Efficacy and Safety of Ultrasound Guided Vacuum-Assisted Excision of Fibroadenomas: A Systematic Review

Background: Ultrasound-guided, vacuum-assisted excision [UGVAE] of fibroadenomas is an emerging minimally invasive procedure. UGVAE is being increasingly employed by breast radiologists and surgeons. It has been approved for this use since 2006 by the National Institute for Health and Clinical Excellence in the UK. The aim of this systematic review is to evaluate the efficacy and safety of this procedure based on the up to date available evidence. Methods: A comprehensive literature search of the Cochrane library, Medline, Embase, PubMed, Google scholar, Trip-database, Internet, and the reference lists of relevant articles was performed. We searched for literature with sizeable cases of therapeutic vacuum-assisted excision of fibroadenomas. All prospective studies with more than 20 study participants were screened. The study quality was assessed using the Grading of Recommendations Assessment, Development, and Evaluation [GRADE] approach. All were in English. Results: Of the four case series that fulfilled the inclusion criteria there were 516 patients in which 520 procedures were performed. Overall complete excision rate was 89.4%. In two of the studies, this was 100% when the lesions were less than 1.5 cm but reduced to 61% when between 1.5 and 2 cm. Most common significant complication was bleeding with 2 cases of pneumothorax. None of the studies can be regarded as high quality going by GRADE approach. Conclusion: Within the limit of the current published case series, UGVAE appears to be an effective and safe procedure for excision of small fibroadenomas of less than 15 mm in size. A well designed case control study or randomized controlled trials with a sufficient sample size is needed to further assess its safety, effectiveness and more importantly, patients’ satisfaction especially in larger sized fibroadenomas.

Amino acid quality indices of the leaves of Clerodendrum volubile

Objective:To evaluate the amino acid profile and quality indices of Clerodendrum volubile(***)***:Dried leaves of *** were blended,defatted and subjected to amino acid analysis using the technicon sequential multi-sample amino acid *** amino acid quality indices which covers for chemical score,essential amino acid index,nutritional index,true digestibility,protein digestibility corrected amino acid score,and digestible indispensable amino acid score were evaluated using standard ***:Amino acid analysis revealed glutamic acid to have the highest concentration,with cysteine having the *** acid had the highest chemical score,this was followed by glycine,histidine and arginine,*** least scores were observed in serine and *** acid had the highest value for true digestibility and protein digestibility corrected amino acid score,with the least observed in *** indispensable amino acid score evaluation showed histidine to have the highest value for infants(birth to 6 months),threonine for children(6 months to 3 years),while isoleucine was observed to have the highest value for older children,adolescents and *** essential amino acid index value was less than 4,while nutritional index value was less than ***:These results indicated the leaves of *** as a potential source of amino acids in the human diet as portrayed by its amino acids profile and qualities.

The Effect of Electrolyte on Dye Sensitized Solar Cells Using Natural Dye from Mango (<i>M. indica</i>L.) Leaf as Sensitizer

Dye-sensitized solar cells (DSSC) were fabricated with mango leaf dye extracts as natural dye sensitizers at pH value of 5.20 and temperature of 18.1°C. Methanol was used as dye-extracting solvent. DSSCs from dye extract of M. indica L. with KMnO4 electrolyte had the highest photocurrent density of 1.3 mA/cm2 and fill factor FF of 0.46 for the sun at its peak. Potassium permanganate (KMnO4) had a photocurrent density of 1.3 mA/cm2 and FF of 0.8 at sundown. Potassium Iodide (KI), Potassium Bromide (KBr) and Mercury Chloride (HgCl2) electrolytes had 0.2 mA/cm2, 0.08 mA/cm2 and 0.02 mA/cm2 photocurrent densities respectively. The fill factors of 0.09, 0.03 and 0.003 respectively for sun overhead while 0.08 mA/cm2, 0.01 mA/cm2 and 0.01 mA/cm2 were the values of photocurrent densities respectively at sundown. The fill factors were 0.02, 0.0006 and 0.003 respectively at sundown. The maximum power Pmax of the DSSCs were 0.5 mW/cm2, 0.10 mW/cm2, 0.01 mW/cm2 and 0.012 mW/cm2 respectively at 1300 h at 1630 h 0.9 mW/cm2, 0.14 mW/cm2, 0.005 mW/cm2 and 0.0015 mW/cm2 respectively.

