Presepsin teardown- pitfalls of biomarkers in the diagnosis and prognosis of bacterial infection in cirrhosis

AIM To evaluate the diagnostic and prognostic value of presepsin in cirrhosis-associated bacterial infections. METHODS Two hundred and sixteen patients with cirrhosis were enrolled. At admission, the presence of bacterial infections and level of plasma presepsin, serum C-reactive protein(CRP) and procalcitonin(PCT) were evaluated. Patients were followed for three months to assess the possible association between presepsin level and short-term *** Present 34.7 of patients had bacterial infection. Presepsin levels were significantly higher in patients with infection than without(median, 1002 pg/m L vs 477 pg/m L, P 1206 pg/m L sensitivity, specificity, positive predictive values and negative predictive values were as follows: 87.5%, 74.5%, 61.8% and 92.7%. The accuracy of presepsin, however, decreased in advanced stage of the disease or in the presence of renal failure, most probably because of the significantly elevated presepsin levels in non-infected patients. 28-d mortality rate was higher among patients with > 1277 pg/m L compared to those with ≤ 1277 pg/m L(46.9% vs 11.6%, P justing with MELD score and leukocyte *** Presepsin is a valuable new biomarker for defining severe infections in cirrhosis, proving same efficacy as PCT. However, it is not a useful marker of short-term mortality.

Smoking is an important environmental factor in inflammatory bowel disease （IBD）, having different effects in ulcerative colitis （UC） and Crohn＇s disease （CD）. A recent meta-analysis partially confirmed previous findings that smoking was found to be protective against ulcerative colitis and, after onset of the disease, might improve its course, decreasing the need for colectomy. However, smoking increases the risk of developing Crohn＇s disease and worsens its course, increasing the need for steroids, immunosuppressants and re-operations. Smoking cessation aggravates ulcerative colitis and improves Crohn＇s disease. Data are however, largely conflictive as well as the potential mechanisms involved in this dual relationship are still unknown. In this review article, the authors review the role of smoking in inflammatory bowel diseases.

Pulmonary embolization as primary manifestation of hepatocellular carcinoma with intracardiac penetration: A case report

Intracardiac manifestation of hepatocellular carcinoma (HCC) is a rare condition and an uncommon finding even at autopsy. Pulmonary tumor embolism as a presenting feature of HCC has been published only twice previously. In our case report, a 63-year-old man presented with high fever and six episodes of recurrent pneumonias during the last half year. Echocardiography was performed, a solid mass was found in the right atrium. Transesophageal echocardiography proved a tumor mass in the inferior vena cava (IVC) extending into the right atrium, abdominal ultrasound revealed tumor mass in the IVC and a solid tumor in the liver. Combined liver and heart surgery was attempted in order to remove the tumor mass from both the liver and the right atrium. Acute cor pulmonale occurred during tumor removal from the right atrium and the patient expired. In addition to local factors the possibility of embolization should arise in the background of recurrent pneumonia. Occult carcinoma must be included in possible causes of recurrent pulmonary embolism. Searching for primary malignancy should include HCC as frequent cause of hypercoagulability. In case of HCC, echocardiography is suggested because of the possibility of expansion in IVC or right atrium and tumor-embolization.

Topographic and lithologic controls behind mountain depopulation in Zlatibor District(Western Serbia)

Mountain depopulation is a worldwide phenomenon observed in all continents. It has varied socio-economic reasons;among others, the low profitability of traditional agriculture, the better job possibilities and the higher level of services in urban settlements. However, it is often recognized that depopulation is related to natural factors such as elevation, slope or lithology. It is also observed that protected areas are frequently established in depopulated mountain regions. Their primary aim is the conservation of nature, but they may help tourism development as well. Tourism, in turn, may slow down or even reverse the process of depopulation. In this study, we investigate the impact of topographic and lithologic factors, namely of karst settings, on mountain demographic processes and the relationship of protected areas and tourism through the example of the northern part of Zlatibor District(Western Serbia). The study area is characterized by mountains and hills at elevations from 200 to 1600 m a.s.l. Our aim is to find GIS-based statistical relationships between topographic, lithologic factors and demographic characteristics. In this area, mountain depopulation started after WWII, and weproved that this process was strongly controlled by topographic factors. The higher and more dissected the area, the more significant is the decrease of the population and the more advanced is the ageing. As a result, population density contrasts are much more pronounced now than 70 years ago. After WWII, depopulation and ageing became gradually more serious on karstic terrains than on non-karst. However, by using compound topographic and lithologic types, we proved that it is not the effect of karst, but the effect of topography. The flow of population from hills and mountains to valleys and basins are closely related to the restructuring of the economic sectors. Special nanostructures in Al-Mg alloys subjected to high pressure torsion

Deformation twins and stacking faults were observed in nanostructure A1-Mg alloys subjected to high pressure torsion. These observations are surprising because deformation twinnings have never been observed in their coarse-grained counterparts under normal conditions. Experimental evidences are introduced on non-equilibrium grain boundaries, deformation twinnings and partial dislocation emissions from grain boundaries. Some of these features can be explained by the results reported from molecular-dynamics simulations of pure FCC metals. Special emphasis is laid on the recent observations of high density hexagonal and rhombic shaped nanostructures with an average size of 3 nm in the A1-Mg alloys processed by high pressure torsion. A possible formation process of these nanostructures is proposed based on molecular-dynamics simulations.

