Case of colonic intussusception secondary to mobile cecum syndrome repaired by laparoscopic cecopexy using a barbed wound suture device

A 27-year-old man with recurrent right lower quadrant pain was admitted to our hospital. Ultrasonography and computed tomography examination of the abdomen revealed a target sign in the ascending colon, which was compatible with the diagnosis of cecal intussusception. The intussusception was spontaneously resolved at that time, but it relapsed 6 mo later. The patient underwent a successful colonoscopic disinvagination; there was no evidence of neoplastic or inflammatory lesions in the colon and terminal ileum. The patient underwent laparoscopic surgery for recurring cecal intussusception. During laparoscopy, we observed an unfixed cecum on the posterior peritoneum(i.e. a mobile cecum). Thus, we performed laparoscopic appendectomy and cecopexy with a lateral peritoneal flap using a barbed wound suture device. The patient's post-operative course was uneventful, and he continued to do well without recurrence at 10 mo after surgery. Laparoscopic cecopexy using a barbed wound suture device is a simple and reliable procedure that can be the treatment of choice for recurrent cecal intussusception associated with a mobile cecum.

Impact of postoperative sexual function on health-related quality of life after robot-assisted radical prostatectomy

Background:We investigated potential disparities in health-related quality of life,particularly concerning urinary function,between patients with preserved and those with impaired sexual function after robot-assisted radical prostatectomy(RARP).Materials and methods:BetweenDecember 2012 and April 2020,704men underwent RARP in our *** study included 155 patients with a preoperative 5-item International Index of Erectile Function(IIEF-5)of≥12 points and an assessable IIEF-5 at 12 months ***-related quality of life was assessed using the 8-item Short-Form Health Survey and Expanded Prostate Cancer Index Composite(EPIC)preoperatively and at 3,6,and 12months postoperatively.A logistic regression analysis andWilcoxon rank sum tests were ***:Patients were grouped according to the median IIEF-5 score 12 months after surgery:those with preserved sexual function(n=71)and those with impaired sexual function(n=84).The mental component summary of the 8-item Short-Form Health Survey was better in the group with preserved sexual function at 6 months postoperatively than in the group with impaired sexual function(p<0.01).In the EPIC,the group with preserved sexual function performed better not only in the sexual domain but also in the urinary domain at all time points comparedwith the groupwith impaired sexual function(p<0.01).In the comparison of the urinary subdomains of the EPIC,there were no significant differences in urinary function or incontinence,but there were significant differences in urinary distress and irritative/obstructive scores(p<0.01).Conclusions:Patients with preserved postoperative sexual function after RARP showed better urinary function than those with impaired sexual ***,preserved sexual function is closely associated with urinary function.

A preliminary study on rectal dose reduction associated with hyaluronic acid implantation in brachytherapy for prostate cancer

Objectives: Hydrogel spacer (HS) was developed to reduce rectal toxicities caused by radiotherapy, but has been reported to cause major adverse events. Our institute has attempted to introduce a hyaluronic acid (HA) as an alternative spacer. This study aimed to compare rectal doses and geometric distributions between the HS and HA implantation in prostate ***: HS and HA were inserted in 20 and 18 patients undergoing high-dose brachytherapy, respectively. The rectum spacer volumes injected were 10 mL and 22 mL, respectively. In the treatment planning system, 13.5 Gy was administered with common catheter positions. The rectal dose indices were assessed between the spacer groups for dosimetry evaluation. Distances between the prostate and rectum and configurations of the spacers were ***: The mean doses irradiated to 0.1 and 2 mL of the rectum were 10.45 Gy and 6.71 Gy for HS, and 6.73 Gy and 4.90 Gy for HA (p<0.001). The mean minimum distances between the prostate and rectum were 1.23 cm and 1.79 cm for HS and HA, respectively (p<0.05). Geometrical configuration comparisons revealed that HA has a higher ability to expand the space than ***: The rectal dose reduction ability of HA is significantly greater than that of HS, suggesting its potential as a new spacer.

Negative refraction of deep water waves through water channel network

A design method of a negative refractive index for free surface water waves in deep water is presented. To obtain a condition of a negative index, an analogy between water waves in a water channel and electric waves in an electric circuit is employed. An analytical solution of waves around a periodic array of rectangular columns is obtained by a circuit analysis. Attention is not focused on columns but a space among columns, and then the space is regarded as the water channel. Therefore, the circuit analysis can be applied to water waves. One channel is treated as a unit structure and a number of channels are periodically cascaded as a network. The negative refraction and backward waves by a network of the water channels are shown. Since the negative refraction might occur even around simple periodic structures, it is meaningful to understand wave phenomenon around offshore structures and develop innovative technology.

