Acute kidney injury due to intravenous detergent poisoning:A case report

BACKGROUND Detergent poisoning mostly occurs through oral ingestion(>85%),ocular exposure(<15%),or dermal exposure(<8%).Reports of detergent poisoning through an intravenous injection are extremely rare.In addition,there are very few cases of renal toxicity directly caused by detergents.Here,we report a unique case of acute kidney injury caused by detergent poisoning through an accidental intravenous injection.CASE SUMMARY A 61-year-old man was intravenously injected with 20 mL of detergent by another patient in the same room of a local hospital.The surfactant and calcium carbonate accounted for the largest proportion of the detergent.The patient complained of vascular pain,chest discomfort,and nausea,and was transferred to our institution.After hospitalization,the patient’s serum creatinine level increased to 5.42 mg/dL,and his daily urine output decreased to approximately 300 mL.Renal biopsy findings noted that the glomeruli were relatively intact;however,diffuse acute tubular injury was observed.Generalized edema was also noted,and the patient underwent a total of four hemodiafiltration sessions.Afterward,the patient’s urine output gradually increased whereas the serum creatinine level decreased.The patient was discharged in a stable status without any sequelae.CONCLUSION Detergents appear to directly cause renal tubular injury by systemic absorption.In treating a patient with detergent poisoning,physicians should be aware that the renal function may also deteriorate.In addition,timely renal replacement therapy may help improve the patient’s prognosis.

A strategy for fabricating microcrystalline cellulose–Ti_(3)C_(2)T_(x)(MCC–MXene)nanocomposite films with high relative permittivity,high thermal conductivity,and excellent mechanical properties was developed.The MCC–MXene nanocomposite film was fabricated by casting a solution containing N,N-dimethylacetamide/lithium chloride(DMAc/LiCl)-soluble MCC and DMAcdispersible MXene nanosheets,followed by humidity control drying.The MXene nanosheets greatly enhanced the permittivity of the nanocomposite films owing to interfacial polarization.Thus,the nanocomposite film with 20 wt.%MXene content achieved a desirable permittivity of 71.4 at 102 Hz(a 770%improvement against that of neat cellulose),while the dielectric loss only increased by 1.8 times(from 0.39 to 0.70).The obtained nanocomposite films with 20 wt.%and 30 wt.%MXene exhibited remarkable in-plane thermal conductivities of 8.523 and 9.668 W∙m^(−1)∙K^(−1),respectively,owing to the uniform dispersion and selfalignment of the MXene layered structure.Additionally,the uniformly dispersed MXene nanosheets in the MCC network with interfacial interaction(hydrogen bonding)and mechanical entanglement endowed the nanocomposite films with excellent mechanical properties and flexibility.Furthermore,the thermal stability,water resistance,and antibacterial properties of the nanocomposite films were effectively improved with the introduction of MXene.Moreover,using DMAc/LiCl as the solvent system not only improves the compatibility between MCC and MXene but also avoids the problem of easy oxidation of MXene in aqueous systems.With the high stability of the MCC–MXene solution and enhanced properties of the MCC–MXene films,the proposed strategy manifests great potential for fabricating natural biomass-based dielectric materials.

Because of the intramural spread of gastric cancer,a sufficient length of a resection margin has to be attained to ensure complete excision of the tumor.There has been debate on an adequate length of proximal resection margin(PRM) and its related issues.Thus,the objective of this article is to review several studies on PRM and to summarize the current evidence on the subject.Although there is some discrepancy in the recommended values for PRM between authors,a PRM of more than 2-3 cm for early gastric cancer and 5-6 cm for advanced gastric cancer is thought to be acceptable.Once the margin is confirmed to be clear,however,the length of PRM measured in postoperative pathologic examination does not affect the patient's survival,even when it is shorter than the recommended values.Hence,the recommendations for PRM length should be applied only to intraoperative decision-making to prevent positive margins on the final pathology.Given that a negative resection margin is the ultimate goal of determining an adequate PRM,development and improvement of reliable methods to confirm a negative resection margin intraoperatively would minimize the extent of surgery and offer a better quality of life to more patients.In the same context,special attention has to be paid to patients who have advanced stage or diffuse-type gastric cancer,because they are more likely to have a positive margin.Therefore,a wider excision with intraoperative frozen section(IFS) examination of the resection margin is necessary.Despite all the attempts to avoid positive margins,there is still a certain rate of positive-margin cases.Since the negative impact of a positive margin on prognosis is mostly obvious in low N stage patients,aggressive further management,such as extensive re-operation,is required for these patients.In conclusion,every possible preoperative and intraoperative evaluation should be thoroughly carried out to identify in advance the patients with a high risk of having positive margins;these patients need careful management with a wider

Cystic Brunner's gland hamartoma in the duodenum is exceedingly rare,although microscopic examination may sometimes reveal a Brunner's gland hamartoma containing dilated ducts in the duodenum.We present a case of large cystic Brunner's gland hamartoma in the duodenum with a long stalk,which is described in light of multidetector-row computed tomography,magnetic resonance imaging,and a modified small bowel series,together with pathologic correlation and differential diagnosis.

