The immune-stromal cell interactions play a key role in health and diseases. In periodontitis, the most prevalent infectious disease in humans, immune cells accumulate in the oral mucosa and promote bone destruction by inducing receptor activator of nuclear factor-κB ligand (RANKL) expression in osteogenic cells such as osteoblasts and periodontal ligament cells. However, the detailed mechanism underlying immune–bone cell interactions in periodontitis is not fully understood. Here, we performed single-cell RNAsequencing analysis on mouse periodontal lesions and showed that neutrophil–osteogenic cell crosstalk is involved in periodontitis-induced bone loss. The periodontal lesions displayed marked infiltration of neutrophils, and in silico analyses suggested that the neutrophils interacted with osteogenic cells through cytokine production. Among the cytokines expressed in the periodontal neutrophils, oncostatin M (OSM) potently induced RANKL expression in the primary osteoblasts, and deletion of the OSM receptor in osteogenic cells significantly ameliorated periodontitis-induced bone loss. Epigenomic data analyses identified the OSM-regulated RANKL enhancer region in osteogenic cells, and mice lacking this enhancer showed decreased periodontal bone loss while maintaining physiological bone metabolism. These findings shed light on the role of neutrophils in bone regulation during bacterial infection, highlighting the novel mechanism underlying osteoimmune crosstalk.

The advantage of endoscopic submucosal dissection(ESD)is that it is less invasive than surgery.ESD is one of the best treatments for older patients as surgery in this age group of patients is difficult.However,it is unclear how much lower the physical stress of ESD is compared with that of surgery.Thus,objective methods are required to assess physical stress in patients who have undergone ESD.The current review of ESD aimed to summarize the recent advancements in the assessment of physical stress during the perioperative period,focusing on changes in energy metabolism and serum opsonic activity(SOA).Based on metabolic changes,resting energy expenditure(REE)was measured using an indirect calorimeter.The stress factor calculated from the REE and the basal energy expenditure computed using the Harris-Benedict equation can be used to assess physical stress.SOA was assessed using the chemiluminescence method,wherein the use of chemiluminescent probes(i.e.,lucigenin and luminol)allowed quantification of reactive oxygen species generated by neutrophils.Using an auto luminescence analyzer,the results were evaluated based on the maximum light emission and area under the emission curve.These quantifiable results revealed the minimal invasiveness of ESD.

AIM: To investigate the safety and utility of an electrocautery dilation catheter for endoscopic ultrasonography(EUS)-guided pancreatic fluid collection drainage.METHODS: A single-center, exploratory, retrospective study was conducted between August 2010 and August 2014. This study was approved by the Medical Ethics Committee of our institution. Informed, written consent was obtained from each patient prior to the procedure. The subjects included 28 consecutive patients who underwent EUS-guided transmural drainage(EUS-TD) for symptomatic pancreatic and peripancreatic fluid collections(PFCs) by fine needle aspiration using a 19-gauge needle. These patients were retrospectively divided into two groups based on the use of an electrocautery dilation catheter as a fistula dilation device; 15 patients were treated with an electrocautery dilation catheter(electrocautery group), and 13 patients were treated with a non-electrocautery dilation catheter(non-electrocautery group). We evaluated the technical and clinical successes and the adverse events associated with EUS-TD for the treatment of PFCs between the two groups.RESULTS: There were no significant differences in age, sex, type, location and diameter of PFCs between the groups. Thirteen patients(87%) in the electrocauterygroup and 10 patients(77%) in the non-electrocautery group presented with infected PFCs. The technical success rates of EUS-TD for the treatment of PFCs were 100%(15/15) and 100%(13/13) for the electrocautery and the non-electrocautery groups, respectively. The clinical success rates of EUS-TD for the treatment of PFCs were 67%(10/15) and 69%(9/13) for the electrocautery and the non-electrocautery groups, respectively(P = 0.794). The procedure time of EUS-TD for the treatment of PFCs in the electrocautery group was significantly shorter than that of the non-electrocautery group(mean ± SD: 30 ± 12 min vs 52 ± 20 min, P < 0.001). Adverse events associated with EUS-TD for the treatment of PFCs occurred in 0 patients and 1 patient for the electrocautery

