Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases

AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

This paper presents a genetic programming based reconfiguration planner for metamorphic modular robots. Initially used for evolving computer programs that can solve simple problems, genetic programming （GP） has been recently used to handle various kinds of problems in the area of complex systems. This paper details how genetic programming can be used as an automatic programming tool for handling reconfiguration-planning problem. To do so, the GP evolves sequences of basic operations which are required for transforming the robot＇s geometric structure from its initial configuration into the target one while the total number of modules and their connectedness are preserved. The proposed planner is intended for both Crystalline and TeleCube modules which are achieved by cubical compressible units. The target pattern of the modular robot is expressed in quantitative terms of morphogens diffused on the environment. Our work presents a solution for self recontlguration problem with restricted and unrestricted free space available to the robot during reconfiguration, The planner outputs a near optimal explicit sequence of low-level actions that allows modules to move relative to each other in order to form the desired shape.

Applying the Density Function Theory (DFT) combined with LCAO basis set and employing the B3LYP hybrid functional, the optimized geometrical parameters, electronic properties, as well as the Infrared and Raman spectra for wurtzite-ZnO structure were investigated. Prior to computing, ZnO thin film prepared by the spray pyrolysis method is characterized by X-ray diffraction using Rietveld refinement. This analysis shows that ZnO has hexagonal wurtzite structure (P63mc) with lattice parameters, a = 3.2467 and c = 5.2151 Åin good agreement with our predicted optimized geometry. Atomic force microscopy (AFM), Raman spectroscopy and UV-Vis-NIR spectrophotometry techniques are used to explore morphological, optical and vibrational properties of the sprayed ZnO thin film. The computed band gap is in excellent agreement with that deduced from UV-Vis transmission . The simulated infrared and Raman spectra were also calculated, and a good agreement with the measured spectra is obtained. Finally, a detailed interpretation of the infrared and Raman spectra is reported.

This paper investigates the performance of different configurations of gas turbine engines.A full numerical model for the engine is *** model takes into account the variations in specific heat and the effects of turbine cooling flow.A lso,the model considers the efficiencies of all component,effectiveness of heat exchangers and the pressure drop in relevant *** model is employed to compare the engine performances in cases of employing intercooler,recuperation and reheat on a single spool gas turbine engine.A comparison is made between single-spool engine and two-spool engine with free power ***,the performance of the eng ine with inter-stage turbine burner is investigated and compared with engine employing the nominal reheat *** engine employing inter-stage turbine bumers produces superior improvements in both net work and efficiency over all other *** effects of ignoring the variations on specific heat of gases and turbine cooling flow on engine performance are *** the variation in specific heat can cause up to 30%difference in net specific *** optimum locations of the intercooler and the reheat combustor are detemined using the numerical model of the *** maximum net specific work is obtained if the reheat combustor is placed at 40%of the expansion *** the other hand,to get maximum efficiency the reheat combustor has to be placed at nearly 10%-20%of the expansion *** optimum location of the intercooler is almost at 50%of the compression section for both maximum net specific work and efficiency.
