Expanding endoscopic boundaries:Endoscopic resection of large appendiceal orifice polyps with endoscopic mucosal resection and endoscopic submucosal dissection

BACKGROUND Large appendiceal orifice polyps are traditionally treated ***,endoscopic mucosal resection(EMR)and endoscopic submucosal dissection(ESD)have been utilized as alternative resection *** To evaluate the efficacy and safety of endoscopic resection techniques for the management of large appendiceal orifice *** This was a retrospective observational study conducted to assess the feasibility and safety of EMR and ESD for large appendiceal orifice *** project was approved by the Baylor College of Medicine Institutional Review *** who underwent endoscopic resection of appendiceal orifice polyps≥1 cm from 2015 to 2022 at a tertiary referral endoscopy center in the United States were *** main outcomes of this study included en bloc resection,R0 resection,post resection adverse events,and polyp *** A total of 19 patients were *** patients were female(53%)and Caucasian(95%).The mean age was 63.3±10.8 years,and the average body mass index was 28.8±*** mean polyp size was 25.5±14.2 mm.74%of polyps were localized to the appendix(at or inside the appendiceal orifice)and the remaining extended into the cecum.68%of polyps occupied≥50%of the appendiceal orifice *** mean procedure duration was 61.6±37.9 *** were resected via endoscopic mucosal resection,endoscopic submucosal dissection,and hybrid procedures in 5,6,and 8 patients,*** pathology was remarkable for tubular adenoma(n=10)[one with high grade dysplasia],sessile serrated adenoma(n=7),and tubulovillous adenoma(n=2)[two with high grade dysplasia].En bloc resection was achieved in 84%with an 88%R0 resection *** the large polyp sizes and challenging procedures,89%(n=17)of patients were discharged on the same day as their *** patients were admitted for post-procedure observation for conservative pain *** patients underwent repeat colonoscopy without evide

Centurial Evolution of an Offshore Mud Deposition Area in the Changjiang (Yangtze) Estuary and Its Links to Environmental and Anthropogenic Activities

Large amounts of sediments originating from the Changjiang(Yangtze)River are deposited in the subaqueous delta and in the adjacent muddy area off the mouth and on the inner shelf of the East China *** terrestrial sediments deposited in these areas not only contain information about the composition and environment of the source area,but they also record changes in anthropogenic activities.A sediment piston core(CJ0702)was extracted from the Changjiang subaqueous depocenter(31.00°N,122.67°E)in a water depth of 22.0 *** core was subsampled at 1–2 cm intervals and analyzed for grain size,clay mineralogy,and major element *** indicate a relatively high sediment accumulation rate of approximately 3.11 cm yr−*** parameters exhibited only minor cyclical fluctuations in the core,which resulted from many *** the past 120 years,the Changjiang River-derived sediment is the primary source of sediment in the offshore mud area without evidence for the Yellow River-derived sediment *** the trunk stream shifted from the North Branch to South Branch,the variations of proxies are controlled by the periodic fluctuation possibly linked to El Niño-Southern Oscillation(ENSO)and the Pacific Decadal Oscillation(PDO).In addition,anthropogenic heavy metal concentrations can be divided into three stages,which coincide well with economic development and environmental protection policies.

Neural tissue engineering,nanotechnology and neuroregeneration are diverse biomedical disciplines that have been working together in recent decades to solve the complex problems linked to central nervous system(CNS)*** is known that the CNS demonstrates a very limited regenerative capacity because of a microenvironment that impedes effective regenerative processes,making development of CNS therapeutics *** the high prevalence of CNS conditions such as stroke that damage the brain and place a severe burden on afflicted individuals and on society,it is of utmost significance to explore the optimum methodologies for finding treatments that could be applied to humans for restoration of function to pre-injury *** vesicles(EVs),also known as exosomes,when derived from mesenchymal stem cells,are one of the most promising approaches that have been attempted thus far,as EVs deliver factors that stimulate recovery by acting at the nanoscale level on intercellular communication while avoiding the risks linked to stem cell *** the same time,advances in tissue engineering and regenerative medicine have offered the potential of using hydrogels as bio-scaffolds in order to provide the stroma required for neural repair to occur,as well as the release of biomolecules facilitating or inducing the reparative *** review introduces a novel experimental hypothesis regarding the benefits that could be offered if EVs were to be combined with biocompatible injectable *** rationale behind this hypothesis is presented,analyzing how a hydrogel might prolong the retention of EVs and maximize the localized benefit to the *** sustained delivery of EVs would be coupled with essential guidance cues and structural support from the hydrogel until neural tissue remodeling and regeneration ***,the importance of including nonhuman primate models in the clinical translation pipeline,as well as the added benefit of m

Thermoelastic vibrations in a thin elliptic annulus plate with elastic supports

In this study, integral operational methods are used to investigate the thermally induced transverse vibration of a thin elliptic annulus plate with elastic supports at both radial *** axisymmetric temperature distribution is determined by the heat conduction differential equation and its corresponding boundary conditions by employing a unified integral transform technique by use of Mathieu functions and modified Mathieu functions. The solution of thermally induced vibration of the plate with both ends encased with elastic supports is obtained by employing an integral transform for double Laplace differential equation. The thermal moment is derived on the basis of temperature distribution, and its stresses are obtained based on resultant bending moments per unit width. The numerical calculations of the distributions of the transient temperature and its associated stress distributions are shown in the figures.
