Objectives:This study aimed to study the characteristics of in-hospital deterioration in patients with congenital heart disease who required rapid response system activation and identify risk factors associated with 1-month ***:We retrospectively analysed data from a Japanese rapid response system registry with 35 participating *** included consecutive patients with congenital heart disease who required rapid response system activation between January 2014 and March *** regression analyses were performed to examine the associations between 1-month mortality and other patient-specific ***:Among 9,607 patients for whom the rapid response system was activated,only 82(0.9%)had congenital heart *** few patients with congenital heart disease were being treated at the cardiology and cardiovascular surgery departments(12.3%and 9.9%,respectively).Moreover,the incidences of rapid-response events after intensive care unit discharge or surgery were low(6.8% and 12.2%,respectively).The most common reason for rapid response system activation was respiratory dysfunction(desaturation:35.4%,tachypnoea:25.6%,and new dyspnoea:19.5%).Rapid response system interventions and intensive care unit transfers were required for 65.9% and 20.7%of patients,*** mortality rate was 1.2% at the end of the rapid response system intervention and 11.0% after 1 ***,decreased respiratory rate and decreased heart rate at rapid response system activation were associated with increased 1-month *** adjusted odds ratio was 1.10(95% confidence interval 1.02–1.19)and 1.02(95% confidence interval,1.00–1.04) for respiratory rate and heart rate,***:Rapid response systems were rarely activated after cardiac surgery and intensive care unit discharge,which were situations with a high risk of sudden deterioration in patients with congenital heart ***,encouraging the use of the rapid response sys

Intestinal Behcet＇s disease in a 38-year-old woman was diagnosed because of the history of recurrent oral aphthous ulcers, erythema nodosum-like eruptions, genital ulcer, and endoscopic findings of esophageal and Ueocolonic punched-out ulcers with colonic longitudinal ulcers. Esophageal lesions and colonic longitudinal ulcers are rarely seen in intestinal Behcet＇s disease. The ulcers of esophagus and ileocolon healed with 3 wk of treatment with prednisolone and mesalazine without any adverse effect. Mesalazine may decrease the total dose of prednisolone required to treat the disease.

Test Generation and Design-for-Testability Based on Acyclic Structure with Hold Registers

We present a method of test generation for acyclic sequential circuits with hold registers. A complete (100% fault efficiency) test sequence for an acyclic sequential circuit can be obtained by applying a combinational test generator to all the maximal time-expansion models (TEMs) of the circuit. We propose a class of acyclic sequential circuits for which the number of maximal TEMs is one, i.e, the maximum TEM exists. For a circuit in the class, test generation can be performed by using only the maximum TEM. The proposed class of sequential circuits with the maximum TEM properly includes several known classes of acyclic sequential circuits such as balanced structures and acyclic sequential circuits without hold registers for which test generation can be also performed by using a combinational test generator. Therefore, in general, the hardware overhead for partial scan based on the proposed structure is smaller than that based on balanced or acyclic sequential structure without hold registers.

AIM To evaluate the prognostic significance of perioperative carcinoembryonic antigen(CEA) and carbohydrate antigen 19-9(CA19-9) levels in stage Ⅱ/Ⅲ gastric *** From a multi-institutional retrospective database compiled by integrating clinical data from nine institutions, data of 998 patients who underwent curative resection for stage Ⅱ/Ⅲ gastric cancer between 2010 and 2014 were retrieved and analyzed. The prognostic impact of the preoperative and postoperative levels and chronological changes in CEA, CA19-9 and their combination were evaluated. To test whether postoperative adjuvant chemotherapy alters the prognostic impact of perioperative CEA and CA19-9 levels, the hazard ratios for mortality were compared between patients who underwent surgery alone and patients who underwent surgery followed by adjuvant *** The prognostic impact of postoperative CEA and CA19-9 was superior to that of the preoperative levels. Multivariable analysis identified high postoperative CEA and CA19-9 levels as independent prognostic factors for overall ***-free survival rates clearly decreased in a stepwise manner in association with postoperative CEA and CA19-9 levels, and patients with high levels of both markers showed significantly poorer prognosis than other patient groups. When we analyzed perioperative changes in serum CEA and CA19-9 levels, patients with high levels before and after surgery had the worst disease-free survival rates among all patient groups. Patients with normalized CEA levels after surgery had a significantly lower disease-free survival rate than those with normal perioperative levels, whereas patients with normalized CA19-9 levels after surgery had equivalent survival to those with normal perioperative levels. The prognostic impact of high CEA levels was observably smaller in patients who underwent adjuvant chemotherapy than in patients who underwent surgery alone, whereas that of high CA19-9 was greater in patients who

Establishing a Calculation Method for Estimating the Normal Circumference of the Proximal Interphalangeal Finger Joints

