Garenoxacin Prophylaxis for Febrile Neutropenia after Chemotherapy in Hematological Malignancies

Background: Febrile neutropenia is one of the most serious adverse events in patients with hematological malignancies and chemotherapy. The routine use of fluoroquinolone prophylaxis in patients with hematological malignancies is controversial. Therefore, we prospectively evaluated the efficacy and safety of prophylactic use of garenoxacin for febrile neutropenia. Patients and Methods: Consecutive adult patients with hematological malignancies who were at risk for chemotherapy-induced neutropenia lasting more than seven days were eligible for present study. They received oral garenoxacin (400 mg daily) from the neutrophil count decreased to less than 1000/μl and continued until the neutropenia had resolved. The primary endpoint was incidence of febrile neutropenia, and the secondary endpoints were the type and incidence of adverse events. Results: We enrolled 46 consecutive patients (median age, 59 years). The underlying diseases comprised acute myeloid leukemia (n = 17), acute lymphoblastic leukemia (n = 3), malignant lymphoma (n = 23), and multiple myeloma (n = 3). There were 23 febrile neutropenia episodes and 2 episodes of bacteremia. There were no grade 3 or 4 adverse events;however serum creatinine levels were significantly elevated after garenoxacin administration. The overall prophylactic efficacy of garenoxacin was 50%, and there were no infection-related deaths. Conclusions: Prophylactic use of garenoxacin is effective and safe in patients with hematological malignancies. (Clinical trial registration number: UMIN000004979).

Endoscopic Examination for Patients with Diarrhea after Allogeneic Stem Cell Transplantation

Objective: Intestinal graft-versus-host disease (GVHD) represents one of the most serious complications of allogeneic stem cell transplantation (allo-SCT). Endoscopic and histological proof is required due to the number of differential diagnoses manifesting as diarrhea. We investigated the safety of endoscopic biopsies, and the role of conducting biopsies and inspections of the terminalileum. Patients: Thirty two colonoscopic examinations and 29 biopsies were performed for 19 patients after allo-SCT in our institute between October 2011 and May 2015. Results: Endoscopic examinations and biopsies were performed safely under the policy of transfusing platelets for thrombocytopenia (3/μL). For biopsied cases, the diagnostic consistency rate with endoscopic findings was 60%, with a tendency toward negative correlations with early examinations after diarrhea onset (25% for 0 - 1 days;62.5% for later) or low-grade GVHD according to Freiburg criteria (41.2% for grade 1, 66.7% for grade 2, 100% for higher). The terminal ileum was inspected with colonoscopy in 13 cases. Endoscopic diagnoses of the ileum were provided in 11 cases and histological diagnoses in 9 cases. Diagnostic consistency for diagnosis of the terminal ileum between endoscopy and histology was 77.8%. Conclusion: Because endoscopic and histopathological findings do not always match, caution is required when focusing on endoscopic findings alone, as there is a risk of misdiagnosis. Extensive inspection of the terminal ileum with biopsy appears useful to identify otherwise undetected lesions. Our data thus support invasive endoscopic examinations for gastrointestinal complications, including ileac inspection and biopsies under appropriate management.

Design of ionic liquids as liquid desiccant for an air conditioning system

Suitable control of the humidity can contribute to electric energy savings. However, the present dehumidification system has many weak points. The liquid desiccant air-conditioning system has recently gained growing interest from the stand point of reducing energy consumption during dehumidification. In order to find the appropriate ionic liquids(ILs) as a desiccant for the liquid desiccant air-conditioner system, we conducted a systematic evaluation of the humidification capability of 16 types of ILs. Among the tested ILs, tributyl(methyl)phosphonium dimethyl phosphate([P4441][DMPO_4]) exhibited the best dehumidification capacity and had a less corrosive effect on four types of metals as possible piping materials. It should be noted that this [P_(4441)][DMPO_4] has a very stable nature and produced no odor while conducting the experiment and storing for over 1 year at room temperature under ambient conditions. Furthermore, it was revealed that a 77%(w/w) aqueous solution of [P4441][DMPO_4] worked as an efficient desiccant liquid for the liquid desiccant air-conditioner system.

Quantitative phase imaging(QPI)with its high-contrast images of optical phase delay(OPD)maps is often used for label-free single-cell *** to other imaging methods,sensitivity improvement has not been intensively explored because conventional QPI is sensitive enough to observe the surface roughness of a substrate that restricts the minimum measurable ***,emerging QPI techniques that utilize,for example,differential image analysis of consecutive temporal frames,such as mid-infrared photothermal QPI,mitigate the minimum OPD limit by decoupling the static OPD contribution and allow measurement of much smaller ***,we propose and demonstrate supersensitive QPI with an expanded dynamic *** is enabled by adaptive dynamic range shift through a combination of wavefront shaping and dark-field QPI *** a proof-of-concept demonstration,we show dynamic range expansion(sensitivity improvement)of QPI by a factor of 6.6 and its utility in improving the sensitivity of mid-infrared photothermal *** technique can also be applied for wide-field scattering imaging of dynamically changing nanoscale objects inside and outside a biological cell without losing global cellular morphological image information.

