Calibration of local magnitude scale for Hindukush continental subduction zone

Hindukush is an active subduction zone where at least one earthquake occurs on daily *** seismic hazard studies,it is important to develop a local magnitude scale using the data of local seismic *** have computed local magnitude scale for Hindukush earthquakes using data from local network belonging to Center for Earthquake Studies(CES)for a period of three years,i.e.2015–2017.A total of 26,365 seismic records pertaining to 2,683 earthquakes with magnitude 2.0 and greater,was used with hypocentral distance less than 600 *** scale developed by using this data comes to be M_(L)=logA+0.929logr+*** magnitude determined through formulated relation was compared with that of standard relation for Southern California and relation developed by the same authors for local network for Northern *** was observed that Hindukush region has high attenuation as compared to that of Southern California and Northern Punjab which implies that Hindukush is tectonically more disturbed as compared to the said regions,hence,seismically more active as *** have calculated station correction factors for our *** correction factors do not show any pattern which probably owes to the geological and tectonic complexity of this *** deviation and variance of magnitude residuals for CES network determined using Hutton and Boore scale and scale developed in this study were compared,it showed that a variance reduction of 44.1%was *** of magnitude residuals for different distance ranges was almost zero which showed that our magnitude scale was stable for all distances up to 600 *** developed magnitude scale will help in homogenization of earthquake *** has been observed that b-value of CES catalog decreases when magnitude is calculated by using newly developed magnitude scale.

Anomalous Ionospheric foF2 Variations Observed Prior to the Dalbandin Earthquake in Pakistan

Ionosphereic foF2 variations are very sensitive to the seismic effect and results of ionospheric perturbations associated with earthquakes seem to very hopeful for short-term earthquake prediction. On January 18,2011 at 20: 23 UT a great earthquake( M = 7. 2)occurred in Dalbandin( 28. 73° N,63. 92° E),Pakistan. In this study,we have tried to find out the features of pre-earthquake ionospheric anomalies by using the hourly day time( 08. 00 a. m.- 05. 00 p. m.) data of critical frequency( foF2) obtained by three vertical sounding stations installed in Islamabad( 33. 78°N,73. 06°E),Multan( 32. 26°N,71. 51°E) and Karachi( 24. 89° N,67. 02° E), Pakistan. The results show the significant anomalies of foF2 in the earthquake preparation zone several days prior to the Dalbandin earthquake. It is also observed that the amplitude and frequency of foF2 anomalies are more prominent at the nearest station to the epicenter as compared to those stations near the outer margin of the earthquake preparation zone. The confidence level for ionospheric anomalies regarding the seismic signatures can be enhanced by adding the analysis of some other ionospheic parameters along with critical frequency of the layer F2.

On October 7,2021,a magnitude 5.9 earthquake struck the Harnai(Baluchistan)region of Pakistan,causing several fatalities and injuries within the epicentral ***-order tectonic deformation in this region is caused by the convergence of the Indian Plate with respect to the Eurasian *** Katwaz Block hinders the motion of the Indian Plate,resulting in the formation of strike-slip *** this study,the P-wave first-motion polarity technique was used to determine the mainshock faulting *** scanning of the polarity solutions was applied to determine the most suitable focal mechanism solution among the available solutions generated by the FOCMEC(focal mechanism)*** nodal planes correspond to different faulting styles(i.e.,thrust and strike-slip faulting).A nodal plane oriented in the NW-SE direction corresponded to a strike-slip mechanism,which was considered to be the fault ***,this earthquake was associated with the Harnai-Karahi strike-slip fault zone owing to the fault strike and direction of *** apparent stress drop,fault length,and moment magnitude of the Harnai earthquake were 35.4 bar,6.1 km,and 5.9,respectively.A lower b-value for the Gutenberg-Richter law was observed prior to the ***α-than b-values(α>b)indicate that this earthquake was governed by large events as opposed to small-magnitude *** Harnai sequence had a decay exponent close to unity,lasted for 145 days,and produced few *** study will help the future hazard mitigation in the region.

Disease monitoring strategies in inflammatory bowel diseases: What do we mean by “tight control”?

In recent years,there has been a critical change in treatment paradigms in inflammatory bowel diseases(IBD)triggered by the arrival of new effective treatments aiming to prevent disease progression,bowel damage and *** insufficiency of symptomatic disease control and the well-known discordance between symptoms and objective measures of disease activity lead to the need of reviewing conventional treatment algorithms and developing new concepts of optimal therapeutic *** treat-to-target strategies,defined by the selecting therapeutic targets in inflammatory bowel disease consensus recommendation,move away from only symptomatic disease control and support targeting composite therapeutic endpoints(clinical and endoscopical remission)and timely *** data suggest that early therapy using a treat-to-target approach and an algorithmic therapy escalation using regular disease monitoring by clinical and biochemical markers(fecal calprotectin and C-reactive protein)leads to improved *** review aims to present the emerging strategies and supporting evidence in the current therapeutic paradigm of IBD including the concepts of“early intervention”,“treat-to-target”and“tight control”*** also discuss the real-word experience and applicability of these new strategies and give an overview on the future perspectives and areas in need of further research and potential improvement regarding treatment targets and(“tight”)disease monitoring strategies.

