type 1 neurofibromatosis is an inherited multisystem neurocutaneous disease predisposing to tumors development. Serious skin and ophthalmologic complications, although rare, can occur throughout life. Furthermore in children, unawareness of early symptoms may delay diagnosis. We report the case of A.t. 8 years old, admitted for exophthalmosis and facial deformity dating back to the age of 2 years. the diagnosis of neurofibromatosis was suspected in the presence of light brown skin spots scattered all over the body and subcutaneous nodules. Ophthalmologic examination revealed bilateral exophthalmosis, eyelids neurofibromas, blepharoptosis, Lisch nodules, corneal edema, and optic atrophy. Head Ct scan clarified the nature and the extent of ophthalmologic lesions. treatment was symptomatic. Neurofibromatosis is rarely reported in children in our setting;it is probably under diagnosed. Clinicians should think of this diagnosis in presence of certain specific symptoms and make a clinical assessment.

In the Kokumbo sub-prefecture, groundwater extraction related to gold panning remains a major concern, not to mention the deterioration of its quality. Among the work carried out on water resources, no scientific interest has been shown in groundwater to characterise it. the objective of this study is to contribute to the knowledge of its physico-chemical quality. In situ measurements and physico-chemical analyses using an inductively coupled plasma optical emission spectrometer (ICP-OES) were carried out on five (5) human-powered pump (HPP) water samples and six (6) well water samples collected during low and high water seasons. the results show that the physico-chemical quality of the water, which is sometimes turbid, is satisfactory in terms of the mineralization of the borehole and well water, and the pH of the boreholes, while the temperatures of the two waters and the pH of the wells do not comply with WHO standards. the levels of major ions are recommended for consumption. the waters are classified as predominantly bicarbonate-calcium and magnesium (73%) in the dry season and in the flood season, with an equal split between bicarbonate-calcium and magnesium (45.5%) and chloride-calcium and magnesium (45.5%). the elimination of materials responsible for the turbidity of certain waters by managers or populations is essential for drinking water use. the risk linked to this element means that these turbid waters are not recommended for drinking water.

Ruthenium complexes present two states of oxidation that are Ru(II) and Ru(III). Both are assumed to present cytotoxic activity at ground state. On the purpose of highlighting their differences, DFt, tD-DFt and NBO have been performed at both Wb97xd/Lanl2dz and B3lyp/Lanl2dz levels. NBO program shows that both groups of ruthenium complexes present almost the same charge of Ru atom. Moreover, they display nearly the same structure of valence orbitals of the ruthenium. However, when it comes to compare their frontier orbitals HOMO and LUMO, we notice that the chloride atom has a great influence on their energy. the lack of Chloride atoms reduces the energy of frontier orbitals regardless of the functional. And the more the number of chloride atoms, the higher the energy. Also, RuCl3terpy and α-RuCl2(Azpy)2 have been discovered to display the best energy suitable for reaction as cytotoxic agents. Yet, both are from groups different. thus, at ground state, there is practically no difference between both groups. However, regarding tDDFt prediction with the determination of vertical electronic affinity VEA and vertical ionization potential VIP both at ground state S and at exciting t1 state, we notice that Ru(II) complexes are not active either in the presence or absence of 3O2 molecule. Here, only Ru(III) complexes are able to react on Guanine through their radical cations or by generating the superoxide radical anion t_***" alt="" />. therefore, the Ru(III) complexes are assumed to be active both at a fundamental state and under the effect of light for photodynamic therapy. We come to conclude that Ru(II) complexes are not active by excitation as their valence electrons are paired thereby making these complexes more stable. We directly measure optical bound states in the continuum(BICs)by embedding a photodetector into a photonic crystal *** BICs observed in our experiment are the result of accidental phase matching between incident,reflected and in-plane waves at seemingly random wave vectors in the photonic band *** measurements were confirmed through a rigorously coupled-wave analysis simulation in conjunction with temporal coupled mode *** mixing between photonic crystal slab modes was observed and described using a plane wave expansion *** ability to probe the field intensity inside the photonic crystal and thereby to directly measure BICs represents a milestone in the development of integrated opto-electronic devices based on BICs.

Context: Neurologic disorders are common in patients on maintenance haemodialysis (MHD). Data in Sub-Saharan Africa are scarce. Aim: to determine the prevalence and associated factors of neurological disorders amongst patients on MHD in Cameroon. Settings and Design: this was a four-month cross-sectional study carried out at the Douala General Hospital (DGH) a tertiary referral hospital in Cameroon. Methods: the diagnosis of neuropathy was made using the Michigan Neuropathy Screening Instrument (MNSI), neuropathic pain with the DN4 score, Restless Legs Syndrome (RLS) with the International Restless Legs Syndrome Study Group questionnaire (IRLSSG) and cognitive disorders with the Mini-Mental State Examination test (MMS). Student t and Chi-square tests were used to compare qualitative and quantitative variables. the level of significance was set at p Results: A total of 157 patients were included with 65% being males. the mean age was 48.8 ± 13.7 years. the main comorbidities were hypertension (90.4%), diabetes (19.1%), hepatitis C (10.5%) and HIV infection (10.8%). the median dialysis vintage was 36 (1 - 178) months. the overall prevalence of neurological disorders was 85.4%. the leading type was sensory polyneuropathy (57.3%), cognitive dysfunction (52.9%), neuropathic pain (23%), and restless legs syndrome (17.8%). Male gender was statistically associated with neuropathic pain, while age ≥ 40 years and diabetes were associated with sensory neuropathy and RLS. Conclusions: the burden of neurological disorders is high among patients on MHD as up to 4/5 of them had neurological involvement dominated by sensory polyneuropathy and cognitive dysfunction.

