Anemia and long-term outcome in adjuvant and neoadjuvant radiochemotherapy of stage Ⅱ and Ⅲ rectal adenocarcinoma:The Freiburg experience(1989-2002)

AIM： To evaluate the long-term outcome of standard 5-FU based adjuvant or neoadjuvant radiochemotherapy and to identify the predictive factors, especially anemia before and after radiotherapy as well as hemoglobin increase or decrease during radiotherapy. METHODs： Two hundred and eighty-six patients with Union International Contre Cancer （UICC） stage Ⅱ and Ⅲ rectal adenocarcinomas, who underwent resection by conventional surgical techniques （low anterior or abdominoperineal resection）, received either postoperative （n = 233） or preoperative （n = 53） radiochemotherapy from January 1989 until July 2002. Overall survival （OAs）, cancer-specific survival （Css）, disease-free survival （DFs）, local-relapse-free （LRs） and distant-relapse-free survival （DRs） were evaluated using Kaplan-Meier, Log-rank test and Cox＇s proportional hazards as statistical methods. Multivariate analysis was used to identify prognostic factors. Median follow-up time was 8 years. REsULTs： Anemia before radiochemotherapy was an independent prognostic factor for improved DFs （risk ratio 0.76, P= 0.04） as well as stage, grading, R status （free radial margins）, type of surgery, carcinoembryonic antigen （CEA） levels, and gender. The univariate analysis revealed that anemia was associated with impaired LRs（better local control） but with improved DFs. In contrast, hemoglobin decrease during radiotherapy was an independent risk factor for DFs （risk ratio 1.97, P= 0.04）. During radiotherapy, only 30.8% of R0-resected patients suffered from hemoglobin decrease compared to 55.6% if R1/2 resection was performed （P= 0.04）. The 5-year OAs, Css, DFs, LRs and DRs were 47.0%, 60.0%, 41.4%, 67.2%, and 84.3%, respectively. significant differences between preoperative and postoperative radiochemotherapy were not found. shakespeare＇s Language strategies in Hamlet

This study investigates the language strategies used by shakespeare in The Tragedy of Hamlet, Prince of Denmark （1603）. Emphasis is given to the type of language or register shakespeare provides his characters with in order to give them genuine or assumed identities. The linguistic register of the three major characters of Claudius, the Ghost, and Hamlet is explored in light of the Elizabethan cultural context. This concern with these three characters stems from the basic assumption of the paper that the play is a struggle between Claudius and the Ghost over Hamlet. Claudius speaks the language of a monarch which is informed by the code of society; the Ghost, who used to speak such language of sovereignty while alive, speaks a language of the dead informed by a newly acquired experience not familiar to human beings. On the other hand, Hamlet＇s use of language is the most peculiar in the play. His register keeps changing according to his tragic growth and his readiness for action. The study indicates that the language functions shakespeare utilizes in Hamlet are numerous. Language is used creatively for a variety of purposes in addition to communication. The playwright employs language as a shield for self-defense, a tool for defining and hiding identity and misleading and manipulating others, a means of search for reality, and a tool for punishment, among other functions. The study findings invite further research into shakespeare＇s use of language in his plays. No detailed in-depth studies of shakespearean language and its implications exist.

“With This Disease, You Take Whatever Chances There Are”—A study on socio-Cultural and Psychological Aspects of Experiments Regarding Huntington’s Disease

Although relatively rare, Huntington’s disease (HD) has fatal consequences. There is no cure for the disease, which leads to an early death. Worldwide, scientists are trying to develop therapeutic methods that could cure the disease, including new molecular gene therapeutic methods. At Lund University, research on HD is now about to step from animal models to trials with humans. The project is special in its design since it involves both medical studies and socio-cultural and psychological research to explore and meet the many challenges that experimental trials with HD patients give rise to. The aim of the present study was to investigate the viewpoints of individuals affected by HD on the issues of participation and exerting influence if taking part in a medical study on gene therapy that has not previously been tested on humans. A total of 16 participants, recruited through the national association for HD and through the neurological clinic at Lund University in sweden, took part in a focus group or in a survey study. A thematic analysis, to explore the transcribed text from the focus groups as well as from the written mail responses, was conducted by means of Nvivo, a program for qualitative data analysis. Results showed three main themes expressing reasons for participation: participation as a last resort, as an activity of hope, and as a way to take responsibility for the development of a cure that will benefit future generations. The responses relating to the question about affected individuals’ view of exerting influence resulted in two themes. The first theme was having a voice when researchers design experiments, which may give a sense of receiving respect. The second was that influence is an essential part of the information process before agreeing to take part in an experimental trail.

The organization of societal conflicts by pavement ants Tetramorium caespitum： an agent-based model of amine-mediated decision making

Ant colonies self-organize to solve complex problems despite the simplicity of an individual ant＇s brain. Pavement ant Tetramorium caespitum colonies must solve the problem of defending the ter- ritory that they patrol in search of energetically rich forage. When members of 2 colonies randomly interact at the territory boundary a decision to fight occurs when： 1） there is a mismatch in nest- mate recognition cues and 2） each ant has a recent history of high interaction rates with nestmate ants. Instead of fighting, some ants will decide to recruit more workers from the nest to the fighting location, and in this way a positive feedback mediates the development of colony wide wars. In ants, the monoamines serotonin （5-HT） and octopamine （OA） modulate many behaviors associated with colony organization and in particular behaviors associated with nestmate recognition and ag- gression. In this article, we develop and explore an agent-based model that conceptualizes how in- dividual changes in brain concentrations of 5-HT and OA, paired with a simple threshold-based de- cision rule, can lead to the development of colony wide warfare. Model simulations do lead to the development of warfare with 91% of ants fighting at the end of 1 h. When conducting a sensitivity analysis, we determined that uncertainty in monoamine concentration signal decay influences the behavior of the model more than uncertainty in the decision-making rule or density. We conclude that pavement ant behavior is consistent with the detection of interaction rate through a single timed interval rather than integration of multiple interactions.

AIC-Based selection of Growth Models: The Case of Piglets from Organic Farming

The selection and comparison of different growth models for describing weight gain of piglets raised in organic farming is investigated by using the Akaike’s Information Criterion (AIC). In total, 49,699 data points of 5188 piglets recorded between 2007 and 2013 were considered. From the day of birth, up to 40 days (i.e. until weaning) the model of von Bertalanffy was favored by the AIC. This model is with 60.32% more likely to truly reflect reality than any other of the analyzed models. Up to 105 days, the two-linear model was favored by the AIC (probability 99.75%). The intersection point of the two-linear model was calculated by 53.8 days, which fitted well to the actual change in the food situations.
