Autoimmune diseases of the central nervous system(CNS) like multiple sclerosis(MS) are characterized by inflammation and demyelinated lesions in white and grey matter regions. While inflammation is present at all stages of MS, it is more pronounced in the relapsing forms of the disease, whereas progressive MS(PMS) shows significant neuroaxonal damage and grey and white matter atrophy. Hence, disease-modifying treatments beneficial in patients with relapsing MS have limited success in PMS. BAF312(siponimod) is a novel sphingosine-1-phosphate receptor modulator shown to delay progression in PMS. Besides reducing inflammation by sequestering lymphocytes in lymphoid tissues, BAF312 crosses the blood-brain barrier and binds its receptors on neurons, astrocytes and oligodendrocytes. To evaluate potential direct neuroprotective effects, BAF312 was systemically or locally administered in the CNS of experimental autoimmune encephalomyelitis mice with distinct grey-and white-matter lesions(focal experimental autoimmune encephalomyelitis using an osmotic mini-pump). Ex-vivo flow cytometry revealed that systemic but not local BAF312 administration lowered immune cell infiltration in animals with both grey and white matter lesions. Ex-vivo voltage-sensitive dye imaging of acute brain slices revealed an altered spatio-temporal pattern of activation in the lesioned cortex compared to controls in response to electrical stimulation of incoming white-matter fiber tracts. Here, BAF312 administration showed partial restore of cortical neuronal circuit function. The data suggest that BAF312 exerts a neuroprotective effect after crossing the blood-brain barrier independently of peripheral effects on immune cells. Experiments were carried out in accordance with german and EU animal protection law and approved by local authorities(Landesamt für Natur, Umwelt und Verbraucherschutz Nordrhein-Westfalen;87-51.04.2010.A331) on December 28, 2010.

BACKgROUND Splanchnic vein thrombosis(SVT)is a major complication of moderate and severe acute *** is no consensus on whether therapeutic anticoagulation should be started in patients with acute pancreatitis and *** To gain insight into current opinions and clinical decision making of pancreatologists regarding SVT in acute *** A total of 139 pancreatologists of the Dutch Pancreatitis Study group and Dutch Pancreatic Cancer group were approached to complete an online survey and case vignette *** threshold to assume group agreement was set at 75%.RESULTS The response rate was 67%(n=93).Seventy-one pancreatologists(77%)regularly prescribed therapeutic anticoagulation in case of SVT,and 12 pancreatologists(13%)for narrowing of splanchnic vein *** most common reason to treat SVT was to avoid complications(87%).Acute thrombosis was the most important factor to prescribe therapeutic anticoagulation(90%).Portal vein thrombosis was chosen as the most preferred location to initiate therapeutic anticoagulation(76%)and splenic vein thrombosis as the least preferred location(86%).The preferred initial agent was low molecular weight heparin(LMWH;87%).In the case vignettes,therapeutic anticoagulation was prescribed for acute portal vein thrombosis,with or without suspected infected necrosis(82%and 90%),and thrombus progression(88%).Agreement was lacking regarding the selection and duration of long-term anticoagulation,the indication for thrombophilia testing and upper endoscopy,and about whether risk of bleeding is a major barrier for therapeutic *** In this national survey,the pancreatologists seemed to agree on the use of therapeutic anticoagulation,using LMWH in the acute phase,for acute portal thrombosis and in the case of thrombus progression,irrespective of the presence of infected necrosis.

Effect of exogenous fibrolytic enzymes on performance and blood profile in early and mid-lactation Holstein cows

The supplementation of exogenous fibrolytic enzymes(EFE) to dairy cows diets could be a strategy to improve fiber degradation in the rumen which is especially important for the early lactating cows characterized by a high milk energy output and an insufficient energy intake. The objective of this study was to examine the effects of a fibrolytic enzyme product(Roxazyme g2 Liquid, 3.8 and 3.9 mL/kg total mixed ration [TMR] DM) supplemented to a TMR on production performance and blood parameters of dairy cows during early(trial 1) and mid-lactation(trail 2). In addition, rumination activity was measured in trial 2. The nutrient digestibility of the experimental TMR was obtained by using wethers. In the digestibility trial, EFE was supplemented at a rate of 4.4 m L/kg Roxazyme g2 Liquid TMR-DM. The TMR contained 60% forage and 40% concentrate(DM basis). Twenty eight 50 ± 16 days in milk(DIM)and twenty six 136 ± 26 DIM Holstein cows were used in two 8-wk completely randomized trails,stratified by parity and milk yield level. One milliliter of the enzyme product contained primarily cellulase and xylanase activities(8,000 units endo-1,4-? glucanase, 18,000 units endo-1,3(4)-? glucanase and 26,000 units 1,4-? xylanase). No differences in digestibility of DM, OM, CP, NDF and ADF were observed(P >0.05) between the control and the EFE supplemented TMR. Addition of EFE to the TMR fed to early(trial 1) and mid-lactation cows(trial 2) did not affect daily dry matter intake(DMI), milk yield,4% fat-corrected milk, energy-corrected milk(ECM), concentration of milk fat, protein, fat-protein-quotients, somatic cell score, energy balance, and gross feed efficiency of early and mid-lactation cows(P >0.05). Mid-lactation cows(trial 2) fed with TMR enzyme showed a tendency of a slightly higher ECM yield(P = 0.09).
