Anti-microbial antibodies in celiac disease:Trick or treat?

AIM： To determine the prevalence of a new set ot anti-glycan and anti-outer membrane protein （anti- OMP） antibodies in a Hungarian cohort of adult Celiac disease （CD） patients. METHODS： 190 consecutive CD patients [M/F： 71/119, age：39.9 （SD：14.1） years], 100 healthy, and 48 gastrointestinal controls were tested for glycan anti-Saccharomyces cerevisiae （gASCA）, anti-laminaribioside （ALCA）, anti-chitobioside, anti-mannobioside, anti-OMP antibodies and major NOD2/CARD15 mutations. Thirty out of 82 CD patients enrolled at the time of diagnosis were re-evaluated for the same antibodies after longstanding gluten-free diet （GFD）. RESULTS： 65.9% of the CD patients were positive for at least one of the tested antibodies at the time of the diagnosis. Except anti-OMP and ALCA, antimicrobial antibodies were exclusively seen in untreated CD; however, the overall sensitivity was low. Any glycan positivity （LR＋： 3.13; 95% CI： 2.08-4.73） was associated with an increased likelihood ratio for diagnosing CD. Significant correlation was found between the levels of anti-glycan and anti-endomysial or anti-transglutaminase antibodies. Anti-glycan positivity was lost after longstanding GFD. Anti-glycan antibody titers were associated with symptoms at presentation, but not the presence of NOD2/CARD15 mutations. Patients with severe malabsorption more frequently had multiple antibodies at diagnosis （P = 0.019）. CONCLUSION： The presence of anti-glycan antibodies in CD seems to be secondary to the impaired small bowel mucosa which can lead to increased antigen presentation. Furthermore, anti-glycan positivity may be considered an additional marker of CD and dietary adherence.

Fractures around the Pelvis, Pelvic Rim &Acetabulum in the Elderly: Report on 54 Cases

The authors report on 34 patients with a pelvic rim disruption who were treated with percutaneous 7.3 mm screws. This technique is less invasive and gives immediate comfort. Screws were placed with conventional fluoroscopic assistance. In four cases, the screw tip was protruding out of the pelvic bone, two malposition screws were in the back, and two were in the front of the pelvic ring. A revision was needed in one case. We also report on 20 elderly patients with a fracture dislocation of the hip that ran into the pelvis. Eight of them were anteromedial dislocations, and the rest were central dislocations of the femoral head. In the group of anteromedial fracture dislocations, the results were excellent, with an open reduction and internal fixation (ORIF) achieved through a retroperitoneal approach. In the subgroup of central fracture dislocations, we performed the same type of osteosynthesis via the retroperitoneal approach. However, we obtained an excellent result for only two cases. We have a couple of results. First, a percutaneous screw fixation for pelvic fractures with fluoroscopic guidance gives excellent results in most cases. Second, central fracture dislocations of the hip perform poorly after osteosynthesis via the retroperitoneal approach. These fractures probably should be offered acute or delayed total hip arthroplasty (THA).

＜正＞ The peptide hormone somatostatin has been shown to inhibit the release of inflammatory neuropeptides from the nerve endings of capsaicin sensitive neurons and thus prevent inflammation. Previous studies indicated that somatostatin exerts its anti-inflammatory effects through the SSTR4 and possibly the SSTR1 receptor. To facilitate large scale screening for pharmacologically active compounds acting on these targets, two new cell lines were created which express the human SSTR1 and SSTR4 receptors. CHO-K1 cells were transfected by alentiviral system containing the human SSTR1 and SSTR4 cDNAs. In the constructs the SSTR cDNA is followedby an internal ribosome entry site allowing separate expression of the enhanced green fluorescent protein from the same mRNA. Stable and uniform expression of the SSTR receptors were demonstrated by RT-PCR. flow cytometry and immunohistochemistry. Binding of somatostatin-14. somatostatin analogue peptid TT-232. the selective SSTR4 ligand KD-5621 and the selective SSTR1 ligand KD-7825 were demonstrated by a radioactive binding assay.

The case of a forty-five year old woman is presented who consulted for spontaneous haematomas of the thighs. The diagnosis of acquired haemophilia A associated to renal cancer was retained. She received anti CD20 monoclonal antibody treatment allowing her to undergo partial nephrectomy 4 months later without major complication. One year after surgery there is no sign of tumour recurrence.

A method for retrieving the waveform of the pressure pulsations from the output of an electronic oscillometer

In the most common version of an oscillometric blood pressure monitor, the output from the pressure transducer, Y(t), is split into two parts, and used for separate determinations of the pressure inside the pneumatic cuff and its fluc-tuating part;the latter is derived by sending Y(t) to a high-pass filter (HPF) and amplifying the fil-tered part to obtain the oscillometric signal O(t). Using a typical HPF-amplifier combination, we show that if p(t), the pulsatile part of the cuff pressure, is defined to be a train of positive-going pulses, O(t) turns out to be rather close but not identical to dp/dt, and to demonstrate that one can easily retrieve p(t) from a record of O(t). This means that, with a small modification, the instrument can provide both p(t) and dp/dt;the practical advantages of this demonstration are pointed out.

波场分离中的一个关键问题是找到一个能将信号与相干噪声相互分离的域。一种名为偏移滤波的新的波场分离算法,能根据信息波至的传播路径和实际时差特征将之分离。这是用正演模拟算子计算信号与相干噪声波至来完成的。采用线性最小二乘反演算法对上述两量作出模型估计,则预测的信号通过对信号模型估计量的正演模拟得到。合成数据和野外资料实例表明:偏移滤波法能改善P波反射与面波、P波反射与管波、P波衍射与S波衍射的分离效果。与常规滤波法相比,偏移滤波法的主要优点是波场分离能力更强,在一般的反演法框架下混合运用任意两类常规变换法进行波场分离的能力也更强,并能通过调控,减少信号与相干噪声间的串音现象。这种方法的局限在于:与常规变换相比,计算量至少增加一个数量级,且对某些波型难以选择合适的模拟算子。
