Ischemic heart disease,factor predisposing to barrett's adenocarcinoma: A case control study

AIM:to define the significance of ischemic heart disease(IHD)(stable angina to infarction) co-existance in barrett esophagus(BE) patients and patients with esophageal adenocarcinoma(AdE).MEtHODS: All BE/AdE patients in Blackpool-Wyre-Fylde area and trikala prefecture identified from medical records. Patient clinical details were obtained from hospital and General Practitioner records. Additional information was gathered from validated ***: Forty(33%) AdE and 83(19%) BE patients had IHD(P = 0.002). Eighteen(15%) AdE and 34(8%) BE patients had suffered a myocardial infarction(P = 0.03). three(3%) AdE and 7(2%) BE patients had severe heart failure(P = 0.82). thirty-nine(47%) BE with IHD and 8(20%) AdE patients with IHD consumed aspirin daily(P = 0.004). Seventh-seven(93%) BE patients with IHD and 36(90%) AdE patients with IHD were on statins(P = 0.86). Logistic regression analysis: AdE was more frequent in the elderly,with long termreflux,long BE and concurrent IHD(odds ratio: 2.086,P = 0.001) not consuming statins. Eighteen(22%) BE patients with IHD [16(84%) with myocardial infarction] vs 33(10%) without IHD died from non-neoplastic causes within 24 mo from BE diagnosis(P = 0.005). CONCLUSION: IHD is more prevalent in AdE than BE patients. Increased prevalence of AdE is related with the presence of myocardial infarction but not severe heart failure,possibly because patients with BE and se-vere IHD have low life expectancy.

Bacterial biota in reflux esophagitis and barrett's esophagus

AIM： to identify the bacterial flora in conditions such as barrett＇s esophagus and reflux esophagitis to determine if they are similar to normal esophageal flora. MEtHODS： Using broad-range 16S rDNA PCR, esophageal biopsies were examined from 24 patients [9 with normal esophageal mucosa, 12 with gastroesophageal reflux disease （GERD）, and 3 with barrett＇s esophagus]. two separate broad-range PCR reactions were performed for each patient, and the resulting products were cloned. In one patient with barrett＇s esophagus, 99 PCR clones were analyzed. RESULtS： two separate clones were recovered from each patient （total = 48）, representing 24 different species, with 14 species homologous to known bacteria, 5 homologous to unidentified bacteria, and 5 were not homologous （〈97% identity） to any known bacterial 16S rDNA sequences. Seventeen species were found in the reflux esophagitis patients, 5 in the barrett＇s esophagus patients, and 10 in normal esophagus patients. Further analysis concentrating on a single biopsy from an individual with barrett＇s esophagus revealed the presence of 21 distinct bacterial species. Members of four phyla were represented, including Bacteroidetes, Firmicutes, Proteobacteria, and Actinobacteria. Microscopic examination of each biopsy demonstrated bacteria in intimate association with the distal esophageal epithelium, suggesting that the presence of these bacteria is not transitory. CONCLUSION： these findings provide evidence for a complex, residential bacterial population in esophageal reflux-related disorders. While much of this biota is present in the normal esophagus, more detailed comparisons may help identify potential disease associations.

AIM: to examine an increased risk of esophageal adenocarcinoma is restricted to patients who develop barrett's esophagus or whether esophagitis per se is a risk factor for ***: A population-based cohort of patients with histological evidence of esophagitis without.barrett's esophagus was constructed using electronic pathology reports relating to all esophageal biopsies in Northern Ireland between 1993 and 1996. Person-years of followup and incident cases of esophageal cancer were calculated by linking the cohort to death files and the Northern Ireland Cancer Registry records. Standardized incidence ratios (SIR) were calculated for esophageal cancers (adenocarcinoma, squamous cell carcinoma (SCC), and histologically unspecified cancers).RESULtS: A total of 2 013 patients in the cohort provided 13 559 patient-years of follow-up (mean follow-up 6.7 years). None of the patients developed adenocarcinoma. three patients developed SCC, and six developed histologically unspecified cancers. the SIR for all esophageal cancers and for SCC were 2.73 (95%CI 1.25-5.19) and 2.93 (95%CI 0.61-8.59), respectively. In a sensitivity analysis in which all unspecified esophageal cancers were treated as adenocarcinomas, the SIR for adenocarcinoma was 2.64 (0.97-5.75).CONCLUSION: the risk of adenocarcinoma is not elevated in patients with histological evidence of esophagitis without.barrett's esophagus; however, these patients may have a moderately increased risk of *** studies are required to confirm these findings,which suggest that.barrett's esophagus, not esophagitis,is the key precursor lesion in the development of adenocarcinoma.

From April to July 2018,a data sample at the peak energy of the t(4 S) resonance was collected with the Belle Ⅱ detector at the SuperKEKB electron-positron *** is the first data sample of the Belle Ⅱ *** Bhabha and digamma events,we measure the integrated luminosity of the data sample to be(496.3±0.3±3.0) pb-1,where the first uncertainty is statistical and the second is *** work provides a basis for future luminosity measurements at Belle Ⅱ.

