transforming growth factor-β(tGF-β)and bone morphogenetic protein(BMP)play important roles in bone *** ubiquitination regulatory factors(Smurfs)regulate tGF-β/BMP signaling via ubiquitination,resulting in degradation of signaling molecules to prevent excessive activation of tGF-β/BMP *** Smurf2 has been shown to negatively regulate tGF-β/Smad signaling,its involvement in BMP/Smad signaling in bone metabolism has not been thoroughly *** the present study,we sought to evaluate the role of Smurf2 in BMP/Smad signaling in bone *** collagen sponges containing 3μg of recombinant human BMP2(rhBMP2)were implanted in the dorsal muscle pouches of wild type(Wt)and Smurf2−/−*** rhBMP2-induced ectopic bone in Smurf2−/−mice showed greater bone mass,higher mineral apposition and bone formation rates,and greater osteoblast numbers than the ectopic bone in Wt *** Wt mice,the ectopic bone consisted of a thin discontinuous outer cortical shell and scant inner trabecular *** contrast,in Smurf2−/−mice,the induced bone consisted of a thick,continuous outer cortical shell and abundant inner trabecular ***,rhBMP2-stimulated bone marrow stromal cells(BMSCs)from Smurf2−/−mice showed increased osteogenic ***2 induced the ubiquitination of Smad1/***/Smad signaling was enhanced in Smurf2−/−BMSCs stimulated with rhBMP2,and the inhibition of BMP/Smad signaling suppressed osteogenic differentiation of these *** findings demonstrate that Smurf2 negatively regulates BMP/Smad signaling,thereby identifying a new regulatory mechanism in bone metabolism.

the human intestinal microbiome plays a major role in human health and diseases, including colorectal cancer. Colorectal carcinogenesis represents a heterogeneous process with a differing set of somatic molecular alterations, influenced by diet, environmental and microbial exposures, and host immunity. Fusobacterium species are part of the human oral and intestinal microbiota. Metagenomic analyses have shown an enrichment of Fusobacterium nucleatum(F. nucleatum) in colorectal carcinoma tissue. Using 511 colorectal carcinomas from Japanese patients, we assessed the presence of F. nucleatum. Our results showed that the frequency of F. nucleatum positivity in the Japanese colorectal cancer was 8.6%(44/511), which was lower than that in United States cohort studies(13%). Similar to the United States studies, F. nucleatum positivityin Japanese colorectal cancers was significantly associated with microsatellite instability(MSI)-high status. Regarding the immune response in colorectal cancer, high levels of infiltrating t-cell subsets(i.e., CD3+, CD8+, CD45RO+, and FOXP3+ cells) have been associated with better patient prognosis. there is also evidence to indicate that molecular features of colorectal cancer, especially MSI, influence t-cell-mediated adaptive immunity. Concerning the association between the gut microbiome and immunity, F. nucleatum has been shown to expand myeloid-derived immune cells, which inhibit t-cell proliferation and induce t-cell apoptosis in colorectal cancer. this finding indicates that F. nucleatum possesses immunosuppressive activities by inhibiting human t-cell responses. Certain micro RNAs are induced during the macrophage inflammatory response and have the ability to regulate host-cell responses to pathogens. Micro RNA-21 increases the levels of IL-10 and prostaglandin E2, which suppress antitumor t-cell-mediated adaptive immunity through the inhibition of the antigen-presenting capacities of dendritic cells and t-cell proliferation in color

Vascular Malformation with Multiple Organized thromboses in Buccal Region

Vascular malformations occur most frequently in the maxillofacial and oral regions. Although many cases of vascular malformations with phlebolithiasis have been reported, only few reports have documented cases of thrombosis formation. We report a rare case of a vascular malformation with multiple thromboses in the left buccal region. A 28-year-old woman had a painless swelling in the left cheek when she consulted our hospital. t1-weighted magnetic resonance imaging revealed a low-signal lesion, having the same intensity as the muscle, in the left buccal region, and t2-weighted imaging revealed a high-signal lesion. Additionally, three tuberous tumours were noted in this lesion showing both a light high signal on t1-weighted imaging and a low signal on t2-weighted imaging. Based on these imaging findings, as well as clinical and pathological findings, the patient was diagnosed with multiple vascular malformations in the left midfacial region. the three tuberous tumours showed necrotic tissue in the central area, which was formed by calcified and concentric fibrous tissue, and vascularization. Because tumorous lesion and typical vascular structure by the Elastica Van Gieson staining were not observed, these tumours were seemed that fibrin thrombus in this lesion formed the organized tissue.

