Propensity score matching analysis for clinical impact of braided-type versus laser-cut-type covered self-expandable metal stents for endoscopic ultrasound-guided hepaticogastrostomy

Background:To prevent stent migration during endoscopic ultrasound-guided hepaticogastrostomy(EUS-HGS),intra-scope channel release technique is important,but is unfamiliar to non-expert *** self-expandable metal stent(SEMS)is an additional factor to prevent stent ***,no compara-tive studies of laser-cut-type and braided-type during EUS-HGS have been *** aim of this study was to compare the distance between the intrahepatic bile duct and stomach wall after EUS-HGS among laser-cut-type and braided-type ***:To evaluate stent anchoring function,we measured the distance between the hepatic parenchyma and stomach wall before EUS-HGS,one day after EUS-HGS,and 7 days after ***,propensity score matching was performed to create a propensity score for using laser-cut-type group and braided-type ***:A total of 142 patients were enrolled in this *** them,24 patients underwent EUS-HGS using a laser-cut-type SEMS,and 118 patients underwent EUS-HGS using a braided-type ***-HGS using the laser-cut-type SEMS was mainly performed by non-expert endoscopists(n=21);EUS-HGS using braided-type SEMS was mainly performed by expert endoscopists(n=98).The distance after 1 day was significantly shorter in the laser-cut-type group than that in the braided-type group[2.00(1.70-3.75)vs.6.90(3.72-11.70)mm,P<0.001].In addition,this distance remained significantly shorter in the laser-cut-type group after 7 *** these results were similar after propensity score matching analysis,the distance between hepatic parenchyma and stomach after 7 days was increased by 4 mm compared with the distance after 1 day in the braided-type *** the other hand,in the laser-cut-type group,the distance after 1 day and 7 days was almost the ***:EUS-HGS using a laser-cut-type SEMS may be safe to prevent stent migration,even in non-expert hands.

Identification of key pathways and gene expression in the activation of mast cells via calcium flux using bioinformatics analysis

Mast cells are the main effector cells in IgE-associated allergic disorders,and we have reported that mucosal mast cells(MMCs)play a more important role in the development of food allergy(FA).IgE with antigen or calcium ionophore stimulation can lead to the activation of MMCs via a calcium-dependent *** purpose of the present study was to identify gene signatures with IgE/antigen(dinitrophenyl-bovine serum albumin,DNP-BSA)or calcium ionophore(A23187)on the activation of *** expressed genes between the two types of samples were identified with microarray *** ontology functional and pathway enrichment analyses of differentially expressed genes were performed using the database for annotation,visualization,and integrated discovery *** results showed that IgE/antigen and A23187 could induce degranulation,increase vacuoles,and elevate the cytosolic calcium concentration in ***,GeneChip analysis showed that the same 134 mRNAs were altered with IgE/DNP-BSA and A23187,suggesting that DNP-BSA/IgE and A23187 affect the same signal pathway partly in *** analysis showed that the data were enriched in NF-κB,TNF,MAPK,transcription factor activity,DNA binding,and nucleic acid binding,suggesting that activation of MMCs is a complex *** results provide new insights on MMCs activation.

Mathematical Modeling of Metal Active Gas (MAG) Arc Welding

In the present paper. a numerical model for MAG (metal active gas) arc welding of thin plate has been developed. In MAG arc welding, the electrode wire is melted and supplied into the molten pool intermittently. Accordingly, it is assumed on the modeling that the thermal energy enters the base-plates through two following mechanisms, i.e., direct heating from arc plasma and 'indirect' heating from the deposited metal. In the second part of the paper, MAG arc welding process is numerically-analyzed by using the model. and the calculated weld bead dimension and surface profile have been compared with the experimental MAG welds on steel plate. As the result. it is made clear that the model is capable of predicting the bead profile of thin-plate MAG arc welding, including weld bead with undercutting.

A Twenty-Year Follow-up Case Study of an Office Worker Who Returned to Work despite Serious Memory Disorder Caused by Herpes Encephalitis

This study followed a 52-year-old male patient, who had suffered from severe impairment in recent memory due to se- quelae of herpes encephalitis, for 20 years. He returned to his highly intellectual work and performed well despite his doctor’s prediction. While the patient showed consistently poor results on various neuropsychological memory tests, he demonstrated incredible performance at work. This case exemplifies an extreme case that declarative memory is formed with the support of semantic memory, procedural memory, and his strong interests. Additionally, it offers lessons that results on memory tests do not necessarily correspond to the actual level of competence. The focal sites were found on both sides of the medial temporal lobe, predominantly on the left side. The T2-weighted magnetic resonance images (MRI) obtained 9 years after the onset confirmed widespread damage to the left brain including parahippocampal gyrus, hippocampus, spindle gyrus, and amygdaloid complex, with microlesions extending from the right parahippocampal gyrus to its antero-interior rim. However, the damage to hippocampus was presumed to be minor.

