Partial Splenic Artery Embolization in Cirrhosis Is a Safe and Useful Procedure

Background: Portal Hypertension is a common complication of cirrhosis. It leads to splenomegaly which manifests with features of hypersplenism. This results in leucopenia which increases the likelihood of sepsis and prevents treatment with interferon. Thrombocytopenia increases the risk of bleeding including variceal bleeds which make the anemia worse. This study was done to determine the usefulness and safety of partial splenic artery embolization (PSAE) in portal hypertension due to cirrhosis. Methods: Patients with PSAE were identified by using International Classification of Diseases (ICD)-10 coding from medical records and their charts were reviewed retrospectively. 25 patients underwent splenic artery embolization at The Aga Khan University Hospital Karachi from November 2000 to December 2016. 18 patients who underwent PSAE for disabling hypersplenism caused by cirrhosis were included. Patients who were under 18 year of age, or in whom PSAE were performed for reasons other than cirrhosis and those with missing records/incomplete data were excluded (n = 7). Information was collected regarding demographic details, procedure indications, nature, technique, clinical efficacy, repeat embolization and complications along with laboratory and radiological investigations. Results: Eighteen patients of cirrhosis with a mean age of 43.47 ± 10.926 years, of which 14 were males, underwent PSAE (19 procedures). Indications were severe hypersplenism which precluded treatment with interferon and ribavirin (n = 8) and recurrent Gastro-oesophageal variceal (GOV) bleeds due to advanced Child-Pugh grade and thrombocytopenia (n = 10). Hematological parameters improved significantly following PSAE. Three out of eight patients successfully completed interferon + ribavirin treatment for hepatitis C (HCV) infection post PSAE, and GOV bleeds stopped in eight out of 10 patients. Imaging with high definition video camera is an important technique to visualize the drilling conditions and to study the physics of complex multiphase flow associated with the hole cleaning *** main advantage of visualizing multiphase flow in a drilling annulus is that the viewer can easily distinguish fluid phases,flow patterns and thicknesses of cutting *** this paper the hole cleaning process which involves the transportation of cuttings through a horizontal annulus was *** two-phase(solid-liquid)and the three-phase(solid-liquid-gas)flow conditions involved in this kind of annular transportation were experimentally simulated and images were taken using a high definition *** the captured images,a number of important parameters like velocities of different phases,heights of solid beds and sizes of gas bubbles were *** different techniques based on an image analysis software and MATLAB coding were used for the *** results were compared to validate the image analyzing *** visualization technique developed in this paper has a direct application in investigating the critical conditions required for efficient hole cleaning as well as in optimizing the mud program during both planning and operational phases of ***,it would be useful in predicting the cuttings transport performance,estimating solid bed height,gas bubble size,and mean velocities of bubbles/particles.

Three new alkyl substituted anthraquinone derivatives, trivially named as symploquinones A-C(Compounds 1-3) were isolated from Symplocos racemosa. The structures of these compounds were determined on the basis of extensive spectroscopic analyses(UV, IR, Mass, ~1 H-and ^(13)C-NMR, and two-dimensional(2D) NMR techniques). The resulting data were also compared with the reported literature. These compounds were then subjected to antibacterial or antibiofilm testing. Compounds 1 and 3 exhibited good antibacterial activity in the concentration range of 160-83 μg·m L^(-1) against Streptococcus mutans, methicillin resistant Staphylococcus aureus and Proteus mirabilis. Both compounds were further screened for anti-biofilm activity, which revealed promising activities at sub-MIC concentrations. None of the compounds were found to be active against Klebsiella pneumoniae.

A Review: Evolutionary Computations (GA and PSO) in Geotechnical Engineering

This study briefly reviews the application of Genetic Algorithm (GA) and Particle Swarm Optimization (PSO) in geotechnical engineering since GA and PSO are widely used in civil engineering. The application of GA and PSO is studied in three popular families of geotechnical problems including unconfined seepage analysis, slope stability analysis, and foundation design. In each category the available results from different studies are reviewed and compared. The comparison of results shows the desirable accuracy in the predicting of optimal values in the process of analysis and design. The presented methods perform successfully in the reviewed problems. However, PSO predicts the optimum values in fewer numbers of iterations, which suggests higher performance in term of implementation/application.