Meta-analysis of lymph node metastasis in Siewert type Ⅰ and Ⅱ t1 adenocarcinomas

AIM to evaluate the incidence of lymph node metastasis （LNM） and its risk factors in patients with Siewert type Ⅰ and type Ⅱ pt1 *** We enrolled 85 patients [69 men, 16 women; median age （range）, 67 （38-84） years] who had undergone esophagectomy or proximal gastrectomy for Siewert type Ⅰ and type Ⅱ pt1 adenocarcinomas. Predictive risk factors of LNM included age, sex, location of the tumor center, confirmed Barrett’s esophageal adenocarcinoma, tumor size, macroscopic tumor type, pathology, invasion depth, presence of ulceration, and lymphovascular invasion. Multivariate logistic regression analysis was used to identify factors predicting LNM. We also evaluated the frequencies of LNM for Siewert type Ⅰ and type Ⅱ pt1 adenocarcinomas in meta-data *** were found in 11 out of 85 patients （12.9%, 95%CI： 5.8-20.0）. Only 1 of the 15 patients （6.6%, 95%CI： 0.0-19.2） who had a final diagnosis of pt1a adenocarcinoma had a positive LNM, whereas 10 of the 70 patients （14.2%, 95%CI： 6.0-22.4） with a final diagnosis of pt1b adenocarcinoma had positive LNM. Furthermore, only one of the 30 patients （3.3%, 95%CI： 0.0-9.7） with a tumor invasion depth within 500 μm from muscularis mucosae had positive LNM. Poor differentiation and lymphovascular invasion were independently associated with a risk of LNM. In meta-data analysis, 12 of the 355 patients （3.3%, 95%CI： 1.5-5.2） who had a final diagnosis of pt1a adenocarcinoma had a positive LNM, whereas 91 of the 438 patients （20.7%, 95%CI： 16.9-24.5） with a fnal diagnosis of pt1b adenocarcinoma had positive *** consider endoscopic submucosal dissection （ESD） is suitable for patients with Siewert type Ⅰ and type Ⅱ t1a adenocarcinomas. For patients with t1b adenocarcinoma, especially invasion depth is within 500 μm from muscularis mucosae with no other risk factor for LNM, diagnostic ESD could be a treatment option according to the overall status of patients and the presence of comorbiditi

Genetic variants involved in gallstone formation and capsaicin metabolism,and the risk of gallbladder cancer in Chilean women

AIM:to determine the effects of genetic variants associated with gallstone formation and capsaicin (a pungent component of chili pepper) metabolism on the risk of gallbladder cancer (GBC).MEtHODS: A total of 57 patients with GBC, 119 patients with gallstones, and 70 controls were enrolled in this study. DNA was extracted from their blood or paraffi n block sample using standard commercial kits. the statuses of the genetic variants were assayed using taqman SNP Genotyping Assays or Custom taqman SNP Genotyping ***:the non-ancestral t/t genotype of apolipoprotein B rs693 polymorphism was associated with a decreased risk of GBC (OR:0.14,95% CI:0.03-0.63). the t/t genotype of cholesteryl ester transfer protein (CEtP) rs708272 polymorphism was associated with an increased risk of GBC (OR:5.04,95% CI:1.43-17.8).CONCLUSION: Genetic variants involved in gallstone formation such as the apolipoprotein B rs693 and CEtP rs 708272 polymorphisms may be related to the risk of developing GBC in Chilean women.

Background: the inability to pass endoscopes beyond strictures is a considerable problem in patients with a colonic stricture. Methods: In patients with bowel obstruction, we have modified the insertion method for double-balloon endoscopy with a long, transnasal decompression tube. Observations: We have succeeded in reaching the proximal side of the stricture from the oral approach across the entire small bowel in a patient. Conclusions: this modified double-balloon enteroscopy is useful for patients with bowel obstruction in whom a long decompression tube is already placed.

to evaluate platelet aggregation in patients with retinal vein occlusion (RVO) by the light scattering method and compare the effects of three antiplatelet d rugs on aggregate formation. Prospective, nonrandomized, interventional clinical trial. (1) Platelet aggregation was measured in 42 patients with untreated bran ch RVO (BRVO), 26 patients with central RVO (CRVO), and 30 healthy control subjects using a light scatte ring platelet aggregometer. Platelet aggregates were classified as small, medium , and large according to light intensity. total light intensities of each aggreg ate size were compared between BR VO, CRVO, and control subjects. (2) In 33 pat ients with RVO, platelet aggregation before and 2 weeks after the administration of ticlopidine, beraprost, or aspirin were compared. (1) there was a statistica lly significant difference (P=0.007 3) between the control subjects (8.3×106 a. u.) and CRVO patients (22.9×.106 a.u.) with respect to small aggregates. there was no statistically significant difference with respect to medium and large agg regate formation between the control subjects and either patient group. (2) Comp ared with aggregates formed in the absence of antiplatelet drugs, ticlopidine si gnificantly inhibited only the formation of small aggregates and beraprost that of all sizes; aspirin did not significantly inhibit the formation of any aggrega te sizes. Increase in small platelet aggregates may be attributable to RVO patho genesis. Beraprost and ticlopidine appear to inhibit small aggregate formation i n RVO patients and may represent effective antiplatelet treatments. the lightsca ttering method is useful to investigate the pathogenesis of RVO and the effects of antiplatelet drugs.