Percutaneous ablation under imaging guidance is a curative treatment that can induce complete tumor necrosis with advantages of minimal invasiveness and a low risk of complications.Thermal ablation,which includes radiofrequency ablation and microwave ablation,is a representative technique that has sufficient antitumor effects in cases of hepatocellular carcinoma with≤3 lesions measuring≤3 cm and preserved liver function.The short-and long-term outcomes of patients are comparable with those achieved with surgical resection.Despite their nonmalignant nature,some benign liver tumors require treatment for symptoms caused by the presence of the tumor and/or continuous enlargement.Ablation may be the treatment of choice because it has lower burden on patients than surgical treatment.This review describes the recent concepts,progress,and limitations of ablation-based treatment for benign liver tumors.

The effects of alternating current poling(ACP)at 80℃on electrical properties of[001]-oriented 0.72 Pb(Mg_(1/3)Nb_(2/3))O_(3)-0.28PbTiO_(3)(PMN-28PT)single crystals(SCs)have been investigated.The square-wave ACP SCs poled at high voltage(HV,5 kV_(rms)/cm)occasionally showed large fluctuations and low opposite values of piezoelectric coefficient(d_(33)=±1370 pC/N)in one plate.This revealed spuriousmode vibrations(SMV)of impedance spectrum.However,after depolarizing and repolarizing the sample with a sine-wave ACP at low voltage(LV,3.5 kV_(rms)/cm),the d_(33) enhanced to be 1720 pC/N(+26%)and did not exhibit large fluctuation or opposite values in one plate any more.The impedance spectrum became clean and the abnormal SMV disappeared.We proposed four possible mechanisms of the SMV,and speculate that the main cause maybe by macro-scale sub-domain structure and/or phase change in the main domain structure and/or phase in the SC plate due to the specific poling conditions not eternal mechanical damage of PMN-PT SCs.This study will be useful to realize a high d_(33) and improve other properties of PMN-PT ACP SC ultrasonic transducers without any SMV for high-frequency medical imaging equipment.

Surgical outcome comparisons of multifocal IOLs of Lentis Comfort LS-313 MF15 and Tecnis Eyhance DIB00V

AIM:To compare the surgical outcomes of a multifocal intraocular lens(IOL;Lentis Comfort LS-313 MF15)with those of an enhanced monofocal IOL(Tecnis Eyhance DIB00V).METHODS:This retrospective study included patients who underwent cataract surgery with LS-313 MF15 or Eyhance IOL implantation.Data regarding patient demographics,surgical records,and ophthalmic examination before the cataract surgery and one and three months postoperatively were collected.Visual acuities,refractive values,defocus curves,contrast sensitivities and subjective symptoms were evaluated.RESULTS:Among the 71 eyes(47 patients)included in this study,32 eyes(20 patients)underwent LS-313 MF15 IOL implantation,and 39 eyes(27 patients)underwent Eyhance IOL implantation.No significant differences were observed in age,axial length,or refractive error between the two groups preoperatively.Furthermore,the distancecorrected and uncorrected distance visual acuities one month postoperatively did not differ between the groups,and both groups had sufficient visual acuities at the distances of 5,1 m,70,50,and 30 cm.Other ophthalmic data,including subjective symptoms based on the 14-item Visual Function Index Questionnaire,monocular defocus curves,contrast sensitivities,and halo and glare,did not differ between the groups three months postoperatively.Moreover,both groups had good outcomes.The spherical equivalent one month postoperatively was significantly myopic in the LS-313 MF15 group compared with that in the Eyhance group(P=0.033);however,this difference was not observed three months postoperatively(P=0.471).CONCLUSION:Comparison of the surgical outcomes of LS-313 MF15 with those of Eyhance with different optical properties reveal that both IOLs show good postoperative outcomes,with no significant differences being noted between the two IOLs.

