Aims: t. det.rmine t.e prevalence and risk fact.rs of heart.urn in Abidjan, a black African cit.. Pat.ent. and Met.ods: cross-sect.onal st.dy from June 15 t. Sept.mber 30, 2003. One t.ousand nine hundred fort. (1940) persons from five healt. zones of t.e cit. Abidjan were included aft.r t.eir informed consent. Socio demographic and clinical paramet.rs were collect.d using a quest.onnaire in an int.rview format. Heart.urn was defined as a discomfort.or burning sensat.on ext.nding from t.e st.rnal manubrium t. t.e base of t.e neck. Fact.rs related t. t.e complaint.were asked such as predisposing fact.rs, habit. (t.bacco, alcohol and coffee int.ke) and body mass index. St.pwise mult.ple logist.c regression analyses were used t. examine associat.ons bet.een t.ese fact.rs and heart.urn. Result.: Among 1940 respondent. (mean age 28 ± 9 years;sex rat.o (M:F) 0.86), heart.urn occurred in 433 persons (once a week in 9.2% of case (178 persons)). Five fact.rs were st.t.st.cally associat.d wit. heart.urn: male sex (p = 0.025, OR = 0.555 [CI95% 0.331 - 0.930]), heart.urn in a family member (p = 0.010, OR = 1.765 [95%CI 1.143 - 2.725]), const.pat.on (p = 0.011, OR = 2.182 [95%CI 11,953,983]), right.lateral decubit.s (p = 0.001, OR = 6.247 [95%CI 2.079 - 18.775]) and aft.r a meal (p = 0.000, OR = 2.643 [95%CI 1.594 4.383]). Conclusion: Heart.urn is common in t.is black African population. Male sex appears t. be less associat.d. Const.pat.on, right.lateral decubit.s and aft.r a meal are t.igger fact.rs for heart.urn. Heart.urn in a family member is a risk fact.r.

Parasympat.et.c funct.on can be assessed by t.e deep breat.ing t.st.(DB) as a simple and reproducible cardiovascular reflex. t.e aim of t.is st.dy is t. use t.is t.st.t. compare t.e vagal response of a young foot.allers group t. of age-mat.hed unt.ained normal subject.. Deep breat.ing t.st.was performed in 2 groups: one of 20 adult.young foot.allers (average age of 19.3 ± 0.6 years), and a second age-mat.hed group of 20 unt.ained subject. (average age of 19.6 ± 0.6 years). Subject. underwent.t.e DB t.st.aft.r 30 min rest.ng in supine posit.on, and responses were expressed as a percent.ge of variat.on of heart.rat. during t.e st.mulation. St.dent.s t.t.st.was used t. evaluat. st.t.st.cal differences among t.e t.o groups for all paramet.rs (vagal response, heart.rat., VO2max….) and considering p t.difference. t.e result. showed t.at.vagal response t. deep breat.ing t.st.was significant.y higher in t.e young foot.allers when compared t. t.e unt.ained cont.ols (72.6% ± 16.2% vs 55.0% ± 12.8%, respect.vely, p = 0.03). t.e basal heart.rat. was significant.y lower in foot.allers t.an in t.e cont.ols group (52.1 ± 7.4 bat.min vs 69.8 ± 14.3 bat.min, p t.e use of t.e simple t.st.of deep breat.ing allowed us t. demonst.at. t.at.adult.young foot.allers have a significant.y lower basal HR and higher parasympat.et.c response in comparison t. unt.ained subject..

t.e t.ree major large-scale, diffuse γ-ray st.uct.res of t.e Milky Way are t.e Galact.c disk,a bulge-like GeV excess t.wards t.e Galact.c cent.r, and t.e Fermi bubble. Whet.er such st.uct.res can also be present.in ot.er normal galaxies remains an open quest.on. M31, as t.e nearest.massive normal galaxy, holds promise for spat.ally-resolving t.e γ-ray emission. Based on more t.an 8 years of Fermi-lat observat.ons, we use(1) disk,(2) bulge, and(3) disk-plus-bulge t.mplates t. model t.e spat.al dist.ibut.on of t.e γ-ray emission from M31. Among t.ese, t.e disk-plus-bulge t.mplate delivers t.e best.fit. in which t.e bulge component.has a t. value 25.7 and a phot.n-index of 2.57 ± 0.17, providing st.ong evidence for a cent.ally-concent.at.d γ-ray emission from M31, t.at.is analogous t. t.e Galact.c cent.r excess. t.e t.t.l0.2–300 GeV γ-ray luminosit. from t.is bulge component.is(1.16 ± 0.14) × 1038 erg s-1, which would require ～ 1.5 × 105 millisecond pulsars, if t.ey were t.e dominant.source. We also search for a Fermi bubble-like st.uct.re in M31 using t.e full dat.set.pass8), but.no significant.evidence has been *** addit.on, a likelihood analysis using only phot.ns wit. t.e most.accurat. reconst.uct.d direct.on(i.e.,PSF3-only dat.) reveals a 4.8 σ point.like source locat.d at.～10 kpc t. t.e nort.west.of t.e M31 disk, wit. a luminosit. of(0.97 ± 0.27) × 1038 erg s-1 and a pho@t.n-$i·nd3 ex of 2.31 ± 0.18. Lacking a count.rpart.on t.e sout.east.side of t.e disk, t.e relation bet.een t.is point.like source and a bubble-like st.uct.re remains elusive.

Met.ods: From 1991 t.rough 2001, all 27 pat.ent. present.ng wit. int.rrup t.d aort.c arch/vent.icular sept.l defect.and post.rior deviat.on of t.e infundi bular sept.m were analyzed. Fift.en pat.ent. wit. t.e smallest.subaort.c areas u nderwent.myect.my or myot.my of t.e infundibular sept.m concomit.nt.wit. int.rru pt.d aort.c arch/vent.icular sept.l defect.repair. Result.: Pat.ent. undergoing myect.my-myot.my(Group I) had significant.y smaller subaort.c diamet.r indexes( 0.83±0.16 cm/m2) when compared wit. t.ose who had only int.rrupt.d aort.c arch/ vent.icular sept.l defect.repair(group 2: 0.99±0.13 cm/m2, P=.012). t.o hospit. l deat.s occurred in group 1, and 1 occurred in group 2.
