Swallowing Function before and after Subtotal Glossectomy and Reconstruction with a Rectus Abdominis Musculocutaneous Flap: A Case Report

Purpose: Swallowing function was in a patient with tongue cancer that was evaluated with video fluorography before and after subtotal glossectomy and reconstruction with a rectus abdominis muscuocutaneous flap. Materials and Methods: A 41-year-old man underwent subtotal glossectomy and retained the unilateral posterior mylohyoid and unilateral stylohyoid muscles. The structure of the flap was evaluated postoperatively. To assess swallowing function, video fluorography was performed before surgery, 1 month after surgery and 1 year after surgery. Testing involved 1) ability to hold 10 mL of liquid in the oral cavity, 2) epiglottis turnover, 3) the presence of aspiration, 4) hyoid bone movement, and 5) maximum width of the esophageal entrance. Results: The flap was protuberant postoperatively. The patient was able to hold the test diet in the oral cavity before and after surgery. Epiglottis turnover was good before surgery but insufficient after surgery. Aspiration during swallowing was not observed before or after surgery. At rest, the hyoid bone sagged postoperatively, in comparison with preoperatively. Hyoid bone movement and width of the esophageal entrance decreased after surgery;however, they demonstrated gradual recovery. Conclusions: For good postoperative swallowing function after subtotal glossectomy, it is necessary to perform reconstruction with protuberant flap and to retain the suprahyoid muscles as much as possible.

Study of High Power ICRF Antenna Design in LHD

In large helical device （LHD）, antenna loadings are different for minority ion cyclotron heating （MICH with 38.47 MHz） and mode-converted ion Bernstein wave heating （MC-IBW with 28.4 MHz）, and it is necessary to improve antenna loading with low heating efficiency to avoid arching on transmission line. To design a new ion cyclotron range of frequencies （ICRF） antenna in LHD, calculation for a simple antenna model is conducted using three-dimensional electrical magnetic code （high frequency structure simulator, HFSS） for an water loading as an imaginary plasma with low heating efficiency. At resonant frequencies, antenna loading is sensitive to strap width, and resonant frequencies are strongly related to strap height. There is no differences of RF current profile on the strap surface between resonant frequency and non-resonant frequency. The strap should be perpendicularly placed against the magnetic field line, since Faraday-shield angle will lead to a decrease in the effective antenna height.

背景许多整形外科手术后（如脂肪移植），脂肪组织开始缺血，导致组织缺氧和营养缺失。然而，很少有研究检测脂肪组织中的缺血和／或缺氧变化的报道。方法切断小鼠体内为腹股沟脂肪垫供血的血管，建立外科手术诱导的缺血模型。利用氧气监视器测量脂肪组织的氧分压，利用全标本染色、免疫组化、流式细胞术和蛋白质印迹法分析缺血改变。作者还检测了体外低氧环境下的细胞寿命。结果3种程度缺血模型（轻度、中度、重度）的氧分压水平分别为正常脂肪组织（50．5±1．3）mmHg的75％、55％、20％。在第28天观察到脂肪组织萎缩伴随实质纤维变性，这取决于缺血的严重程度。在第1天观察到中度和重度缺血诱导低氧诱导因子1仪和纤维生长因子2表达的提高，中度和重度缺血还可导致脂肪组织中的退行性改变（如细胞凋亡、坏死、巨噬细胞侵噬）。死亡细胞包括脂肪细胞，血管内皮细胞，血源性细胞，但不包括脂肪源性干／祖细胞。退行性改变后可观察到再生性改变，包括血管形成、脂肪形成和细胞的增殖（包括脂肪源性干／祖细胞、血管内皮细胞和血细胞）。笔者还发现，严重缺氧时在体外用实验方法分化的脂肪细胞发生凋亡和／或坏死，而脂肪源性干／祖细胞仍然存活。结论脂肪组织严重缺血／缺氧导致退行性改变，继而发生相应的组织重构。在缺血环境下，脂肪细胞很容易死亡，而脂肪源性干／祖细胞却非常活跃，并有助于组织修复。
