Skeletal muscle mass and quality before preoperative chemotherapy influence postoperative long-term outcomes in esophageal squamous cell carcinoma patients

BACKGROUND Previous reports have focused on muscle mass as a prognostic factor in esophageal *** To investigate how preoperative body type influences the prognosis of patients with esophageal squamous cell carcinoma who underwent neoadjuvant chemotherapy(NAC)and *** The subjects were 131 patients with clinical stage Ⅱ/Ⅲ esophageal squamous cell carcinoma who underwent subtotal esophagectomy after *** muscle mass and quality were calculated based on computed tomography images prior to NAC,and their statistical association with long-term outcomes was examined retrospectively in this case-control *** The disease-free survival rates in the low psoas muscle mass index(PMI)group vs the high PMI group were 41.3%vs 58.8%(P=0.036),*** the high intramuscular adipose tissue content(IMAC)group vs the low IMAC group,the disease-free survival rates were 28.5%vs 57.6%(P=0.021),*** overall survival(OS)rates for the low PMI group vs the high PMI group were 41.3%vs 64.5%(P=0.008),respectively,and for the high IMAC group vs the low IMAC group,they were 29.9%vs 61.9%(P=0.024),*** of the OS rate revealed significant differences in patients aged 60 years or older(P=0.018),those with pT3 or above disease(P=0.021),or those with lymph node metastasis(P=0.006),aside from PMI and *** analysis demonstrated that pT3 or above hazard ratio(HR):1.966,95%confidence interval(CI):1.089-3.550,(P=0.025),lymph node metastasis(HR:2.154,95%CI:1.118-4.148,P=0.022),low PMI(HR:2.266,95%CI:1.282-4.006,P=0.005),and high IMAC(HR:2.089,95%CI:1.036-4.214,P=0.022)were significant prognostic factors for esophageal squamous cell *** muscle mass and quality before NAC in patients with esophageal squamous cellcarcinoma are significant prognostic factors for postoperative OS.

Development and installation of bedload monitoring systems with submerged load cells

Bedload governs riverbed channel variations and morphology, it is necessary to determine bedload discharge through an arbitrary cross section in a mountain fiver. A new system with submerged load cells has been developed to directly measure bedload discharge. The system consists of：（1） an iron box which is 1 m long, 0.5 m wide and 0.1 m in depth, （2） two submerged load cells 0.7 m apart, （3） a pressure sensor and, （4） an electromagnetic velocity meter. This system has been designed to exclude the effect of the hydraulic pressure of water on direct measurements of bedload particle weight. Initial tests in a laboratory were conducted to examine the accuracy of measurements with the system under aerial conditions. The system has been installed in the supercritical flume in Ashi-arai-dani River of the Hodaka Sedimentation Observatory of the Disaster Prevention Research Institute （DPRI） of Kyoto University to obtain bedload discharge under natural conditions. Flume tests were conducted in this channel by artificial supply of uniform sediment particles of several grain sizes. The average velocity of the sediment particles near the bed was estimated using cross-correlation functions for weight waves obtained by the two load cells, Bedload discharge calculations were based on time integration of the product of sediment velocity and sediment weight obtained by the two load cells. This study clarifies the reasons why bedload measurements are difficult, and provides some solutions using the monitoring systems with submerged load cells through the field measurements. Additionally, the applicability of bedload measurement with the submerged load cells is explained based on experimental artificial sediment supply data.

This article describes a case we experienced in which good postsurgical facial profiles were obtained for a patient with jaw deformities associated with facial asymmetry,by implementing surgical planning with SimPlant *** this method,we conducted LF1 osteotomy,intraoral vertical ramus osteotomy（IVRO）,sagittal split ramus osteotomy（SSRO）,mandibular constriction and mandibular border *** only did we obtain a class I occlusal relationship,but the complicated surgery also improved the asymmetry of the frontal view,as well as of the profile view,of the *** virtual operation using three-dimensional computed tomography（3D-CT）could be especially useful for the treatment of patients with jaw deformities associated with facial asymmetry.