Open Prostatectomy in the Management of Benign Prostate Hyperplasia in a Developing Economy

Background: Open prostatectomy (OP) is still relatively common in developing countries and remains a useful benchmark against which the minimal access surgical techniques are compared. This study aims to document the indications and outcomes of OP in a typical developing economy. Method: The records of patients with benign prostate hyperplasia (BPH) who had OP in our university teaching hospital between July 2004 and June 2014 were retrospectively reviewed. Some analyzed parameters include the demographic characteristics, indications, pre-operative work-up, anaesthetic techniques, OP type, complications, histopathology results and follow-up duration. Results: A total of 247 cases were studied. Mean age was 67 years while the commonest surgery indication was recurrent acute urinary retention. Average prostate specific antigen (PSA) was 8.4 ng/ml while hypertension was the most common comorbidity (44.1%). Regional anaesthesia was mainly used (79.4%) while retropubic prostatectomy was the commonest OP type done (58.7%). The enucleated specimen weighed above 60 g in 91.9% of cases. All our patients were able to micturate spontaneously with urine stream above 20 mls/second on follow-up one week after discharge. Mean duration of hospital admission and follow-up were 7 days and 9 months respectively. Complications occurred in 90 patients (36.4%), of which surgical site infections were the commonest (9.8%). There was 0.4% mortality. Histopathology results showed BPH (95.5%), (incidental) prostate adenocarcinoma (2.4%) or prostatic intra-epithelial neoplasia (2.1%). Conclusion: OP remains an important therapeutic option for management of BPH in developing countries partly due to relatively large prostate size and presence of BPH complications from late presentationin many patients. The surgery is efficient and has relatively low morbidity and minimal mortality.

Improving Diabetes Care by Data Feedback and Diabetes Templates

Importance: Health information technology has been used to improve diabetes care and outcomes. With the implementation of our diabetes registry, we discovered several flaws in the data. Objective: The aim of this paper is to demonstrate whether improving diabetes templates in electronic medical records associated with data feedback, improves process and quality outcomes for patients with diabetes. Methods: We redesigned our chronic diseases templates and clinical flow, built a diabetes registry and used the data for feedback to educate providers, staff and address inconsistencies. A total of 724 active diabetic patients were identified in October 2009 (pre-implementation) and 731 active diabetic patients were identified in June 2011 (post-implementation). Results: The results showed an improvement in the process outcomes of ordering hemoglobin A1C every 6 months and a microalbumin every 12 months (p-value 0.05). Discussion: Data feedback and lessons learned were instrumental to our practice change.

Neonatal Cutaneous Myiasis: A Mistaken Identity for Impetigo

Introduction: Myiasis is a parasitic infestation of the animal body by fly larvae that grow inside the host and feed from its tissue. Myiasis occurs when eggs laid by female flies come in contact with the human skin where they feed and develop into the larvae. Greater numbers are noted in the less-developed countries where environmental hygiene remains a problem. Case Report: A case of a previously healthy 10-day old female neonate delivered in a specialist hospital but was infested by the larval forms of the blowfly with over 50 papules widely distributed to all parts of the body excluding the diaper area which was initially mistaken for impetigo. Conclusion: This case is to heighten the index of suspicion in this category of babies especially in endemic settings and reiterate the need to handle newborn clothing with better care such as ironing especially where such articles are aired in the open.

Intrigues and Challenges Associated with COVID-19 Pandemic in Nigeria

Coronavirus disease 2019 (COVID-19) pandemic has emerged as a global health crisis, with 3,855,788 infected persons and 256,862 deaths worldwide as of May 9, 2020. In Nigeria, the first case of the pandemic was reported by the Nigerian Centre for Disease Control on February 27, 2020. Between the dates when the index case was reported and May 9, 2020, the nation has recorded a total of 4151 confirmed COVID-19 cases from 25,951 samples screened and 745 (18%) cases discharged with 128 deaths indicating a case fatality rate of 3.1%. Thirty-four (34) States and the Federal Capital Territory have recorded coronavirus disease. The most affected States in Nigeria is Lagos (epicentre of COVID-19) with 1764 cases, followed by 576 cases in Kano states and only one COVID-19 case in Anambra State 42 days since the last report of index case. Demographically, a total of 2828 male subjects have been infected representing 68% and 1323 female subjects representing 32%. The age group 31 - 40 years is mostly affected accounting for 24%. The number of people with travel history is 210 (5%), 947 (23%) contacts, 2618 (63%) without epidemiological link and 376 (9%) with an incomplete information. Nigeria is currently witnessing community transmission of COVID-19. Some observed issues aiding community transmission of COVID-19 in Nigeria are: the distrust of some Nigeria citizens towards government on COVID-19 management, poverty, religious beliefs, ignorance on face mask sharing, low level of informed populace, misconceptions, stigmatization of infected individuals, poor health facilities, inadequate testing Centre, shortage of health workers, poor treatment among others. Effective people’s health preventive behaviour and community-based health policy and strategies to mitigate these challenges are therein suggested.

