Prophylactic transcatheter arterial embolization reduces rebleeding in non-variceal upper gastrointestinal bleeding: A meta-analysis

BACKGROUND Despite the improvement in the endoscopic hemostasis of non-variceal upper gastrointestinal bleeding(NvUGIB),rebleeding remains a major *** To assess the role of prophylactic transcatheter arterial embolization(PTAE)added to successful hemostatic treatment among NvUGIB *** We searched three databases from inception through October 19th,*** controlled trials(RCTs)and observational cohort studies were *** compared patients with NvUGIB receiving PTAE to those who did not get *** outcomes were rebleeding,mortality,reintervention,need for surgery and transfusion,length of hospital(LOH),and intensive care unit(ICU)*** the quantitative synthesis,odds ratios(ORs)and weighted mean differences(WMDs)were calculated with the random-effects model and interpreted with 95%confidence intervals(CIs).RESULTS We included a total of 3 RCTs and 9 observational studies with a total of 1329 patients,with 486 in the intervention *** was associated with lower odds of rebleeding(OR=0.48,95%CI:0.29–0.78).There was no difference in the 30-d mortality rates(OR=0.82,95%CI:0.39–1.72)between the PTAE and control *** who underwent PTAE treatment had a lower chance for reintervention(OR=0.48,95%CI:0.31–0.76)or rescue surgery(OR=0.35,95%CI:0.14–0.92).The LOH and ICU stay was shorter in the PTAE group,but the difference was non-significant[WMD=-3.77,95%CI:(-8.00)–0.45;WMD=-1.33,95%CI:(-2.84)–0.18,respectively].CONCLUSION PTAE is associated with lower odds of rebleeding and any reintervention in ***,further RCTs are needed to have a higher level of evidence.

Strategies in training deep learning models to extract building from multisource images with small training sample sizes

Building extraction from remote sensing data is an important topic in urban studies and the deep learning methods have an increasing role due to their minimal requirements in training data to reach outstanding *** aimed to investigate the original U-Net architecture’s efficiency in building segmentation with different number of training images and the role of data augmentation based on multisource remote sensing data with varying spatial and spectral resolutions(Worldview-2[Wv2],Worldview-3[Wv3]images and an aerial orthophoto[ORTHO]).When the trainings and predictions were conducted on the same image,U-Net provided good results with very few training images(validation accuracies:94-97%;192 images).Combining the ORTHO’s and Wv2’s training data for prediction on Wv3 provided poor results with low F1-score(0.184).However,the inclusion of only 48 Wv3 training images significantly improved the F1-score(0.693),thus,most buildings were correctly ***,using only independent reference data(other than the target image)is not enough to train an accurate *** our case,the reference from WW2 and ORTHO images did not provide an acceptable basis to train a good model,but a minimal number of training images from the targeted Wv3 improved the accuracy(F1-score:69%).

Anthropogenic Pseudokarstic Depressions on Mount Bocskor(Bakony Region, Hungary)

Surface forms above the mine(ármin mine) of Mount Bocskor(Southern Bakony, Hungary) were examined. We made contour maps, plan maps, morphological maps and atectonic grike(cave) maps of some of the forms and their surroundings. We examined the distribution of the depth, length, elongation ratio in case of some depressions, the relation between the depth and the diameter of some depressions, the relationship between depression group directions and mine cut directions, the standard deviation of the direction differences of depression groups and of their depressions. The forms of the mountain related to surface mining can be separated to open and closed. The former are trenches and stairs, the latter are circular, elongated, and complex depressions. The formation of these forms can be related to the balancing movements of the vault over the mountain’s mine. At thin vault, stairs develop by collapses, while at the atectonic fissures of thicker vault, trenches and depressions are formed at the surface. In areas bordered by sinking(subsidence through) and downwardly cohesive faults, depression groups of diverse features are arranged in the marginal bands. Elongated depressions are formed at atectonic blocks bounded by dispersing faults in non-banded distribution. Where there is a superficial deposit, atectonic fissures can also be inherited directly by collapse to the surface and form depressions. They can also form indirectly over atectonic fissures by compaction, subsequent collapse and/or suffosion of the superficial deposit. The results of the study make it possible to analyse the material loss due to mining on the vault if the atectonic structures of the vault are partly or completely covered by superficial deposit.

Efficiency of local minima and GLM techniques in sinkhole extraction from a LiDAR-based terrain model

The aim of this paper was to study reliable automated delineationpossibilities of karst sinkholes using a LiDAR-based digital terrain model(DTM) with pixel-based classifications. We applied two approaches toextract sinkholes: (1) general linear modeling (GLM) with morphometricindices derived from DTM;(2) and a local minima-based delineationusing only LiDAR DTM as the input layer. The outcome of the localminima was significantly different from the reference ones but found allthe sinkholes without previous knowledge of the area. The GLM-basedoutcome did not differ statistically from the reference. Results showedthat these approaches were efficient in detecting sinkholes based onLIDAR derivatives, and can be used for risk assessment and hazardpreparedness in karst areas: GLM had an overall accuracy of 89.5% andlocal minima had an accuracy of 92.3%;both methods identifiedsinkholes but also had commission errors, identifying depressions assinkholes.