Size does not determine the grade of malignancy of early invasive colorectal cancer

AIM： To clarify the clinicopathological characteristics of small and large early invasive colorectal cancers （EI-CRCs）, and to determine whether malignancy grade depends on size. METHODS： A total of 583 consecutive EI-CRCs treated by endoscopic mucosal resection or surgery at the National Cancer Center Hospital between 1980 and 2004 were enrolled in this study. Lesions were classified into two groups based on size： small （≤10 ram） and large （〉10 ram）. Clinicopathological features, incidence of lymph node metastasis （LNM） and risk factors for LNM, such as depth of invasion, lymphovascular invasion （LVI） and poorly differentiated adenocarcinoma （PDA） were analyzed in all resected specimens. RESULTS： There were 120 （21%） small and 463 （79%） large lesions. Histopathological analysis of the small lesion group revealed submucosal deep cancer （sin： 1〉1000 μm） in 90 （75%） cases, LVI in 26 （22%） cases, and PDA in 12 （10%） cases. Similarly, the large lesion group exhibited submucosal deep cancer in 380 （82%） cases, LVI in 125 （27%） cases, and PDA in 79 （17%） cases. The rate of LNM was 11.2% and 12.1% in the small and large lesion groups, ***： Small EI-CRC demonstrated the same aggressiveness and malignant potential as large cancer.

Coexpression of Escherichia coli RNase III in silkworm cells improves the efficiency of RNA interference induced by long hairpin dsRNAs

Long hairpin dsRNA transcribed from chromosomal DNA can induce RNA interference in Bombyx mori cells, although its gene silencing efficiency is lower than that of exogenously introduced double-stranded RNAs （dsRNAs）. To solve this problem, we monitored the nuclear cytoplasmic translocation of the transcribed hairpin dsRNA and analyzed the processing efficiency into mature small interfering RNA （siRNA）. Northem blot analysis revealed that the transcribed hairpin dsRNAs were spliced and transported into the cytoplasm, but were not effectively diced into siRNAs. Interestingly, RNAi with hairpin dsRNAs from genome-integrated IR transgene was stimulated by the coexpression of Escherichia coli RNase III, although this exogenous enzyme seemed to bring about nonspecific cleavage of cellular mRNA.

Application of an impact plate-Bedload transport measuring system for high-speed flows

To achieve the sustainable use of dams, the development of methods for sediment management in reservoirs is required. One such method includes the use of Sediment Bypass Tunnels(SBTs) to divert sediment around a dam, thereby preventing sedimentation in the reservoir. However, SBTs are prone to severe invert abrasion caused by the high sediment flux. Therefore, it is necessary to establish a measurement system of the sediment transport rate in these tunnels. One system to measure sediment transport in rivers is the Swiss plate geophone, which can register plate vibrations caused by particle impact. In Japan, the Japanese pipe microphone is used, and sediment transport is measured based on the sound emitted by the particle impact. In this study an attempt was made to optimize the advantages of both systems by fixing a microphone and an acceleration sensor to a steel plate. The results of calibration experiments with this new system are presented and compared with the existing methods. It was found that the acceleration sensor can detect sediment particles larger than 2 mm in ***, a new parameter, referred to as the detection rate, was introduced to describe the correlation between the actual amount of sediment and the registered output. Analysis of Endoscopic Sphenopalatine Artery Surgery for Refractory Epistaxis

Background: This study aimed to determine the safety and efficacy of sphenopalatine artery (SPA) surgery in patients with refractory epistaxis and to identify factors associated with the indications for surgery to assist clinicians in making prompt and appropriate decisions regarding SPA surgery for refractory epistaxis. Methods: We analyzed 97 consecutive patients with nasal bleeding hospitalized in our institution between April 2009 and March 2018. Clinicopathological characteristics and clinical outcomes were analyzed from retrospective chart reviews. Results: Of the 97 patients, 28 (29 sides) required SPA surgery. There were no differences in sex, anti-coagulant drug or antiplatelet use, hemoglobin concentration, or platelet count between Group A (patients who required surgery) and Group B (patients who did not require surgery). Age (58 vs. 67 years, p = 0.038) and severity of comorbidity (0 vs. 1, p = 0.039) were significantly lower in the surgery group. Patients who were younger and had lower Charlson Comorbidity Index (CCI) scores had more requirements for surgery than those who were older and had higher CCI scores. There was no significant difference between the groups with respect to the proportion of patients receiving blood transfusions;however, the length of hospitalization was significantly longer in Group A than in Group B (8.9 vs. 8 days, p = 0.038). Success rate (non-rebleeding rate) was 89%, comparable to that reported in previous studies. Conclusions: Endoscopic SPA surgery was found safe and effective method with few complications. Younger patients with lower CCI scores were found appropriate for SPA surgery for refractory epistaxis due in part to a lower risk of anesthesia.

