目的采用肾包膜下种植方案建立人前列腺癌异种移植动物模型.方法 15只SCID雄鼠按随机表分组法随机分为3组,每组5只.应用组织重组技术,将人前列腺癌新鲜组织和新出生BALB/c雄鼠精囊间质（SVm）在体外重组,显微外科条件下种植于SCID鼠肾包膜下,4周后观察肿瘤种植率并称重、计算体积.检测血清前列腺特异性抗原（PSA）值.用细胞角蛋白（CK）8/18、波形蛋白（vimentin）单克隆抗体特异标记人前列腺上皮和基质成纤维细胞,用P63单克隆抗体标记上皮基底膜证实其是否为前列腺癌组织.另将人前列腺癌新鲜组织单独种植于SCID鼠肾包膜下作为对照.结果 15只SCID鼠78只次组织种植中无一只死亡.4周后,重组种植组成瘤率为100%（39/39）,单独种植组为94.1%（37/39）,差异无统计学意义（P＞0.05）.新生瘤体为实体状、不规则、黄/白色,隆出肾脏表面和（或）嵌入肾实质.重组种植组瘤体生长旺盛,单独种植组瘤体较为平坦,肿瘤大小及重量在两组间差异有统计学意义[（9.7±3.1）mm^3比（6.8±2.0）mm^3;（12.1±3.6）μg比（8.2±2.2）μg,均P＜0.01].重组种植组血清PSA值高于单独种植组,但两组间差异无统计学意义（P＞0.05）.CK8/18、vimentin单抗标记证实新生瘤体为人源性前列腺组织,P63单抗标记显示上皮基底膜消失证实为前列腺癌.结论肾包膜下种植可高效率建立人前列腺癌异种移植动物模型.本模型含有肿瘤基质成分,可为体内研究前列腺癌发病和病程演变中间质-上皮细胞相互作用提供技术平台.

OBJECTIVES Elderly patients show a higher incidence of ischemic and bleeding events after percutaneous transluminal coron-ary intervention(PCI).We sought to investigate outcomes in elderly patients treated with antithrombotic strategy guided by bleeding and ischemic risks after revascularization with last generation everolimus-eluting stent(EES).mEThODS Prospective multicenter registry including patients over 75 years revascularized with EES and antithrombotic ther-apy guided by clinical presentation,PCI complexity and PRECISE DAPT ***-primary safety endpoints were:(1)composite of cardiac death,myocardial infarction and stent thrombosis and;(2)bleeding(BARC 2-5).Primary efficacy endpoint was target lesion revascularization.A matched group of patients revascularized with current drug-eluting stents and no such tailored antith-rombotic therapy was used as *** Finally,1064 patients were included in SIERRA-75 cohort,80.8±4.2 years,36.6%women,71%acute coronary syn-dromes(ACS)and 53.6%complex ***-primary safety endpoint of major adverse cardiovascular events was met in 6.2%,co-primary safety endpoint of bleeding in 7.8%and primary efficacy endpoint of TKLR in 1.5%.The multivariable adjusted model showed no significant association of the prescribed short/long dual antiplatelet therapy(DAPT)durations with any endpoint suggesting a well tailored *** stent thrombosis reported in the subgroup with 1-3 months DAPT *** compared to control group,bleeding BARC 2-5 was significantly lower in SIERRA-75 group(7.4%vs.10.2%,P=0.04)as well as the net safety-efficacy endpoint(14.3%vs.18.5%,P=0.02).CONCLUSIONS In elderly population,the application of this risks-adjusted antithrombotic protocol after revascularization with last generation EES seems to be associated with an improved prognosis in terms of ischemic and bleeding outcomes.

Background and Aims: To better understand nonalcoholic steatohepatitis (NASh) disease progression and to evaluate drug targets and compound activity, we undertook the devel-opment of an in vitro 3D model to mimic liver architecture and the NASh environment. methods:We have developed an in vitro preclinical 3D NASh model by coculturing primary hu-man hepatocytes, human stellate cells, liver endothelial cells and Kupffer cells embedded in a hydrogel of rat collagen on a 96-well plate. A NASh-like environment was induced by ad-dition of medium containing free fatty acids and tumor ne-crosis factor-α. This model was then characterized by biochemical, imaging and transcriptomics analyses. Results:We succeeded in defining suitable culture conditions to main-tain the 3D coculture for up to 10 days in vitro, with the lowest level of steatosis and reproducible low level of inflammation and fibrosis. NASh disease was induced with a custom me-dium mimicking NASh features. The cell model exhibited the key NASh disease phenotypes of hepatocyte injury, steatosis, inflammation, and fibrosis. hepatocyte injury was highlighted by a decrease of CYP3A4 expression and activity, without loss of viability up to day 10. moreover, the model was able to stimulate a stable inflammatory and early fibrotic environ-ment, with expression and secretion of several cytokines. A global gene expression analysis confirmed the NASh induc-tion. Conclusions:This is a new in vitro model of NASh dis-ease consisting of four human primary cell-types that exhibits most features of the disease. The 10-day cell viability and cost effectiveness of the model make it suitable for medium throughput drug screening and provide attractive avenues to better understand disease physiology and to identify and characterize new drug targets.