受天然醌蛋白中吡咯并喹啉醌（PQQ）的"加成-消除"催化机理启发,含PQQ的复合纳米材料近年来被越来越多地用作仿生多相电催化剂。然而,现有手段仍难以精准测量PQQ官能团的催化本征活性,极大地延缓了高效PQQ电催化剂的研发。本项工作提出,通过记录统计单个PQQ修饰的碳纳米管碰撞碳纤维微电极时产生的瞬态电信号,可以计算得到PQQ活性位的转化频率（tOF）。结果显示,PQQ官能团能够显著提升多个生物体系的催化性能[1,2],单个PQQ活性位的tOF值可高达100次每秒以上。这类新型单碳管碰撞法为研究单分子水平的催化本征活性提供了有力手段。

this quasi-experimental study compared the results of a traditional model of physical therapy(Pt)care to a Pt wellness model known as *** traditional model included discharge from Pt with a home exercise program(HEP)to be self-administered with or without the addition of a community-based exercise *** wellness model included participation in a Pt-designed and supervised group exercise program(GroupHab class)in an outpatient clinical setting following discharge from *** t-tests were used to compare the number of falls,exercise frequency,and exercise duration between the two groups.A repeated measures,analysis of variance(RM-ANOVA)compared changes in balance confidence scores both within and between groups,and a multivariate analysis of variance(MANOVA)analyzed group differences across multiple quality of life ratings using the *** data were analyzed at the 0.05 alpha level using SPSS 24 statistical *** results showed a significantly greater reduction in recurrent falls among the GroupHab wellness group compared to the HEP group(t=2.811,p=0.009).the resulting odds ratio for subsequent falls was 2.2 among HEP participants and 0.2 among GroupHab *** adherence was also greater for those who participated in the GroupHab *** documented greater exercise frequency(t=-3.253,p=0.002)and more exercise minutes(t=-7.188,pthan those who participated in the *** comparing changes in the participants’balance confidence,we found an average increase of 5%among GroupHab participants compared to a 6%decrease among HEP participants(F=16.877,pthough our multivariate analysis of the SF-20 scores revealed no significant difference overall(F=0.768,p=0.73),the univariate analyses showed significantly greater improvements among GroupHab participants in selected areas of physical *** results suggest that at-risk older adults who are discharged into a functionally-based group exercise

the characteristics of AIDS is that HIV infection induces CD4+ t cell defects that includes a quantitave CD4+ t cells depletion and a loss of t helper cells function leading to a progressive immune deficiency. Early report indicated that this immune deficiency was irreversible, even with the antiretroviral therapies. More recently, however, clinical benifits of hightly active antiretroviral therapies (HAARt) are increasingly evidenced by resolving opportunistic infections and malignancies, as well as declining hospitalization and mortality rates. this suggests that potent and sustained suppression of viral replication, at least to some extent, is associated with reconstitution of the immune system even in adult patients treated at advanced stages of the disease. Such immune reconstitution which was demonstrated firstly by a French reseach group includes (the group leader is Pr Brigitte Autran)(1) a rapid rise in CD4+ and CD8+ t cells followed by a slower CD4+ t cell increase; (2) a rapid rise in memory CD4+ t cells during the first three months of treatment by late increase in naive t cells coexpressing CD45RA and CD62L molecules after 3 months of efficient antiviral treatment;(3) a significant reduction of CD4 and CD8 activation markers in parallel to plasma virus load reduction;(4) a restoration in CD4+ t cell reactivity to recall antigens. these observations open new perspectives for the understanding of CD4+ t cells deficiency and therapeutic strategies of HIV infection. In the present review we will address some of the questions raised by immune restoration with HAARt when administered at advanced stages of the disease.

Background: Achieving and maintaining glycemic targets are a challenge for health practitioners around the world. We aimed to study the factors associated with prolonged poor diabetes control in the cohort of T2DM patients monitored and treated in the Department of Internal Medicine at the Yalgado OUEDRAOGO Teaching Hospital in Ouagadougou in order to optimize therapeutic education in these patients. Methodology: This was a descriptive and analytical cross-sectional study combining retrospective data collection from the last year of patient follow-up and prospective collection of some information. The study included all diabetic patients, aged at least 18 years old, followed and treated in the Department of Internal Medicine at the Yalgado OUEDRAOGO Teaching Hospital between January 1, 2010 and December 31, 2018 following a systematic random sampling with a sampling step of 10. The variables collected were sociodemographic, anthropometric, lifestyle, cardiovascular risk factors and diabetes-related characteristics. To determine the risk factors associated with prolonged poor glycemic control, we performed modeling using logistic regression. All variables associated with prolonged poor glycemic control, in bivariate logistic regression with a p-value less than 0.20 were included in the full model. Later, we used a stepwise descending method to obtain the final model, which was then tested by a receiver operating characteristic (ROC) curve. The significance threshold was set at 5%. Raw and fitted Odds-Ratio (OR) and 95% confidence interval were presented. Results: 270 patients were included. Prolonged poor control of diabetes mellitus was observed in 73.70%. the mean age was 55.97 years (SD: ±11.52) and the sex ratio was 0.6 in favor of female. the mean time since diabetes mellitus diagnoses was 5.85 years (SD: ±5.15).