Risk factors for barrett’s oesophagus and oesophageal adenocarcinoma:Results from the FINBAR study

AIM:to investigate risk factors associated with barrett's oesophagus and oesophageal ***:this all-Ireland population-based case-control study recruited 224 barrett's oesophagus patients,227 oesophageal adenocarcinoma patients and 260 *** participants underwent a structured interview with information obtained about potential lifestyle and environmental risk ***:Gastro-oesophageal reflux was associated with barrett's [OR 12.0(95% CI 7.64-18.7)] and oesophageal adenocarcinoma [OR 3.48(95% CI 2.25-5.41)].Oesophageal adenocarcinoma patients were more likely than controls to be ex-or current smokers [OR 1.72(95% CI 1.06-2.81)and OR 4.84(95% CI 2.72-8.61)respectively] and to have a high body mass index [OR 2.69(95% CI 1.62-4.46)].No significant associations were observed between these risk factors and barrett's *** but not vegetables were negatively associated with oesophageal adenocarcinoma [OR 0.50(95% CI 0.30-0.86)].CONCLUSION:A high body mass index,a diet low in fruit and cigarette smoking may be involved in the progression from barrett's oesophagus to oesophageal adenocarcinoma.

Efficacy and safety of liquid nitrogen cryotherapy for treatment of barrett's esophagus

AIM to evaluate the efficacy and safety of liquid nitrogen cryotherapy as a primary or rescue treatment for BE,with and without dysplasia,or intramucosal adenocarcinoma (IMC).MEtHODS this was a retrospective,single-center study carried out in a tertiary care center including 45 patients with BE who was treatment-na?ve or who had persistent intestinal metaplasia(IM),dysplasia,or IMC despite prior ***'s mucosa was resected via EMR when clinically appropriate,then patients underwent cryotherapy until eradication or until deemed to have failed *** biopsies were taken at standard *** From 2010 through 2014,33 patients were studied regarding the efficacy of ***,29 patients (88%) responded to cryotherapy,with 84% having complete regression of all dysplasia and *** eradication of cancer and dysplasia was seen in 75% of subjects with IMC; the remaining two subjects did not respond to *** cryotherapy,15 patients with high-grade dysplasia (HGD) had 30% complete regression,50% IM,and 7% low-grade dysplasia (LGD); one subject had persistent *** eradication of dysplasia occurred in all 5 patients with *** 5 patients with IM,complete regression occurred in 4,and IM persisted in *** 136 cryotherapy sessions amongst 45 patients,adverse events included chest pain (1%),stricture (4%),and one gastrointestinal bleed in a patient on dual antiplatelet therapy who had previously undergone *** Cryotherapy is an efficacious and safe treatment modality for barrett's esophagus with and without dysplasia or intramucosal adenocarcinoma.

Gastric intestinal metaplasia is associated with gastric dysplasia but is inversely correlated with esophageal dysplasia

AIMto determine which clinical factors might be associated with gastric intestinal metaplasia (IM) in a North American population. MEtHODSPathology and endoscopy databases at an academic medical center were reviewed to identify patients with and without gastric IM on biopsies for a retrospective cohort study. Patient demographics, insurance status, and other clinical factors were reviewed. RESULtSFour hundred and sixty-eight patients with gastric IM (mean age: 61.0 years ± 14.4 years, 55.5% female) and 171 without gastric IM (mean age: 48.8 years ± 20.8 years, 55.0% female) were compared. the endoscopic appearance of atrophic gastritis correlated with finding gastric IM on histopathology (OR = 2.05, P = 0.051). Gastric IM was associated with histologic findings of chronic gastritis (OR = 2.56, P P = 0.015), gastric dysplasia (OR = 6.11, P = 0.038), and gastric cancer (OR = 6.53, P = 0.027). Histologic findings of barrett’s esophagus (OR = 0.28, P = 0.003) and esophageal dysplasia (OR = 0.11, P = 0.014) were inversely associated with gastric IM. tobacco use (OR = 1.73, P = 0.005) was associated with gastric IM. CONCLUSIONPatients who smoke or have the endoscopic finding of atrophic gastritis are more likely to have gastric IM and should have screening gastric biopsies during esophagogastroduodenoscopy (EGD). Patients with gastric IM are at increased risk for having gastric dysplasia and cancer, and surveillance EGD with gastric biopsies in these patients might be reasonable.

***’s esophagus with the intestinal predominant mucin phenotype is considered to have a higher malignant potential than that with the gastric predominant mucin *** purpose of this prospective study was to investigate the prevalence of and risk factors for barrett’s esophagus with the intestinal predominant mucin phenotype in patients undergoing *** and methods.A total of 1699 consecutive patients undergoing esophagogastroduodenoscopy were enrolled in the study.A targeted biopsy was performed when endoscopically observed columnar-appearing esophagus was stained with crystal *** sample,histologically evidenced as barrett’s esophagus,was immunohistochemically evaluated and categorized as of either gastric or intestinal predominant mucin *** the patients were requested to complete the structured questionnaire indicating their symptoms and food consumption *** of and risk factors for barrett’s esophagus with and without the intestinal predominant mucin phenotype were *** of 1668 patients,629(37.7%) were found to have endoscopic barrett’s *** 333 out of 1668 patients(19.9%) ,histological studies were diagnostic of barrett’s *** hundred and six of these 333 patients(31.8%) had the intestinal predominant mucin ***,male gender and the presence of hiatal hernia were confirmed by multivariate analysis as the independent predictors for the presence of barrett’s esophagus with the intestinal predominant mucin ***’s esophagus with the intestinal predominant mucin phenotype was immunohistochemically found in 6.4% of all study *** age,male gender and the presence of hiatal hernia were the risk factors for the presence of barrett’s esophagus with the intestinal predominant mucin phenotype.
