Doppler measurements in fetal descending aorta and umbilical artery can predict borderline oxygenation in pre-eclampsia and HELLP syndrome

Objective: To determine diagnostic utility of Doppler measurements in fetal descending aorta and umbilical arteries in predicting intrauterine growth retarded (IUGR) fetuses at risk in pregnancies complicated by preeclampsia and/or HELLP syndrome. Methods: Doppler measurements were taken in fetal descending aorta and umbilical arteries of 53 patients with pre-eclampsia, 10 of whom had HELLP syndrome using fetometry (ACUsON 128XP/10, 3.5 MHz probe). These values were compared with those of 44 appropriate-for-gestational age singleton pregnancies (AGA). Doppler Indices (Resistance Index (RI), systolic/Diastolic (s/D) ratio) and end-diastolic flows were related to fetal heart rate (FHR) during contraction stress test, to cord blood parameters (pH, Base Excess) and to Apgar scores. Results: In contrast to AGA fetuses, IUGR fetuses had decreased end-diastolic flow and an increase of Doppler Indices significantly earlier in the descending aorta (p sed RI’s, s/D ratios and a decrease of end-diastolic flow in fetal aorta were significantly correlated to frequency of FHR decelerations during contraction stress tests, pH, Base Excess (p scores in IUGR fetuses. Conclusion: A decrease in end-diastolic flow paralleled with an increase in Doppler indices in fetal descending aorta reflect oxygen deprivation in IUGR fetuses during pre-eclamptic pregnancies with or without HELLP syndrome. While ductus venosus and umbilical artery are more frequently used nowadays to determine fetal oxygen deprivation, Doppler measurements in fetal descending aorta provide additional information for early detection of fetuses at risk for IUGR in pregnancies complicated with pre-eclampsia and/or HELLP

Clinical Nurses’Research Capacity Building in Practice—A systematic Review

The aim of this study was to identify and evaluate evidence of clinical nurses’ research capacity building in practice. A systematic review of studies of nurses’ research capacity building in practice was performed. The quality of the articles was evaluated and reflected on in accordance with the Quality Assessment and Validity Tool for Correlation studies. The literature searches identified a total of 4748 abstracts and titles. Eight quantitative studies were included in the evaluation. Three themes emerged from the analysis: Failure to ensure research quality and standards, Developing a research culture and Collaboration and organization of research utilization. The first theme has one sub-theme: Lack of knowledge about how to increase research utilization. The second theme is based on three sub-themes: Ability to identify clinical problems, changing nurses’ attitudes to research and research supervision. Finally, the third theme has one sub-theme: Funding as a success factor. In conclusion, research capacity building requires the development of research competence to generate knowledge that enhances quality and patient safety. Nurse leaders are essential for establishing evidence-based practice and a research culture, thus enhancing nurses’ scientific attitudes and capacity.

Following votes by the Pliensbachian Working Group, the Jurassic subcommission and the International Commission on stratigraphy, lUGs ratified the proposed Global Boundary stratotype section and Point （GssP） for the base of the Pliensbachian stage （Lower Jurassic） at the base of bed 73b in the Wine Haven section, Robin Hood＇s Bay, Yorkshire Coast, UK. This level contains the characteristic ammonite association Bifericeras donovani Dommergues and Meister and Apoderoceras sp. Complementary data include： a） strontium-isotope stratigraphy, based on analysis of belemnites which yield a calcite 87sr/^86sr ratio for the boundary level of 0. 707425 and data supporting interpretation of continuous sedimentation; b） Belemnite oxygen-isotope data indicate a significant temperature drop （-5 ℃） across the boundary at this locality; c） A Transgressive systems Tract （TsT） initiated in the Aplanaturn subzone （uppermost sinemurian） continues into the Lowermost Pliensbachian （Taylori subzone）; it forms part of a transgressive facies cycle sensu Graciansky et al. （1998）; d） The Upper sinemurian to lowermost Plensbachian at Wine Haven section has a predominantly norreal magnetic polarity, but two discrete reversed polarity magnetozones are present. The first spans much of the latest sinemurian Aplanatum subzone. It terminates 〈0.5m below the sinemurian-Pliensbachian boundary and may prove a valuable chronostratigraphic marker The second extends from the latest Oxynotum subzone probably through to the lower part of the Raricostatum subzone.