Lower back pain is one of the most common medical problems in the world and it is experienced by a huge percentage of people *** to its ability to produce a detailed view of the soft tissues,including the spinal cord,nerves,intervertebral discs,and vertebrae,Magnetic Resonance Imaging is thought to be the most effective method for imaging the *** semantic segmentation of vertebrae plays a major role in the diagnostic process of lumbar *** is difficult to semantically partition the vertebrae in Magnetic Resonance Images from the surrounding variety of tissues,including muscles,ligaments,and intervertebral discs.U-Net is a powerful deep-learning architecture to handle the challenges of medical image analysis tasks and achieves high segmentation *** work proposes a modified U-Net architecture namely MU-Net,consisting of the Meijering convolutional layer that incorporates the Meijering filter to perform the semantic segmentation of lumbar vertebrae L1 to L5 and sacral vertebra ***-colour mask images were generated and used as ground truth for training the *** work has been carried out on 1312 images expanded from T1-weighted mid-sagittal MRI images of 515 patients in the Lumbar Spine MRI Dataset publicly available from Mendeley *** proposed MU-Net model for the semantic segmentation of the lumbar vertebrae gives better performance with 98.79%of pixel accuracy(PA),98.66%of dice similarity coefficient(DSC),97.36%of Jaccard coefficient,and 92.55%mean Intersection over Union(mean IoU)metrics using the mentioned dataset.

Autism spectrum disorder:difficulties in diagnosis and microRNA biomarkers

We performed a PubMed search for microRNAs in autism spectrum disorder that could serve as diagnostic biomarkers in patients and selected 17 articles published from January 2008 to December 2023,of which 4 studies were performed with whole blood,4 with blood plasma,5 with blood serum,1 with serum neural cell adhesion molecule L1-captured extracellular vesicles,1 with blood cells,and 2 with peripheral blood mononuclear *** of the studies involved children and the study cohorts were largely *** of the studies had performed microRNA sequencing or quantitative polymerase chain reaction assays to measure microRNA *** five studies had used real-time polymerase chain reaction assay to validate microRNA expression in autism spectrum disorder subjects compared to *** microRNAs that were validated in these studies may be considered as potential candidate biomarkers for autism spectrum disorder and include miR-500a-5p,-197-5p,-424-5p,-664a-3p,-365a-3p,-619-5p,-664a-3p,-3135a,-328-3p,and-500a-5p in blood plasma and miR-151a-3p,-181b-5p,-320a,-328,-433,-489,-572,-663a,-101-3p,-106b-5p,-19b-3p,-195-5p,and-130a-3p in blood serum of children,and miR-15b-5p and-6126 in whole blood of *** important limitations were identified in the studies reviewed,and need to be taken into account in future *** studies are warranted with children and adults having different levels of autism spectrum disorder severity and consideration should be given to using animal models of autism spectrum disorder to investigate the effects of suppressing or overexpressing specific microRNAs as a novel therapy.

MicroRNAs as potential diagnostic biomarkers for bipolar disorder

Abnormal expression of microRNAs is connected to brain development and disease and could provide novel biomarkers for the diagnosis and prognosis of bipolar disorder. We performed a PubMed search for microRNA biomarkers in bipolar disorder and found 18 original research articles on studies performed with human patients and published from January 2011 to June 2023. These studies included microRNA profiling in bloodand brain-based materials. From the studies that had validated the preliminary findings,potential candidate biomarkers for bipolar disorder in adults could be miR-140-3p,-30d-5p,-330-5p,-378a-5p,-21-3p,-330-3p,-345-5p in whole blood, miR-19b-3p,-1180-3p,-125a-5p, let-7e-5p in blood plasma, and miR-7-5p,-23b-5p,-142-3p,-221-5p,-370-3p in the blood serum. Two of the studies had investigated the changes in microRNA expression of patients with bipolar disorder receiving treatment. One showed a significant increase in plasma miR-134 compared to baseline after 4 weeks of treatment which included typical antipsychotics, atypical antipsychotics, and benzodiazepines. The other study had assessed the effects of prescribed medications which included neurotransmitter receptorsite binders(drug class B) and sedatives, hypnotics, anticonvulsants, and analgesics(drug class C) on microRNA results. The combined effects of the two drug classes increased the significance of the results for miR-219 and-29c with miR-30e-3p and-526b* acquiring significance. MicroRNAs were tested to see if they could serve as biomarkers of bipolar disorder at different clinical states of mania, depression, and euthymia. One study showed that upregulation in whole blood of miR-9-5p,-29a-3p,-106a-5p,-106b-5p,-107,-125a-3p,-125b-5p and of miR-107,-125a-3p occurred in manic and euthymic patients compared to controls, respectively, and that upregulation of miR-106a-5p,-107 was found for manic compared to euthymic patients. MicroRNAs as potential biomarkers for diagnosis of post-traumatic stress disorder
