, staff willingness to listen carefully and help patients;regular progress update on condition is in place with p value (0.000, 0.005, 0.003) respectively. Conclusion: IVF outcome isn’t significantly related to where induction medication was taken either at IVF center or at the home because infertile couples are keen to comply with all instructions on how to take medications with correct route, correct dose and on time. Recommendations: Empower the role of nurse as a health educator for women undergoing IVF about importance of stage of induction and take medications with correct route, correct dose and on time on outcome of IVF;other studies should also be conducted in other centers to furt

Diagnostic value of dobutamine stress Doppler tissue imaging in diabetic patients with suspected coronary artery disease

Background: Coronary artery disease (CAD) is often silent in diabetic patients, and it is typically in ad-vanced stages of development by the time it manifests. Various forms of stress testing have been investigated to detect obstructive CAD in diabetes mellitus. Ob-jectives: To assess the diagnostic value of dobutamine stress pulsed-wave Doppler tissue imaging (DTI) compared with standard wall motion analysis in de-tection of myocardial ischemia in diabetic patients with suspected CAD. Methods: The study comprised 46 diabetic patients with suspected CAD underwent dobutamine stress echocardiography (DSE) with DTI within 4 weeks before coronary angiography(CA). Dobutamine infusion started at 5 μ/kg/min and in-creased up to 40 μ/kg/min with additional atropine during submaximal heart rate responses. In addition to wall-motion score index (WMSI) analysis, pulsed- wave DTI examination of basal and mid segments of posteroseptal, lateral, anterior, inferior and antero- septal walls was performed. Myocardial velocities were measured at rest in the apical 4, 3 and 2-cham- ber views. The measurements were repeated at low dose (10 - 15 μ/kg/min) and at peak stress (40 μ/kg/min). DTI measurements included peak systolic velocity (S), peak early diastolic velocity (E) and peak late diastolic velocity (A) and the results were com-pared to WMSI analysis. Patients were classified into two groups according to CA results;group (I) di-abetics with positive CA (n = 27) and group (II) di-abetics with negative CA (n = 19). Results: There was no significant difference between the two groups in duration of diabetes, global WMSI at rest or the ? changes (stress-rest/rest) of WMSI (P > 0.05). Global S and global E were significantly lower in group I compared to group II at peak stress (11.3 ± 3.7 cm/sec vs. 14.5 ± 2.2 cm/sec, p ely. Mastectomy Scar Boost Results in Low Risk of Locoregional Recurrence in the Setting of Close or Involved Surgical Margins

Background: Several Phase III randomized trials have demonstrated improved local control and survival for post-mastectomy radiotherapy in patients with high-risk pathologic features. Close or involved surgical margins were not included as high-risk in these protocols, but have been associated with increased risk of local failure;however, the impact of a boost dose following chestwall radiotherapy in this setting remains to be determined. Methods: Retrospective single-institution outcomes analysis for patients with close or involved surgical margins treated with post-operative radiotherapy is followed by a boost. Results: Between 2003 and 2011, 34 patients were identified for inclusion in the present study. The median chestwall dose was 5040 cGy (range 5000 - 5040) and median boost dose was 1080 cGy (900 - 1620). At a median follow-up of 38.4 months (10.2 - 115.6;with 29% more than 5 years), 28 patients were alive without evidence of recurrence, 3 were alive with recurrent disease (1 chestwall), and 3 had died (none with recurrent disease). The 3-year local control, disease-free survival, and overall survivals were 96.9%, 93.9%, and 93.1%, respectively. Conclusion: Chestwall radiotherapy plus boost results in low risk of early locoregional recurrence for women with close or involved surgical margin(s) at mastectomy. Further investigation of PMRT with or without boost in this setting is warranted.

NUMERICAL AND EXPERIMENTAL SURVEY OF CENTRIFUGAL MACHINE COMPONENTS

Although centrifugal compressors have been in use for over a century, much empiricism is still present in the compressor(or pump) design process. Therefore, a better understanding is needed of the flow process inside their components(impeller, diffuser including vaneless and vaned, and volute casing). The article reviewed process for the experimental and numerical published researches concerning this issue and calculated the 3 D flow pattern in Krain’s centrifugal impeller using CFX Tascflow code with CFX Turbogrid for grid generation. Higher number of grid points is used and the tip clearance is simulated using five grid points. The results show good agreement with that measured by Krain. The CFX Tascflow code can be applied to the analsis of the complicated flow structures in centrifugal impeller.

On the Performability of Hierarchical Wireless Networked Control Systems

This paper investigates the performability of hierarchical Wireless Networked Control Systems (WNCS). The WNCS studied can operate in two modes: passive supervisor and active supervisor. It is first shown that the Markov models for both modes are identical. Performability models are then developed and a case study shows how to use these models to help make design decisions. More specifically, it is observed that the performability of a passive supervisor system increases in time while that of an active supervisor system decreases in time.

Behavior and Flexural Prediction of Special Cementitious Bonding Material for Fiber Reinforced Strengthening Systems

Strengthening of RC structures with externally bonded FRP （fiber reinforced polymers） has become an important challenge in civil engineering. Epoxy is the main bonding agent used so far, but in the case of a fire, it is subjected to complete loss of his bonding capabilities. Mineral based composites strengthening systems consist of FRPs and a cementitious bonding agent which form a repair or strengthening system that is more compatible with the concrete substrata, and roved its efficiency. The current research introduces the use of a special cementitious material ＂Grancrete＂ as a bonding agent. Test results of 32 T-section RC beams strengthened with various FRG （fiber reinforced Grancrete） strengthening systems are presented. The results demonstrated that most of the specimens were likely to fail by debonding of the FRP from the concrete either at the ends or at intermediate flexural cracks. This paper presents an in-depth study aimed at the development of a better understanding of debonding failures in RC beams strengthened with externally bonded FRP systems. Different analytical models, published in the literature for plate end debonding, are reviewed and compared to test results. The results also demonstrated that when using U-wraps, the specimens were likely to fail by FRP sheet rupture.
