Xanthogranulomatous inflammation (XGI) is a disease of unknown origin, most frequently described in the kidney and gallbladder; its localization in the colorectal tract is extremely rare. The extension of the typical inflammatory process to the surrounding tissues may lead to misdiagnosis as cancer. We report the case of a 56-year-old woman presenting to the Emergency Department with pain, increased levels of α1 and α2 proteins and C-reactive protein (17.5 mg/dL; normal value 0-0.5), and a palpable mass, localized in the right lower quadrant of the abdomen. A computed tomography scan showed a large right cecal mass with necrotic areas, local inflammation of retroperitoneal fat, and enlargement of local lymph nodes. Because of the high suspicion of colic abscess as well as malignancy and worsening of the clinical condition, the patient underwent right colectomy after 4 d of antibiotic treatment. Pathology revealed xanthogranulomatous inflammation involving the ileocecal valve. We review the reports of large bowel tract XGI in the international literature.

An <i>in Vitro</i>Evaluation of Pressure Generated by Programmed Intermittent Epidural Bolus (PIEB) or Continuous Epidural Infusion (CEI)

PIEB has been reported as being superior to CEI for labor analgesia. The aim of this study was to measure the pressures generated by two commercially available pumps (CADD PIB, Smiths and Gemstar, Hospira) when delivering PIEB or CEI at 2 commonly used rates of infusion (10 or 5 mL). The two pumps were set to deliver fluid at four rates (CEI: 5 or 10 mL/h;PIEB: 5 or 10 mL every h) and connected to a pressure transducer (PCB Piezotronics 1500 connected to a NI USB-6251 Screw Terminal) to determine the pressures applied during each infusion. The peak pressure generated during the PIEB mode was consistently higher when compared to the CEI mode in both pumps. When comparing the two pumps in the PIEB 10 mL every hour mode, the peak pressures were approximately the same, while the pressure pattern differed. For each cycle the pressure generated by Gemstar oscillated from 25 mmHg to 0 mmHg;whereas with the CADD pump the baseline pressure was above 0. The Gemstar pump piston frequency (1.2 Hz) was twice as high as the CADD (0.5 Hz), so the volume delivered per cycle was lower. In the PIEB 5 mL every hour mode the peak pressures followed the same wave patterns. However, unlike the 10 mL mode, the pump piston frequency was approximately the same in both pumps. In both the CEI 5 or 10 mL/h modes, the CADD peak pressure was four times greater than the Gemstar. Both cycled between their respective peak pressures and 0 mmHg. Since the peak pressure of the CADD was approximately the same with PIEB and CEI, we speculated that the most important factor affecting the distribution of the solution in the epidural space was not the peak pressure per se, but the mode of delivery of the bolus.

Although the rate of patients reporting satisfaction is generally high after joint replacement surgery, up to 23% after total hip replacement and 34% after total knee arthroplasty of treated subjects report discomfort or pain 1 year after surgery. Moreover, chronic or subacute inflammation is reported in some cases even a long time after surgery. Another open and debated issue in prosthetic surgery is implant survivorship, especially when related to good prosthesis bone ingrowth. Pulsed Electro Magnetic Fields(PEMFs) treatment, although initially recommended after total joint replacement to promote bone ingrowth and to reduce inflammation and pain, is not currently part of usual clinical practice. The purpose of this review was to analyze existing literature on PEMFs effects in joint replacement surgery and to report results of clinical studies and current indications. We selected all currently available prospective studies or RCT on the use of PEMFs in total joint replacement with the purpose of investigating effects of PEMFs on recovery, pain relief and patients’ satisfaction following hip, knee or shoulder arthroplasty. All the studies analyzed reported no adverse effects, and good patient compliance to the treatment. The available literature shows that early control of joint inflammation process in the first days after surgery through the use of PEMFs should be considered an effective completion of the surgical procedure to improve the patient’s functional recovery.

Food allergy is a common and increasing problem worldwide. The newly-found knowledge might provide novel experimental strategies, especially for laboratory diagnosis. Approximately 20% of the population alters their diet for a perceived adverse reaction to food, but the application of double-blind placebo-controlled oral food challenge, the ＂gold standard＂ for diagnosis of food allergy, shows that questionnaire-based studies overestimate the prevalence of food allergies. The clinical disorders determined by adverse reactions to food can be dassified on the basis of immunologic or nonimmunologic mechanisms and the organ system or systems affected. Diagnosis of food allergy is based on clinical history, skin prick tests, and laboratory tests to detect serum-food specific IgE, elimination diets and challenges. The primary therapy for food allergy is to avoid the responsible food. Antihistamines might partially relieve oral allergy syndrome and IgE-mediated skin symptoms, but they do not block systemic reactions. Systemic corticosteroids are generally effective in treating chronic IgE-mediated disorders. Epinephrine is the mainstay of treatment for anaphylaxis. Experimental therapies for IgE-mediated food allergy have been evaluated, such as humanized IgG anti-IgE antibodies and allergen specific immunotherapy.

Objective:To evaluate in vitro effects of Tagetes minuta L. essential oil(TEO) on L3 Anisakis larvae type 1. Methods:In order to evaluate the potential use of Tagetes minuta essential oil against L3 Anisakis larvae three different media were tested:1) a saline solution(SS); 2) an industrial marinating solution; 3) sunflower seeds oil(SO). For each media and concentrations of TEO(0.1%,0.5%,1.0% and 5.0% v/v),20 parasites were introduced into plastic Petri dishes(diameter 90 mm) and maintained at room temperature. As controls,larvae were maintained without TEO under identical experimental conditions in SS,MS and SO. A total of 900 larvae were tested. The normalized mean viability,LT100,LT50 and the percentage of inactivation at 24 hours were calculated. Results:In vitro tests revealed a complete inactivation of parasites in saline solution after 2 hours with 5% and 1% of TEO. In marinating solution,a complete inactivation of parasites was observed after 4 hours at all concentrations used. A slower activity for all TEO concentration was reported in SO. Conclusions:The results obtained,showing a strong activity against Anisakis larvae,confirm TEO as a larvicidal agent in the treatment of human anisakidosis and in the industrial marinating process.

Assessment of Potential Nutrient Release from Phosphate Rock and Dolostone for Application in Acid Soils

Finding alternative local sources of plant nutrients is a practical, low-cost, and long-term strategy. In this study, laboratory column experiments were conducted in a completely randomized design to evaluate the feasibility of using phosphate rock and dolostone as fertilizers or acid-neutralizing agents for application in tropical acid soils. The dissolution rates of different particle-size fractions(0.063–0.25, 0.25–0.5, and 0.5–2 mm) of both rocks were studied by citric acid solution at p H 4 and 2 and water, with extraction times of 1, 3, 5, 7, 12, 24, 72, 144, 240, and 360 h. The results showed that the dissolution of both rocks depended on the particle size,leaching solution, and extraction time. The dissolution rate of rock-forming minerals increased as the specific surface area increased,corresponding to a decrease in particle size. In all cases, the release kinetics was characterized by two phases: 1) a first stage of rapid release that lasted 24 h and would ensure short-term nutrient release, and 2) a second stage of slow release after 24 h, representing the long-term nutrient release efficiency. Both rocks were suitable as slow-release fertilizers in strongly acid soils and would ensure the replenishment of P, Ca, and Mg. A combination of fine and medium particle-size fractions should be used to ensure high nutrient-release efficiency. Much work could remain to determine the overall impact of considerable amounts of fresh rocks in soils.