A Development of Rom-Exercise Machine Using Linkages(Design of Leg Mechanism by Walking Simulation)

In this study, the design of an automatic ROM-Exercise machine that is constructed witha planar multililnk mechamsm consisting of only revolute pairs is investigated. Namely, equations thatdetermine the minimum moving spaces and relative positions of link required to construct the legmechanism are formulated with consideration of transform functions. For the leg mechanism that isconstructed with a planar eleven-link mechanism, arrangements of each link and optimum linkprofiles avoided mutual interferences among moving links are determined wb consideration of therelative locations of each link in the same plane during a cycle of motion of the mechanism. Based onthe above analytical results, an automatic ROM-Exercise machine that performs within a minimum moving spaces is proposed as a prachcal example. ms study is carried out as part of the students'computcr education to the graduation thesis, in order to improve their creativity and machine designtechnology skills in coniunction with educational advantages. Significam educational results areobtained by using the design techniques mentioned above.

Evaluation of Fever and Its Related Factors during 4 Weeks after Primary Total Hip Arthroplasty

The purpose of this study was to describe the incidence and characteristics of postoperative fever (POF;≥38℃) and clarify the correlation between POF and febrile-related factors during 4 weeks after primary total hip arthroplasty (THA). A total of 56 patients were included in the study (67 THAs). The preoperative diagnosis for all patients was osteoarthritis. The average follow-up period was 90 months. No patients developed deep infection during the follow-up period. The maximum mean peak daily temperature was 37.7℃ on the day of the surgery and the next day. Fifty-two percent of patients experienced POF, with most developing a maximum temperature (MT) on the day of surgery and the latest occurring by postoperative day (POD) 3. Only C-reactive protein (R = 0.384, p = 0.001) among the febrile-related factors had a weak correlation with MT. Four hips showed a POF of >38℃ later than POD 7;all four of these patients were diagnosed with a urinary tract infection. POF is common and may be part of a normal inflammatory response to tissue injury after THA. More than half of patients may develop POF within 1 week, with most developing MT on POD 0 and the latest occurring by POD 3. In addition, urinary tract infections might be an initial reason for POF in the subsequent 3 weeks.

AIM:To evaluate the efficacy of pegylated interferon α-2b(peg-IFNα-2b) plus ribavirin(RBV) therapy in Japanese patients with chronic hepatitis C(CHC) genotype Ib and a high viral ***:One hundred and twenty CHC patients(58.3% male) who received peg-IFNα-2b plus RBV therapy for 48 wk were *** virological response(SVR) and clinical parameters were ***:One hundred(83.3%) of 120 patients completed 48 wk of treatment.53 patients(44.3%) achieved *** virological response(EVR) and end of treatment response(ETR) rates were 50% and 73.3%,*** clinical parameters(SVR vs non-SVR) associated with SVR,ALT(108.4 IU/L vs 74.5 IU/L,P = 0.063),EVR(76.4% vs 16.4%,P 56 years),SVR in females was significantly lower than that in males(17% vs 50%,P = 0.0262).EVR and adherence to Peg-IFN were demonstrated to be the main factors associated with ***:Peg-IFNα-2b plus RBV combination therapy demonstrated good tolerability in Japanese patients with CHC and resulted in a SVR rate of 44.3%.Treatment of elderly female patients is still challenging and maintenance of adherence to peg-IFNα-2b is important in improving the SVR rate.

Ice accretion is the phenomenon that super-cooled water droplets impinge and accrete on a body. It is well known that ice accretion on blades and airfoils leads to performance degradation and severe accidents. For this reason, experimental investigations have been carried out using flight tests or icing tunnels. However, it is too expensive, dangerous, and difficult to set actual icing conditions. Hence, computational fluid dynamics is useful to predict ice accretion. A rotor blade is one of jet engine components where ice accretes. Therefore, the authors focus on the ice accretion on a rotor blade in this study. Three-dimensional icing phenomena on the rotor blade of a commercial axial blower are computed here, and ice accretion on the rotor blade is numerically investigated.

Infrasound observations at Syowa Station,East Antarctica:Implications for detecting the surface environmental variations in the polar regions

Characteristic infrasound waves observed at Antarctic stations demonstrate physical interaction involving environmental changes in the Antarctic continent and the surrounding oceans. A Chaparraltype infrasound sensor was installed at Syowa Station （SYO; 39°E, 69°S）, East Antarctica, as one of the projects of the International Polar Year （IPY2007-2008）. Data continuously recorded during the three seasons in 2008-2010 clearly indicate a contamination of the background oceanic signals （microbaroms） with peaks between 4 and 10 s observed during a whole season. The peak amplitudes of the microbaroms have relatively lower values during austral winters, caused by a larger amount of sea-ice extending around the Lutzow-Holm Bay near SYO, with decreasing ocean wave loading effects. Micro- baroms measurements are useful tool for characterizing ocean wave climate, complementing other oceanographic and geophysical data. A continuous monitoring by infrasound sensors in the Antarctic firmly contributes to the Comprehensive Nuclear-Test-Ban Treaty （CTBT） in the southern high latitude, together with the Pan-Antarctic Observations System （PAntOS） under the Scientific Committee on Antarctic Research （SCAR）. Detailed measurements of the infrasound waves in Antarctica, consequently, could be a new proxy for monitoring regional environmental change as well as the temporal climate variations in the polar regions.