Performance Improvements in Temperature Reconstructions of 2-D Tunable Diode Laser Absorption Spectroscopy(TDLAS)

Performance improvement was attained in data reconstructions of 2-dimensional tunable diode laser absorption spectroscopy(TDLAS). Multiplicative Algebraic Reconstruction Technique(MART) algorithm was adopted for data reconstruction. The data obtained in an experiment for the measurement of temperature and concentration fields of gas flows were used. The measurement theory is based upon the Beer-Lambert law, and the measurement system consists of a tunable laser, collimators, detectors, and an analyzer. Methane was used as a fuel for combustion with air in the Bunsen-type burner. The data used for the reconstruction are from the optical signals of 8-laser beams passed on a cross-section of the methane flame. The performances of MART algorithm in data reconstruction were validated and compared with those obtained by Algebraic Reconstruction Technique(ART) algorithm.

Irrigation system and land use effect on surface water quality in river, at lake Dianchi, Yunnan, China

The surface water samples were collected in river Dahe and its tributaries, which flow into severely eutrophic lake Dianchi, Yunnan Province, China, in order to elucidate factors controlling water quality fluctuations. The temporal and spatial distribution of water quality tendency was observed. The water quality of each river is dependent on the hydrology effect such water gate and circulating irrigation system. We must consider the hydrology effect to accurately understand water quality variations of river in this study field. In river without highly circulating irrigation system or water gate effect, the downstream nitrate nitrogen （NO3-N） concentration increase occurred in area dominated by open field cultivation, whereas the NO3-N concentration was constant or decreased in area dominated by greenhouse land use. This result suggests that greenhouse covers the soil from precipitation, and nitrate load of greenhouse could be less than that of open field cultivation while the rainfall event. In the upper reaches of river, where is dominated by open field cultivation, there were no sharp increase dissolved molybdate reactive phosphorus and total phosphorus concentration, but P load was accumulated in the lower reaches of river, whose predominant land use is greenhouse. Although the P sources is unclear in this study, greenhouse area may have potential of P loads due to its high P content in greenhouse soil. Considering hydrology effect is necessary to determine what the major factor is influencing the water quality variation, especially in area with highly complicated irrigation system in this studying site.

Oxidative stress responses of submerged macrophyte Vallisneria asiatica to different concentrations of cyanobacteria

In a 10-day aquarium experiment, this investigation examines macrophyte restoration in eutrophic Lake Taihu, the physiological effects of different plant biomass levels and of increasing natural cyanobacterial concentrations on a submerged macrophyte, Vallisneria asiatica. Cyanobacterial stress suppressed the superoxide dismutase （SOD） activity of the plant＇s leaves and induced the catalase （CAT） and peroxidase （POD） activities of its roots. The soluble protein content in V. asiatica decreased with an increase in natural cyanobacterial concentrations, whereas the malonaldehyde （MDA） increased significantly at chlorophyll a （Chl a） concentrations of 222 and 262 μg/L in water. V. asiatica adapted to the stress caused by cyanobacterial concentrations by adjusting its antioxidant defense system to remove the excessive reactive oxygen species when the algal Chl a concentration was 〉109 μg/L. Additionally, high biomass of V. asiatica （2 222 g FW/m^2） can inhibit the reproduction of cyanobacteria more significantly than low biomass （1 111 g FW/m^2）. High biomass of V. asiatica increased the oxidative stress in an individual plant when the initial Chl a concentration in the water reached 222 and 262 μg/L, as expressed by the increased MDA in leaves, compared with low biomass of K asiatica. This provides a basis for controlling cyanobacterial concentrations and V. asiatica biomass for the recovery of V. asiatica in eutrophic Lake Taihu.
