National trends in resection of the distal pancreas

AIM:To investigate national trends in distal pancreatectomy(Dp) through query of three national patient care ***:From the Nationwide Inpatient Sample(NIS,2003-2009),the National Surgical Quality Improvement project(NSQIp,2005-2010),and the Surveillance Epidemiology and End Results(SEER,2003-2009) databases using appropriate diagnostic and procedural codes we identified all patients with a diagnosis of a benign or malignant lesion of the body and/or tail of the pancreas that had undergone a partial or distal *** of laparoscopy was defined in NIS by the International Classification of Diseases,Ninth Revision correspondent procedure code;and in NSQIp by the exploratory laparoscopy or unlisted procedure current procedural terminology *** SEER,patients were identified by the International Classification of Diseases for Oncology,Third Edition diagnosis codes and the SEER program Code Manual,third edition procedure *** analyzed the databases with respect to trends of inpatient outcome metrics,oncologic outcomes,and hospital volumes in patients with lesions of the neck and body of the pancreas that underwent operative ***:NIS,NSQIp and SEER identified 4242,2681 and 11 082 Dp resections,***,laparoscopy was utilized in 15%(NIS) and 27%(NSQIp).No significant increase was seen over the course of the *** was performed for malignancy in 59%(NIS) and 66%(NSQIp).Neither patient Body mass index nor comorbidities were associated with operative approach(p = 0.95 and p = 0.96,respectively).Mortality(3% vs 2%,p = 0.05) and reoperation(4% vs 4%,p = 1.0) was not different between laparoscopy and open *** complications(10% vs 15%,p pital costs [44 741 dollars,interquartile range(IQR) 28 347-74 114 dollars vs 49 792 dollars,IQR 13 299-73 463,p = 0.02] and hospital length of stay(7 d,IQR 4-11 d vs 7 d,IQR 6-10,p paroscopy was *** and two year sur

Androgen receptor functions in prostate cancer development and progression

The androgen receptor （AR） is critical for the normal development ofprostate and for its differentiated functions. The consistent expression of AR in prostate cancer （pCa）, and its continued activity in pCa that relapse after androgen deprivation therapy （castration-resistant prostate cancer （CRpC））, indicate that at least a subset of these genes are also critical for pCa development and progression. This review addressed AR regulated genes that may be critical for pCa, and how AR may acquire new functions during pCa development and progression.

Extraintestinal heterotopic gastric tissue simulating acute appendicitis

We describe the case of a 68-year-old otherwise healthy male who presented to our emergency room with signs and symptoms of acute appendicitis. Exploratory surgery revealed a normal appendix. Further examination revealed an enlarged lymph node-like mass of tissue near the appendix, in the ileocecal mesentery. This mass was removed and was found to be inflamed heterotopic gastric tissue. Although reports of heterotopic gastric tissue in the literature are common, we believe that this case represents the first report of inflamed heterotopic gastric tissue simulating appendicitis.

Kaempferia parviflora ethanol extract improves self-assessed sexual health in men:a pilot study

Background： Sexual health positively correlates with overall wellbeing. Existing therapeutics to enhance male sexual health are limited by factors that include responsiveness, adherence and adverse effects. As the population ages, safe and effective interventions that preserve male sexual function are needed. published research suggests that various preparations of Kaempferia parviflora, a plant in the Zingiberaceae （ginger） family, support cardiovascular health and may ameliorate erectile function. Objective： The aim of this study was to examine the effects of KaempMaxTM, an ethanol extract of the K. parvi]lora rhizome, on erectile function in healthy middle-aged and older men. Design, setting, participants and interventions： We conducted an open-label, one-arm study on 14 generally healthy males aged 50-68 years with self-reported mild erectile dysfunction, who were not using pre- scription treatments. participants took 100 mg KaempMaxTM daily for 30 days. Main outcome measures： Evaluations were conducted at baseline and on the final study assessment. primary efficacy analyses included the International Index of Erectile Function （IIEF）; secondary efficacy analyses included the Global Assessment Question about erectile function. Results： Thirteen participants completed the 30-day study. Supplementation with KaempMaxTM resulted in statistically significant improvements in erectile function, intercourse satisfaction and total scores on the IIEF questionnaire. KaempMaxTM was well tolerated and exhibited an excellent safety profile. Conclusion： Our results suggest that KaempMaxTM may improve erectile function in healthy middle-aged and older men. While the effects were not as pronounced as what might be seen with prescription med- ication, most participants found them satisfactory. Growth hormone releasing peptide 2 reverses anorexia associated with chemotherapy with 5-fluoruracil in colon cancer cell-bearing mice

The cancer-associated anorexia-cachexia syndrome is observed in 80% of patients with advanced-stage cancer, and is one of the major obstacles in chemo- therapy. Ghrelin is a orexigenic hormone that has been proposed to prevent anorexia. Aim of the study was to determine whether the addition of the ghrelin agonist growth hormone releasing peptide 2 (GHRp-2) to cytotoxic therapy with 5-fluoruracil (5-FU) prevents the anorexia associated with chemotherapy in cancer cachectic mice. Thirty-three BALB/c female tumourbearing mice were randomized to receive a solution containing: (a) placebo; (b) GHRp-2; (c) 5-FU; or (d) 5-FU + GHRp-2. Ten BALB/c no tumour-bearing mice received placebo solution. Food intake and survival were checked. Six hours after the drug injection the cumulative food intake was signifi cantly increased in mice treated with the combination of 5-FU + GHRp-2 versus the 5-FU alone (p = 0.0096). On day 3, the cumulative food intake of mice treated with GHRp-2,5-FU and 5-FU + GHRp-2 signifi cantly increased com- pared with naive and vehicle groups (p = 0.0007, p = 0.0038 and p = 0.0166, respectively). The median survival time was longer in 5-FU + GHRp-2 treated mice than in those with 5-FU, although it was not signifi cant (18 d versus 15.5 d, p = 0.7). For the fi rst time, we demonstrated that the addition of GHRp-2 to cytotoxic therapy with 5-FU improved appetite in tumour-bearing mice with anorexia/cachexia syndrome in early stage. These data suggest that GHRp-2 may improve the effi cacy of therapy and the quality of life of cancer patients thank to the amelioration of their nutritional state.