Management of inflammatory bowel disease with Clostridium difficile infection

To address the management of Clostridium difficile (C. difficile) infection (CDI) in the setting of suspected inflammatory bowel disease (IBD)-flare. METHODSA systematic search of the Ovid MEDLINE and EMBASE databases by independent reviewers identified 70 articles including a total of 932141 IBD patients or IBD-related hospitalizations. RESULTSIn those with IBD, CDI is associated with increased morbidity, including subsequent escalation in IBD medical therapy, urgent colectomy and increased hospitalization, as well as excess mortality. Vancomycin-containing regimens are effective first-line therapies for CDI in IBD inpatients. No prospective data exists with regards to the safety or efficacy of initiating or maintaining corticosteroid, immunomodulator, or biologic therapy to treat IBD in the setting of CDI. Corticosteroid use is a risk factor for the development of CDI, while immunomodulators and biologics are not. CONCLUSIONStrong recommendations regarding when to initiate IBD specific therapy in those with CDI are precluded by a lack of evidence. However, based on expert opinion and observational data, initiation or resumption of immunosuppressive therapy after 48-72 h of targeted antibiotic treatment for CDI may be considered.

Wound dehiscence with continuous versus interrupted mass closure of transverse incisions in children with absorbable suture:a randomized controlled trial

Introduction Numerous meta-analyses done on adults suggest superiority of continuous mass closure technique,but any such study does not exist for the pediatric age *** results in adults cannot be applied to pediatrics because of numerous physiologic and anatomic *** This is a single-blinded,randomized controlled trial,1:1 parallel groups,that compares the frequency of dehiscence between the interrupted and continuous mass closure techniques for transverse incisions in pediatric *** age range was from birth to 12 *** sampled 350 patients undergoing emergency or elective exploratory laparotomies in our pediatric surgery *** randomization was used and only the patients remained blinded during the *** group was closed with interrupted mass closure(group A)and the other group with continuous mass closure technique(group B).We had to drop 50 patients for not meeting the inclusion *** The wound dehiscence rate for group A was 1.34%(4 patients)and for group B was 3.0%(9 patients).Significance was calculated usingχ^(2)(p<0.156).The global wound dehiscence rate was 4.34%(13 patients).The maximum number of patients dehisced on the fifth postoperative day,while the range was 4-11 *** only statistically significant confounding factor was wound classification(p<0.002).Discussion Statistically there is no significant difference between interrupted and continuous mass closures techniques in terms of wound *** dirty wounds are at a maximum risk of developing wound dehiscence irrespective of the technique *** need to strictly adhere to the basic principles of closure especially when dealing with dirty *** registration number TCTR20150318001.

Effect of invasion by Hyptis suaveolens on plant diversity and selected soil properties of a constructed tropical grassland

Quality of care in inflammatory bowel diseases: What is the best way to better outcomes?

Inflammatory bowel disease(IBD) is a lifelong, progres-sive disease that has disabling impacts on patient's lives. Given the complex nature of the diagnosis of IBD and its management there is consequently a large economic burden seen across all health care systems. Quality in-dicators(QI) have been created to assess the different fa?ades of disease management including structure, process and outcome components. Their development serves to provide a means to target and measure quality of care(Qo C). Multiple different QI sets have been published in IBD, but all serve the same purpose of trying to achieve a standard of care that can be attained on a national and international level, since there is still a major variation in clinical practice. There have been many recent innovative developments that aim to improve Qo C in IBD including telemedicine, home biomarker assessment and rapid access clinics. These are some of the novel advancements that have been shown to have great potential at improving Qo C, while offloading some of the burden that IBD can have vis-a-vis emergency room visits and hospital admissions. The aim of the current review is to summarize and discuss available QI sets and recent developments in IBD care including telemedicine, and to give insight into how the utilization of these tools could benefit the Qo C of IBD patients. Additionally, a treating-to-target structure as well as evidence surrounding aggressive management directed at tighter disease control will be presented.

Positioning of old and new biologicals and small molecules in the treatment of inflammatory bowel diseases

The past decade has brought substantial advances in the management of inflammatory bowel diseases(IBD). The introduction of tumor necrosis factor(TNF) antagonists, evidence for the value of combination therapy, the recog-nition of targeting lymphocyte trafficking and activation as a viable treatment, and the need for early treatment of high-risk patients are all fundamental concepts for current modern IBD treatment algorithms. In this article, authors review the existing data on approved biologicals and small molecules as well as provide insight on the current positioning of approved therapies. Patient stratification for the selection of specific therapies, therapeutic targets and patient monitoring will be discussed as well. The thera-peutic armamentarium for IBD is expanding as novel and more targeted therapies become available. In the absence of comparative trials, positioning these agents is becoming difficult. Emerging concepts for the future will include an emphasis on the development of algorithms which will facilitate a greater understanding of the positioning of novel biological drugs and small molecules in order to best tailor therapy to the patient. In the interim, anti-TNF therapy remains an important component of IBD therapy with the most real-life evidence and should be considered as first-line therapy in patients with complicated Crohn's disease and in acute-severe ulcerative colitis. The safety and efficacy of these ‘older' anti-TNF therapies can be optimized by adhering to therapeutic algorithms which combine clinical and objective markers of disease severityand response to therapy.
