目的探讨过敏性紫癜(Henoch-Schonlein purpura,HSP)患儿合并肺炎支原体(Mycoplasma pneumoniae,MP)感染的临床表现及实验室检查特点,分析此类患儿的诊治策略及预后情况。方法对本院就诊的112例HSP患儿的临床资料进行回顾性分析,同时选取70例体检健康儿童为对照组。检测各组儿童MP感染情况,对比分析不同分组患儿MP阳性率。采集患儿静脉血,测定th17细胞和treg细胞在样本中的百分比,对比不同分组儿童免疫功能的差异。结果在112例HSP患儿中,35例感染MP,感染率31.25%。70例对照组儿童中只有8例感染MP,感染率11.43%,两组的MP感染率差异显著。在HSP患儿中,73例为首次发病,39例为2次或多次发病。首次发病组中MP阳性率为16.44%,而复发组中MP阳性率高达58.97%,两组之间MP阳性率差异显著。在65例男性HSP患儿中,20例为MP阳性,占30.77%;47例女性患儿中,15例为MP阳性,占31.91%。性别间MP阳性率无显著差异。1~5岁患儿MP阳性率为47.37%,5~9岁为32.26%,而>9岁为19.35%,不同年龄间MP阳性率无统计学差异。春季MP阳性率为53.85%,夏季为13.33%,秋季为18.75%,冬季为33.33%,不同季节间MP阳性率差异显著。MP阳性患儿中,皮肤型占14.29%,腹型5.71%,关节型17.14%,肾型14.29%,混合型48.57%。MP阴性患儿中,皮肤型占10.39%,腹型22.08%,关节型23.38%,肾型3.9%,混合型40.26%。腹型和肾型在两组间差异显著(P0.05)。MP阳性患儿首发症状均为皮疹,腹痛47.71%,呕吐28.57%,消化道出血14.29%,关节肿痛57.14%。MP阴性患儿首发症状均为皮疹,腹痛37.66%,呕吐22.08%,消化道出血7.79%,关节肿痛32.47%。关节肿痛在两组间有显著差异(P0.05)。MP阳性患儿31.43%并发紫癜性肾炎,MP阴性患儿15.58%并发紫癜性肾炎,差异无统计学意义(P>0.05)。MP阳性组患儿th17细胞百分率为(3.64±0.56)%,treg细胞百分率为(10.57±1.04)%。MP阴性组患儿th17细胞百分率为(2.63±0.55)%,treg细胞百分率为(12.45±1.77)%。健康组儿童th17细胞百分率为(1.79±0.31)%,treg细胞百分率为(14.88±1.69)%。MP阳性组患儿th17细胞百分率显著高于MP阴性组患儿及健康组儿童,treg细胞百分率显著低于MP阴性组患儿及健康组儿童,差异有统计学意义(Pth17细胞比例升高,treg细胞比例下降,这可能与疾病的发展和严重程度有关。同时,MP感染在不同季节的阳性率差异提示环境因素可能对感染有一定影响。尽管在紫癜性肾炎的并发率上差异无统计学意义,但MP阳性患儿的腹型和肾型发病率较高,需引起临床重视。

Hyponatremia is a frequent complication of advanced cirrhosis with ascites associated with increased morbidity and mortality. It is caused by an impairment in the renal capacity to eliminate solute-free water and is considered to be related to persistent secretionof vasopressin despite low serum osmolality. this nonosmotic release of vasopressin is mediated by the autonomic nervous system, which senses the underfilling of arterial vascular component. this reduction of effective arterial blood volume is closely related to the development of ascites. Although the short-time effects of vasopressin V2 receptor antagonists(vaptans) on hyponatremia and ascites have been repeatedly reported, their effects on the long-term management of cirrhotic ascites have not been established yet. Considering that their effects on water diuresis and their safety are limited by severe underfilling state of patients, cautious approaches with adequate monitoring are needed to advanced cirrhosis. Proper indication, adequate doses and new possibility of combination therapy should be explored in the future controlled study. As hyponatremia is frequent obstacle to ascites management, judicious combination with low-dose diuretics may decrease the incidence of refractory ascites. Although vaptans show much promise in the treatment of advanced cirrhosis, the problem of high cost should be solved for the future.

this study investigated variations in nitrogen dioxide(NO_(2))levels in Ukraine during two significant periods:the COVID-19 pandemic lockdown in 2020 and the armed conflict with Russia in *** and reprocessed Sentinel 5P data products were utilized for the analysis.A machine learning model was employed to generate a business-as-usual NO_(2)time series that accounted for meteorological *** the nine most populous cities in Ukraine,during the lockdown in 2020 we observed a moderation of increases in NO_(2)levels during the lockdown compared to the pre-lockdown *** at the same months during the conflict period in 2022,we identified much more significant reductions in NO_(2)level in these cities,averaging 12.1%for original and 18.1%for reprocessed *** our examination of major urban areas,we observed reductions in NO_(2)levels in areas surrounding coal power plants damaged or destroyed by the *** the major urban areas in Ukraine,we conclude that changes in daily anthropogenic activities due to the conflict-related events had more substantial impacts on NO_(2)levels than did COVID-19 lockdown.

