Prospective controlled study on the effects of polyethylene glycol in capsule endoscopy

Risk factors for bleeding after endoscopic submucosal dissection for gastric lesions

AIM:To assess risk factors for bleeding after gastric endoscopic submucosal dissection(ESD) and to develop preventive ***:This retrospective study was performed in a tertiary referral center.A total of 328 patients underwent ESD for 398 gastric neoplasms between July 2007 and June *** main outcome was association between post-ESD bleeding and the following:age;sex;comorbidities;daily use of medicine potentially related to gastric injury/bleeding;location,size,and histological depth of lesions;ulceration;experience of operator coagulating the ulcer floor,and duration of *** also determined the relationship between the location of post-ESD bleeding and risk factors for ***:Univariate analysis revealed significant risk factors:tumor location [odds ratio(OR),2.86;95% CI:1.21-6.79,P=0.024],coagulator experience(OR,4.29;95% CI:1.43-12.86,P=0.009),and medicine potentially related to gastric injury/bleeding(OR,2.80;95% CI:1.14-6.90,P=0.039).Multivariate logistic regression analysis confirmed significant,independent risk factors:tumor in lower third of stomach(OR,2.47;95% CI:1.02-5.96,P=0.044),beginner coagulator(OR,3.93;95% CI:1.29-11.9,P=0.016),and medicine(OR,2.76;95% CI:1.09-6.98,P=0.032).We classif ied cases of post-ESD bleeding into two groups(bleeding at the ulcer margin vs bleeding at the center) and found that bleeding at the margin occurred more frequently with beginner coagulators compared with experts(OR,16.00;95% CI:1.22-210.59,P=0.040).CONCLUSION:Beginner coagulators,tumor in the antrum,and medicines were significant risk factors for post-ESD *** at the ulcer margin frequently occurred with beginner operators.

Impact of the Coronavirus Pandemic on Family Well-Being: A Rapid and Scoping Review

The coronavirus disease of 2019 (COVID-19) has had a serious impact not only on the society, economy, and medical system, but also on the families and family members affected by it. This scoping review aimed to describe the effects of the COVID-19 pandemic on family well-being. Original articles in English published between January 2020 and August 2021 that examined the association between COVID-19 and family well-being, were searched on MEDLINE and CINAHL. The literature search was conducted using Mesh or CINAHL Subject Headings on COVID-19 and families. Of the 923 references extracted from MEDLINE and CINAHL, this review included 25 references based on exclusion criteria. The largest number of articles examined the impact of COVID-19 on family caregivers’ mental health, followed by its impact on family relationships. The pandemic reportedly worsened family relationships and functioning, increasing domestic violence. The increased burden of caregiving for children and older adults due to COVID-19 was a risk factor for poor physical, psychological, and social health among family caregivers. The workplace environments and health conditions of workers involved with COVID-19 patients affected their family members’ physical, psychological, and social health. The social and economic impact of the pandemic could change the internal family system and the permeability of its boundaries, necessitating strategies to maintain an open family system. Additionally, family caregivers are at high risk for poor mental health and need a provision of psychosocial support. Moreover, devising strategies to improve workplace environments and alleviate health issues of workers involved with COVID-19 patients would be crucial for better mental health among their family members.

Characteristic features of ghrelin cells in the gastrointestinal tract and the regulation of stomach ghrelin expression and production

Ghrelin was isolated as an endogenous ligand for the GH secretagogue receptor from the rat stomach. Although physiological effects of ghrelin have been revealed by numerous studies, the regulation of stomach ghrelin remains obscure, and the factor that directly regulates ghrelin expression and production has not been identified. Here, we show some data regarding the characteristic features of ghrelin cells and the regulation of stomach ghrelin. In the gastrointestinal tract, ghrelin cells were identified as opened- and closed-type cells, and it was found that the number of ghrelin cells decreased from the stomach to the colon. The postnatal change in number of ghrelin cells in the stomach showed a sexually dimorphic pattern, indicating a role of estrogen in the regulation of stomach ghrelin. In vitro studies revealed that estrogen stimulated both ghrelin expression and production and that treatment with formestane, an aromatase (estrogen synthetase) inhibitor, decreased ghrelin expression level. On the other hand, leptin was found to inhibit both basal and estrogen-stimulated ghrelin expression. Moreover, both aromatase mRNA- expressing cells and leptin cells were found to be located close to ghrelin cells in the gastric mucosa. Furthermore, we found an inverse relationship between gastric ghrelin and leptin levels in a fasting state, and we revealed relative changes in expression of gastric ghrelin, estrogen and leptin in the postnatal rats. We propose that gastric estrogen and leptin directly regulate stomach ghrelin and that the balance control through gastric estrogen and leptin contributes to the altered ghrelin expression level in some physiological states.

