Feasibility of pancreatectomy following high-dose proton therapy for unresectable pancreatic cancer

AIM To review surgical outcomes for patients undergoing pancreatectomy after proton therapy with concomitant capecitabine for initially unresectable pancreatic *** From April 2010 to September 2013,15 patients with initially unresectable pancreatic cancer were treated withproton therapy with concomitant capecitabine at 1000 mg orally twice daily. All patients received 59.40 Gy(RBe) to the gross disease and 1 patient received 50.40 Gy(RBe) to high-risk nodal targets. There were no treatment interruptions and no chemotherapy dose reductions. Six patients achieved a radiographic response sufficient to justify surgical exploration,of whom 1 was identified as having intraperitoneal dissemination at the time of surgery and the planned pancreatectomy was aborted. Five patients underwent resection. Procedures included:Laparoscopic standard pancreaticoduodenectomy(n = 3),open pyloris-sparing pancreaticoduodenectomy(n = 1),and open distal pancreatectomy with irreversible electroporation(IRe) of a pancreatic head mass(n = 1). ReSULTS The median patient age was 60 years(range,51-67). The median duration of surgery was 419 min(range,290-484),with a median estimated blood loss of 850 cm^3(range,300-2000),median ICU stay of 1 d(range,0-2),and median hospital stay of 10 d(range,5-14). Three patients were re-admitted to a hospital within 30 d after discharge for wound infection(n = 1),delayed gastric emptying(n = 1),and ischemic gastritis(n = 1). Two patients underwent R0 resections and demonstrated minimal residual disease in the final pathology specimen. One patient,after negative pancreatic head biopsies,underwent IRe followed by distal pancreatectomy with no tumor seen in the specimen. Two patients underwent R2 resections. Only 1 patient demonstrated ultimate local progression at the primary site. Median survival for the 5 resected patients was 24 mo(range,10-30).CONCLUSION Pancreatic resection for patients with initially unresectable cancers is feasible after

Planning a road weather information system with GIS

The road weather information system （RWIS）, which collects and monitors weather and pavement surface conditions, has been proven effective to support winter road maintenance by improving safety, mobility, and effi- ciency. Although the geographic information systems are being widely applied for facility siting, traditional practices of sitting RWIS stations still heavily rely on the experi- ences of maintenance and operation personnel, which is time-consuming and subjective. This study develops a linear model to determine the optimal RWlS locations subject to three selection criteria： weather, traffic condition, and distance to existing RWIS sites, while considering practical constraints to satisfy specific requirements of various agencies （e.g., different weights of weather/traffic factors, various available budgets, etc.）.

Purpose: To examine possible association between heart irradiation and Overall Survival (OS) in lung SBRT patients and to compare observed associations with cardiac toxicity models previously derived in LA-NSCLC patient studies. Materials and Methods: 197 Patients treated with lung SBRT at Mayo Clinic Arizona were selected for this IRB-approved study. Multivariate Cox model with Akaike Information Criterion (AIC) was used to select patient specific covariates associated with OS. Deep Rank-Based Average Pooling Network for Covid-19 Recognition

(Aim)To make a more accurate and precise COVID-19 diagnosis system,this study proposed a novel deep rank-based average pooling network(DRAPNet)model,i.e.,deep rank-based average pooling network,for COVID-19 recognition.(Methods)521 subjects yield 1164 slice images via the slice level selection *** the 1164 slice images comprise four categories:COVID-19 positive;community-acquired pneumonia;second pulmonary tuberculosis;and healthy *** method firstly introduced an improved multiple-way data ***,an n-conv rankbased average pooling module(NRAPM)was proposed in which rank-based pooling—particularly,rank-based average pooling(RAP)—was employed to avoid ***,a novel DRAPNet was proposed based on NRAPM and inspired by the ***-CAM was used to generate heatmaps and gave our AI model an explainable analysis.(Results)Our DRAPNet achieved a micro-averaged F1 score of 95.49%by 10 runs over the test *** sensitivities of the four classes were 95.44%,96.07%,94.41%,and 96.07%,*** precisions of four classes were 96.45%,95.22%,95.05%,and 95.28%,*** F1 scores of the four classes were 95.94%,95.64%,94.73%,and 95.67%,***,the confusion matrix was given.(Conclusions)The DRAPNet is effective in diagnosing COVID-19 and other chest infectious *** RAP gives better results than four other methods:strided convolution,l2-norm pooling,average pooling,and max pooling.

employment and Patient Satisfaction after Liver Transplantation

Background and Aims:This study serves to revisit the effects of liver transplantation(LT)on employment in an era of improving survival outcomes post-transplant,and to identify areas of improvement in the transplant process to better optimize post-LT employment and patient ***:Prospectively,patients who had undergone LT at a single tertiary LT center were surveyed in person and by *** outcomes included employment rate pre-and post-LT,annual salary,weekly hours worked,barriers to re-employment,and patient ***:Responses were collected and analyzed from 121 patients who underwent ***-LT,68(56.1%)reported full-time employment,13(10.7%)part-time employment,and 40(33.1%)***-LT,26(21.4%)reported continued full-time employment,18(14.9%)part-time employment,and 77(63.6%)*** weekly work hours decreased post-LT(16.1 h/week vs.39.9 h/week).Mean annual salaries decreased post-LT(17 earning salary≥$40,000 vs.56 earning salary≥$40,000).These outcomes differed from patient pre-LT expectations,with 81.0%of previously employed patients believing they would return to employment,resulting in decreased patient *** working physically demanding jobs pre-LT were less likely to return to *** cited for lack of return to full employment included early fatigue and difficulty regaining physical ***:Re-employment rates remain low post-LT,which is particularly true for patients working physically active *** is a significant barrier to re-employment and increased physical rehabilitation post-LT may prove to be *** should be given realistic expectations about return to employment prior to their LT.