Microbial Contents and Antibiotics Susceptibilities from Hand Washing Stations during COVID-19 Pandemic in Lagos, Nigeria

The COVID-19 pandemic put every government in the globe on red alert;safety protocol and long-standing infection prevention and control (IPC) measures such as hand and oral hygiene, social/ physical distancing, use of face mask were rigorously enforced. This study evaluated microbial (bacterial, fungal) contents of rinsate, bowl water, water from tap, and the neighborhood drainage in selected hand washing stations in Agege, Alimosho, Mainland, Island and Shomolu local government areas (LGAs) in Lagos, Nigeria. The identities of bacterial isolates were confirmed with 16S rRNA sequencing while the fungi were identified by colonial appearances. The antibiotics susceptibility testing (AST) of the bacteria against structurally unrelated antibiotics was performed and interpreted according to standard guidelines. Sixty-seven (67) bacterial and fifty-one (51) fungal isolates were recovered from mainly water bucket with tap and rinsates across all the LGAs. There were marked microbial loads (some too numerous to count at 10-3 dilution) across Alimosho, Lagos Island and Mainland. Forty (40) representative bacterial isolates were selected for 16S rRNA sequencing. The occurrence of microbial isolates in the samples was at varying degrees;Klebsiella spp. (37.5%), Bacillus spp. (32.5%), Enterobacter spp. (17.5%), were the predominant bacteria while Aspergillus spp. (46.2%), yeasts (34.6%), Sporothrix schenckii (11.5%) and Penicillium spp. (7.7%) constituted the fungal isolates. The biological weapon, B. anthracis was recovered from a water bucket with tap in Alimosho. The antibiotics susceptibility testing of the bacteria showed high degrees of resistance profile;45 (69.2%) to ampicillin, 41.5% amoxicillin/clavulanate while 47.6% were resistant to two (2) or more antibiotics. Antioxidant and chemical properties of essential oil extracted from blend of selected spices

Objective:To investigate the chemical properties of essential oil extracted from blends of selected Nigerian spices as well as its antioxidant protective potentials against free radical in ***:Essential oil was extracted from selected spices blend consisting of Monodora myristica,Myristica fragrans,Tetrapleura tetraptera,and Aframomum sceptrum using a Clevenger type *** obtained was subjected to phytochemical and gas chromatography-mass spectrometer analysis as well as analyzed for antioxidant activity which covers for 1,1-diphenyl-2 picrylhydrazyl,nitric oxide scavenging activities and reducing ***:Gas chromatography-mass spectrometer analysis revealed over 50 compoundfs withα-phellandrene being the most predominant compound(27.32%),which was followed by(-)-β-bourbonene(15.78%)and 5-(1-methylethyl)-α-phellandrene(11.80%).Phytochemical analysis showed high flavonoid content and a lower phenolic *** oil showed a dose like dependent effect on the1-diphenyl-2 picrylhydrazyl and nitric oxide scavenging activities,these activities increased with increasing *** same was also observed for the reducing power properties of the ***:The antioxidant activities exhibited by the essential oil in vitro signify its protective potential against free *** chemical constituents,α-phellandrene in particular and the studied phytochemicals may be responsible for these ***,in vivo study is needed to further authenticate this potency.

Direct Calculation of Unsteady-State Weymouth Equations for Gas Volumetric Flow Rate with Different Friction Factors in Horizontal and Inclined Pipes

Direct calculations of unsteady-state Weymouth equations for gas volumetric flow rate occur more frequently in the design and operation analysis of natural gas systems. Most of the existing gas pipelines design procedures are based on a particular friction factor and steady-state flow analysis. This paper examined the behavior of different friction factors and the need to develop model analysis capable of calculating unsteady-state gas flow rate in horizontal and inclined pipes. The results show different variation in flow rate with Panhandle A and Panhandle B attaining stability in accurate time with initial unsteadiness at the instance of flow. Chen and Jain friction factors have opposition to flow with high flow rate: The prediction also reveals that Colebrook-White degenerated to Nikuradse friction factor at high Reynolds number. The horizontal and inclined flow equations are considerably enhanced on the usage of different friction factors with the aid of Matlab to handle these calculations.
