Predictors of Spontaneous Bacterial Peritonitis in Patients with Cirrhotic Ascites

Background and Aims:Spontaneous bacterial peritonitis(SBP)is a serious complication of liver cirrhosis and a prognostic model to predict it is *** study was designed to test the ability of different laboratory tests and the new scoring system by Wehmeyer and colleagues(consisting of age,C-reactive protein(CRP)and platelet count)to predict ***:Three-hundred patients admitted to the National Liver Institute,University of Menoufia,Egypt(2015-2016)with liver cirrhosis and ascites were included in our *** was diagnosed if ascetic neutrophil count was≥250/μL with no sign of secondary ***:The patient population had age range of 29-81 years old,was 60%men and showed a majority(91.7%)with primary cause of liver disease being hepatitis *** univariate analysis,associations with age,total bilirubin,aspartate aminotransferase level,creatinine level,international normalized ratio,model for endstage liver disease score,total leucocytic count,platelet count and CRP level were *** multivariate analysis,independent predictors were age,platelet count and CRP level(p=0.004,0.013 and<0.001,respectively).CRP at a cut-off point≥13.5 mg/L could predict SBP(sensitivity of 86.4%and specificity of 66.0%).Wehmeyer's SBP scoring system was predictive(p<0.001);only 4%of patients with 0 score developed SBP(CRP cut-off of 30 mg/L),while 92.8%with score of 3 or 4 developed *** using our modified Wehmeyer score with CRP cut-off value of 13.5 mg/L,no patient with 0 score developed ***:Age,CRP level and platelet count are independent predictors for SBP and a scoring system including them could easily predict the *** diagnosis could be excluded in patients with 0 score,using CRP cut-off value of 13.5 mg/L.

Detection of Polymorphisms of DNA Repair Genes (XRCC1 and XPC) in Prostate Cancer

Prostate cancer is a common disease with a multifactorial and complex etiology. It is the most common male malignancy and the second leading cause of death in many countries. The widespread use of PSA testing has increased the detection of this cancer at earlier stages, although this diagnostic method has proved to be insufficient to identify the disease. DNA in most cells is regularly damaged by endogenous and exogenous mutagens. At least four main partially overlapping damage repair pathways operate in mammals. Common polymorphisms in DNA repair genes may alter protein function and an individual’s capacity to repair damaged DNA;deficits in repair capacity may lead to genetic instability and carcinogenesis. In the present study, we investigated the genotypic distribution of XRCC1 and XPC polymorphisms and its association with prostate cancer risk, pathological staging and Gleason’s scoring. The present study was conducted in the departments of Clinical Pathology, Pathology, and Urology Faculty of Medicine, Alexandria University-Egypt. A total number of 50 patients with pathologically confirmed prostate cancer and 50 age-matched control subjects were enrolled in this study. The diagnosis was made on the basis of histopathological findings, following radical prostatectomy or transurethral resection of the prostate (TURP). Genomic DNA was extracted from peripheral blood using QIAamp blood DNA isolation kits. PCR followed by enzymatic digestion of the PCR products for (XRCC1, XPC) was used for the genotyping of these polymorphisms. Statistical analyses were performed using SPSS statistics version 20. The genotype frequencies of the studied polymorphisms in all the samples (n = 100), PC patients (n = 50) and healthy controls (n = 50) were consistent with the Hardy-Weinberg equilibrium distribution (p-value > 0.05). In the present study, we investigated the biochemical alterations and gene expression of carbohydrate and lipid metabolism after oral administration of vitamin A and E for 2 months in diet-induced obese Wistar rats. Vitamin A and E administration reduced significantly the increase in body weight and food intake and normalized the alterations in lipid profiles in obese rats compared to normal rats. Moreover, both vitamins decreased the fat accumulation in liver tissues of obese rats. Finally, they up-regulated mRNA expression of Pyruvate Kinase (PK) and Glucose Transporter-2 (GLUT-2), and increased lipolysis and cholesterol metabolism through up-regulation of lipoprotein lipase (LPL), Sterol Responsible Element Binding Protein-1a (STREBP-1a) and STREBP-1c mRNA expression. In conclusion, vitamin A and E regulate gene expression of carbohydrate and lipid metabolism, and also ameliorate changes associated with obesity induced by high fat diet in Wistar rats.

BACKGROUND Pancreatic cystic lesions(PCLs) are common in clinical practice. The accurate classification and diagnosis of these lesions are crucial to avoid unnecessary treatment of benign lesions and missed opportunities for early treatment of potentially malignant *** To evaluate the role of cyst fluid analysis of different tumor markers such as cancer antigens [e.g., cancer antigen(CA)19-9, CA72-4], carcinoembryonic antigen(CEA), serine protease inhibitor Kazal-type 1(SPINK1), interleukin 1 beta(IL1-β), vascular endothelial growth factor A(VEGF-A), and prostaglandin E2(PGE2)], amylase, and mucin stain in diagnosing pancreatic cysts and differentiating malignant from benign *** This study included 76 patients diagnosed with PCLs using different imaging modalities. All patients underwent endoscopic ultrasound(EUS) and EUS-fine needle aspiration(EUS-FNA) for characterization and sampling of different *** The mean age of studied patients was 47.4 ± 11.4 years, with a slight female predominance(59.2%). Mucin stain showed high statistical significance in predicting malignancy with a sensitivity of 87.1% and specificity of 95.56%. It also showed a positive predictive value and negative predictive value of 93.1% and 91.49%, respectively(P < 0.001). We found that positive mucin stain, cyst fluid glucose, SPINK1, amylase, and CEA levels had high statistical significance(P < 0.0001). In contrast, IL-1β, CA 72-4, VEGF-A, VEGFR2, and PGE2 did not show any statistical significance. Univariate regression analysis for prediction of malignancy in PCLs showed a statistically significant positive correlation with mural nodules, lymph nodes, cyst diameter, mucin stain, and cyst fluid CEA.
