Partial Splenic Artery Embolization in Cirrhosis Is a Safe and Useful Procedure

Background: Portal Hypertension is a common complication of cirrhosis. It leads to splenomegaly which manifests with features of hypersplenism. This results in leucopenia which increases the likelihood of sepsis and prevents treatment with interferon. Thrombocytopenia increases the risk of bleeding including variceal bleeds which make the anemia worse. This study was done to determine the usefulness and safety of partial splenic artery embolization (PSAE) in portal hypertension due to cirrhosis. Methods: Patients with PSAE were identified by using International Classification of Diseases (ICD)-10 coding from medical records and their charts were reviewed retrospectively. 25 patients underwent splenic artery embolization at The Aga Khan University Hospital Karachi from November 2000 to December 2016. 18 patients who underwent PSAE for disabling hypersplenism caused by cirrhosis were included. Patients who were under 18 year of age, or in whom PSAE were performed for reasons other than cirrhosis and those with missing records/incomplete data were excluded (n = 7). Information was collected regarding demographic details, procedure indications, nature, technique, clinical efficacy, repeat embolization and complications along with laboratory and radiological investigations. Results: Eighteen patients of cirrhosis with a mean age of 43.47 ± 10.926 years, of which 14 were males, underwent PSAE (19 procedures). Indications were severe hypersplenism which precluded treatment with interferon and ribavirin (n = 8) and recurrent Gastro-oesophageal variceal (GOV) bleeds due to advanced Child-Pugh grade and thrombocytopenia (n = 10). Hematological parameters improved significantly following PSAE. Three out of eight patients successfully completed interferon + ribavirin treatment for hepatitis C (HCV) infection post PSAE, and GOV bleeds stopped in eight out of 10 patients. An asymmetrical multilevel inverter with minimum voltage stress and fewer components for photovoltaic renewable-energy system

The enhanced power quality provided by multilevel inverters(MLIs)has made them more appropriate for medium-and high-power applications,including photovoltaic ***,a prevalent limitation involves the necessity for numerous switches and increased voltage stress across these switches,consequently increasing the overall system *** address these challenges,a new 17-level asymmetrical MLI with fewer components and low voltage stress is proposed for the photovoltaic *** innovative MLI configuration has four direct current(DC)sources and 10 *** on the trinary sequence,the proposed topology uses photovoltaics with boost converters and fuzzy logic controllers as its DC *** equations are used to calculate cru-cial parameters for this proposed design,including total standing voltage per unit(TSVPU),cost function per level(CF/L),component count per level(CC/L)and voltage stress across the *** comparison is conducted by considering switches,DC sources,TSVPU,CF/L,gate driver circuits and CC/L with other existing MLI *** analysis is carried out under various conditions,encompassing different levels of irradiance,variable loads and modulation *** reduce the total harmonic distortion of the suggested topology,the phase opposition disposition approach has been *** suggested framework is simulated in MATLAB®/Simulink®.The results indicate that the proposed topology achieves a well-distributed stress profile across the switches and has CC/L of 1.23,TSVPU of 5 and CF/L of 4.58 and 5.76 with weight coefficients of 0.5 and 1.5,*** values are not-ably superior to those of existing MLI *** results demonstrate that the proposed topology maintains a consistent output at varying irradiance levels with FLCs and exhibits robust performance under variable loads and diverse modulation ***,the total harmonic distortion achieved with phase opposition disposi

The software development process mostly depends on accurately identifying both essential and optional ***,user needs are typically expressed in free-form language,requiring significant time and human resources to translate these into clear functional and non-functional *** address this challenge,various machine learning(ML)methods have been explored to automate the understanding of these requirements,aiming to reduce time and human ***,existing techniques often struggle with complex instructions and large-scale *** our study,we introduce an innovative approach known as the Functional and Non-functional Requirements Classifier(FNRC).By combining the traditional random forest algorithm with the Accuracy Sliding Window(ASW)technique,we develop optimal sub-ensembles that surpass the initial classifier’s accuracy while using fewer *** results demonstrate that our FNRC methodology performs robustly across different datasets,achieving a balanced Precision of 75%on the PROMISE dataset and an impressive Recall of 85%on the CCHIT *** datasets consistently maintain an F-measure around 64%,highlighting FNRC’s ability to effectively balance precision and recall in diverse *** findings contribute to more accurate and efficient software development processes,increasing the probability of achieving successful project outcomes.