本研究以国际玉米小麦改良中心(CIMMYT)、国际干旱地区农业研究中心(ICARDA)、法国Agropolis研究所和中国农业科学院作物科学研究所提供的数据为基础,建立了包括134个ssR引物、2457个普通小麦基因型的指纹图谱数据库。利用荧光标记法的分析结果,用代表性基因型在某一位点的扩增片段作为银染法的读带依据,开发了ssR分子标记试剂盒,包括46对ssR引物及其PCR反应程序、代表性基因型的DNA样品、592个等位变异的代表性基因型清单及试剂盒使用说明等。该数据库的建立和试剂盒的开发,为利用ssR分子标记技术进行小麦种质遗传多样性研究,实现小麦遗传资源和信息资源全球共享提供了重要工具和技术平台。

In an experiment carried out at the Prague Asterix Laser system at laser intensities relevant to shock ignition conditions(I>10^(16) W/cm^(2)),the heating and transport of hot electrons were studied by using several complementary diagnostics,i.e.,K_(α)time-resolved imaging,hard x-ray filtering(a bremsstrahlung cannon),and electron *** with differing composition from low Z(parylene N)to high Z(nickel)were used in multilayer planar targets to produce plasmas with different coronal temperature and collisionality and modify the conditions of hot-electron *** variety of available diagnostics allowed full characterization of the population of hot electrons,retrieving their conversion efficiency,time generation and duration,temperature,and angular *** obtained results are shown to be consistent with those from detailed simulations and similar inertial confinement fusion *** on the measured data,the advantages,reliability,and complementarity of the experimental diagnostics are discussed.

Laser–plasma interaction(LPI)at intensities 1015–1016 W·cm^-2 is dominated by parametric instabilities which can be responsible for a significant amount of non-collisional absorption and generate large fluxes of high-energy nonthermal *** a regime is of paramount importance for inertial confinement fusion(ICF)and in particular for the shock ignition *** this paper we report on an experiment carried out at the Prague Asterix Laser system(PALs)facility to investigate the extent and time history of stimulated Raman scattering(sRs)and two-plasmon decay(TPD)instabilities,driven by the interaction of an infrared laser pulse at an intensity^1.2×1016 W·cm^-2 with a^100μm scalelength plasma produced from irradiation of a flat plastic *** laser pulse duration(300 ps)and the high value of plasma temperature(~4 ke V)expected from hydrodynamic simulations make these results interesting for a deeper understanding of LPI in shock ignition *** results show that absolute TPD/sRs,driven at a quarter of the critical density,and convective sRs,driven at lower plasma densities,are well separated in time,with absolute instabilities driven at early times of interaction and convective backward sRs emerging at the laser peak and persisting all over the tail of the ***-scattering sRs,driven at low plasma densities,is also clearly *** results are compared to fully kinetic large-scale,two-dimensional ***-in-cell results,beyond reproducing the framework delineated by the experimental measurements,reveal the importance of filamentation instability in ruling the onset of sRs and stimulated Brillouin scattering instabilities and confirm the crucial role of collisionless absorption in the LPI energy balance.

Psychometric Properties of the swedish Version of the Fear of Complications Questionnaire

Objectives: To translate and evaluate the psychometric properties of the swedish version of the Fear of Complications Questionnaire. Design: Cross-sectional study design and scale development. settings: Totally, 469 adults (response rate 63.5%) with Type 1 diabetes completed the questionnaires. Participants were recruited from two university hospitals in sweden. Participants: Eligible patients were those who met the following inclusion criteria: diagnosed with Type 1 diabetes, diabetes duration of at least 1 year and aged at least 18 years. Methods: The Fear of Complications Questionnaire was translated using the forward-backward translation method. Factor analyses of the questionnaire were performed in two steps using both exploratory and confirmatory factor analysis. Convergent validity was examined using the Hospital Anxiety and Depression scale and the Fear of Hypoglycaemia Fear survey. Internal consistency was estimated using Cronbach’s alpha. Results: Exploratory factor analysis supported a two-factor solution. One factor contained three items having to do with fear of kidney-related complications and one factor included the rest of items concerning fear of other diabetes-related complications, as well as fear of complications in general. Internal consistency was high Cronbach’s alpha 0.96. The findings also gave support for convergent validity, with significant positive correlations between measures (r = 0.51 to 0.54). Conclusion: The clinical relevance of the identified two-factor model with a structure of one dominant subdomain may be considered. We suggest, however a one-factor model covering all the items as a relevant basis to assess fear of complications among people with Type 1 diabetes.

The prevalence of pediatric obesity continues to increase worldwide,bringing with it various metabolic,functional,social,and psychological complications.1 Both the prevention and treatment of childhood obesity must be based on multidisciplinary approaches combining nutrition,physical activity(PA),and psychological support.2 As an essential element of these multicomponent strategies,regular physical activity has been acknowledged as having beneficial effects on children's and adolescents,body composition,physical fitness,and metabolic profile,as well as on their health-related quality of life,social and psychological health,and academic achievement.3,4 Given that only a relatively small proportion of children and adolescents meet the public health recommendations for PA,5 and given that weight loss exercise-based interventions suffer from a high attrition rate,there is a clear need for appropriate PAs.