Improved image denoising via RAISR with fewer filters

In recent years,accurate Gaussian noise removal has attracted considerable attention for mobile applications,as in smart *** conventional denoising methods have the potential ability to improve denoising performance with no additional ***,we propose a rapid post-processing method for Gaussian noise removal in this *** matching and 3D filtering and weighted nuclear norm minimization are utilized to suppress *** these nonlocal image denoising methods have quantitatively high performance,some fine image details are lacking due to the loss of high frequency *** tackle this problem,an improvement to the pioneering RAISR approach(rapid and accurate image super-resolution),is applied to rapidly post-process the denoised *** gives performance comparable to state-of-the-art super-resolution techniques at low computational cost,preserving important image structures *** modification is to reduce the hash classes for the patches extracted from the denoised image and the pixels from the ground truth to 18 filters by two improvements:geometric conversion and reduction of the strength *** addition,following RAISR,the census transform is exploited by blending the image processed by noise removal methods with the filtered one to achieve artifact-free *** results demonstrate that higher quality and more pleasant visual results can be achieved than by other methods,efficiently and with low memory requirements.

Interaction between Topographic Conditions and Entrainment Rate in Numerical Simulations of Debris Flow

Debris flow simulations are useful for predicting the sediment supplied to watersheds from upstream areas. However, the topographic conditions upstream are more complicated than those downstream and the relationship between the topographic conditions and debris flow initiation is not well understood. This study compared the use of several entrainment rate equations in numerical simulations of debris flows to examine the effect of topographic conditions on the flow. One-dimensional numerical simulations were performed based on the shallow water equations and three entrainment rate equations were tested. These entrainment rate equations were based on the same idea that erosion and the deposition of debris flows occur via the difference between the equilibrium and current conditions of debris flows, while they differed in the expression of the concentration, channel angle, and sediment amount. The comparison was performed using a straight channel with various channel angles and a channel with a periodically undulating surface. The three entrainment rate equations gave different amounts of channel bed degradation and hydrographs for a straight channel with a channel angle greater than 21° when water was supplied from upstream at a steady rate. The difference was caused by the expression of the entrainment rate equations. For channels with little undulation, the numerical simulations gave results almost identical to those for straight channels with the same channel angle. However, for channels with large undulations, the hydrographs differed from those for straight channels with the same channel angle when the channel angle was less than 21°. Rapid erosion occurred and the hydrograph showed a significant peak, especially in cases using the entrainment equation expressed by channel angle. This was caused by the effects of the steep undulating sections, since the effect increased with the magnitude of the undulation, suggesting that a debris flow in an upstream area develo

We studied the effect of salinity on nail-hold- ing power in wood construction. In saline solution, the holding power of nails was less than in purified water. With the increase of salt concentration, the surface and side nail- holding power of the wood specimens both declined, but the differences between salinity treatments were not sig- nificant. However, compared to the surface and side nail- holding power, the power on the edge was generally less and the difference was not obvious in different salt con- centrations. In the same salt concentration, with the extension of the processing cycle, the performance of holding power of nails showed a downward trend, expect the temporary rise in the middle.

Degradation rates and products of fluticasone propionate in alkaline solutions

The apparent degradation rate constant of fluticasone propionate(FLT) in 0.1 M NaOH:methanol=1:1 at 37 °C was previously reported to be 0.169 ± 0.003 h^(-1), and four degradation products(products 1–4) were observed in the solution. The aims of the present study were to assess the degradation rates of FLT in other alkaline solutions and clarify the chemical structures of the four degradation products in order to obtain basic data for designing an enema for inflammatory bowel disease. The apparent degradation rate constants in 0.05 M NaOH and 0.1 M NaOH:CH_3CN=1:1 were 0.472 ± 0.013 h^(-1) and 0.154 ± 0.000 h^(-1)(n=3), respectively. The chemical structures of products 1–4 in 0.1 M NaOH:methanol=1:1 were revealed by nuclear magnetic resonance(NMR)and mass spectrometry data. The chemical structure of products 2 was that the 17-position of the thioester moiety of FLT was substituted by a carboxylic acid. The degradation product in 0.1 M NaOH:CH_3CN=1:1 was found to be product 2 based on ~1H NMR data. The degradation product in 0.05 M NaOH was considered to be product 2 based on the retention time of HPLC. These results are useful for detecting the degradation products of FLT by enzymes of the intestinal bacterial flora in the large intestine after dosing FLT as an enema.
