Interaction between Caregiving Ability and Home Discharge Using Severity Classification upon Discharge

Background: The interaction between self-care and caregiving ability in home discharged stroke patients is unclear. Purpose: This study investigates the interaction between caregiving ability and individual self-care activities for post-stroke patients upon home discharge based on their motor-Functional Independence Measure (m-FIM) score. Methods: A total of 2626 stroke patients registered in the Japanese Rehabilitation Database were retrospectively analyzed. Extracted data were divided into three groups based on the m-FIM score at discharge and classified into two groups based on their discharge destination (home discharge, non-home discharge). After the data were modified as mean-centered values, a hierarchical multiple regression analysis evaluated the change in variance amount (ΔR2) using discharge destination as the dependent variable. Model 1 used two independent variables (Rankin Scale and cognitive FIM score), model 2 used two independent variables (caregiving ability and individual self-care), which were added to model 1, and model 3 used an interaction term value, which was added to model 2. Furthermore, a simple slope analysis was performed for these interaction effects. Results: The ΔR2 exhibited six and five self-care items in the moderate and mild groups, respectively. The interaction was significant on simple slope analysis in the moderate group for six self-care items (except dressing upper body, toileting, and bowel management) and in the mild group for three self-care items, including dressing upper body, bladder management, and climbing stairs). Conclusion: This study suggests the need for intervention, especially bladder management and single-foot standing ability during step climbing, in stroke patients discharged to home. In addition, caregiving ability is one of the factors that should be considered in mild group.

Investigate Targeted Factors to Achieve Prediction Goal in Stroke Convalescence in Terms of Causal Relationships of Prediction Error

Background and Purpose: To investigate target functional independence measure (FIM) items to achieve the prediction goal in terms of the causal relationships between prognostic prediction error and FIM among stroke patients in the convalescent phase using the structural equation modeling (SEM) analysis. Methods: A total of 2992 stroke patients registered in the Japanese Rehabilitation Database were analyzed retrospectively. The prediction error was calculated based on a prognostic prediction formula proposed in a previous study. An exploratory factor analysis (EFA) then the factor was determined using confirmatory factorial analysis (CFA). Finally, multivariate analyses were performed using SEM analysis. Results: The fitted indices of the hypothesized model estimated based on EFA were confirmed by CFA. The factors estimated by EFA were applied, and interpreted as follows: “Transferring (T-factor),” “Dressing (D-factor),” and “Cognitive function (C-factor).” The fit of the structural model based on the three factors and prediction errors was supported by the SEM analysis. The effects of the D- and C-factors yielded similar causal relationships on prediction error. Meanwhile, the effects between the prediction error and the T-factor were low. Observed FIM items were related to their domains in the structural model, except for the dressing of the upper body and memory (p < 0.01). Conclusions: Transfer, which was not heavily considered in the previous prediction formula, was found in causal relationships with prediction error. It is suggested to intervene to transfer together with positive factors to recovery for achieving the prediction goal.

Discrimination of hie and hiesho using toe blood pressure in young women

OBJECTIVE： In Western medicine, hie （cold sensation） is thought as a subjective complaint. Consequently, despite women＇s complaints of health problems due to hie, such as distress and decreased quality of life, it is not diagnosed as a disease or given active treatment. However, hie sometimes requires emergency treatment or is a symptom of other disorders. Thus, diagnosing hie and eliminating distress due to hie are clinically significant. This research aimed to discriminate between hie and hiesho （cold disorder） and quantify hie by using objective assessments in addition to subjective assessments. METHODS： A questionnaire was administered to 159 healthy young women without any underlying disease that could cause hie, followed by assessment of vascular function. These data were examined through exploratory factor analysis, exploratory cluster analysis, and multivariate logistic analysis. Two- and three-group comparisons were made using the Mann-Whitney test and KruskaI-Wallis test, respectively, followed by multiple comparisons using Scheffe＇s method.RESULTS： The hie （including hiesho） and normal groups comprised 81 （51%） and 78 （49%） subjects, respectively. The questionnaire was a low reliability screening method for determining the severity of hie symptoms. As an index of discrimination of hie and hiesho, we determined the toe-brachial index （TBI） to be 0.81 and toe systolic blood pressure （SBP） to be 87.73 mmHg with a linear function （TBI=0.007 5xtoe SBP＋0.152, adjusted R2=0.829, F（1,79）=388.79, P〈0.001）. CONCLUSION： Toe SBP and TBI were suggested to be capable of discriminating and quantifying symptoms when used in combination with a questionnaire.