Objective: To estimate the normal circumference of the interphalangeal (IP) thumb joint and the proximal IP (PIP) joints from the index finger to the little finger using individual physical parameters such as body height and body weight. Methods: The maximum size on the ring gauge was recorded which caused resistance when the IP joint of the thumb or PIP joints of the index to small fingers were passed through the gauge. RG was defined as the ring gauge number with the same diameter as the circumference of the IP thumb joint and PIP finger joints from the index finger to the little finger. There were 300 healthy subjects and 600 hands. Univariate and multivariate analyses were used to analyze the relationships between RG and the parameters of age, sex, height, weight, and dominant hand, and regression equations were derived. Results: Age, sex and body weight were all statistically significant predictive factors of RG in all fingers. R2, the coefficient of determination in the regression equation, was almost 0.7 for all fingers, indicating that a moderate or strong correlation was observed between RG and the regression equation. Conclusions: The circumference of the IP thumb joint and PIP finger joints from the index finger to the little finger can be estimated in healthy individuals using a calculation formula that takes age, sex, body height and body weight into account.

Efficacy of MRP8/14 as a Marker of Disease Activity in Rheumatoid Arthritis

Objective: Early and accurate evaluation of the presence and activity of synovitis is extremely important in the diagnosis and treatment of rheumatoid arthritis. Myeloid related protein 8/14 (MRP8/14), also known as calprotectin or S100A8/A9 is considered as a sensitive marker for local inflammatory activity in rheumatoid arthritis. The aim of this study is to demonstrate the efficacy of MRP8/14 as a marker of disease activity in RA. Methods: Thirty-one patients with diagnosis of RA who received treatment without biological drugs at our institution were included in this study. Serum MRP8/14, CRP and MMP-3 were tested in all patients. Disease activity was evaluated using DAS28-CRP and SDAI. Ultrasonography was performed on the wrists and MCP joints of both hands using semi-quantitative scale of power Doppler signal. The sum of scales in joints was calculated as the PD score. The correlation of MRP8/14 with serum biomarkers, disease activity and ultrasonography examination was investigated. Result: Serum MRP8/14 was strongly correlated with CRP (r = 0.63) and MMP-3 (r = 0.69). A correlation was observed between serum MRP8/14 and DAS28-CRP (r = 0.53) and SDAI (r = 0.66). No significant correlation was found between PD scores and MRP8/14. Conclusion: This study demonstrated that MRP8/14 is correlated with evaluated disease activity and markers of serum inflammatory response in patients not using biological drugs. MRP8/14 is considered an effective new method for objective evaluation of synovitis in RA.

健康成人吞咽液体过程中舌压变化的研究

目的研究健康成人生理性吞咽液体过程中的舌压变化规律,为临床评估吞咽功能和诊断吞咽障碍提供可能.方法 13名健康成年男性（24～31岁）志愿者直立坐位吞咽5 ml水,用舌压传感器测量吞咽过程中舌与硬腭中线前、中和后部以及硬腭两侧后部接触产生舌压的变化,同时将麦克风记录吞咽音的发生时间做为参考时间.比较吞咽时各位点舌压变化相关参数.结果正常吞咽时,硬腭中线上舌压由前向后逐渐发生[（-0.40±0.22）s、（-0.36±0.21）s、（-0.24±0.18）s],硬腭两侧后部舌压[（-0.38±0.23）s、（-0.40±0.23）s]与硬腭中线前部舌压产生时间相似（P＞0.05）;以上各位点舌压峰值出现时间[（-0.12±0.24）s、（-0.16±0.22）s、（-0.13±0.21）s、（-0.16±0.23）s、（-0.17±0.23）s]接近（P＞0.05）,且早于吞咽音发生时间（P＜0.05）,舌压消失时间接近[（0.32±0.23）s、（0.27±0.21）s、（0.23±0.16）s、（0.33±0.31）s、（0.33±0.29）s]（P＞0.05）,且晚于吞咽音发生时间（P＜0.05）.无论是舌压持续时间、舌压峰值时间、舌压回落时间、舌压峰值及舌压积分值,均是腭中线后部的数值最小,而腭后部双侧上述参数相似（P＞0.05）.结论吞咽时舌与硬腭接触各位点产生的舌压存在精确协调性;舌压传感器为无创简单评估吞咽功能和诊断吞咽障碍提供了可能.

目的研究吞咽过程中舌压传感器的应用是否对屈曲传感器所记录的喉部运动波形产生影响.方法选取12名健康成年男性为受试者,嘱所有受试者端坐位吞咽5 m1水,比较同一受试者在同时贴附属曲传感器和舌压传感器以及仅贴附属曲传感器而不贴附舌压传感器两种情况下吞咽屈曲传感器的记录参数.结果统计显示,无论贴附舌压传感器与否,屈曲传感器所记录吞咽过程中喉部运动波形的同一标志时间点彼此之间的差异均无统计学意义（P＞0.05）,而且屈曲传感器记录的喉部运动各时期持续时间也不因贴附舌压传感器而发生变化（P＞0.05）.结论吞咽时舌压传感器的贴附不影响屈曲传感器记录的喉部运动,为将来同步应用舌压传感器和屈曲传感器测定吞咽过程中舌压生成与喉部舌骨运动生理协同性研究提供了基础.