Amelanotic malignant melanoma of the esophagus:Report of two cases with immunohistochemical and molecular genetic study of KIT and PDGFRA

The author reports herein two cases of amelanotic malignant melanoma of the esophagus.Case 1 is an 87-year-old woman who was admitted to our hospital because of nausea and vomiting.Endoscopic examination revealed an ulcerated tumor of the distal esophagus,and a biopsy was taken.The biopsy showed malignant polygonal and spindle cells.No melanin pigment was recognized.Immunohistochemically,the tumor cells were positive for melanosome(HMB45),S100 protein,KIT and Platelet derived growth factor receptor-α(PDG-FRA).The patient was treated by chemotherapy and radiation,but died of systemic metastasis 12 mo after the presentation.Case 2 is a 56-year-old man presenting with dysphagia.Endoscopic examination revealed a polypoid tumor in the middle esophagus,and a biopsy was obtained.The biopsy showed malignant spindle cells without melanin pigment.Immunohistochemically,the tumor cells were positively labeled for melanosome,S100 protein,KIT and PDGFRA.The patient refused operation,and was treated by palliative chemotherapy and radiation.He died of metastasis 7 mo after the admission.In both cases,molecular genetic analyses of KIT gene(exons 9,11,13 and 17) and PDGFRA gene(exons 12 and 18) were performed by the PCR direct sequencing method,which showed no mutations of KIT and PDGFRA genes.This is the first report of esophageal malignant melanoma with an examination of the expression of KIT and PDGFRA and the mutational status of KIT and PDGFRA genes.

The CCR4-NOT complex is a highly conserved,multifunctional machinery controlling mRNA metabolism.Its components have been implicated in several aspects of mRNA and protein expression,including transcription initiation,elongation,mRNA degradation,ubiquitination,and protein modification.In this review,we will focus on the role of the CCR4-NOT complex in mRNA degradation.The complex contains two types of deadenylase enzymes,one belonging to the DEDD-type family and one belonging to the EEP-type family,which shorten the poly(A)tails of mRNA.We will review the present state of structure-function analyses into the CCR4-NOT deadenylases and summarize current understanding of their roles in mRNA degradation.We will also review structural and functional work on the Tob/BTG family of proteins,which are known to interact with the CCR4-NOT complex and which have been reported to suppress deadenylase activity in vitro.

Primary esophageal combined carcinoma is very rare.The authors herein report 2 cases.Case 1 was a combined squamous cell carcinoma and small cell carcinoma,and case 2 was a combined squamous cell carcinoma,adenocarcinoma,and small cell carcinoma.Case 1 was a 67-year-old man with complaints of dysphagia.Endoscopic examination revealed an ulcerated tumor in the middle esophagus,and 6 biopsies were obtained.All 6 biopsies revealed a mixture of squamous cell carcinoma and small cell carcinoma.Both elements were positive for cytokeratin,epithelial membrane antigen,and p53 protein,and had high Ki-67 labeling.The small cell carcinoma element was positive for synaptophysin,CD56,KIT,and platelet-derived growth factor-α(PDG-FRA),while the squamous cell carcinoma element was not.Genetically,no mutations of KIT and PDGFRA were recognized.The patient died of systemic carcinomatosis 15 mo after presentation.Case 2 was a 74-year-old man presenting with dysplasia.Endoscopy revealed a polypoid tumor in the distal esophagus.Seven biopsies were taken,and 6 showed a mixture of squamous cell carcinoma,small cell carcinoma,and adenocarcinoma.The 3 elements were positive for cytokeratins,epithelial membrane antigen,and p53 protein,and had high Ki-67 labeling.The adenocarcinoma element was positive for mucins.The small cell carcinoma element was positive for CD56,synaptophysin,KIT,and PDGFRA,but the other elements were not.Mutations of KIT and PDGFRA were not recognized.The patient died of systemic carcinomatosis 7 mo after presentation.These combined carcinomas may arise from enterochromaffin cells or totipotential stem cell in the esophagus or transdifferentiation of one element to another.A review of the literature was performed.
