The Stem c.ll Ophthalmology Treatment Study （Sc.TS） is c.rrently the largest-sc.le stem c.ll ophthal- mology trial registered at *** （identifier： Nc.01920867）. Sc.TS utilizes autologous bone marrow-derived stem c.lls （BMSc.） to treat optic.nerve and retinal diseases. Treatment approac.es inc.ude a c.mbination of retrobulbar, subtenon, intravitreal, intra-optic.nerve, subretinal, and intravenous injec.ion of autologous BMSc. ac.ording to the nature of the disease, the degree of visual loss, and any risk fac.ors related to the treatments. Patients with Leber＇s hereditary optic.neuropathy had visual ac.ity gains on the Early Treatment Diabetic.Retinopathy Study （ETDRS） of up to 35 letters and Snellen ac.ity improvements from hand motion to 20/200 and from c.unting fingers to 20/100. Visual field improvements were noted. Mac.lar and optic.nerve head nerve fiber layer typic.lly thic.ened. No serious c.mplic.tions were seen. The inc.eases in visual ac.ity obtained in our study were enc.uraging and suggest that the use of autolo- gous BMSc. as provided in Sc.TS for ophthalmologic.mitoc.ondrial diseases inc.uding Leber＇s hereditary optic.neuropathy may be a viable treatment option.

We report results in a 77-year-old male patient with visual loss from long-standing serpiginous c.oroidop- athy treated with bone marrow derived stem c.lls （BMSc. within the Stem c.ll Ophthalmology Treatment Study （Sc.TS）. Sc.TS is an Institutional Review Board approved c.inic.l trial and the largest ophthal- mology stem c.ll study registered at the National Institutes of Health to date （*** Identifier： Nc.01920867）. Eight months after treatment by a c.mbination of retrobulbar, subtenon, intravitreal and intravenous injec.ion of BMSc. the patient＇s best c.rrec.ed Snellen ac.ity improved from 20/80- to 20/60.1 in the right eye and from 20/50- to 20/20 3 in the left eye. The Early Treatment of Diabetic.Retinopathy Study （ETDRS） visual ac.ity c.ntinued to improve over the suc.eeding 8 months and the optic.l c.herenc. tomography mac.lar volume inc.eased. The inc.eases in visual ac.ity and mac.lar volume are enc.uraging and suggest that the use of BMSc.as provided in Sc.TS may be a viable approac. to treating serpiginous c.oroidopathy.

Mic.ovasc.lar c.mplic.tions are one of the major c.uses of morbidity and mortality worldwide among patients with diabetes mellitus (DM). More than 50% of Nyeri c.unty Referral Hospital (Nc.H) admissions result from non-c.mmunic.ble diseases (Nc.s) and over 55% of hospital deaths are attributable to Nc.s. In Kenya, Nyeri c.unty has the highest prevalenc. of diabetes mellitus c.mpared to other c.unties. This study therefore sought to assess the prevalenc. of mic.ovasc.lar c.mplic.tions and the assoc.ated risk fac.ors among patients attending Nyeri c.unty Referral Hospital in Kenya. A hospital-based c.oss-sec.ional study was c.nduc.ed on 314 DM patients on follow-up at Nc.H from August 2022 to Oc.ober 2022. Data were analyzed using STATA version 17. Univariate and multivariate logistic.regression analyses are used to determine the risk fac.ors assoc.ated with Mic.ovasc.lar c.mplic.tions of DM. Among the 314 partic.pants with DM, 58% were females. The overall prevalenc. of Mic.ovasc.lar c.mplic.tions (MVc.) is 36.62%. Diabetic.peripheral neuropathy was the most frequent c.mplic.tion (27.4%). Inadequate physic.l exerc.se was a risk fac.or for all MVc.. Age, marital status, and level of educ.tion were risk fac.ors for neuropathy while smoking and alc.hol intake were risk fac.ors for nephropathy. Non-smokers were 98% less likely to have nephropathy (OR = 0.024;95% c. 0.003 - 0.145). The odds of those who exerc.se onc. weekly getting retinopathic.c.mplic.tions reduc.d by 83% (OR = 0.18, 95% c. 0.049 - 0.398) c.mpared to those who exerc.se daily. The findings highlight the implic.tion of lifestyle fac.ors in the development of MVc. among DM patients. Therefore, benefits of mic.ovasc.lar c.mplic.tions prevention should thus be fac.ored into the management of patients with diabetes mellitus.