Background:Despite considerable advances in preoperative imaging,up to one-third of patients operatively explored for hepatic colorectal metastases are unexpectedly found to harbor unresectable intrahepatic or extrahepatic ***:The current study is a prospective,blinded study comparing utility of[18F]2-fluoro-2-deoxyglucose positron emission tomography(18F-FDG-PeT)to computed tomography(CT)and CT arterial portography(CTAP)as preoperative ***:The 125 planned subjects were *** seen on FDG-PeT alone changed therapy for 23 of 125 patients(18%).FDG-PeT confirmed other radiologic findings in 16 cases(13%),for an overall influence on therapy in 39 cases(31%).FDG-PeT was the most sensitive diagnostic imaging test for extrahepatic cancer;it was 80-90%sensitive for extrahepatic cancer and 70-90%*** the 28 cases of unresectable disease due to extrahepatic disease,FDG-PeT findings solely changed therapies in 16 cases(57%)and influenced therapy in seven other cases(25%).Of the 21 unresectable cases due to extent of intrahepatic disease,FDG-PeT did not solely change therapy in ***,FDG-PeT had the lowest sensitivity for hepatic sites compared with CT or *** particular,small(er tumors were particularly poorly detected by *** area under the receiver operating characteristic(ROC)curve for small tumors was 0.58 and for patients on chemotherapy it was 0.66,a modest improvement over no ***:FDG-PeT is an important test for preoperative staging of patients with hepatic colorectal metastases,affecting treatment decisions in nearly one-third of *** high yield is due mainly to detection of extrahepatic *** is therefore recommended in patients with extrahepatic lesions suspected to be disseminated cancer or those with high risk for extrahepatic *** is not a good test for identification of small tumors in the liver.

The~790 ka Australasian(micro)tektite strewn field is one of the most recent and best-known examples of impact ejecta emplacement as the result of a large-scale cratering event across a considerable part of earth's surface(>10%in area).The Australasian strewn field is characterized by a tri-lobe pattern consisting of a large central distribution lobe,and two smaller side lobes extending to the west and ***,we report on the discovery of microtektite-like particles in sedimentary traps,containing abundant micrometeorite material,in the Sør Rondane Mountain(SRM)range of east *** thirty-three glassy particles display a characteristic pale yellowcolor and are predominantly spherical in shape,except for a single dumbbell-shaped *** vitreous spherules range in size from220 to 570μm,with an average diameter of~370μ*** compares relatively well with the size distribution(75–778μm)of Australasian microtektites previously recovered from the TransantarcticMountains(TAM)and located ca.2500–3000 km fromthe *** addition,the chemical composition of the SRM particles exhibits limited variation and is nearly identical to the‘normal-type’(i.e.,e systematics for a single batch of SRM particles(n=26)strongly support their affiliation with TAMmicrotektites and the Australasian tektite strewn field in ***,Sr isotope ratios and Nd model ages suggest that the target material of the SRM particles was composed of a plagioclase-or carbonate-rich lithology derived from a Paleo-or Mesoproterozoic crustal *** affiliation to the Australasian strewn field requires long-range transportation,with estimated great circle distances of ca.11,600 km from the hypothetical source crater,provided transportation occurred along the central distribution *** is in agreement with the observations made for the Australasian microtektites recovered from Victoria Land(ca.11,000 km)and Larkman Nunatak(ca.12,000 km),w

How to impute study-specific standard deviations in metaanalyses of skewed continuous endpoints?

AIM： To compare four methods to approximate mean and standard deviation （SD） when only medians and interquartile ranges are ***： We performed simulated meta-analyses on six datasets of 15, 30, 50, 100, 500, and 1000 trials, respectively. Subjects were iteratively generated from one of the following seven scenarios： five theoreti-cal continuous distributions [Normal, Normal （0, 1）, Gamma, exponential, and Bimodal] and two real-life distributions of intensive care unit stay and hospital stay. For each simulation, we calculated the pooled estimates assembling the study-specific medians and SD approximations： Conservative SD, less conservativeSD, mean SD, or interquartile range. We provided a graphical evaluation of the standardized *** show which imputation method produced the best estimate, we ranked those differences and calculated the rate at which each estimate appeared as the best, second-best, third-best, or ***： Our results demonstrated that the best pooled estimate for the overall mean and SD was provided by the median and interquartile range （mean standardized estimates： 4.5 ± 2.2, P = 0.14） or by the median and the SD conservative estimate （mean standardized estimates： 4.5 ± 3.5, P = 0.13）. The less conservative approximation of SD appeared to be the worst method, exhibiting a significant difference from the reference method at the 90% confidence level. The method that ranked first most frequently is the interquartile range method （23/42 = 55%）, particularly when data were generated according to the Standard Normal, Gamma, and exponential distributions. The second-best is the conservative SD method （15/42 = 36%）, particularly for data from a bimodal distributionand for the intensive care unit stay variable. CONCLUSION： Meta-analytic estimates are not signi-fcantly affected by approximating the missing values ofmean and SD with the correspondent values for medianand interquartile range.