KDM2B promotes cell viability by enhancing DNA damage response in canine hemangiosarcoma

Epigenetic regulators have been implicated in tumorigenesis of many types of cancer;however,their roles in endothelial cell cancers such as canine hemangiosarcoma(HSA)have not been studied.In this study,we find that lysine-specific demethylase 2 b(KDM2 B)is highly expressed in HSA cell lines compared with normal canine endothelial cells.Silencing of KDM2 B in HSA cells results in increased cell death in vitro compared with the scramble control by inducing apoptosis through the inactivation of the DNA repair pathways and accumulation of DNA damage.Similarly,doxycycline-induced KDM2 B silencing in tumor xenografts results in decreased tumor sizes compared with the control.Furthermore,KDM2 B is also highly expressed in clinical cases of HSA.We hypothesize that pharmacological KDM2 B inhibition can also induce HSA cell death and can be used as an alternative treatment for HSA.We treat HSA cells with GSK-J4,a histone demethylase inhibitor,and find that GSK-J4 treatment also induces apoptosis and cell death.In addition,GSK-J4 treatment decreases tumor size.Therefore,we demonstrate that KDM2 B acts as an oncogene in HSA by enhancing the DNA damage response.Moreover,we show that histone demethylase inhibitor GSK-J4 can be used as a therapeutic alternative to doxorubicin for HSA treatment.

Background: A correlation has been found between self-care and caregiving ability in home discharged stroke patients. However, few studies have explored the interaction between individual self-care and caregiving ability. Purpose: This study aimed to investigate the interaction between individual self-care and caregiving ability in home discharged stroke patients. Participants and Methods: A total of 2688 stroke patients registered in the Japanese Rehabilitation Database (JRD) were retrospectively analyzed. Patients were classified into two groups based on their discharge destination (home discharge, non-home discharge), and the data were modified as mean-centered values. After a correlation analysis was performed between all the collected data, a hierarchical multiple regression analysis evaluated the interaction using discharge destination as the dependent variable. Model 1 used two independent variables (Rankin scale and cognitive functional independence measure score), model 2 used two independent variables (caregiving ability and individual self-care), and model 3 used a mean-centering value, which was added to model 2. Background: A correlation between self-care and gait in stroke patients has been shown. However, there are few reports suggesting an interaction between self-care and gait. Purpose: To investigate the interaction between locomotion and improvements in self-care. Participants and Methods: We retrospectively analyzed 3034 stroke patients who were registered in the Japanese Rehabilitation Database. Using their data, patients were classified into three groups (severe, moderate, slight) based on the motor functional independence measure upon admission, and data were modified as mean-centered values. We performed a correlation analysis to evaluate the relationship among all the collected data. Subsequently, a hierarchical multiple regression analysis was performed to evaluate interaction using the self-care motor score from the Functional Independent Measure (FIM) as the dependent variable. We give some new genus-3 universal equations for Gromov-Witten invariants of compact symplectic manifolds. These equations were obtained by studying relations in the tautological ring of the moduli space of2-pointed genus-3 stable curves. A byproduct of our search for genus-3 equations is a new genus-2 universal equation for Gromov-Witten invariants.

BACKGROUND Ischemic gastritis is a clinically rare and highly fatal disease that occurs when the hemodynamics of a patient with vascular risk is disrupted.Early diagnosis and treatment are possible only with upper endoscopy after symptom appearance.We report seven cases of ischemic gastritis and its clinical features,prognosis,and indicators that may help in early detection.CASE SUMMARY Of the seven patients,six had vascular risk and five died within 2 wk of diagnosis.Their symptoms included hematemesis and hypotension.Although surgery is a choice for radical treatment,not all patients were tolerant.For such patients,conservative treatment was selected,but all of them died.In contrast,patients who underwent repeat endoscopy showed improved mucosal findings,suggesting that this improvement may not affect prognosis.Some ischemic changes such as wall thickening,mural emphysema,and fluid retention in the stomach were observed before diagnosis through endoscopy and computed tomography(CT).The CT scan can be effective for early detection,and improvement in circulatory failure and aggressive treatment may save the lives of patients with this disease.CONCLUSION The characteristic CT findings enable early detection of ischemic gastritis.Early diagnosis increases the chance of survival if early therapeutic intervention and improvement of circulatory dynamics can be achieved in this highly fatal disease.

Common bile duct stones are among the most common conditions encountered by endoscopists.Therefore,it is well researched;however,some items,such as indications for endoscopic papillary balloon dilatation(EPBD),safety of EPBD and endoscopic sphincterotomy in patients receiving dual antiplatelet therapy or direct oral anticoagulant,selection strategy for retrieval balloons and baskets,lack adequate evidence.Therefore,the guidelines have been updated with new research,while others remain unchanged due to weak evidence.In this review,we comprehensively summarize the standard methods in guidelines and new findings from recent studies on papillary dilation,stone retrieval devices,difficult-to-treat cases,troubleshooting during the procedure,and complicated cases of cholangitis,cholecystolithiasis,or distal biliary stricture.