Effects of water saturation and loading rate on direct shear tests of andesite

For estimating the long-term stability of underground framework,it is vital to learn the mechanical and rheological characteristics of rock in multiple water saturation ***,the majority of previous studies explored the rheological properties of rock in air-dried and water saturated conditions,as well as the water effects on compressive and tensile *** this study,andesite was subjected to direct shear tests under five water saturation conditions,which were controlled by varying the wetting and drying *** tests were conducted at alternating displacement rates under three vertical *** results reveal that the shear strength decreases exponentially as water saturation increases,and that the increase in shear strength with a tenfold increase in displacement rate is nearly constant for each of the vertical *** on the findings of the shear tests in this study and the compression and tension tests in previous studies,the influences of both water saturation and loading rate on the Hoek-Brown failure criterion for the andesite was *** results indicate that the brittleness index of the andesite,which is defined as the ratio of uniaxial compressive strength to tensile strength,is independent of both water saturation and loading rate and that the influences of the water saturation dependence and the loading rate dependence of the failure criterion can be converted between each other.

Strigolactones(SLs)are plant hormones that regulate the branching of plants and seed germination stimulants of root parasitic *** root parasites are a great threat to agricultural production,the use of SL agonists could be anticipated to provide an efficient method for regulating root parasites as suicidal germination inducers.A series of phenoxyfuranone-type SL mimics,termed debranones,has been reported to show potent bioactivities,including reduction of the tiller number on rice,and stimulation of seed germination in the root parasite Striga *** exert both activities,two substituents on the phenyl ring of the molecules were important but at least a substituent at the 2-position must be an electron-withdrawing ***,little is known about the effect of the properties of the substituents at the 2-position on ***,we found that different substituents at the 2-position give different preferences for *** debranones were more effective than the others and SL agonist GR24 for inhibiting rice tiller but far less effective in the induction of *** ***,nitrile and methyl derivatives clearly stimulated the germination of *** *** their IC_(50) values were 100 times higher than that of GR24 in the receptor competitive binding assay,their physiological activities were approximately 1/10 of *** differences could be due to their uptake in plants and/or their physicochemical stability under our experimental *** findings could support the design of more potent and selective SL agonists that could contribute to solving big agricultural issues.

this paper addresses the conservation laws in finite brittle solids with microcracks. the discussion is limited to the 2-D cases. First, after considering the combination of the Pseudo-traction Method and the indirect Boundary Element Method, a versatile method for solving multi-crack interacting problems in finite plane solids is proposed, by which the fracture parameters （SIF and path-independent integrals） can be calculated with a desirable accuracy. Second, with the aid of the method proposed, the roles the conservation laws play in the fracture analysis for finite microcracking solids are studied. It is concluded that the conservation laws do play important roles in not only the fracture analysis but also the analysis of damage and stability for the finite microcracking system. Finally, the physical interpretation of the M-integral is discussed further. An explicit relation between the M-integral and the crack face area, i.e., M = GS, has been discovered using the analytical method, which can shed some light on the Damage Mechanics issues from a different perspective.

Strigolactones （SLs） are a novel class of plant hormones and rhizosphere communication signals, although the molecular mechanisms underlying their activities have not yet been fully determined. Nor is their application in agriculture well developed. the importance of plant hormone agonists has been demonstrated in both basic and applied research, and chemicals that mimic strigolactone functions should greatly facilitate strigolactone research. Here, we report our discovery of a new phenoxyfuranone compound, 4-Br debranone （4BD）, that shows similar activity to that of the major strigolactone （SL） analog GR24 in many aspects of a biological assay on plants. 4BD strongly inhibited tiller bud outgrowth in the SL-deficient rice mutant d10 at the same concentration as GR24, with no adverse effects, even dur- ing prolonged cultivation. this result was also observed in the Arabidopsis thaliana SL-deficient mutants max1, max3, and max4. However, the application of 4BD to the Arabidopsis SL-insensitive mutant max2 induced no morphological changes in it. the expression of SL biosynthetic genes was also reduced by 4BD treatment, probably via negative feed- back regulation. However, in a seed germination assay on Striga hermonthica, a root parasitic plant, 4BD showed far less activity than GR24. these results suggest that 4BD is the first plant-specific strigolactone mimic.

Gut-liver axis in liver cirrhosis: How to manage leaky gut and endotoxemia

A "leaky gut" may be the cutting edge for the passage of toxins, antigens or bacteria into the body, and may play a pathogenic role in advanced liver cirrhosis and its complications. Plasma endotoxin levels have been admitted as a surrogate marker of bacterial translocation and close relations of endotoxemia to hyperdynamic circulation, portal hypertension, renal, cardiac, pulmonary and coagulation disturbances have been reported. Bacterial overgrowth, increased intestinal permeability, failure to inactivate endotoxin,activated innate immunity are all likely to play a role in the pathological states of bacterial translocation. therapeutic approach by management of the gut-liver axis by antibiotics, probiotics, synbiotics, prebiotics and their combinations may improve the clinical course of cirrhotic patients. Special concern should be paid on anti-endotoxin treatment. Adequate management of the gut-liver axis may be effective for prevention of liver cirrhosis itself by inhibiting the progression of fibrosis.

the dynamical behavior of a localized traveling wave (LtW) in a binary fluid layer heated from below confined in a rectangular cell with the intermediate aspect ratio Γ=12 and the separation ratio ψ=-0. 11 is studied by using the two-dimensional numerical simulation of the full hydrodynamic equations. We find that a counterpropa-gating wave (CPW) occurs at the onset of convection and is modulated in space-time. the modulated traveling wave (MtW) plays an important role in the transition to the LtW state, and the LtW state depends on the fluid parameters and exists in a narrow interval of the reduced Rayleigh number. the transition from LtW to an extended traveling wave (EtW) or stationary overturning convection (SOC) state occurs when r exceeds a certain critical value. Our results agree with those observed in experiments.