Microstructure effect on nanohardness distribution for medium-carbon martensitic steel

Nanoindentation measurement was performed to evaluate the local hardness distribution for quenched and tempered martensitic steel with ferrite-cementite constituent. The ratio of standard deviation to the average nanohardness was 15.4% when nanoin-dentation tests were carried out at the peak load of 1000 μN, while that of the Vickers hardness was only 1.5% at 9.8 N. Electron backscatter diffraction analysis and SEM ob-servation all showed that the large scattering of nanohardness did not depend on the crystallographic orientation of each grain but from the inhomogeneous microstructure in the sub-micron scale such as cementite distribution. Compared with the results on tung-sten single crystals with different surface orientations as well as another martensite proc-essed by modified-ausforming with the same chemical composition but more homogene-ous cementite distribution, the nanohardness showed smaller scattering, supporting the conclusion.

Small-scale analysis of brittle-to-ductile transition behavior in pure tungsten

Tungsten as a material exhibits broad and increasingly important applications;however,the characterization of its ductile-to-brittle transition(BDT)is currently limited to large-scale scenarios and destructive *** this study,we overcome this challenge by implementing small-scale techniques to provide a comprehensive understanding of the BDT behavior of pure *** order to predict the failure mode at various temperature ranges,the practical fracture analysis diagram has been proposed to describe the resistance to shear flow and cracking behavior with *** temperature nano-indentation tests have provided the inherent mechanical responses corresponding to the maximum shear stress at various temperatures,which is required for dislocation activities in an atomic scaled activation *** one hand,atomistic simulations have provided the temperature dependence of brittle fracture stress,where the atomic bonds break due to intergranular or intragranular *** considered four tungsten specimens having various microstructures prepared using different processing conditions of cold-rolling and post-annealing,and their BDT ranges were inferred using the obtained fracture analysis diagram with the statistical data *** fracture analysis diagram of each specimen obtained were compared with the direct observation of fracture responses in macroscopic mechanical tests,which conclusively indicated that the small-scale inherent mechanical properties are greatly relevant to the macroscopic BDT behavior in pure *** on the BDT estimations by small-scale characterization,we provided further insights into the factors affecting the BDT behavior of tungsten,focusing on the contributions of different types of dislocations.

Computed tomography-guided transpulmonary radiofrequency ablation for hepatocellular carcinoma located in hepatic dome

AIM： To determine the feasibility of performing computed tomography （CT）-guided transpulmonary radiofrequency ablation （RFA） for hepatocellular carcinoma （HCC） located in the hepatic dome. METHODS： A total of seven patients with HCC comprising seven nodules located in the hepatic dome were treated from April 2004 to December 2004. CTguided transpulmonary RFA was performed using a cool-tip type electrode （Radionics Company） based on a standardized energy protocol. All tumors located in the hepatic dome were not detectable by the usual ultrasound （US） methods. The lesion diameters ranged from 15 to 27 mm. RESULTS： RFA was technically feasible in all the patients. The puncture procedure was performed twice or less and the total average performance time was 40.6 min. Local tumor control was achieved in all the patients. The necrosis diameter ranged from 25 to 35 mm. The mean follow-up period was 9.6 （7-14 mo） mo. There was no local recurrenceat the follow-up points. Pneumothorax requiring pleural drainage was the main complication, which was observed in two of the seven patients （28.6%）. However, it improved with chest drainage tube, and the tube could be removed within 2-3 d. No other major complications were observed.CONCLUSION： CT-guided puncture is useful for the treatment of tumors located in the hepatic dome which are hardly detectable by US, even though pneumothorax sometimes may occur as a complication. In the cases with adhesion in the pleura for which artificial pleural effusion methods are not appropriate, CT-guided RFA is thus considered to be an alternative treatment for HCC located in the hepatic dome.

Thermoelectric properties of Indium doped skutterudite thick film synthesized by a facile technique of electrochemical deposition

Doped/filled skutterudites are much studied materials due to their excellent thermoelectric ***,their synthesis and preparation is *** work synthesized indium(In)doped cobalt triantimonide(CoSb_(3))skutterudite thick films using a facile electrochemical deposition technique through chronoamperometric steps for 2 *** nominal composition of In element is found in the range of 0.55e0.23 for a stoichiometric condition of In doped CoSb_(3)thick *** early crystal growth of In doped films shows instantaneous nucleation and is controlled by the charge transfer process with diffusion coefficient,D of 10^(-5)cm^(2)/*** incorporation of In into the interstitial sites of CoSb_(3)cages is evident from the lattice constant(a)expansion as observed in *** optimum Seeback coefficient(S)of the 0.5 mmol In doped CoSb_(3)thick film is89.84 mV/K at 282 K,due to an increase in the carrier concentration(n~10^(20)cm^(-3)).The negative S is due to the electron donor behaviour of the ***,high electrical conductivity,s value(14.26 kS/m)contributes to a power factor(S2s)increment of 115.11 mW/(m$K2).The result shows a promising thermoelectric property of doped skutterudite synthesized by electrochemical deposition technique.
