Toward an operating speed profile model for rural two-lane roads in Egypt

The geometric design, especially the horizontal and vertical alignments, of rural two-lane highway facilities is considered one of the most important factors affecting the quality of traffic service and safety. A consistent highway geometric design is defined to be one that conforms to the driver＇s expectations. In order to calculate the main measures of design consistency, an accurate operating speed profile model for road alignment is needed. Studies have shown that operating speed models are country dependent due to varying demographics, driver attitudes, habits, etc. This paper develops a speed profile model for two-lane rural roads in Egypt. This includes the development of operating speed models for horizontal curves and tangents, as well as the study of the characteristics and relationships between acceleration and deceleration rates before and after horizontal curves. The study uses a desert rural two-lane, two-way road sections that connect the city of Sohag with the city of Hurghada, in Upper Egypt. All geometric characteristics were obtained from high- way authority. Speed data regarding individual drivers traveling on selected two-lane rural road sections were sampled using an on-board GPS system. The study confirms the finding in previous research that curvature is the most important factor in determining the speed on horizontal curves. Moreover, tangent length is the most important factor in determining operating speeds at tangents. The acceleration and deceleration characteristics were derived to gain an understanding of the behavior of individual vehicles traveling through curves of varying radii and lengths as well as preceding tangent length. Several operating speed models were developed for tangents and curves as well as for acceleration and deceleration rates. Incredibly effective, these models can be used for design consistency evaluations.

Effect of speed hump characteristics on pavement condition

Speed humps are the most common type of traffic calming devices due to their low cost and easy installation. However, in many Egyptian roads, considerable number of these humps is randomly placed without proper engineering studies and justifications. Deteri- oration of pavement condition is observed near these humps. This paper presents a case study applied to collect and analyze visual inspection data for the reason of evaluating the impact of speed humps on pavement condition on intercity rural roads. The paper used 52 speed humps located in an intercity two-lane, two-way road that connects two cities, Tahta and Gerga, in Upper Egypt. The total length of this road is about 34 km. Pavement condition index （PCI）, in road sections, near speed humps in the two directions of travel were calculated from the visual inspection measurements. The characteristics of each speed hump （width, height, and distance from preceding hump） were measured. Using statistical analyses, the correlations between the pavement conditions and hump char- acteristics were examined. Regression analysis models were developed to represent the relationships between pavement conditions and hump characteristics. Generally, the re- sults proved that the pavement conditions are greatly influenced by the presence of speed humps and hump characteristics.

通行能力预测是公路设施规划、设计与运营中的基础环节.本文基于埃及曼努菲亚省12条农村双车道路段数据,研究了公路线形等几何特征对通行能力的影响以及线形由切线变为平曲线时通行能力的损失.所选取的路段线形包括直线和随后的平曲线.同时,采集了各路段上车流量及速度数据,利用基于基本图的外推法研究流量和密度关系.路段上不同车型的车辆均转换为小客车当量.此外,针对不同情形(切线、平曲线及对应流量损失),分别建立了最佳回归模型.结果表明,切线型道路的关键自变量是路宽、肩宽以及切线长;平曲线道路的关键变量是曲线半径和路宽.通行能力和几何特征关系最佳模型的自变量为曲线半径.本文的模型可用于分析评价农村双车道道路的通行能力,尤其是评价文中所研究路段的通行能力.

Cartilage tympanoplasty in the treatment of adhesive otitis media with and without Eustachian tube balloon dilatation

Purpose:To compare cartilage tympanoplasty(CT)combined with eustachian tube balloon dilatation(ETBD)and cartilage tympanoplasty alone as a surgical treatment modality for adhesive otitis media(AdOM)in terms of graft healing,audiological outcomes,and impact on life style,using Chronic Otitis Media Outcome Test 15(COMOT-15).Methods:50 patients with AdOM were randomly classified into 2 groups:25 patients for cartilage tympanoplasty only(CT group)and 25 patients for cartilage tympanoplasty combined with eustachian tube balloon dilatation(CT t ETBD group).Clinical outcomes in both groups were compared at 3 and 6 months of follow ***:There was no significant difference in graft healing between the two *** COMOT-15 scores significantly decreased in both groups with a significant difference between the groups with regard to the decrease in COMOT-15 scores at 3 and 6 months of follow-up(P<0.05).Hearing improvement was achieved,as the mean preoperative ABG was 26.5±5.4 and 27.1±4.6 dB,and the mean postoperative ABG at 6 months was 19.4±4.4 and 14.6±3.9 dB in the CT and the CT t ETBD groups,*** difference in the magnitude of ABG reduction in the two groups was significant at 3 and 6 months of postoperative follow-up(P<0.05)in favour of the CT t ETBD ***:ETBD can increase the success rate of cartilage tympanoplasty in patients with AdOM by enhancing the audiological outcome and quality of life.