General regular shaped diagrid structures can express diverse shapes because braces are installed along the exterior faces of the structures and the structures have no columns. However, since irregular shaped structures have diverse variables, studies to assess behaviors resulting from various variables are continuously required to supplement the imperfections related to such variables. In the present study, materials elastic modulus and yield strength were selected as variables for strength that would be applied to diagrid structural systems in the form of Twisters among the irregular shaped buildings classified by Vollers and that affect the structural design of these structural systems. The purpose of this study is to conduct sensitivity analysis for axial rotation diagrid structural systems according to changes in brace angles in order to identify the design variables that have relatively larger effects and the tendencies of the sensitivity of the structures according to changes in brace angles and axial rotation angles.

We report that the corrosion resistance of a Mg-Sn-based alloy with MgzSn precipitates can be considerably improved by surface modification using pulsed electron beam treatment.The alloy subjected to a pulse electron beam treatment showed a modified surface layer with a thickness of〜12 μm,appearing more resistant to corrosion attack than the bare surface of the alloy.In 0.6 M NaCl solution,the alloys with and without the surface modification exhibited average corrosion rates of 4.3 and 8.1 mm y^-1,respectively.The improved corrosion resistance was attributed to reduced cathodic activation,resulting from the surficial reduction of relatively noble Mg2Sn precipitates.

Duplex nanoporous Cu was successfully fabricated by dealloying a dual-phase Mg-Cu precursor alloy consisting of intermetallic Mg2Cu and MgCu2.The duplex nanoporous Cu with embedded nanoporous struts exhibited highly enhanced strength compared to the typical monolithic nanoporous Cu under both compressive and flexural test conditions at room temperature;the duplex np-Cu sample exhibited a 12 times higher compressive strength and a 40 times greater flexural strength than the monolithic np-Cu sample.Factors responsible for the strength enhancement in the duplex nanoporous Cu are discussed.

Compressive deformation behavior of an indirect-extruded Mg-8Sn-1Al-1Zn alloy

The medium and warm deformation behaviors of an indirect-extruded Mg-8Sn-IAI-IZn alloy were investigated by compression tests at temperatures between 298 and 523 K and strain rates of 0.001-10 s-1. It was found that the twinning-slip transition temperature was strain rate dependent, and all the true stress-true strain curves could be divided into two groups： concave and convex curves. Associated microstructural investigations indicated that the dynamic recrystallization （DRX） be- havior of the alloy varied with deformation conditions. At high strain rate and low temperature, dynamically recrystallized grains preferentially nucleated and developed in the twinned regions, indicating that twinning-induced DRX was dominant. While, at low strain rate, DRX developed extensively at grain boundaries and twins, and the process of twinning contributed to both oriented nucleation and selective growth. For the studied alloy, cracks mainly initiated from the shear band and twinning lamellar over the ranges of temperature and strain rate currently applied.

Facile Synthesis of Porous Carbon Nitride Spheres with Hierarchical Three-Dimensional Mesostructures for CO_(2) Capture

Porous carbon nitride(CN)spheres with partially crystalline frameworks have been successfully synthesized via a nanocasting approach by using spherical mesoporous cellular silica foams(MCFs)as a hard template,and ethylenediamine and carbon tetrachloride as precursors.The resulting spherical CN materials have uniform diameters of ca.4μm,hierarchical three-dimensional(3-D)mesostructures with small and large mesopores with pore diameters centered at ca.4.0 and 43 nm,respectively,a relatively high BET surface area of~550 m^(2)/g,and a pore volume of 0.90 cm^(3)/g.High-resolution transmission electron microscope(HRTEM)images,wide-angle X-ray diffraction(XRD)patterns,and Raman spectra demonstrate that the porous CN material has a partly graphitized structure.In addition,elemental analyses,X-ray photoelectron spectra(XPS),Fourier transform infrared spectra(FT-IR),and CO_(2) temperature-programmed desorption(CO_(2)-TPD)show that the material has a high nitrogen content(17.8 wt%)with nitrogen-containing groups and abundant basic sites.The hierarchical porous CN spheres have excellent CO_(2) capture properties with a capacity of 2.90 mmol/g at 25℃and 0.97 mmol/g at 75℃,superior to those of the pure carbon materials with analogous mesostructures.This can be mainly attributed to the abundant nitrogen-containing basic groups,hierarchical mesostructure,relatively high BET surface area and stable framework.Furthermore,the presence of a large number of micropores and small mesopores also enhance the CO_(2) capture performance,owing to the capillary condensation effect.
