Neoadjuvant Chemotherapy for Transitional Cell Carcinoma of the Bladder： A Single Centre Experience

Urothelial cancers usually recur distantly rather than loco-regionally. In patients with pT2 and pT3/pT4 tumors, local recurrence has been observed in 3-4% and 11-16%, respectively, whereas distant failure has occurred in 10-27% and 19-35%, respectively. Despite local therapy most patients with muscle invasive transitional cell carcinoma （TCC） of the bladder die of systemic relapse, indicating a need for effective adjunctive systemic treatment. We determined whether neoadjuvant chemotherapy improved overall survival. This study evaluated the role of neoadjuvant combination chemotherapy with gemcitabine/cisplatin （GC） in improving the outcome of this group of patients. A total of 44 patients （84% Male, 16% Female） with newly diagnosed bladder cancer （T3-4, N0-2, M0） were subjected to initial 3 cycles of GC, then managed according to response. Patients were assessed clinically after each cycle and by Interim CT scan after 3 cycles of chemotherapy and those who achieved complete or partial response underwent radical cystectomy. We enrolled 63 patients, 19 of whom were found to be ineligible; thus, 44 were assigned to receive neoadjuvant chemotherapy followed by surgery. Average size of the largest tumor was greater that 30mm in 77% patients. According to Computed Tomographic findings 70% patients belonged to Stage T4A. The overall response rate to GC was 50%, and incomplete response was achieved in 25% whereas 25% patients were lost to follow up. Twenty two patients who had complete response, underwent cystectomy and diversion. It was observed that those patients who underwent radical cystectomy with ureterosigmoidostomy had an increased serum creatinine in comparison to patients who had ileal conduit. The size of the effect is modest and combination chemotherapy can be administered safely without adverse outcomes resulting in delayed local therapy. The challenge of achieving sustainable economic development with a secure environmental system is a global challenge faced by both developed and developing *** Efficiency(EE)is crucial in achieving sustainable economic growth while reducing ecological *** research utilizes the Slack-Based Measure Data Envelopment Analysis(SBM-DEA)and the Malmquist-Luenberger Index(MLI)method to evaluate EE and productivity changes from 1995 to 2020 across G20 *** study uses four different input-output bundles to gauge the impact of renewable and non-renewable energy consumption and carbon emissions on EE and productivity *** study results show that including renewable energy consumption improves the average EE from 0.783 to 0.8578,but energy productivity declines from 1.0064 to *** bad output(carbon emissions)in the estimation process enhances renewable EE and productivity change,resulting in an average EE of 0.6678 and MLI of *** change is identified as the primary determinant of energy productivity growth in scenarios 1 and 2,while technical efficiency determines energy productivity change in scenarios 3 and *** Kruskal-Wallis test reveals a significant statistical difference between the mean EE and MLI scores of G20 countries.

面向南亚和东南亚的小语种机器翻译,目前已有不少研究团队开展了深入研究,但作为巴基斯坦官方语言的乌尔都语,由于稀缺的数据资源和与汉语之间的巨大差距,有针对性的汉乌机器翻译方法研究非常稀少。针对这种情况,提出了基于Transformer的融合乌尔都语词性序列的汉乌神经机器翻译模型。首先利用Transformer对目标语言乌尔都语的词性序列进行预测,然后将翻译模型的预测结果和词性序列模型的预测结果相结合进行联合预测,从而实现语言知识到翻译模型的融入。在现有小规模汉乌数据集上的实验表明,所提方法在数据集上的BLEU值相较于基准模型提升了0.13,取得了较为明显的效果。

In this paper, a new statistical averaging technique is proposed for finding an optimal solution to a multi-objective linear fractional programming problem (MOLFPP) and multi-objective linear programming problem (MOLPP) by using new arithmetic averaging method and new geometric averaging method. It is significantly noticeable same characteristics among all the technique while taking maximum or minimum among all optimized values for multi-objective functions using simplex algorithm. The characteristics provided from the problems are verified by the numerical examples.

为解决物理和化学方法改良盐渍土工程性质时工程耗能高、环境负效应扰动大的问题,利用高原寒区盐渍环境发育的原生脲解型微生物对细粒硫酸盐渍土进行固化试验,采用微观试验和三轴压缩试验对微生物固化盐渍土的强度特征进行了研究。结果表明:细粒硫酸盐渍土被微生物固化后,抗剪强度最高可提升2.9倍,增强机制由黏聚强度的提高程度所决定;固化土的初始切线模量和刚度得到提升,抗变形能力得到增强。该研究结果对增强固化土的稳定性、承载力等具有理论研究和实践指导意义。

目的观察成人腹壁下血管穿过腹股沟区域时的直径、血流速度、血管位置、血管长度,为精索静脉曲张双flow-through超引流技术提供解剖依据。方法2023年7月至2023年11月在武汉大学中南医院对30例年龄在20~30岁之间的成年男性进行B超检查,测量腹壁下静脉,腹壁浅静脉直径、血流速度,计量资料以均数±标准差(±s)表示,并进行文献查阅,查出腹壁下血管在腹股沟区域的走行及精索静脉曲张时的静脉管径及流速。并且在两具人体标本上进行精索内静脉-腹壁下静脉双flow-through超引流模拟手术,总结经验,得出较好的手术方法。组间比较采用t检验。结果通过B超测得的腹壁下动脉直径[(2.3±0.2)mm]大于腹壁浅动脉[(1.4±0.2)mm,t=10.312,P0.05],腹壁下静脉流速[(1.77±0.23)cm/s]小于腹壁浅静脉流速[(3.03±0.58)cm/s,t=-6.017,P<0.01]。结论精索静脉曲张腹壁下静脉双flow-through超引流技术临床应用价值较高。

Endoscopy has become increasingly fundamental in the management of patients with inflammatory bowel disease(IBD). It is required for diagnosis, assessment of therapeutic response, postoperative follow up and in the surveillance of dysplasia. With rapid advances in technology, including high definition colonoscopy and chromoendoscopy, questions have arisen regarding the most appropriate surveillance and management strategies of colorectal neoplasia in IBD. We aim to review current surveillance strategies, explore the utility of new technologies, and examine the role of endoscopic resection, with the aim of clarifying these questions.

In this paper,a comparative study for kernel-PCA based linear parameter varying(LPV)model approximation of sufficiently nonlinear and reasonably practical systems is carried *** matrix inequalities(LMIs)to be solved in LPV controller design process increase exponentially with the increase in a number of scheduling *** kernel functions are used to obtain the approximate LPV model of highly coupled nonlinear *** error to norm ratio of original and approximate LPV models is introduced as a measure of accuracy of the approximate LPV *** examples conclude the effectiveness of kernel-PCA for LPV model approximation as with the identification of accurate approximate LPV model,computation complexity involved in LPV controller design is decreased exponentially.
