Evaluation of Functional and Radiological Outcome of Arthroscopic-Assisted Anatomical Coracoclavicular (CC) and Acromioclavicular (AC) Ligament Reconstruction in Chronic AC Joint Dislocation

Introduction: Acromioclavicular (AC) joint dislocation is a common shoulder injury, comprising 9% - 12% of shoulder girdle injuries. Optimal management remains challenging, with treatment decisions guided by the Rockwood classification system. Controversies surround grade III injuries, necessitating further classification. Non-operative treatment has shown favorable outcomes, while surgical interventions vary. Anatomical coracoclavicular reconstruction (ACCR) has demonstrated biomechanical advantages over traditional methods. Arthroscopic techniques offer advantages, minimizing deltoid detachment and allowing concurrent pathology identification. This study evaluates the outcomes of arthroscopic-assisted ACCR in chronic AC joint dislocation. Surgical Technique: Arthroscopic-assisted ACCR involves meticulous portal placement, tendon graft harvesting, diagnostic arthroscopy, and coracoid exposure. The clavicle tunnels were made to mimic the conoid and trapezoid ligament positions, using FibreTape#2 loop and Dog Bone Button for correct placement against the coracoid base, and passing the semitendinosus graft through to reconstruct the conoid ligament, reduction done and graft follow through for anatomical reconstruction. Methods: A retrospective cohort study at Hospital Kuala Lumpur analyzed 35 patients undergoing arthroscopic-assisted ACCR for Rockwood grade III - V AC joint dislocations. Inclusion criteria encompassed trauma ≥ 3 weeks prior, no prior shoulder injuries, and ≥12-month follow-up. Functional and radiological assessments utilized ASES scores and coracoclavicular distances, respectively. Statistical analysis employed descriptive statistics and logistic regression. Results: The mean age was 38.9 years (SD 11.26), and 34 of 35 patients were male. Grade IV injuries were predominant (37.1%). Waiting time for surgery averaged 234.9 days. Controlled mutual quantum entity authentication using entanglement swapping

In this paper, we suggest a controlled mutual quantum entity authentication protocol by which two users mutually certify each other on a quantum network using a sequence of Greenberger–Horne–Zeilinger（GHZ）-like states. Unlike existing unidirectional quantum entity authentication, our protocol enables mutual quantum entity authentication utilizing entanglement swapping; moreover, it allows the managing trusted center（TC） or trusted third party（TTP） to effectively control the certification of two users using the nature of the GHZ-like state. We will also analyze the security of the protocol and quantum channel.

AIM:There is some evidence of functional superiority of colonic J-pouch over straight coloanal anastomosis (CM) in ultralow anterior resection (ULAR) or intersphincteric resection. On the assumption that colonic J-pouch anal anastomosis is superior to straight CM in ULAR with upper sphincter excision (USE: excision of the upper part of the internal sphincter) for low-lying rectal cancer, we compare functional outcome of colonic J-pouch vsthe straight CM. METHODS: Fifty patients of one hundred and thirty-three rectal cancer patients in whom lower margin of the tumors were located between 3 and 5 cm from the anal verge received ULAR including USE from September 1998 to January 2002. Patients were randomized for reconstruction using either a straight (n = 26) or a colonic J-pouch anastomosis (n = 24) with a temporary diverting-loop ileostomy. All patients were followed-up prospectively by a standardized questionnaire [Fecal Inco-ntinence Severity Index (FISI) scores and Fecal Incontinence Quality of Life (FIQL) scales]. RESULTS: We found that, compared to straight anastomosis patients, the frequency of defecation was significantly lower in J-pouch anastomosis patients for 10 mo after ileostomy takedown. The FISI scores and FIQL scales were significantly better in J-pouch patients than in straight patients at both 3 and 12 mo after ileostomy takedown. Furthermore, we found that FISI scores highly correlated with FIQL scales. CONCLUSION: This study indicates that colonic J-pouch anal anastomosis decreases the severity of fecal incontinence and improves the quality of life for 10 mo after ileostomy takedown in patients undergoing ULAR with USE for low-lying rectal cancer.