Detection of HER-2/neu Amplification on Fine Needle Aspirates of Breast Cancer Using Fluorescence in Situ Hybridization

The accuracy of diagnostic assays for HER-2/neu in breast cancer is extremely important as HER-2/neu status is essential in tailoring adjuvant and/or neoadjuvant treatment in every patient. FNAC is widely practiced in Egypt in preoperative diagnosis of breast cancer for its low cost and high diagnostic accuracy. Since the determination of HER-2/neu protein expression on cytological preparations was previously found to be unreliable for clinical use, we opted for the assessment of HER-2/neu status in fine needle aspirates using FISH. The main objective of this study was to evaluate the reliability of HER-2/neu status assessment by FISH on fine needle aspirates of breast cancers by comparing the results with IHC and FISH on FFPE tissue sections obtained from corresponding surgically resected specimens. Fine needle aspirates from 40 breast cancer patients with pathologically confirmed breast cancer were included in the study. They were submitted for HER-2/neu evaluation by FISH. After surgery, the corresponding FFPE sections were evaluated for HER-2/neu by FISH and by IHC. FNAs from 11 cases proved to be amplified by FISH, while 29 cases were not amplified. Apart from two cases that showed lack of signals, all specimens evaluated by FISH on the corresponding FFPE sections showed matched results. The Measurement of Agreement between FISH on FNAs and FISH on FFPE sections was 86.7%, while that between FISH on FNAs and IHC was 72.5%. The high concordance rate in the present study between FISH evaluation of HER-2/neu gene amplification on FNAC samples and their corresponding FFPE samples indicate that FISH may be a reliable technique for HER-2/neu assessment on FNAs. Furthermore, FISH on FNAs gave us better hybridization signals than their corresponding FFPE tissue sections. Finally, we also conclude that all score (2+) cases by IHC should be reevaluated by FISH which is crucial for the patient management.

HER-2/Neu Status in Gastric Carcinomas in a Series of Egyptian Patients and Its Relation to Ki-67 Expression

Objective: HER-2/neu status in gastric carcinomas (GCs) has not been studied before in the Egyptian population. Materials and Methods: Seventy-three GCs were evaluated for the expression of HER-2/neu and Ki-67 using immunohistochemistry (IHC). Fluorescence in situ Hybridization (FISH) was done for HER2/neu IHC score 2+ cases. Results: Out of the 73 gastric carcinomas, 23 (31.5%) were score 0, 17 (23.3%) were score 1+, 23 (31.5%) were score 2+, and 10 (13.7%) were score 3+. FISH analysis revealed that the HER-2/neu gene was amplified in 11 out of the 23 cases (47.8%) scored 2+ by IHC. Therefore, the overall HER-2/neu positivity rate was 28.8% and it was significantly associated with higher T-stage and lymphovascular invasion (LVI). The Ki-67 expression rate ranged between 10% and 100%, with 84.9% of the cases (n = 62) featuring high Ki-67 scores. High Ki-67 score was significantly associated with male sex, tumor grade, and number of positive nodes. HER-2/neu protein expression correlated significantly with Ki-67 score. Twenty tumors showed combined HER-2/neu positivity and high Ki-67 score and were significantly associated with higher T-stage and occurrence of LVI, implying a more aggressive behavior. Conclusions: The rate of HER-2/neu positive GCs in our series simulates universal rates, thereby mandating routine evaluation of HER-2/neu status in all GCs submitted to our laboratory to benefit from trastuzumab therapy. Further studies on a larger number of GCs are required to prove that the concurrent HER-2/neu positivity and high Ki-67 score are markers of worse prognosis.

Structural and Tectonic Evolution of El-Faiyum Depression,North Western Desert,Egypt Based on Analysis of Landsat ETM+,and SRTM Data

False color Landsat ETM＋（bands 7, 4, 2 in RGB） and SRTM images of the EI-Faiyum depression, Egypt, highlights major NE-SW faults and other lineaments trending NW-SE and N-S. Airborne magnetic data reveal some E-W subsurface faults which are not recognized on Landsat ETM＋ and SRTM images. Ratio images （5/7, 3/1, 4/3） and （5/7, 5/1, 4） were used for lithological discrimination of different rock types. The EI-Faiyum area is occupied by sedimentary rocks of Middle Eocene, Upper Eocene, Oligocene, Lower Miocene, Pliocene, Pleistocene and Holocene. Understanding the structural regime of EI-Faiyum depression in the Western Desert of Egypt is significant in relation to the neotectonics of the Nile Delta and to the development of new communities west of the delta. The area is covered by Late Cretaceous, Middle to Late Eocene deposits, Oligocene deposits （comprising fluviatile sediments and basalt sheets） and Pliocene and Quaternary alluvium and sand deposits. New geological and structural maps were prepared at a scale of 1 ： 100 000 using Landsat ETM＋ and SRTM images coupled with extensive field work. The major structural trends are E-W, ENE, NS and NW--the same trends observed elsewhere in North Africa from NW Sudan to NE Egypt including north and central Sinai. EI-Faiyum depression is a structurally-controlled tectonic basin, marked by northeast-striking faults along its northern margin. These faults are parallel to similar faults of the Syrian arc fold belt, which includes a group of related faults, folds, push-up structures, and basins. Qarun Lake is located in the northern part of EI-Faiyum depression, at a right-step between two strands of the NE striking fault system. We have suggested elsewhere that EI-Faiyum depression is a pull-apart basin related to extension between these mismatched strands of strike-slip faults.