Kinetic-compartmental modelling of potassium-containing cellulose feedstock gasification

Biomass is of growing interest as a secondary energy source and can be converted to fuels with higher energy density especially by pyrolysis or gasification. Understanding the mechanism and the kinetics of biomass pyrolysis (thermal decomposition) and gasification (conversion of organic material to gases) could be the key to the design of industrial devices capable of processing vast amounts of biomass feedstock. In our work real product components obtained in pyrolysis were took into consideration as well as char and oil as lumped components, and the kinetic constants for a biomass model compound (cellulose) pyrolysis and gasification were identified based on a proposed simplified reaction mechanism within a compartment model structure. A laboratory scale reactor was used for the physical experiments containing consecutive fast pyrolysis and gasification stages using alkali metal (K) containing feedstock, which has a significant effect on the cellulose pyrolysis and gasification. The detailed model was implemented in MATLAB/Simulink environment, and the unknown kinetic parameters were identified based on experimental data. The model was validated based on measurement data, and a good agreement was found. Based on the validated first principle model the optimal parameters were determined as 0.15 mL/min steam flow rate, and 4% K content.

An effective Building Neighborhood Green Index model for measuring urban green space

Urban green space forms an integral part of urban *** urban green space is of substantial importance for urban planning and *** the drawbacks of previous urban green space index models,which established either through a grid method or green distribution,and the difficulty of the validation process of earlier urban green space index models,this study exploits the advantages of multisource high-resolution remote sensing data to establish a Building Neighborhood Green Index(BNGI)*** model which analyzes the spatial configuration of built-up areas and the vegetation is based on the building-oriented method and considers four parameters–Green Index(GI),proximity to green,building sparsity,and building *** BNGI with GI in different types of characteristic building regions,it was found that BNGI evaluates urban green space more *** was also found that high-rise low-sparsity area has a lower mean value of BNGI(0.56)as compared with that of low-rise low-sparsity neighborhood(0.62),whereas mean GI(0.24)is equal for both *** characteristics of urban building and green types into consideration,BNGI model can be effectively used in many fields such as land suitability analysis and urban planning.

A Nearly Analytical Discrete Method for Wave-Field Simulations in 2D Porous Media

The nearly analytic discrete method(NADM)is a perturbation method originally proposed by Yang et al.(2003)[26]for acoustic and elastic waves in elastic *** method is based on a truncated Taylor series expansion and interpolation approximations and it can suppress effectively numerical dispersions caused by the discretizating the wave equations when too-coarse grids are *** the present work,we apply the NADM to simulating acoustic and elastic wave propagations in 2D porous *** method enables wave propagation to be simulated in 2D porous isotropic and anisotropic *** experiments show that the error of the NADM for the porous case is less than those of the conventional finite-difference method(FDM)and the so-called Lax-Wendroff correction(LWC)*** three-component seismic wave fields in the 2D porous isotropic medium are simulated and compared with those obtained by using the LWC method and exact *** characteristics of wave propagating in porous anisotropic media,computed by the NADM,are also reported in this *** numerical results illustrate that the NADM provides a useful tool for large-scale porous problems and it can suppress effectively numerical dispersions.

Inland-coastal demographic transformations in a karst area:a case study of the surroundings of Krka National Park(Croatia)

According to geographic possibilism,environment does not determine socio-economic ***,the environment sets certain constraints and limitations for *** do natural settings influence population dynamics?In this study,we examine this question using the example of?ibenik-Knin County,which includes Krka National Park(NP)with a GIS-based statistical *** following parameters represent natural factors:elevation,slope,distance from water,and proportion of karst *** addition,distance from Krka NP was also added as an independent seminatural factor(despite the fact that it is a social construction).The dependent social parameters are population density for selected historical dates,population change,year of maximum population,natural increase,migration,and age *** analysis units are the 199 settlements found in the county,and the 20 local administrative units(LAU2).The study period can be divided into five phases:from the first census in 1857 to 1921,there was a slow increase in most settlements;then a relatively constant phase up to 1961(some settlements began shrinking in this period);a slow decrease until 1991;arapid decrease during the next decade;and relative stabilisation to present *** processes were also accompanied by significant inner and outer *** for the relationships between natural and demographic factors,we found that although the coastal-inland area dichotomy is dominant in the study area,elevation has the highest correlation with most population ***,distance from water is also significantly correlated with population change and migration in certain *** the inland areas,there are statistically significant differences between the demographic processes of fully-karstic and partially-karstic areas,while in the coastal zone,the karst effect is overshadowed by other social *** impact of Krka NP on the demographic characteristics of the surrounding settlements was statisti