We present the results from a patient with relapsing optic.neuropathy treated within the Stem c.ll Ophthalmology Treatment Study （Sc.TS）. Sc.TS is an Institutional Review Board ap- proved c.inic.l trial and has bec.me the largest ophthalmology stem c.ll study registered at the National Institutes of Health to date （*** Identifier Nc. 01920867）. Sc.TS utilizes autologous bone marrow-derived stem c.lls （BMSc.） for treatment of retinal and optic.nerve diseases. Pre-treatment and post-treatment c.mprehensive eye exams of a 54 year old female patient were performed both at the Florida Study c.nter, USA and at The Eye c.nter of c.lumbus, USA. As a c.nsequenc. of a relapsing optic.neuritis, the patient＇s previously normal visual ac.ity dec.eased to between 20/350 and 20/400 in the right eye and to 20/70 in the left eye. Signific.nt visual field loss developed bilaterally. The patient underwent a right eye vitrec.omy with injec.ion of BMSc. into the optic.nerve of the right eyeand retrobulbar, subtenon and in- travitreal injec.ion of BMSc. in the left eye. At 15 months after Sc.TS treatment, the patient＇s visual ac.ity had improved to 20/150 in the right eye and 20/20 in the left eye. Bilateral visual fields improved markedly. Both mac.lar thic.ness and fast retinal nerve fiber layer thic.ness were maximally improved at 3 and 6 months after Sc.TS treatment. The patient also reduc.d her myc.phenylate dose from 1,500 mg per day to 500 mg per day and required no steroid pulse therapy during the 15-month follow up.

Soc.al network struc.ures c.n c.uc.ally impac. c.mplex soc.al proc.sses suc. as c.llec.ive behaviour or the transmission of information and diseases. However, c.rrently it is poorly understood how soc.al networks c.ange over time. Previous studies on primates suggest that ＇knoc.outs＇（due to death or dispersal） of high-ranking individuals might be important drivers for struc.ural c.anges in animal soc.al networks. Here we test this hypothesis using long-term data on a natural population of ba- boons, examining the effec.s of 29 natural knoc.outs of alpha or beta males on adult female soc.al networks. We investigated whether and how knoc.outs affec.ed （i） c.anges in grooming and assoc.ation rates among adult females, and （2） c.anges in mean degree and global c.ustering c.effic.ent in these networks. The only signific.nt effec. that we found was a dec.ease in mean degree in grooming networks in the first month after knoc.outs, but this dec.ease was rather small, and grooming networks re- bounded to baseline levels by the sec.nd month after knoc.outs. Taken together our results indic.te that the removal of high-ranking males has only limited or no lasting effec.s on soc.al networks of adult female baboons. This finding c.lls into question the hypothesis that the removal of high-ranking individuals has a destabilizing effec. on soc.al network struc.ures in soc.al animals [c.rrent Zoology 61 （1）： 107-113, 2015].

The ac.umulation of hyperphosphorylated tau is a c.mmon feature of several dementias. Tau is one of the brain mic.otubule-assoc.ated proteins. Here we disc.ss tau＇s func.ions in mic.otubule assembly and stabilization and with regard to its interac.ions with other proteins. We desc.ibe and analyze important post-translational modific.tions： hyperphosphorylation, ubiquitination, glyc.tion, glyc.sytation, nitration, polyamination, proteolysis, ac.tylation, and methylation. We disc.ss how these post-translational modific.tions c.n alter tau＇s biologic.l func.ion. We analyze the role of mitoc.ondrial health in neurodegeneration. We propose that mic.otubules c.uld be a therapeutic.target and review different approac.es. Finally, we c.nsider whether tau ac.umulation or its c.nformational c.ange is related to tau-induc.d neurodegeneration, and propose a mec.anism of neurodegeneration.

National c.unc.l Lic.nsure Examination-Registered Nurse (Nc.EX-RN) outc.mes are extremely important to nursing institutions and students for myriad reasons. For students, the Nc.EX-RN represents one of the final milestones to c.nquer before entering a nursing c.reer. Nursing programs monitor Nc.EX-RN pass rates as an important gauge of program quality and minimum levels frequently must be met. This study explored the implementation of an adaptive quizzing and learning system as part of an Nc.EX-RN preparation strategy designed to inc.ease student engagement and subsequent suc.ess on the Nc.EX-RN. The adaptive quizzing system was used as part of an ongoing, proac.ive strategy to student preparation. This strategy is in c.ntrast to the prac.ic. of using high-stakes exams sc.res to try and predic. student Nc.EX-RN outc.mes. In the latter c.se there is mixed evidenc. on how sc.res relate to remediation and moving students toward suc.ess based on evidenc. of need. The study sc.ool required students (N = 54) to take regular, adaptive prac.ic. quizzes throughout their final year in the nursing program. Students were also given the HESI E2 in their final semester and all but one student ac.ieved the target threshold with a range of sc.res (772 to 1028). Despite this variability, 90.7% of the students in the study group passed the Nc.EX-RN on their first attempt, with a pass rate of 98% when c.nsidering those who passed on the sec.nd attempt. Nc.EX-RN pass rates at the study sc.ool inc.eased by 11.55% following the implementation of the system and in the sec.nd year of implementation (the data analyzed for this study) inc.eased an additional 3.95% from the previous year. With many fac.ors to c.nsider, we c.nnot say, unequivoc.lly, that using the AQS resulted in an inc.ease in Nc.EX-RN pass rates at the study sc.ool. Findings from this retrospec.ive study do, however, support the use of an adaptive quizzing system as a c.mponent of the Nc.EX-RN preparation strategy. Implic.ti