Safety and efficacy of surgical treatments for axillary osmidrosis: a retrospective cohort study comparing conventional open excision with cartilage-shaver closed curettage

Preparation of hemicellulolic oligosaccharides from Chamaecyparis obtuse （Hinoki） slurry using commercial enzymes

Wood biomass is anticipated to serve as a substitute for carbon source, which has no feedstock competition with foods. Biomass is commonly used for the production of bio-ethanol by a series of processes such as pretreatment, enzymatic degradation, and fermentation. Hemicellulose, constituting 20 wt-% - 40 wt-% ofbiomass materials, contains various kinds of saccharides known to be bioactive substrates. Practical usage of hemicellulose is generally limited to its conversion to bio-ethanol. Here, we aimed to prepare hemicellulolic oligosaccharides, more valuable products other than ethanol. Therefore, the Hinoki slurry was treated with lime at room temperature for 3 h, and then neutralized with HCI. The resulting sample was treated with 13 types of commercial enzymes, and the saccharides produced in the supernatant were evaluated. The result showed that the commercial enzyme Cellulase SS （Nagase ＆ Co., LTD.） effectively degraded the slurry to produce disaccharides and trisaccharides. Analysis of sugar components by liquid chromatography/mass spectrography （LC/MS） after the derivation with ethyl 4- aminobenzoate （ABEE） showed that mannobiose, manno- Nose, and cellobiose were the major oligosaccharides. These results indicate valuable oligosaccharides can be successfully produced from Hinoki softwood slurry.

Book Review of “The Secret of Organogermanium―Amazing Power to Renew Life”

The organogermanium (Ge-132), namely Asaigermanium (Ge-132) was synthesized by Dr. Kazuhiko Asai about over 50 years ago (1967). This Ge-132 has important functions to cure the patients with many serious diseases like cancers and also has functions to activate the physiological actions of microorganisms and organisms like bacteria, animals/humans and plants. Namely, the Ge-132 plays a very important role in many kinds of organisms. At present, we are in COVID-19 crisis. In order to ease the crisis, the governments in the world vaccinate people against COVID-19. However, this vaccination seems to be not every. The Ge-132 as a different treatment for COVID-19 will probably have a good effect on COVID-19. Its probability can be judged from our experiments/experience and evidential data by researchers who have gone before ourselves. The probable contents are described in this book.

Prospective randomized controlled trial investigating the type of sutures used during hepatectomy

AIM: To determine whether absorbable sutures or non-absorbable sutures are better in preventing surgical site infection (SSI), in this paper we discuss the results of a randomized clinical trial which examined the type of sutures used during hepatectomy. METHODS: All hepatic resections performed from January 2007 to November 2008 at the Department of Surgery at Iizuka Hospital in Japan were included in this study. There were 125 patients randomly assigned to an absorbable sutures (Vicryl) group or non-absorbable sutures (Silk) group. RESULTS: SSI was observed in 13.6% (17/125) patients participating in this study, 11.3% in the Vicryl group and 15.8% in the Silk group. Incisional SSI including superficial and deep SSI, was observed in 8% of the Vicryl group and 9.5% of the Silk group. Organ/ space SSI was observed in 3.2% of the Vicryl group and 6.0% of the Silk group. There were no significant differences, but among the patients with SSI, the period for recovery was significantly shorter for the Vicryl group compared to the Silk ***: The incidence of SSI in patients receiving absorbable sutures and silk sutures is not significantly different in this randomized controlled study; however, the period for recovery in patients with SSI was significantly shorter for absorbable sutures.

Caterpillars(Lepidoptera and Symphyta larvae)employ diverse visual defensive tactics,and effectiveness of such tactics may be highly dynamic across time due to seasonal changes in the predator assemblages and their ***,this has rarely been studied especially in tropical *** we assessed temporal changes in the defensive value of caterpillar color and shape,using six types of plasticine dummy caterpillars:three colors(green,black,and white)x two shapes(curled and straight).These dummy caterpillars were deployed five times over different seasons in tropical forests of Xishuangbanna(China)and,as a comparison,twice in a temperate forest of Hirosaki(Japan).The colors and shapes of dummy caterpillars simulate visual traits of black sawfly larvae which take the curled resting posture in tropical rainforests of Xishuangbanna,apparently masquerading excrements commonly found on plants,while in Hirosaki there is no black-curled sawfly larvae and few excrements on *** found no significant effects of caterpillar colors or shapes on predation in *** contrast,black and curled caterpillars received significantly lower predation by birds in Xishuangbanna constantly across ***,we were unable to provide evidence that the black-curled sawfly larvae are masquerading as *** of the dummy caterpillars also affected the predation by ants and parasitoid wasps at certain *** is the first report on ecological function of the curled posture of sawfly larvae,and we demonstrated the importance to assess the temporal dynamics of predation and effectiveness of defensive tactics in tropical forests.