Diagnosing various diseases such as glaucoma,age-related macular degeneration,cardiovascular conditions,and diabetic retinopathy involves segmenting retinal blood *** task is particularly challenging when dealing with color fundus images due to issues like non-uniformillumination,low contrast,and variations in vessel appearance,especially in the presence of different ***,the speed of the retinal vessel segmentation system is of utmost *** the surge of now available big data,the speed of the algorithm becomes increasingly important,carrying almost equivalent weightage to the accuracy of the *** address these challenges,we present a novel approach for retinal vessel segmentation,leveraging efficient and robust techniques based on multiscale line detection and mathematical *** algorithm’s performance is evaluated on two publicly available datasets,namely the Digital Retinal Images for Vessel Extraction dataset(DRIVE)and the Structure Analysis of Retina(STARE)*** experimental results demonstrate the effectiveness of our method,withmean accuracy values of 0.9467 forDRIVE and 0.9535 for STARE datasets,aswell as sensitivity values of 0.6952 forDRIVE and 0.6809 for STARE ***,our algorithmexhibits competitive performance with state-of-the-art ***,it operates at an average speed of 3.73 s per image for DRIVE and 3.75 s for STARE *** is worth noting that these results were achieved using Matlab scripts containing multiple *** suggests that the processing time can be further reduced by replacing loops with *** the proposed algorithm can be deployed in real time *** summary,our proposed system strikes a fine balance between swift computation and accuracy that is on par with the best available methods in the field.

In today’s rapidly evolving landscape of communication technologies,ensuring the secure delivery of sensitive data has become an essential *** overcome these difficulties,different steganography and data encryption methods have been proposed by researchers to secure *** of the proposed steganography techniques achieve higher embedding capacities without compromising visual imperceptibility using LSB *** this work,we have an approach that utilizes a combinationofMost SignificantBit(MSB)matching andLeast Significant Bit(LSB)*** proposed algorithm divides confidential messages into pairs of bits and connects them with the MSBs of individual pixels using pair matching,enabling the storage of 6 bits in one pixel by modifying a maximum of three *** proposed technique is evaluated using embedding capacity and Peak Signal-to-Noise Ratio(PSNR)score,we compared our work with the Zakariya scheme the results showed a significant increase in data concealment *** achieved results of ourwork showthat our algorithmdemonstrates an improvement in hiding capacity from11%to 22%for different data samples while maintaining a minimumPeak Signal-to-Noise Ratio(PSNR)of 37 *** findings highlight the effectiveness and trustworthiness of the proposed algorithm in securing the communication process and maintaining visual integrity.

This study aims to demystify the role of green energy and green technology in establishing the nexus between behavioural intentions of tourists,technologies,and digital payments by using Perceived value(PV),Compatibility(CO),Perceived Enjoyment(PE),and Social Influence(SI)as a predictor variables,Trust(TR)and Satisfaction(SA)as a mediating variables and Behavioural Intentions(BI)as an outcome *** the empirical estimation,we employ smart PLS-SEM,TAM(Technology Acceptance Model)and SPSS and Tested the LCC *** findings suggest that green energy and perceived value have the highest positive impact on tourists’trust towards digital payments followed by compatibility,social influence and perceived ***,tourists’satisfaction and green technology is one of the important determinants of choosing any digital mode of payment,is mostly influence by perceived value,perceived enjoyment,compatibility and social ***,if we choose between trust and satisfaction,trust plays a significant role in exploring the behavioural intentions of tourists about green energy and green technology followed by tourists’*** addition,Tourists’trust and satisfaction are highly correlated and influence each *** study offers novel policy implications in terms of use of green technology and green energy in enhancing trust and satisfaction of tourists in order to deeper understanding of different dimensions of digital payments and M-wallets,and allowing them to explore the long-term value inherent of digital payments and M-wallets.

Travelling wave solutions have been played a vital role in demonstrating the wave character of nonlinear problems arising in the field of ocean engineering and *** describe the propagation of the nonlinear wave phenomenon in the ocean(for example,wind waves,tsunami waves),a variety of evolution equations have been suggested and investigated in the existing *** paper studies the dynamic of travelling periodic and solitary wave behavior of a double-dispersive non-linear evolution equation,named the Sharma-Tasso-Olver(STO)*** evolution equations with double dispersion enable us to describe nonlinear wave propagation in the ocean,hyperplastic rods and other mediums in the field of science and *** analyze the wave solutions of this model using a combination of numerical simulations and Ansatz *** analysis shows that the travelling wave solutions involve a range of parameters that displays important and very interesting properties of the wave *** relevance of the parameters in the travelling wave solutions is also *** simulating numerically,we demonstrate how parameters in the solutions influence the phase speed as well as the travelling and solitary ***,we discuss instantaneous streamline patterns among the obtained solutions to explore the local direction of the components of the obtained solitary wave solutions at each point in the coordinate(x,t).
