＜正＞PURPOSE: High-dose preoperative radiotherapy has shown promising results in the treatment of low rectal cancer（LRC）. Laparoscopic procedure provides an enhanced view of the pelvic cavity that is not available by conventional method. We developed a sphincter-saving procedure combining the advantages of preoperative radiotherapy and laparoscopy for preserving anatomic and physiologic anal function. Our purpose is to show detailed information regarding procedure and early results. METHODS: From February 1996 to August 1997, eleven patients with LRC underwent the operation. Mean age was 56.1 years. Tumor located 4～8 cm from anal verge. All patients were treated with high-dose preoperative irradiation （4500～5040 cGy） over a period of 5 weeks. All were given our procedure four and a half to eight weeks after completion of radiation. Left colon and rectum were mobilized by laparoscopic oncologic dissection down to puborectalis muscle, after which specimen retrieval and coloanal anastomosis were performed transanally. We concluded with temporary transverse colostomy for fecal diversion. Careful follow-up of 4 to 21 months has so far been given. RESULTS: Every case was successful without any significant complication, and all revealed advantages of laparoscopy. There was one case of complete remission following radiation, and "five were stage Ⅰ, four stage Ⅱ, and one stage Ⅲ. Margin from the tumor was 1～5 cm distally, 19～32 cm proximally, 0.3～3.5 cm laterally. We had one local recurrence 11 months after operating. CONCLUSION: We have termed our procedure ’Laparoscopic Abdominal Transanal （LATA） Proctosigmoidectomy with coloanal anastomosis （LATA resection）’, and propose it as a functional and curative alternative for LRC while continuing to evaluate its efficacy.

AIM To investigate the rates of pretransplantation fetalmaternal microchimerism(MC) and its effect on rejection in children receiving maternal liver grafts. METHODS DNA or blood samples before liver transplantation(LT) were available in 45 pediatric patients and their mothers. The presence of pretransplantation MC to non-inherited maternal antigens(NIMAs)(NIMA-MC) in the peripheral blood was tested using nested PCRsingle-strand conformation polymorphism analysis for the human leukocyte antigen(HLA)-DRB1 alleles. NIMA-MC was successfully evaluated in 26 of the 45 children. Among these 45 pediatric LT recipients,23 children(51.1%) received transplants from maternal donors and the other 22 from non-maternal *** Among these 26 children,pretransplantation NIMAMC was detected in 23.1%(n = 6),6.1(range,0.8-14) years after birth. Among the children with a maternal donor,the rate of biopsy-proven cellular rejection(BPCR) was 0% in patients with NIMA-MC positivity(0/3) and those with HLA-DR identity with the mother(0/4),but it was 50% in those with NIMA-MC negativity(5/10). Patients with NIMA-MC positivity or HLA-DR identity with the mother showed significantly lower BPCR rate compared with NIMA-MC-negative patients(0% vs 50%,P = 0.04). NIMA-MC-positive patients tended to show lower BPCR rate compared with NIMAMC-negative patients(P = 0.23). CONCLUSION The presence of pretransplantation NIMA-MC or HLADR identity with the mother could be associated with BPCR-free survival in pediatric recipients of LT from maternal donors.

Room temperature deformation behavior of A1-8Fe-2Mo-2V-1Zr alloy produced by spray forming and subsequent warm-extrusion at 693 K and/or rolling at 423 K was investigated in terms of tensile test and microstructural *** the specimen processed by spray forming and warm extrusion, the microstructure consisting of equiaxed grains with the average grain size of 500 nm in the matrix phase and uniformly dispersed fine intermetallic A13(Fe, Mo, V, Zr) and A16(Fe, Mo, V, Zr) phases less than 100 nm was characterized. It was revealed that subsequent warm rolling after warm extrusion promotes precipitation of a fine dispersoid from the supersaturated matrix phase. Warm rolling was found to be effective to increase the yield and tensile strength in the high strain rate regime. Elongation of the warm rolled specimen showed highe r value than extruded specimen over the whole strain rate region studied.

To the Editor： Although rare, multiple adverse events associated with anti-tuberculosis （TB） medication can occur simultaneously in patients with underlying diseases. Here, we describe a 76-year-old man with several preexisting disorders in whom anti-TB medication caused multiple, simultaneous adverse events, including Stevens-Johnson syndrome （SJS）, hypothermia, and lactic acidosis.

Interstitial lung disease （ILD） is a comprehensive term referring to a group of lung diseases affecting the interstitium of the lung. Idiopathic pulmonary fibrosis （IPF） is the most common idiopathic ILD, and nonspecific interstitial pneumonia （NSIP） is the second most common. As the name suggests, NSIP is diagnosed atter many other diseases are excluded. The main pathological finding in NSIP is homogeneous interstitial inflammation with or without fibrosis. NSIP can be categorized by cellular type or fibrotic type, according to the grade of inflammation and fibrosis. The cellular type has mostly inflammatory lesions with good responses to steroid, but the fibrotic type has a large proportion of fibrosis mixed with inflammatory lesions and a relatively poor response to steroid treatment So far, the exact mechanism underlying idiopathic lED has not been clarified. Determining key regulators of these ILDs will be helpful in the diagnosis and development of novel drugs for ILD.

研究植物源性发酵乳杆菌对免疫系统生理活性、抗高血糖和促进肠蠕动的影响。β-内啡肽对免疫系统有重要作用,阿片-促黑素细胞皮质素原(POMC)是β-内啡肽的前体物质,为了探讨发酵乳杆菌对免疫功能的影响,检测小鼠下丘脑和脑垂体中POMC的基因表达水平。通过喂食发酵乳杆菌时间和浓度的不同及测量β-内啡肽表达水平的方式,来分析血液中β-内啡肽的变化情况。此外,测定了三种小鼠模型的血糖变化,即常规的WT小鼠模型、D-葡萄糖诱导的急性高血糖模型和链唑霉素(STZ)诱导的糖尿病模型,以证实发酵乳杆菌的降血糖效果,并对降血糖效果进行了验证。通过检测血糖随喂食发酵乳杆菌浓度和时间而变化的情况,已证实给药浓度和时间对血糖浓度有很好的影响。另外,为证实发酵乳杆菌对肠蠕动的影响测定了胃肠通过时间,证明发酵乳杆菌对肠蠕动有极好的促进作用。通过此项研究,证实发酵乳杆菌对提高人体免疫系统的生理活性、抗高血糖和促进小肠蠕动有极佳的效果。