Gamma irradiation-induced effects on the properties of TiO_2 on fluorine-doped tin oxide prepared by atomic layer deposition

The effect of gamma irradiation with different doses(25–75 kGy) on TiO_2 thin films deposited by atomic layer deposition has been studied and characterized by X-ray diffraction(XRD),photoluminescence measurements,ultraviolet–visible(UV–Vis) spectroscopy,and impedance *** XRD results for the TiO_2 films indicate an enhancement of crystallization after irradiation,which can be clearly observed from the increase in the peak intensities upon increasing the gamma irradiation *** UV–Vis spectra demonstrate a decrease in transmittance,and the band gap of the TiO_2 thin films decreases with an increase in the gamma irradiation *** Nyquist plots reveal that the overall charge-transfer resistance increases upon increasing the gamma irradiation *** equivalent circuit,series resistance,contact resistance,and interface capacitance are measured by simulation using Z-view *** present work demonstrates that gamma irradiation-induced defects play a major role in the modification of thestructural,electrical,and optical properties of the TiO_2 thin films.

H2and H∞Controller Design of Twin Rotor System (TRS)

Control engineering had been the core of all engineering fields all the time. As the name depicts, control of different parameters of various industrial or commercial equipment like plants, vehicles, aircrafts and etc is obtained. Robust and optimal control of these equipments plays a vital role. This paper presents a design of H2 and H∞ control for a Twin Rotor System (TRS). TRS is a multi input multi output (MIMO) nonlinear system. The main objective is to control the angular position of the lever bar of TRS. It is having strong coupling between inputs and outputs. The model is first linearized and then controllers are designed to control the positions of lever bar. Simulations are made in MAT- LAB/SIMULINK. Model parameters are also provided in the end.

Sustainable Development in the Context of Major Infrastructure Projects in United Kingdom

The present paper is aimed at reviewing sustainable development and sustainability approach for infrastructure projects in the United Kingdom. It is imperative that major infrastructure projects (MIPs) adhere to the principles of sustainable development in order to promote sustainability. This requires identifying sustainable strategies that are capable of serving as a guide to inculcating sustainability into major infrastructural projects. The current paper examines ways of inculcating sustainability into infrastructure projects bearing in mind that construction, maintenance and the way we use facilities have significant impacts on the environment. In addition to the fact that, decision making tools on methods of inculcating sustainability into infrastructure project appear too complex to stakeholders;and in most cases they do not provide stakeholders the necessary information required to make a good judgement. Hence, the present paper relies on desk study to gather existing data on infrastructure project and sustainable development. Existing data are obtained from books, scholarly articles and the WebPages of municipal authorities in the UK. Amongst other findings, the paper reveals that the utilization of environmental impact statements and environmental assessment documents at the formative stage of projects will aid the assessment of the level of sustainability to be achieved in any infrastructure development.

研究背景及目的放大内镜是一种可观察到普通内镜难以观察的粘膜微细结构的内镜技术,可将图像放大150倍。Kudo分类法是Kudo等人提出的基于结肠粘膜腺管开口类型(Ⅰ型-Ⅴ型)的分类方法。本文的研究目的是通过Meta分析研究放大内镜依据Kudo腺管开口类型分类法对肿瘤性息肉检测的准确度。固定效应模型或随机效应模型用于基于同质性和异质性的敏感度和特异度的研究。研究方法计算机检索2012年1月至2012年8月间的Pubmed、Embase等数据库,发现6篇符合纳入标准的文献,纳入息肉1697枚。各文献中,结直肠息肉在放大内镜下观察,采用Kudo分类法分类,以组织病理学为金标准,并计算其敏感度和特异度。结果纳入的6篇文献中共有1697枚结直肠息肉,分辨其为肿瘤性息肉或非肿瘤性息肉。首先,息肉在放大内镜下依据Kudo腺管开口类型进行分类,然后取活检进行组织病理学诊断,以病理学为金标准,单独计算每篇文献中的敏感度和特异度,分别应用固定效应模型和随机效应模型进行合并后的敏感度和特异度的计算及总体区间估计,合并后的敏感度(95%CI)为0.92%(0.90-0.93),特异度(95%CI)为0.81%(0.77-0.84),SROC曲线下的面积为0.9632。Kudo分类法是一种发现肿瘤性结肠息肉的有效方法。结论Kudo分类法是一种充满前景而准确性高的诊断方法。通常,Ⅰ型和Ⅱ型腺管开口类型多为非肿瘤性病变,Ⅲ型、Ⅳ型及Ⅴ型腺管开口类型多为肿瘤性病变,而Ⅲ型和Ⅴ型腺管开口类型又可进一步分为ⅢS、ⅢL亚型和Ⅵ、VN亚型。粘膜腺管开口类型及病变深度的确定,可帮助临床操作者对是否行内镜下切除做出判断。如果是良性息肉或微小息肉,患者即可免于手术的痛苦。放大内镜对腺管开口类型的检出率不是100%。