Introduction: high postprandial triglyceride levels and obesity are associated with coronaropathy, and influence the pathogenesis and progression of atherosclerosis. Objective: To evaluate the effect of immediate acute physical exercise on postprandial hyperlipaemia in individuals with central obesity. methods: Study included sedentary, normolipidemic individuals aged 18 - 30 years, with waist circumference >80 in for women and >94 for men. The physical exercise was performed for 45 minutes with 55% - 60% of the heart rate reserve. Triglyceride level tests were performed after fasting, and at time intervals of 1h and 3 h after lipid overload, a week before and immediately after a session of physical exercise. Results: Thirty-six individuals were evaluated (mean age 23.74 ± 4.05 years, 88.2% women, BmI = 28.4 ± 3.4). Fasting and post-prandial triglyceride levels were similar before and after the exercise (p = 0.87). A smaller rise in triglyceride levels was observed in the 1st hour in obese individuals (5%) compared with over-weight individuals (21%) p = 0.001. Conclusion: In individuals with central obesity, moderate intensity physical exercise performed immediately before lipid overload does not reduce the hyperlipidemic peak.

Patients who have disease of the biliary tract commonly present with acute right upper quadrant pain, nausea or vomiting, mid-epigastric pain, and/or jaundice. Etiologies include inflammation with or without infection, noninflammatory disorders, and benign or malignant neoplasms of the gallbladder or bile ducts. Ultrasound (US) is now accepted as the initial imaging modality of choice for the work-up of suspected biliary tract disease. A retrospective study was carried out at King Abdulaziz University hospital to discuss the protocol of ultrasound scanning in demonstrating incidence and complication of gall-bladder (GB) pathologies. Known cases of GB pathologies (100 patients) were surveyed by ultrasound using spatial digital iU22 Philips Convex probe 3.5 mhz. All patients were evaluated with ultrasonogphy following the international scanning guidelines and protocols. The age of the patients is between (9 - 90) years, 68 Patients (68%) were females and 32 patients (32%) were males. Range of age group of accumulation for gallstone presence was (35 - 50) years in females and above 50 years in males. Incidence of gallbladder pathologies are 59% (female 46% and 13% male). Incidence of gallstone is 37%, and ratio of incidence is between male to female 1:3. Other pathologies of gallbladder were found to be acute cholecystities 12%, chronic cholocystities 5%, sludge 2%, carcinoma of the gall-bladder 1%, gall-bladder polyps 1% and Emphysematous choleycystities 1%. Ultransonography is a single imaging modality sufficient for evaluation of patient with suspected gallbladder pathologies (gallstone) which can provide information about the presence of gallstone and more over about site and cause of biliary tract obstruction. Ultrasound is highly sensitive and specific means for diagnosis of the gallbladder disorders.

Background: The chemical composition of gallstones is essential to study aetiopathogenesis of gallstone disease. Objective: To determine the composition of gallstones in a Sudanese population. Patients and methods: It describes an extensive quantitative analysis of gallstones from patients presented with symptomatic gall stone disease and treated by cholecystectomy after the acceptance of the pre-given informed consent in Khartoum teaching hospital in the period between Jan 2010 and Dec 2010. Using a pretested questionnaire data collected from and analyzed statistically by SPSS computer program version 21. Results: Data are analyzed from 94 patients (six males and 88 females). Cholesterol stones showed a significantly higher cholesterol content than pigment stones (p = 0.0042), though not significantly higher than mixed stones. Their phospholipids content and inorganic phosphates were higher than in the other types of stones and oxalate content was significantly elevated in comparison with mixed stones (p = 0.0402). In mixed stones, the cholesterol, bile acids, and bilirubin were intermediate between cholesterol and pigment stones, whereas triglycerides were significantly more than pigment stones (p = 0.0007). Bilirubin (p = 0.0021) and bile acids (p = 0.0016) were significantly higher than cholesterol stones (p = 0.0001) and (p = 0.0001) respectively. however, they contained the lowest amounts of sodium, potassium, magnesium, and oxalate. In pigment stones, bilirubin was significantly higher (p = 0.0001) than both groups. Conclusion: Collaborations between surgeons, nutritionists, biochemists, and physicians should be stimulated in future studies to define the different types of gall stones in different areas in Sudan and the relevancy of such types with diets’ traditions.

A normal hall subgroup N of a group G is a normal subgroup with its order coprime with its index. Schur- Zassenhaus theorem states that every normal hall subgroup has a complement subgroup, that is a set of coset representatives h which also forms a subgroup of G. In this paper, we present a framework to test isomorphism of groups with at least one normal hall subgroup, when groups are given as multiplication tables. To establish the framework, we first observe that a proof of Schur-Zassenhaus theorem is constructive, and formulate a necessary and sufficient condition for testing isomorphism in terms of the associated actions of the semidirect products, and isomorphisms of the normal parts and complement parts. We then focus on the case when the normal subgroup is abelian. Utilizing basic facts of representation theory of finite groups and a technique by Le gall （STACS 2009）, we first get an efficient isomorphism testing algorithm when the complement has bounded number of generators. For the case when the complement subgroup is elementary abelian, which does not necessarily have bounded number of generators, we obtain a polynomial time isomorphism testing algorithm by reducing to generalized code isomorphism problem, which asks whether two linear subspaces are the same up to permutation of coordinates. A solution to the latter can be obtained by a mild extension of the singly exponential （in the number of coordinates） time algorithm for code isomorphism problem developed recently by Babai et al. （SODA 2011）. Enroute to obtaining the above reduction, we study the following computational problem in representation theory of finite groups： given two representations ρandτ- of a group h over Zp^d, a prime, determine if there exists an automorphism ： Фh→ h, such that the induced representation p Ф= ρ o Ф and τ are equivalent, in time poly（｜h｜, p^d）.