Characterization of Ga-doped ZnO Nanorods Synthesized via Microemulsion Method

Gallium-doped ZnO （GZO） nanorods were synthesized by microemulsion method with different types of surfactants. The phase and morphology of the above nanorods were investigated by scanning electron microscopy （SEM）, X-ray diffraction （XRD）, high resolution transmission electron microscopy （HR-TEM） and X-ray photoelectron spectroscopy （XPS）. SEM observations show that the ZnO nanorods have diameters around 70-200 nm and lengths up to several micrometers. The room temperature photoluminescence （PL） spectrum of GZO nanorods exhibited a sharp and strong ultraviolet bandgap at 383 nm and a relatively weaker emission associated with the defect level. Resistivity of GZO nanorods synthesized with sodium lauryl sulfate （SLS） and sodium benzene sulfonate （SBS） surfactants showed 2.84 Ω m and 14.2 Ω m, respectively.

Performance of pulsed plasma thruster at low discharge energy

As the size of satellites scales down, low-power and compact propulsion systems such as the pulsed plasma thruster(PPT) are needed for stabilizing these miniature satellites in orbit. Most PPT systems are operated at 2 J or more of discharge energy. In this work, the performance of a PPT with a side-fed, tongue-flared electrode configuration operated within a lower discharge energy range of 0.5-2.5 J has been investigated. Ablation and charring of the polytetrafluoroethylene propellant surface were analyzed through field-effect scanning electron microscopy imaging and energy-dispersive X-ray spectroscopy. When the discharge energy fell below 2 J, inconsistencies occurred in the specific impulse and the thrust efficiency due to the measurement of the low mass bit. At energy ≥2 J, the performance parameters are compared with other PPT systems of similar configuration and discussed in depth.

AIM: Expression of heat shock proteins (HSPs) is frequently up-regulated in hepatocellular carcinoma (HCC), which evolves from dysplastic nodule (DN) and early HCC to advanced HCC. However, little is known about the differential expression of HSPs in multistep hepatocarcinogenesis. It was the purpose of this study to monitor the expression of HSPs in multistep hepatocarcinogenesis and to evaluate their prognostic significance in hepatitis B virus (HBV)related ***: Thirty-eight HCC and 19 DN samples were obtained from 52 hepatitis B surface antigen-positive Korean patients. Immunohistochemical and dot immunoblot analyses of HSP27, HSP60, HSP70, HSP90, glucoseregulated protein (GRP)78, and GRP94 were performed and their expression at different stages of HCC development was statistically ***: Expression of HSP27, HSP70, HSP90, GRP78, and GRP94 increased along with the stepwise progression of hepatocarcinogenesis. Strong correlation was found only in GRP78 (Spearman's r= 0.802). There was a positive correlation between the expressions of GRP78, GRP94, HSP90, or HSP70 and prognostic factors of HCC. Specifically, the expression of GRP78, GRP94, or HSP90 was associated significantly with vascular invasion and intrahepatic ***: The expressions of HSPs are commonly up-regulated in HBV-related HCCs and GRP78 might play an important role in the stepwise progression of HBVrelated hepatocarcinogenesis. GRP78, GRP94, and HSP90 may be important prognostic markers of HBV-related HCC, strongly suggesting vascular invasion and intrahepatic metastasis.

BACKGROUND The optimal treatment for heart failure(HF)is a combination of appropriate *** the disease using only medical therapy is difficult in patients with HF,severe hypercapnia,and *** patients should first receive ventilator support followed by pulmonary rehabilitation(PR).CASE SUMMARY We report two cases in which arterial blood gas(ABG)improved and PR was possible with appropriate ventilator *** patients with extreme obesity complaining of worsening dyspnea–a 47-year-old woman and a 36-year-old man both diagnosed with HF–were hospitalized because of severe hypercapnia and *** proper medical treatment,hypercapnia and desaturation resolved in neither case,and both patients were transferred to the rehabilitation department for *** the time of the first consultation,the patients were bedridden because of *** demand was successfully reduced once noninvasive ventilation was *** the patients’dyspnea gradually improved to the point where they could be weaned off the ventilator during the daytime,they started engaging in functional training and aerobic *** 4 mo of followup,both patients were able to perform activities of daily living and maintain their lower body weight and normalized ABG *** Symptoms of patients with obesity and HF may improve once ABG levels are normalized through ventilator support and implementation of PR.

To improve humor circulation the latest V4c Visian implantable collamer lens （ICL） was designed with a0.36-ram Aquapon. This design also eliminates the need to perform peripheral iridectomy before ICL implantation. Several authors reported rates of secondary surgical intervention related to insufficient vault in the presence or absence of cataract formation and excessive vault in the presence or absence of elevated intraocular pressure （IOP） after ICL insertion. Here, we report a case of rapid progression of a cataract to a hypermature state after implantation of an ICL with an Aquaport.
